Kicking off the project, our team will coordinate with all parties (i.e. Grant County, utilities, permitting agencies, and constituent groups). USI will develop a rapid response communication plan with input from Grant County. Project progress reports will be submitted to Grant County Highway Department showing progression in accordance with the proposed schedule. The three-span steel beam bridge, built circa 1959, carries Stone Road over Lugar Creek. The bridge is located southeast of Marion, Grant County, Indiana. The three-span bridge has a total length of 86 feet. The 20-foot wide bridge and roadway is narrow for two lanes of traffic. This bridge is not listed on the Indiana Historic Bridge Inventory; no special exception will be required.

1) >> EXISTING CONDITIONS A review of the October 19, 2015, NBIS Structural Inventory and Appraisal report revealed that the superstructure, including the deck, is in poor condition, and that the substructure is in fair condition. Overall, the bridge has an unofficial sufficiency rating of 14.8 (Generally, anything under 50 is recommended for replacement) and is labeled “Structurally Deficient”, making the bridge an excellent candidate for replacement. USI personnel, after visiting the site on March 23, 2016, agree with the bridge inspection’s recommendation to replace the bridge.

According to FEMA FIRM maps, Stone Road floods during the Q100 storm event, posing a hazard/obstacle for the 1,600 plus vehicles (per the 2015 SI&A Report) traveling Stone Road daily. We noted during our site visit that sight distance is limited by the hill in the southeast quadrant of the bridge.

The slopes of Lugar Creek are steep, eroded and unstable. Several trees along the bank have fallen, and several trees have exposed roots and will probably fall in the near future.

This bridge could be replaced with either a single-span or three-span bridge. A single-span bridge is best for bridges surrounded by woods, as is the case at the Bridge #73 crossing. Whereas, the piers of the three-span have a tendency to catch debris, causing scour problems and maintenance issues.

The potential drawback to a single-span bridge is the deeper structure typically requires the road to be raised two or three feet. Raising the road can increase impacts to surrounding properties and add significant cost to the project. However, some of this potential cost is offset by the savings of not having piers.

At the preliminary stages of the project, during the structure size and type phase, USI will provide an economical evaluation of a three-span bridge option versus a single-span bridge option and develop a recommendation, with the County’s input.

1) >> LEVEL 1 DESIGN - ROADWAY/BRIDGE GEOMETRY - As previously sited, the horizontal sight distance is impaired by the hill in the southeast quadrant of the bridge. The geometry of the horizontal curve is sufficient for a 50 mph speed, the curve itself is not the issue (the design speed is 40 mph per the RFP).

The existing bridge approach, consisting of 10-foot lanes and 1-foot aggregate shoulders, and the 20-foot wide bridge, do not meet Indiana Design Manual Requirements. We recommend that both the bridge and approach be widened to provide a safer passage. As an added bonus, this will also improve the horizontal sight distance.

As mentioned earlier, the FEMA FIRM Map (Above) shows that Stone Road floods during a Q100 storm event. However, upon further research, Stone Road may not flood. An extensive hydraulic study of Lugar Creek shows that the Q100 elevation is one foot below Stone Road. This evaluation is based on the best available GIS information. One of our first steps will be evaluating the hydraulics of the bridge. If the road floods, the new bridge will be designed to eliminate the flooding as required per the Request for Proposal.

2) >> PERMITTING - An IDEM Section 401 Water Quality Permit and a USACE Section 404 Regional General Permit will be required since riprap will be installed on the spill-through slopes, below the Ordinary High Water Mark, to protect the bridge end bents. With a drainage area of approximately 29 square miles, an IDNR Construction in a Floodway Permit will be required (IDNR regulates urban stream crossings with stream drainage areas greater than 1 sq. mi.). An IDEM Rule 5 Permit will be required if the scope of the project results in an impacted area more than 1 acre, which is likely.

4) >> RIGHT-OF-WAY - Additional right-of-way will be required for the project. The road and bridge will be widened, and riprap will extend up and downstream. USI meets with the right-of-way engineering team early in the process to help identify potential issues/risks with the design. This philosophy identifies problems early so that they do not perpetuate to become large problems later. It also helps the buying process, which Grant County is providing.

5) >> MAINTENANCE OF TRAFFIC (MOT) - Based on the location of the project and the low traffic count (1,630 ADT in 2013 per the SI&A report), road closure is recommended. There are two potential detour routes: 1) Eastern Loop via 38th Street, CR 350 E, and Monroe Pike. 2) Western Loop (through town) via 38th Street, Lincoln Blvd, and Monroe Pike. The Eastern Loop seems like the more logical choice. However there is a sign on CR 350 E that states no truck traffic is allowed. We will investigate why traffic is not allowed. The Western Loop would also be a reasonable choice, but it is located in a residential area and would likely create traffic congestion, especially along Lincoln Blvd.

6) >> BID PROCESS - During the bid process, USI has a few secrets that help ensure the County secures the best bid for the project:

3) >> UTILITIES - Numerous utilities are located within or near the project limits, including the following:

a. >> Vectren owns an underground gas line which runs along the west side of the road. The gas main is attached to the bridge coping. Because the bridge is being re-placed, the gas main will need to be relocated.

b. >> AT&T’s underground phone line runs along the east side of the road. The phone line is also attached to the bridge coping. It will need to be relocated.

c. >> There are no overhead utility lines that conflict with the project.

***SPECIAL NOTE*** In our experience, gas line insulation frequently contains asbestos. It is important that

the asbestos investigation includes the utility insulation.

USI coordinates early in the design process to help identify the conflicts and resolutions as soon as possible. This helps ensure the rest of the project goes smoothly and keeps the project on schedule.

To make the transition from design to construction smoother, USI has the design utility coordi-nator continue to coordinate the utilities through construction. This helps eliminate communi-cation gaps or miscommunication - eliminating headaches for the County.

USI’s “BEST PRACTICES” During the Bid Process1) We send the plans to three or more plan rooms.

2) We work around the INDOT letting schedule so that the project is not competing with the INDOT projects. (We have found that we receive less interest in projects if they are bid around the same time as an INDOT letting because contractors do not have enough time to put together the INDOT and County bid packages).

3) We send the notice to bidders to a select group of contractors that are known to do good work in the area including any that the County recommends.

4) We reach out to contractors one week prior to the bid and a few days before the bid to find out if there are any concerns and to ensure that at least three bids will be submitted. We firmly believe that the County will get a fair bid price if they receive at least three bids.

USI will attend the bid opening, review the bid tabs, and make a recommendation. We have a formal checklist that, once complete, we will provide to the County showing whether the bid package contains all required components. As a part of the bid review, USI also evaluates the bid item prices versus Engineers Estimate to ensure that the bid prices are balanced.

7) >> COST SAVINGS! $$$$ - Two different strategies have been evaluated: 7a. >> Make improvements - Increase the visibility through the horizontal curve and widen the shoulders to satisfy the Indiana Design Manual. The total project cost is estimated to be be-tween $1,250,000 and $1,350,000. These project costs include construction cost, engineering, periodic construction inspection and land cost. 8) >> SCHEDULE AND TIMELINE - We understand how important the schedule is for the County’s budget planning. Unless something unusually comes up during the design, the propose schedule in the RFP is reasonable (project construction in the summer 2017). The critical path is the Indiana DNR permit, which could take up to 6 months for approval. The proposed schedule on the next page outlines the task and approximate timeframes.

At the onset of the project, USI evaluates the entire project to identify potential risks to the schedule and budget. These items get extra attention, throughout the design, so that they don’t derail the project’s schedule or budget

George provides experience in a wide range of bridge engineering, roadway engineering, and environmental engineering fields. He is experienced in hydraulic analysis, structural design, road design, bridge inspection, bridge load rating, environmental mitigation, and environmental permitting. George has been involved with the design of numerous local, state, and federally funded vehicular bridge rehabilitation and new construction projects.

In addition to his transportation project management responsibilities, George serves as an experienced Team Member on USI’s Commercial Underwater/In-Water Bridge Inspection Team. For the past 5 years, he has assisted our NBIS and Underwater inspection teams with tasks including: regular bridge inspections, scour inspections, commercial underwater SCUBA bridge inspections, confined space and special inspections, and bridge load ratings analysis.

With his expertise, George served as the project manager for various successful multi-million dollar bridge and roadway projects throughout Indiana George’s immediate up-front “hands-on” philosophy provides swift coordination with our clients. His approach has mitigated serious impacts to projects, creating ON-TIME and IN-BUDGET successes.

Shelby County, IN - On-Call Bridge Replacement ProjectsGeorge serves as the project manager for replacing existing bridges, as assigned, for Shelby County. The first three assignments included, single-span, concrete bridges will be replaced due to structural deterioration and inadequate hydraulic capacity. The bridges will be replaced with side-by-side concrete box beam bridges with a waterproofing membrane and asphalt overlay.

The bridge replacement designs provide an economical solution, tailored to fit each project site. Throughout the replacement process, each bridge required 401/404 permitting. With George’s permitting expertise and relationships with the Indiana Agencies, the permitting process has been a swift and “stress-free” asset to Shelby County—keeping ALL projects in-scope and on-schedule. The locally funded bridges are anticipated for spring 2016 completion.

DeKalb County, IN - Bridge #80 Rehabilitation ProjectGeorge serves as the project manager for the rehabilitation and widening of DeKalb County Bridge #80. DeKalb County Bridge #80 is a single-span bridge with a deteriorating superstructure on deep end bents with exposed piles. DeKalb County Bridge #80’s superstructure will be replaced and widened. The existing exposed piles are in fair condition. To extend the life-cycle of the piles, USI is using an innovative resin-based encasement product to halt corrosion and

prolong the service life. In turn, this cost effective solution saves DeKalb County future budgets and investments! This project is waiting for funding approval and is expected to be complete in spring 2017.

Hendricks County, IN - Bridge #19 New Bridge ReplacementGeorge served as the assistant project manager for the replacement of Hendricks County Bridge #19. The existing bridge was too small for the stream and caused flooding of upstream fields and over-the-road flow during storms.

USI prepared a structure size and type analysis for Hendricks County which compared the cost of different bridge types to their hydraulic performance. The replacement bridge is a single-span, bulb-tee bridge with deep end bents. This bridge was designed to eliminate over-the-road flooding issues. This structure type was chosen over a slightly less expensive flat-slab option which kept the over-the-road flow.

The hydraulically sensitive crossing required an Indiana Department of Natural Resources permit, with modeling, to prove the new structure would not cause flooding. In addition, the project required Rule 5 permitting and 401/404 permits. George worked closely with Hendricks County and the permitting agencies to ensure that the process stayed on schedule and contained costs to the original bridge replacement budget. Hendricks County Bridge #19 is currently under construction and set for a spring 2016 completion date. The engineer’s preliminary estimated construction cost for this project was $568,000. The actual bid price was $487,000 --- a savings of $80,000!

Clark County Bridge #95 – Wieseka Hill over Indian CreekClark County, INGeorge served as the assistant project manager for the newly constructed Clark County Bridge #95. Alongside the bridge replacement, a realigned road was designed to cost-effectively smooth out the “S” curve and to reduce the road flooding frequency and minimize potential scouring. The replacement bridge is a durable single-span, reinforced concrete slab with tall abutments built on rock. The wingwalls were extended with MSE walls, this alternative was a more cost effective option. The road realignment required a stream removal and stream relocation, which USI designed, permitted, and helped guide Clark County while they constructed it.

Construction was phased so that the relocated stream was vegetated before the existing stream was removed, the new bridge was built while the existing bridge remained open to traffic, and then the approaches were tied-in. While the approaches were tied-in, the road was closed for a short period. USI helped Clark County find a cost-effective resolution to a complex problem. From replacing the bridge and realigning the roadway to stream and tree mitigation, USI went above and beyond to deliver a quality, economical product for the county.

Mike has over 25 years’ experience in bridge engineering, inspection and construction, focusing on the design and analysis of bridge structures utilizing reinforced concrete, pre-cast/pre-stressed concrete, and steel for INDOT projects. Mark serves as the USI Survey Manager and has managed over 150 completed projects on our On-Call Survey Contracts, as well as, hundreds of INDOT, LPA, and Local completed teaming projects.

Mark’s responsibilities include all design surveys, location control route surveys, land title and boundary surveys and he oversees the electronic data collection, and records research. He has years of experience throughout Indiana involving local, state, and federally-funded projects.

Jake oversees the right-of-way and land acquisition department and is responsible for vital aspects of USI’s R/W engineering services. Having worked over a decade with (INDOT) and Local Public Agencies, Jake’s breadth of experience ranges from Right-of-Way Plan Preparation to land acquisition.

Hamilton County Bridge #301

GRS Abutment

The existing concrete box culverts over Jay Ditch were replaced because of inadequate hydraulic capacity and deteriorating condition. The crossings are located along the same stream, two miles east of the Town of Sheridan, Indiana in Hamilton County.

The bridges are single-span, reinforced concrete slabs. The new bridges will modernize the roadway geometry along 226th Street. The new structures improve the hydraulics of Jay Ditch, eliminating over-the-road flow during the 100-year storm and reducing backwater.

Both new bridges are supported by Geosynthetic Reinforced Soil (GRS) Abutments, an economical bridge support method advocated through FHWA’s Every Day Counts Initiative, which “is a state-based model to identify and rapidly deploy proven but underutilized innovations to shorten the project delivery process, enhance roadway safety, reduce congestion and improve environmental sustainability”. The GRS abutments consists of compacted granular fill, geotextile fabric, and prefabricated masonry blocks. GRS abutments provide an alternative to traditional deep foundation bridge support methods and can be used to accelerate bridge construction schedules and possible eliminate the use of a crane. The bridge project had a budget of $845,000.00 and an anticipated completion date of spring 2016.

USI completed the design of this locally funded bridge rehabilitation project located in Syracuse Indiana. The existing side-by-side box beams with an asphalt overlay were replaced with side-by-side box beams with a concrete deck. The sidewalk was widened and new bridge rail was installed.

The existing substructure and sheet piling retaining walls were retained making the project tricky. Part of the mudwall had to be cut to make space for the new box beams and approach slab. New concrete wingwalls were built on the existing timber pile and new helical screw piles. Helical piles were used because of the close proximity of the aerial electric lines, which made it impossible for a crane to drive new piles. Cables tying the existing sheet piling retaining wall into deadman piles had to be avoided since the sheet piling was retained.. Additionally, sheet piling and modular block walls were installed along the causeway to eliminate the steep embankments, which were severely eroding and causing issues with the road. Coir logs and erosion control blankets with a special blend of slope stabilizing seed mixture was installed to help stabilize the embankment slopes and to help the area look more natural. Engineers Estimate: $430,000

Kosciusko County Bridge #278

Clark County Bridge #95

USI designed the recently constructed Clark County Bridge #95 new bridge replacement, which carries Wieska Hill Road over Indian Creek. Additionally, USI’s construction management team supervised and inspected the construction part-time during critical operations.

Before the improvement project, Wieska Hill Road over Indian Creek featured an unsafe “S” bend curve at the base of a steep hill making it especially hazardous to navigate during inclement weather. Indian Creek is prone to flash flooding through the project area; in turn, caused frequent roadway flooding and the bridge to scour.

The locally funded, Bridge #95 and road realignment was designed to cost-effectively smooth out the “S” curve, diminish roadway flooding frequency, and reduce potential scouring. The replacement bridge is a durable single-span, reinforced concrete slab with tall abutments built on rock. The wingwalls were extended with MSE walls – creating a more cost effective and innovative approach.

The roadway realignment required a stream removal and relocation. USI designed, permitted, and helped guide Clark County during the various vital construction elements within this project. Construction was phased so the new relocated stream was vegetated before the existing stream was removed. Due to the amount of traffic through the area, the new bridge was built while the existing bridge remained open to traffic.

The existing 3 span bridge was replaced because of its deteriorating condition and because it is too narrow for the newly reconstructed County Road 300 East. The bridge is located 0.5 mile north of County Road 700 S, east of Pendleton, Indiana.

The new 102-foot span bulb-tee bridge improves the hydraulics of the bridge crossing. It lowers the water velocity thereby reducing the potential for scour. It also eliminated two piers, which would frequently catch debris and required constant maintenance and were constantly monitored for scour.

The crossing was widening substantially to match the newly reconstructed roadway, which makes it easier for farm equipment to cross over the creek and typically reduces accident frequency. The new bridge has 11-foot lanes and 7-foot, 4-inch shoulders. Newly installed guardrail associated with the bridge project improved safety across the bridge. The bridge project had a budget of $550,000.00. Completion Date: 2014

Madison County Bridge #172

Tipton County Bridge #59

The Tipton County Board of Commissioners selected USI Consultants, Inc. to design a replacement structure for County Bridge No. 59 carrying CR400E over Schlater Ditch. The existing bridge was a single span concrete beam structure on old deep concrete abutments, was heavily deteriorated, restricted the channel flow, and was narrow. USI designed a single span (59’-0”), side by side, prestressed concrete box beam bridge on a 50 degree skew with spill thru slopes to help improve the hydraulics of the bridge site. Once again, USI utilized right of entry agreements with the surrounding property owners, along with working within the legal drain easement of Schlater Ditch, to allow the project to be designed and constructed in one year. Final Construction Cost: $233,000

Tipton County Bridge #60

The Tipton County Board of Commissioners selected USI Consultants, Inc. to replace County Bridge No. 60 carrying CR200E over ASKEN Ditch. The existing bridge was a single span concrete beam structure on deep stone abutments, which was scheduled for replacement. USI designed a single span (70’-9”), side by side, prestressed concrete box beam bridge with spill thru slopes to help improve the hydraulics of the bridge. USI also utilized right of entry agreements with the surrounding property owners, along with working within the legal drain easement of ASKEN Ditch, to move the project ahead quickly. Final Construction Cost: $242,000
