Welcome to Livewell Southwest, I hope you have an enjoyable practice placement experience. My role is to support you, your Practice Supervisor and Practice Assessor whilst on placement, therefore please do not hesitate to get in touch if you have any placement queries or concerns.

I am a Registered General Nurse by background with clinical experience in Coronary Care, Acute Medicine, Elderly Care and Minor Injuries. I have been in this role since July 2011 and am based on the top floor of the Beauchamp Centre at Mount Gould. My contact details are [email protected], tel 01752 437012 or 07920 085658.

Please take the time to read though this booklet as it provides all the essential information for placement guidance and preparation within Livewell Southwest. For specific information about your allocated placement please visit the placement profile via POW on POPPI. Taking the time to view your placement profile will aid you in the planning of your learning experience.

Welcome to Livewell Southwest. My role as the Placement and Development Facilitator is to work alongside Helen in supporting learners on their placement.

I have been a Registered Adult Nurse for 15 years having started my career in coronary care and cardiology, and have also worked in acute settings caring for general medical patients and patients requiring elective surgical procedures. Prior to this role I worked as a Community Nurse before becoming the first person to work as a Clinical Facilitator for District Nursing and the trainee Nursing Associates.

An overview of Livewell Southwest

Livewell Southwest, formerly known as Plymouth Community Healthcare, is an independent social enterprise which provides integrated health and adult social care services for people in Plymouth, and community healthcare for people in South Hams and West Devon. Livewell Southwest also provides some specialist services for those living in Devon and Cornwall. Recent acquisitions and mergers have meant that Livewell Southwest is gaining national and international attention for providing a unique range of services in the South West of England.

The organisation officially formed on 1 October 2011 as an independent health services provider. Working as part of the NHS family, the organisation initially provided health services to the Plymouth community only, but has since taken over the adult social care assessment service from Plymouth City Council and community health services in Kingsbridge, Ivybridge and Tavistock.

To reflect these additional services, Plymouth Community Healthcare changed its name in January 2016 to Livewell Southwest. The new name reflects the operational growth across South Hams and West Devon and that we are here to help people to live well by providing excellent quality care. While our name has changed, we remain part of the NHS family and operate under the same ethos: we support people to lead independent, healthy lives.

The organisation provides inpatient care and community services, including mental health and child services at various sites across Plymouth, Ivybridge, Kingsbridge and Tavistock.

Student Support Strategy

Livewell Southwest is committed to supporting students from all fields of health including nursing, allied health, psychology and social work. You will be supported by a key staff member; a practice supervisor for nursing students and an educator for allied health students. During your placement you will work closely with your allocated supervisor / educatorto facilitate your practice assessments. You may also be supported by other team members who will provide feedback to your Practice / Academic Assessor regarding your progress.

As part of your placement, we strongly recommend students arrange time and visits within other Livewell Southwest services and teams to broaden placement and learning experience. Our staff encourage all students to take responsibility for their own learning pathway and development, although guidance and support, if required, is willingly provided.

All of our placements are audited regularly in line with NMC standards to ensure they all continue to provide a high quality learning experience. Placement evaluation is a vital part of the audit process and so please ensure you complete the placement evaluation on POPPI at the end of your placement and any local evaluation you may be given.

Role of Placement Development Team

Within Livewell Southwest, you will find the concept of the placement development team (PDT) firmly embedded. The team consists of Helen Sykes as undergraduate placement lead for Livewell Southwest, Tracy Jones as placement and development facilitator, and a team of academic staff from the university. Practice and academic staff work in partnership to prepare practice supervisors and assessors / educators for students, as well as being available for the support of any student / placement concerns and queries regarding practice and assessment. You will find the same structure of placement support is available across all the organisations that are in the footprint of Plymouth University.

The role of the team is to support you whilst on placement should you encounter any concerns or difficulty. However, the presence of the PDT does not deflect from your personal tutor and they are obviously still available for support if needed whilst in practice.

Placement Expectations

Students are expected to abide by our organisational policies and procedures. Therefore please ensure you read and discuss with your practice supervisor / educator the policies listed at the end of this booklet.

In Livewell Southwest we expect students to be professional at all times. This includes appropriate behaviour in relation to dress code / uniform, mobile phone use and timely reporting of any sickness / absence, which needs to be reported to both university and placement using the main team number.

We expect our students to be respectful and courteous towards all service users, their families / carers and their placement areas.

If you have any special occupational health recommendations /issues and / or specific learning differences which may impact upon your practice, you are strongly advised to disclose these to your practice supervisor / placement at the earliest opportunity. For students who have specific learning differences, it is also wise to arrange a visit to the practice setting with your study skills tutor prior to the start of your placement. Early discussion of these needs will allow your placement to facilitate any necessary reasonable adjustment.

It is also advisable to come prepared for your placement in terms of your prior learning and experiences so that placements can facilitate any outstanding opportunities.

Smoking

Please note that smoking is not permitted on any part of Livewell premises, nor whilst wearing uniform. If you are a smoker please familiarise yourself with our tobacco policy. Anyone found to be smoking on Livewell premises will potentially face disciplinary consequences.

Raising Concerns / Safeguarding

If you have any concern about patient safety and / or standards of care, which potentially impact upon patient welfare, or if poor care delivery / standards are witnessed, you must raise this as a matter of urgency.

In the first instance, discuss with your Practice Supervisor / Assessor and / or PDT Practice Lead, who will advise of the next steps. In the absence of the PDT Practice Lead, please raise concerns with Dawn Slater, Director of People & Professionalism. Alternatively, if you are unsure about what you have witnessed, please discuss with our Safeguarding team (Michelle & Liz) who will advise of the appropriate action.

Your personal tutor should also be made of aware any concerns /safeguarding alert you have raised so that they may support you with any necessary statements/interviews.

Our patient’s welfare is paramount and it is inappropriateand unprofessional to ignore any concern for fear of penalization within your assessments of practice.

On rare occasions, it may be necessary to remove students from a clinical area whilst an investigation of practice standards is conducted.

Raising a concern / safeguarding alert takes courage and you will require support and so please remember the sources that are available to you.

Incidents / Reporting whilst on Placement

Any incident or accident that involves a student whilst on placement must be reported. Please inform your practice supervisor / placement immediately of the situation so that your safety, health and wellbeing can be ensured.

All incidents and accidents are reported via our electronic system and your practice supervisor / team will help you to complete this.

Your personal tutor and the PDT Practice Lead must also be made of aware of any incident / accident involving a student.

It is the responsibility of the placement supervisor / educator to confirm students have completed the mandatory e-learning / mandatory training below; this is provided by the university. Please review this with your placement as part of your orientation.

Allied Health students should complete the placement induction checklist with their educator, which will have been emailed to you, ideally on the first day of placement but certainly within the first week.

Orientation & Induction / Essential Training

In addition to this, all students will be required to attend some elements of training considered essential for placement practice.You will have been notified of dates and sessions pre-placement. If, for any reason, you are unable to attend the session / date given to you, you must notify the Placement / Undergraduate team as soon as possible.

Students will need to complete SystmOne training, which is our electronic records system. Some areas, which interface with Social Care, use CareFirst6 in place or in addition to SystmOne. Where required, your practice supervisor / educator will arrange training and access for you.

Breakaway techniques may also be arranged for you depending on your level and placement area.

Contact / Next of Kin Details

At times your placement may need to contact you. This may be to check on your welfare if there has been miscommunication about your whereabouts. Therefore, to aid this, please complete the emergency contact and next of kin details at the back of this booklet. Your placement will only keep your details until the placement ends and in sharing this information a lot of time can be saved.

Lone Working

It may be considered appropriate as part of your learning experience that you undertake independent care activity. This may include caring for a small non-complex group of patients within the community. Therefore, please ensure you discuss lone working as part of your orientation with your supervisor / educator. 16

If planning to undertake lone working, and where you will be using your transport to do so, please ensure you complete the lone working checklist at the end of this booklet.

Professional Training & Development

Livewell Southwest offers training opportunities to its staff through the Professional Training & Development department. This training is also available to students whilst on placement within the organisation. Students are firstly advised to discuss their learning needs with their practice supervisor / educator, following which you can book training by contacting Professional Training & Development, tel 01752 435150, email [email protected]. You can also visit the team on the top floor of the Beauchamp Centre.

Some training which is practice / competence based, such as tissue viability and continence care are booked directly with the teams or via our Livewell Training Academy and the Professional Training Administration team will be happy to advise you. However, due to limited places, please be aware that sometimes training will only be available to those students who will be working with the organisation upon graduation.

The full list of available training can be found on the Livewell Southwest website. Students may also book training outside of any placement block via the Training Academy (www.livewellswacademy.co.uk), however this route will incur a cost.

Once training is booked, if for any reason you are unable to attend please contact the Professional Training & Development department. Training sessions often have limited places and a cancellation place can be offered to someone else. 17

With the exception of teams / placements at Derriford, all parking on Livewell Southwest sites is free of change. However, parking is often extremely limited, particularly when attending the Beauchamp centre for training. If you are visiting the Beauchamp Centre, please do not park under the roofed section of the main entrance as this constitutes a fire risk to anyone in the building.

In general, when travelling to your placement please car share or use the local buses / park & ride services wherever possible. Please ensure that when parking you only use the marked parking spaces and, wherever possible, use the main car parks. Please do not park in disabled spaces unless you have a blue badge.

Failure to attend training without prior cancellation will result in the Placement team informing placements / supervisors, and may request a record of non-attendance is entered within your assessment documentation.

Identification

Livewell Southwest magnetic ID badges will not be issued to students. It is expected that students will wear their university photo ID at all times during placement. However, for those students issued with a swipe access card, this may be worn instead for identification purposes.

ID badges should only be worn with the clip provided and not be displayed from a lanyard.

Swipe access cards are issued to all hosted students and those students who are based in a building that requires swipe access. You will have been notified pre-placement if this applies to you.

A swipe access card requires a photograph and you will need to visit our estates team at Mount Gould to enable a team member to take your photo and create your card. We are unable to use your photo on record with the university due to these often being of poor quality. If you have a problem with your swipe access card during your placement please email [email protected]

Access to your current placement only will be given but this can be reset remotely on request for any subsequent placements within Livewell Southwest. Therefore, you should keep your swipe access card safe for possible future use.

If you lose your badge this must be reported immediately to your practice supervisor/placement manager and Estates so the card can be cancelled and a new one issued.

Information Technology

IT registration for studentsAll students are required to have an active IT account to enable access to our intranet and electronic systems. An NHS email account is also required. Your IT account can be accessed from any placement within Livewell Southwest or University Hospitals Plymouth NHS Trust.

You will need to complete either the network account or RA01 form depending on whether a smart card is to be issued, and you will have been advised of this pre-placement. More about smartcards later.

The network account formThis allows basic IT access and NHS email to be set up. Your National Insurance number is required to complete this form. Students on once-only placements will be required to complete this form. These forms will be sent to you pre-placement for completion with your practice supervisor / educator on the first day, or you may wish to complete during any pre-placement visit. Once completed, forms should be sent to [email protected] where your application will be processed.

The RA01 formThe RA01 form allows a smartcard to be issued for SystmOne access. This will only need to be completed by “hosted” Mental Health Nursing students only as they will be returning for more than one placement.

The RA01 requires your National Insurance number and two forms of photographic ID (i.e. passport and driving licence) and20

How to Access your IT AccountOn completion of your network or RA01 form you will need to contact IT service desk from placement on ext. 37000 to activate your IT account and obtain your username and password. IT will not contact you with these details. The initial password given must be changed at first login.

Passwords expire every 90 days and must meet the following criteria:

• A minimum of 8 characters• At least 1 uppercase character• At least 1 lowercase character• At least 1 number or special character, (e.g. *, #, %,!)• Not any of your last 3 passwords• And not contain your first name, surname or account name

NHSmail account / registrationIt is important to register for your NHSmail account in order to receive staff communications and, where applicable, your SystmOne username and password. Your email address will stay the same throughout the duration of your NHS career, even if your organisation changes. Once you have activated your IT account you can self-register for NHSmail from a trusted connection i.e. from placement / NHS site. 21

an addressed form of identification (i.e. utility bill) or one form of photographic ID and two forms of address. Address forms of ID are only acceptable when seen in their original state and must be have been issued within the past three months prior to completion of RA01.

Please refer to the back of the RA01 form for further accepted forms of ID (you will have been sent this in your welcome email).

The process for registering your NHSmail account is:

• Access the NHSmail web portal: www.nhs.net • Click the ‘Register here’ button • Locate your own entry in the NHS Directory by entering your

name • Confirm that you have selected the correct entry • Agree to abide by the Acceptable Use Policy • Choose your email address from the options supplied • Enter your security questions and their associated answers • You should choose your security questions and answers

carefully as they are used to verify your identity. You should not use questions and answers that can be easily guessed. For example, ‘what is my first name?’ or ‘what is the capital of Spain?’

• Review your account details

When you have completed these steps, you will need to contact the ICT Service Desk on (4)37000 on the following working day for a temporary password for your account. You will then need to change this to your own unique password when you first log in to NHSmail.

Your password must be a minimum of 8 characters, not be based on your username, and must contain characters from three of the following four categories:

• Uppercase alphabetic characters (A-Z) • Lowercase alphabetic characters (a-z) • Arabic numerals (0-9) • Non-alphanumeric characters, for example: ! $ # % • Spaces or commas are not permitted.

The ICT Service Desk provides you with a central point for reporting all ICT and telecoms related issues and requests. The ICT Service Desk is staffed from 8am til 5pm, Monday to Friday and can be contacted using one of the following methods:

Further IT information is available on TechNET, accessible via the intranet site. Here, you’ll find links to access the Service Portal for extensive help and guidance across a range of IT services. You can easily access TechNET from the desktop shortcut available on all organisational PCs or from the link on the StaffNET homepage.

Always ensure you follow all organisational policies regarding use of IT in relation to information governance, security, confidentiality, internet and NHSmail use and telecommunications.

Students who will be returning for subsequent placements, i.e. hosted students, will be issued with a smartcard to enable to access to SystmOne. Smartcards will usually be issued at the time of your SystmOne training.

The smartcard requires a photograph and arrangements for this to be taken will either be made as part of the induction session in year 1, or by visiting our Estates team in the Admin block at Mount Gould. The same photo will be used for both smartcard and swipe access (if needed).

Students who are on placement for a once-only period will be given access to SystmOne via a username and password. This username and password will only be issued once training has been completed. Your username and passwords will be emailed to your NHS account. Therefore, it is essential that NHS mail is set up at the earliest opportunity.

Information for Smartcard users

Information stored on your SmartcardYour photo, name and unique user identity number are visible on your Smartcard. In addition, the electronic chip within the card contains encrypted information that permits you to access some IT systems and services. Your Smartcard does not contain any of your personal or identity data.

Security and confidentiality

You have a duty to keep patient information secure and confidential at all times. Your Smartcard provides you with the appropriate level of access to the healthcare information that you need in order to carry out your role. Your smartcard is a token of your identity and any access to patient data using the card will be auditable to you.

You should therefore treat your Smartcard as you would a credit or debit card – never share your passcode, always keep it safe, secure and use it appropriately.

If you encounter any of the above situations, notify your mentor /placement and contact the IT helpdesk on 37000. The helpdesk will pass your query to the RA team who will contact you to provide assistance.

Your Smartcard is a national token of your identity, which is not specific to a particular organisation. You should retain your card if leaving an organisation for use in other health and social care settings, unless you are leaving healthcare permanently.

How do I change my passcode?You can change your passcode yourself by accessing the ‘My Profile’ section in Care Identity Service (CIS).

If you require assistance, you need to ask an RA Manager, Agent, Sponsor or Local Smartcard Administrator to help you. Alternatively, you can contact your local IT helpdesk to raise a call for assistance.

What do I do if my card is used by someone else?If your card is used by someone else:

a) inform your Registration Authority manager / agent and mentor / placement immediately; b) change your passcode.

Never share your Smartcard. All transactions made when your Smartcard is in use are recorded for audit purposes. Access is audited, and you will be held accountable for any inappropriate transactions made using your Smartcard, whether you were responsible for them or not.

How long are my Smartcard certificates valid?The certificates on your Smartcard allow you to authenticate your identity and connect to NHS Spine systems and services. These expire after two years. You will be prompted to renew your certificates by the system close to expiry.

Can I renew my own certificates?You can renew your Smartcard certificates yourself within the 28 day renewal period. You will receive a pop-up prompt when it is time for you to renew your certificates; this will take you through the self-renewal process.

You cannot self-renew your certificates once they have expired. If your certificates expire you will need to log a call with the IT helpdesk for assistance from your local RA service.

Please note: Every third renewal requires RA assistance. They may choose to issue you with a new card at that time.

How do I unlock my card?If you have locked your Smartcard, you will need assistance from the IT helpdesk or our local RA administrator.

Your smartcard will lock itself if there is no activity after 28 days. Therefore, in between placements is it very likely your smartcard will need to be unlocked.

Printing / Faxing whilst on Placement

Permanent staff will print and fax via fob access. The issue of a fob is not standard practice for students but teams / placements may have spare fobs which students may use whilst on placement. Otherwise, students may obtain a fob from the IT dept.

Students returning to Livewell Southwest for Placement

Returning / hosted students will need to ensure they have done the following pre-placement:

• Check IT account is still active by contacting IT on 01752 437000

• Contact IT to have your countersigning rights for SystmOne changed to new placement

• Have registered for NHS email and checked that this can still be accessed

• Contact our local IT administrator to unlock smart card if not used within last 28 days

• Contact the Estates team to have ID /swipe card access changed to relevant placement area
