Innovative Partnerships In 2003, Pfizer became the first non-consulting based company to create a cross-border volunteer program, the Pfizer Global Health Fellows (GHF) program. Over a decade later, the GHF program is still recognized as a best-in-class model across industries.

Pfizer’s commitment to increasing access to health care starts with our commitment to research and extends through some of the most innovative donation and grant programs ever launched. Pfizer, through the GHF program, has found a way to help address pressing health needs in the developing world by making our greatest resources, our highly talented, committed and trained people, available to support organizations around the world.

Through the program the best minds from Pfizer and its partners work together to bring about meaningful and systematic improvements in health service delivery for people in greatest need. During assignments Fellows transfer their professional pharmaceutical and business expertise in ways that promote access, quality and efficiency of health care while building insights on health care delivery challenges in developing country settings.

Global Health Fellows are selected, in part, for their strengths in teaching and training and their willingness to see their work carried on by local teams. As a Global Health Fellow, you will be recognized by your peers and counterparts around the world as not only a representative of Pfizer, but as an ambassador for the industry and your home country.

This flagship program has survived a test of sustainability and demonstrated powerful returns through positive stakeholder impact and strong employee engagement. While Pfizer has numerous country volunteer programs, GHF is the only international volunteer program that is open to participation by Pfizer employees worldwide. Since launching, over 300 Fellows from more than 25 Pfizer country locations have served in 44 countries. To date, Fellows have completed an estimated 340,000 hours of skills-based volunteerism with local partners throughout the developing world.

The following policy is a guideline for all GHF participants to ensure compliance and safety in accordance with Pfizer standards.

The GHF program operates according to the enclosed policies to ensure the safety and security of all program participants during every stage of their Fellowship experience. The policy outlines a number of important actions Fellows must take to prepare for Fellowship. The policy also includes important guidelines while on assignment in the Fellowship country. As the program requires, changes to policy are updated and communicated with current Fellows, their Pfizer managers and partner organizations.

ASSIGNMENT

Global Health Fellows Program The Global Health Fellows (GHF) program is Pfizer’s signature international corporate volunteer program. Fellows are Pfizer colleagues who are selected through a competitive application and interview process. Fellows work with nonprofit organizations for a three to six month specialized assignment.

Colleagues who go on assignment must maintain full-time status throughout the assignment period and as part of their career path at Pfizer. The GHF program is managed by Pfizer’s Corporate Responsibility Team and is a core part of the company’s social responsibility platform.

Pfizer Ambassadorship, Conduct and Requirements Pfizer expects that Global Health Fellows will embody the Pfizer values throughout the GHF experience.

The GHF program team expects participating colleagues to behave in a highly professional manner towards the GHF Program team, other Fellows, nongovernmental organization partners, community clients, stakeholders or Pfizer staff at all times. This experience is not just between you and your host country; it is also between you and the GHF program team, your Pfizer manager and home team. Unprofessional behavior may result in disciplinary action up to and including immediate removal from the program.

The GHF program team expects that Global Health Fellows will complete the following during their participation in the program.

-Participate in GHF program led fellowship orientation sessions and onboarding prior to fielding; -Complete all of the items on the GHF Checklist Additional details to be provided before deployment, activities include, but are not limited to:

-Working with your Pfizer Manager and your NGO Partner Manager to agree upon Fellowship dates

-Applying for relevant visa for Fellowship country location, following guidance provided by the GHF Program team.

-Developing your Fellowship scope of work with your NGO Partner and upload a draft, initial (upon arrival in-country), midpoint and final assignment reports;

-Submitting three health stories focused on work in the field, following guidance from CR Communications, to be provided in orientation;

-Participating in GHF program led class debrief session upon return to Pfizer;

-Presenting your final presentation to Pfizer home team, that includes the GHF program team upon return from assignment;

-Participating in post-Fellowship survey and one-year later survey.

Fellows will be evaluated on the above activities by their GHF partner organization and GHF program team as part of their Pfizer global performance annual review. Specific details will be reviewed during orientation.

GLOBAL HEALTH FELLOWS PROGRAM FUNDING STRUCTURE The GHF Program is one of Pfizer’s signature Corporate Responsibility programs and Pfizer is pleased to offer this unique international corporate volunteering program to Pfizer colleagues globally. To support colleagues participation in this volunteer opportunity, Pfizer covers the majority of the costs associated with Fellowships but there will be some personal expenses incurred by the Fellow. Below is a high level summary of the costs funded by the Pfizer GHF Program and the items funded by the Fellow’s Pfizer Department/Division. More detail is provided in the corresponding sections of this GHF Policy. It is important to note the GHF Program is not a full scale business relocation program and will not cover all expenses related to the Fellow’s time in their host country.

Funded by the GHF Program

1. GHF Training and Orientation, including travel to New York, if the Fellow is not based out of the New York Headquarters office;

2. Temporary medical and dental benefits if the Fellow’s current benefits program does not provide any benefits coverage outside the Fellow’s home country;

3. Expenses associated with a visa or work permit for the Fellow’s host country only (if the Fellowship host country requires a visa);

4. GHF Field Visits in Fellowship Country; 5. Personal Goods Allowance (USD$500.00 per Fellow); 6. Housing for the duration of one’s Fellowship; 7. Daily transportation to and from the Fellow’s Partner office in the host country.

Funded by the Fellow’s Pfizer Department/Division

1. Payroll/Compensation for the Fellow; 2. Roundtrip airfare to the Fellow’s host country; 3. Any recurring fees/expenses the Fellow may incur related to their Pfizer role, if such

services are not suspended while on Fellowship (i.e. monthly fees for any Pfizer services/technology, etc.).

Fellowship Assignment, Location and Length Fellowship assignments refer to the work the Fellow will undertake with GHF partner organizations in their volunteer capacity.

Assignment location refers to the host country where the Fellowship is based. Fellows may travel within their host country as part of their core responsibilities while on assignment. All travel must be reviewed and approved in partnership with GHF partner organization, GHF program team and Pfizer security team over the course of the Fellow’s fellowship experience. All Fellows must adhere to the Field Trip and Personal Travel Protocols throughout their Fellowship (please see field trip and personal travel protocols section).

Global Health Fellows assignments range from a minimum of three months to a maximum of six months. Fellowship assignment length is determined by the GHF program team and GHF program partner based on objective needs in advance of colleague selection. Pfizer colleagues are recruited and selected to serve for a specific time period during the recruitment process.

GLOBAL HEALTH FELLOWS POLICY GUIDE

Pfizer Confidential

Global Health Fellows will not be permitted to stay in their host country beyond six months (or 183 days). Fellows are not permitted to shorten or extend their Fellowship assignment. (Please see Letter of Assignment Section).

Training and Orientation The GHF Program team will provide Fellows with an orientation to the GHF program and Fellowship preparation training through class training sessions with various guest speakers and alumni. Participation is mandatory for all participants accepted into the GHF program.

Global Health Fellows will be provided a more in-depth orientation by the GHF partner organization they have been selected to perform an assignment with. This will be arranged directly between the partner organization and the selected Fellow. Please note, this formal training likely will not occur until the Fellow has arrived in their host country.

Letter of Assignment The selected Pfizer colleague accepts participation in the Global Health Fellows program by agreeing to the terms of and conditions in the Letter of Assignment. The Letter of Assignment outlines the terms and conditions of the Global Health Fellows program that are specific to the Fellow’s assignment and are in addition to, and not in place of, the terms and conditions of your employment in your home country.

The Global Health Fellow signs and submits the completed letter to the GHF Program Operations Manager. The signed letter is shared with the GHF Partner organization and the colleague’s Pfizer manager.

PAYROLL AND TIME AWAY FROM WORK

Pfizer Job Status Global Health Fellows must be full-time employees at the time of deployment and during the length of their assignment in accordance with appropriate application criteria. A Global Health Fellowship is an extension of a colleague’s Pfizer position. Fellows maintain status as a full-time employee during the Fellowship tenure and participation in the program will not influence or adversely affect it. Colleagues who have been given notice of job loss due to restructuring or reorganization as a result of business conditions should not apply for a Global Health Fellowship. If during the program application/selection process, or after selection, but before deployment, you are identified as being impacted by a company restructuring or reorganization you will be required to withdraw from the program. If your position is affected by an immediate restructuring or reorganization during the Fellowship tenure, you will be required to return from assignment immediately.

Payroll Global Health Fellows remain on the designated home location payroll system.

Work Hours and Time Away From Work Fellowship working hours will be based on host country laws and customs. Fellows are expected to follow the office hours that their GHF partner organization field office follows. In practice, Fellows are expected to work whatever additional time is necessary to carry out their Fellowship objectives and duties effectively.

Fellows will be responsible for recording time away from their Fellowship office, which includes local holidays, sick time, and field visits and sharing this information with their GHF partner organization manager, Pfizer manager and GHF program team on a monthly basis. Instructions on this process and associated Fellowship requirements are provided during GHF program orientation.

Vacation The Global Health Fellow will follow the GHF partner organization’s local public holiday schedule.

Global Health Fellows are encouraged to take personal and/or vacation time prior to the first day of Fellowship or after last day of Fellowship. T h i s t im e is counted against the Global Health Fellow’s home country vacation accrual.

Accrued vacation or personal time will be applied to any time off from work taken after the last day at Pfizer and before the fellowship begins. Similarly, accrued vacation or personal time will be applied to any time off from work taken after the last day of the Fellowship and before the first day back at Pfizer. Fellows are not permitted to take vacation time during the Fellowship assignment period.

Home Leave Global Health Fellows are expected to remain in the assignment country for the entire length of the assignment to perform Fellowship-related duties.

Home leave is not permitted or provided.

Emergency and Compassion Leave This leave is permitted to attend to the death or terminal illness or acute medical or psychological condition of an immediate family member in the Global Health Fellow’s home country. The Fellow will inform the GHF program team, Pfizer Manager and GHF partner organization of the situation immediately.

In this emergency leave situation, Pfizer will reimburse international flight to the Fellow’s home country. No other expenses incurred during such a leave period, will be reimbursed by Pfizer (e.g., including, but not limited to: ground transportation, meals and lodging, etc.).

Pfizer’s Bereavement Policy permits up to a maximum of seven (7) consecutive calendar days off with pay at your base rate of pay in the event of death of an immediate family member. TRAVEL INFORMATION – GHF TRAVEL POLICY

Travel to GHF Program Orientation in New York Global Health Fellows Program Orientation will take place at Pfizer New York Headquarters in New York in February 2017. Exact dates will be communicated to colleagues selected for a Fellowship in December 2016. Travel costs and accommodations for GHF Program Orientation will be funded by the GHF Program Team. Global Health Fellows must book their travel to GHF Program Orientation in New York according to guidance from the GHF Program team including utilizing specific calling instructions and using the GHF program meeting number. Fellows should not book travel to GHF orientation with a corporate or personal credit card. The GHF Program will not be responsible for any expenses that a Fellow incurs that are due to the Fellow not following the GHF Travel Policy. Pfizer standard travel policy as outlined in Pfizer Corporate Procedure #301 would apply for booking this flight.

The lowest cost, most direct route should be selected without consideration for airline frequent flyer programs or other incentives.

Business Class seating is permitted on flights of six hours or more in total duration.

Reservations should be made at least 14 days in advance of departure.

The Global Health Fellow must speak to a Pfizer-sponsored BCD travel agent to book travel to GHF Program Orientation and request a round-trip ticket (lowest cost, most direct route) to New York at the time of the initial flight booking on airlines sanctioned by Pfizer’s corporate travel group. Fellows are not permitted to choose travel itineraries based on a preferred airline carrier.

Travel to Fellowship Assignment The Fellow’s home Pfizer Department/Division is responsible for funding international round trip airfare to the Fellowship host location. The Fellow should utilize their Pfizer home country travel system and procedures to book this international flight, similar to the process for booking other Pfizer business travel. The Fellow should confirm approval of the flight cost with their Pfizer manager before booking the flight.

It is highly recommended that travel to/from home/host country occur two to three days before the Fellowship begins to enable the Fellow to adjust to host/home location. The first day of Fellowships should start on that Monday after arrival, the last day of Fellowships should be on a Friday.

If the Fellow chooses to take vacation or personal time before the first day of Fellowship or after the last day of Fellowship, it is considered vacation time against Pfizer accrual. All vacation time must be approved by the Fellow’s Pfizer manager prior to deployment. (Please see Vacation Section)

Fellowship accommodations begin up to two days prior to the start of Fellowship and ends up to two days after the last day of Fellowship. The Fellow is responsible for local accommodation before or after the Fellowship accommodation dates. (Please see the Vacation section)

If any changes are made to the reservation for personal reasons after ticketing, the Fellow will be responsible for all additional charges. If any changes are necessary authorization is required from the Fellow’s Pfizer manager to change the ticket. The Fellow will notify the GHF Program team in advance if any changes impact the Fellowship duration.

The expense for transition costs (ground transportation and/or meals) to and from the fellow’s home country airport at the start and end of the Global Health Fellows assignment may be subsidized by the GHF Program through the Personal Goods Allowance (See Personal Goods Section).

If the Fellow elects to schedule a layover during any connections to/from assignment, the Fellow is responsible for all associated costs during the layover (hotel/transportation and/or meals). The time taken is considered personal or vacation time against Pfizer accrual.

Visas and Work Permits To work in a volunteer location, Global Health Fellows first must satisfy the immigration and visa requirements of the country where the volunteer assignment is located.

Each host country has different visa requirements so it is imperative that Global Health Fellows follow the guidance provided by the GHF Program team coordinate directly with the GHF partner organization and the volunteer assignment country’s consulate to ensure that Fellows are applying for the appropriate visa. The visa application process can take several weeks, so please allow enough processing time to obtain necessary visa(s) before assignment deployment date.

Global Health Fellows must work directly with the GHF partner organization before finalizing any travel arrangements to the assignment location. (Please see the GHF Travel Policy section)

Many countries require an individual traveling on a visa to show proof of leaving the country within the designated length of the visa. This is typically demonstrated by a round-trip airline ticket. (See GHF Travel Policy)

Page 13: PFIZER GLOBAL HEALTH FELLOWS POLICY GLOBAL HEALTH FELLOWS POLICY AS OF AUGUST 1, 2016 NOTE: As the program requires, ... Pfizer Confidential ASSIGNMENT Global Health Fellows Program

GLOBAL HEALTH FELLOWS POLICY GUIDE

It is the responsibility of the Fellow to obtain required visa(s) needed for entry into the fellowship host country location. It is strongly suggested to go to the assignment country’s consulate in person (not a parent, spouse or administrative assistant) to obtain the visa.

Fellows should purchase their entry visa using personal credit card or cash and MUST keep their receipt for reimbursement. The Fellow will be reimbursed for visa(s), any transportation or mailing costs needed to obtain a visa by their GHF partner organization upon arrival. DO NOT use Pfizer Visa services or Corporate Credit Card. More information on this process will be reviewed during GHF program orientation.

Passports

Global Health Fellows traveling internationally to their host-country must have a valid passport. If a new passport is needed, please allow enough time to obtain this and other documents that will be required for travel.

Global Health Fellows must make sure that their passports are valid for at least one year from the start date of the assignment. In many cases, one may not be able to receive a valid visa if the passport is due to expire in less than one year. Any costs associated in obtaining a passport can be subsidized through the GHF Program Personal Goods Allowance (See Personal Goods Allowance section).

GHF Assignment Related Field Trips Many Fellows will have the opportunity to attend a conference and/or travel into rural areas within their host country as part of their core objectives. These field trips, which are within the Fellow’s host country, will be reviewed and approved on a trip-by- trip basis by Pfizer Regional Security (safety review) and t h e GHF Program team (alignment with Fellowship objectives and goals review), at least 5 business days prior to the departure date. Field trips outside the Fellow’s host country are not permitted. The GHF Program funds these field trips and Fellows will be provided with more information on how to submit field trip requests during GHF program orientation. Fellows must provide all of the requested information (an overview to be provided during orientation) when making a field trip request. If not all of this information is provided in the initial request email, the Fellow will need to add on additional time for review. The GHF Program team, cannot guarantee a 5-day turnaround if not all information is provided initially and/or in the case of a holiday.

Personal Travel If a Fellow elects to travel on their personal time (weekends or local holidays), the GHF Program team asks the Fellow to inform the GHF Operations Manager and their Regional Security contact of the dates and location of their travel. This is to ensure that in the case of an emergency that the Program Team and Pfizer Global Security can contact Fellows throughout their assignment.

Corporate Credit Card

Fellows should not use their corporate credit card to cover costs during the GHF assignment. All GHF work-related costs are covered through an approved Fellowship budget between the GHF program team and GHF partner organization. (See Housing, Transportation, Travel Policy, Visa and Work Permits and Personal Goods Allowance Sections for more information). A Fellowship

budget overview will be provided during GHF program orientation.

It is the responsibility of the Global Health Fellow to clear all corporate credit card Pfizer expenses prior to the volunteer assignment. Since Fellows do not have access to Pfizer’s expense reporting systems while on assignment, the Fellow should work with their Pfizer Manager to confirm a delegate and process is in place to submit any emergency and ongoing expenses that may be charged to their corporate card while they are on Fellowship. The colleague will remain liable for any late fees, interest or penalties resulting from late expense submission. The GHF Program team will not be responsible for any of these related expenses.

Health Examinations & Vaccinations

Global Health Fellows are strongly encouraged to have complete health examinations prior to deployment. Some GHF partner organizations may require physical examinations prior to approving the assignment. Vaccinations against infectious diseases as well as preventive medications and physical examinations may be recommended or required before entering an assignment location. This information should be obtained directly from the Fellow’s physician, GHF Partner Organization and/or the Pfizer Occupational Health and Wellness Center.

Global Health Fellows should take note that many vaccinations and medications take time to be effective and must be given well in advance of the actual health examination.

Some countries require proof of vaccination; therefore an international health record should be prepared by your doctor after you have received the vaccination. This proof of vaccination should be carried with you while on assignment.

Depending on your home location, vaccinations and preventive medications are generally available through the Pfizer Occupational Health and Wellness Centers. Additional personal expenses and/or anti-malarial medication may be subsidized through the Personal Goods Allowance.

It is recommended that Global Health Fellows traveling with a pre-existing medical condition travel with a letter from their health care provider detailing the condition and medications the Global Health Fellow must take. Prescriptions should remain in their originally labeled containers and the Global Health Fellow should know the generic name of any prescriptions. If possible, you should bring enough prescription medication and related over-the-counter essentials from the home country to last the length of the assignment.

In some instances (depending on the prescription plan the Global Health Fellow is enrolled in) Pfizer Global Benefits can coordinate an override through the Fellow’s medical plan to request up to six months of prescriptions prior to departure. The GHF Program team can provide fellows with a Pfizer Global Benefits point of contact to discuss prescription benefits. The GHF Program team cannot provide any medical or insurance guidance for Fellows. Fellows are responsible for seeking medical guidance from a medical professional and for speaking with their Pfizer benefits administrator or health plan representative to determine what is covered under their current plan while they are on Fellowship and to discuss, prior to departure, any prescription benefits requests for extended travel.

Security

The GHF program considers personal safety and security a top priority. The GHF program team partners with New York Headquarters’ global and regional security teams, and local country security officers to track closely with all Fellows in the field.

It is recommended that Fellows not take valuable items on assignment. Damaged or stolen items will not be reimbursed by the GHF Program.

Pfizer has arranged for Fellows to receive travel safety briefings by email from the Pfizer Global Security Operations Center (GSOC). If during the Fellowship a medical or security incident occurs, the Fellow will follow security protocols outlined below, which will also be covered during GHF program orientation: If the Fellow is a victim of a crime or unsafe situation/event during their Fellowship they should:

1. Let the appropriate colleagues know right away

This includes: your GHF Partner, Pfizer Regional Security, GSOC, the GHF Program team in New York and the local police (if the situation requires it)

2. The GHF Program team will then coordinate a call to determine if any next steps are necessary

Fellows are required to comply with guidance provided by Pfizer Security and the GHF Program Team throughout their Fellowship assignment.

Pfizer’s global and regional security team is not responsible for the general oversight of a Fellow’s visitor/companion/family members who elects to visit a Fellow during their fellowship. Pfizer will not be responsible for assuming costs associated with local medical care, hospitalization or costs related to an emergency evacuation for Fellow’s family members/companions/visitor.

Visitors (Family Members/Dependents/Companions/Friends)

Fellows are allocated a studio or one-bedroom apartment or in some cases GHF partner organization owned shared guest accommodation (See Fellowship Housing section for more information). Pfizer does not provide financial support for additional accommodation or utilities for visitors of Global Health Fellows. Therefore, family members/dependents are not permitted to live with the Fellow for the duration of the assignment.

Similarly, Fellows are discouraged from having visitors (family members/ dependents/companions/friends) during the assignment period. In the event participants have a visitor, guests are only permitted according to the guidelines below:

Fellows are required to notify the GHF program team and regional security contact of any visitors/companions to a Fellow during the assignment, at least two weeks in advance of their visit. In providing notice, GHF participants are also required to disclose the name of their visitor(s) and the duration of their visit.

Pfizer discourages travel/visitation of visitors under the age of 10 as a safety precaution.

Visitor(s) will be permitted to stay with the Fellow in their Fellowship housing, for duration not to exceed two weeks during the length of the participants’ assignment. Visitors can be a challenging distraction for Fellows during busy work schedules and commitments.
