Objectives: Potential Learner outcomes: a. What has been achieved to date? Updates on Scottish Integrated Care Pathways for Falls & Bone Health with Telehealth, New Zealand Community Exercise Programme, National AFFINITY Project and Irish Hip Fracture Database b. What now needs to be the focus? Learning what works and doesn’t work from real life examples of Falls and Bone Health programmes/pathways as they have evolved c. How can we make it happen together? Multi-disciplinary Teams / individuals from various locations will be invited to work within their work-stream of choice to share best practice (poster/oral presentations) that are linked to the Integrated Falls and Bone Health system JIGSAW and consult to the proposed FRAMEWORK Please see programme for the day overleaf Learn from approaches taken to improving and scaling up falls and bone health services in Ireland and abroad Network with other disciplines, agencies and sectors, sharing knowledge and experiences, enhancing their capabilities and strengthening communities of practice Engage with the national AFFINITY project regarding the supports required to enable local implementation Attendees will have the opportunity to: Questions to be addressed: To promote the implementation of integrated falls and bone health services locally and nationally.
