TESTING OF CONSTRUCTION MATERIALS AND/OR CONSTRUCTED FACILITIES WILL BE REQUIRED AS OUTLINED IN THE GOVERNING SPECIFICATIONS AS NECESSARY TO MEET THE REQUIREMENTS OF THE PERMITTING/APPROVING AGENCIES. ALL TESTING IS TO BE DONE BY A FLORIDA LICENSED TESTING LABORATORY AND THE COST OF THESE TESTS IS INCLUDED IN THE CONTRACTOR'S UNIT PRICES. 5. FIELD REVIEWS OF CONSTRUCTED FACILITIES WILL BE REQUIRED DURING THE CONSTRUCTION PROCESS. THE CONTRACTOR IS TO NOTIFY THE ENGINEER A MINIMUM OF 24 HOURS PRIOR TO THE REQUESTED TIME FOR REVIEW OF THE COMPLETED ITEMS. PRESSURE AND LEAKAGE TESTING OF WATER SYSTEMS 5.0.9. LAMPING AND TELESPECTION OF SANITARY SEWER SYSTEM FACILITIES MUST BE COMPLETED PRIOR TO SCHEDULING FIELD REVIEW. 3. FINAL GRADING IS TO BE INCLUDED IN THE UNIT PRICES. GRADING IS TO BE IN

6. UTILITY CROSSINGS ARE TO BE MARKED WITH A PVC PIPE EACH END INDICATING

THE CROSSING TYPE AND SIZE(S) ON THE PVC PIPE WITH A PERMANENT WATER

PROOF MARKER.

7. WATER AND SEWER SERVICE LOCATIONS ARE TO BE MARKED WITH A NAIL AND

TIN TAB IN THE PAVEMENT 18 FEET FROM THE SERVICE AND PERPENDICULAR

TO THE ROAD RIGHT-OF-WAY LINE.

8. THE CONTRACTOR WILL BE REQUIRED, AS PART OF THE UNIT PRICES, TO

STREET SWEEP/VACUUM THE CURB AND ROADWAY SURFACE UPON NOTICE TO

PROCEED FROM OWNER.

9. THE CONTRACTOR WILL BE REQUIRED, AS PART OF THE UNIT PRICES, TO CLEAN

ITEMS. ANY COSTS ASSOCIATED WITH COMPLIANCE IS INCLUDED IN THE UNIT PRICES.

2. AT NO ADDITIONAL COST TO THE OWNER, THE CONTRACTOR SHALL PROVIDE ACCESS AND ALLOW FOR

COORDINATION SERVICES TO ACCOMMODATE UTILITIES, CONDUIT AND/OR SLEEVE CONSTRUCTION BY

OTHERS. ANY UTILITIES, CONDUITS AND/OR SLEEVES INSTALLED BY OTHERS AND DAMAGED BY THE

CONTRACTOR SHALL BE REPAIRED AT THE CONTRACTOR'S EXPENSE.

3. IF REQUIRED BY OWNER OR GOVERNMENTAL AGENCY HAVING JURISDICTION OVER AFFECTED

ROADWAYS, THE CONTRACTOR WILL PROVIDE, AS PART OF HIS BASE BID FOR PAVEMENT SYSTEMS, A

MAINTENANCE OF TRAFFIC PLAN. ALSO INCLUDED WILL BE THE LABOR, EQUIPMENT AND MATERIALS

REQUIRED TO IMPLEMENT THAT PLAN. ALL WORK MUST BE DONE IN ACCORDANCE WITH AGENCY

REQUIREMENTS AND ANY REQUIRED PERMITS OR APPROVALS MUST BE APPLIED FOR AND OBTAINED

BY THE CONTRACTOR.

4. TESTING OF CONSTRUCTION MATERIALS AND/OR CONSTRUCTED FACILITIES WILL BE REQUIRED AS

OUTLINED IN THE GOVERNING SPECIFICATIONS AS NECESSARY TO MEET THE REQUIREMENTS OF THE

PERMITTING/APPROVING AGENCIES. ALL TESTING IS TO BE DONE BY A FLORIDA LICENSED TESTING

LABORATORY AND THE COST OF THESE TESTS IS INCLUDED IN THE CONTRACTOR'S UNIT PRICES.

5. FIELD REVIEWS OF CONSTRUCTED FACILITIES WILL BE REQUIRED DURING THE CONSTRUCTION

PROCESS. THE CONTRACTOR IS TO NOTIFY THE ENGINEER A MINIMUM OF 24 HOURS PRIOR TO THE

REQUESTED TIME FOR REVIEW OF THE COMPLETED ITEMS. CONTRACTOR TO PROVIDE ALL LABOR,

EQUIPMENT AND MATERIALS NEEDED TO ACCOMMODATE THE FIELD REVIEW. THESE REVIEWS

INCLUDE BUT ARE NOT LIMITED TO:

5.0.1. DRAINAGE PIPE AND STRUCTURE REVIEW

5.0.2. SUB-GRADE REVIEW

5.0.3. CURB REVIEWS

5.0.4. BASE REVIEWS

5.0.5. PUMP OUT AND LAMPING OF DRAINAGE SYSTEM

5.0.6. ASPHALT REVIEWS

5.0.7. SEMI-FINAL AND FINAL REVIEWS

5.0.8. PRESSURE AND LEAKAGE TESTING OF WATER SYSTEMS

5.0.9. LAMPING AND TELESPECTION OF SANITARY SEWER SYSTEM

FACILITIES MUST BE COMPLETED PRIOR TO SCHEDULING FIELD REVIEW. IF A FIELD REVIEW FAILS

TO MEET THE AGENCY REQUIREMENTS AND AN ADDITIONAL REVIEW IS REQUIRED THE CONTRACTOR

WILL BE RESPONSIBLE TO PAY ANY FEES ASSOCIATED WITH THE ADDITIONAL REVIEW.

6. NO SEPARATE PAYMENT SHALL BE PROVIDED TO THE CONTRACTOR FOR EARTHWORK ASSOCIATED

WITH THIS CONTRACT. ALL COSTS ASSOCIATED WITH SITE PREPARATION NECESSARY TO EXECUTE

THE WORK SET FORTH IN THE CONTRACT SHALL BE INCLUDED IN THE ITEMS PROVIDED FOR IN THE

UNIT PRICE BID SCHEDULE. ANY EXCESS MATERIAL REMAINING UPON COMPLETION OF CONSTRUCTION

SHALL BE REMOVED TO AN ACCEPTABLE LOCATION AT THE EXPENSE OF THE CONTRACTOR. AT THE

OWNER'S OPTION HE MAY IDENTIFY A LOCATION FOR THIS MATERIAL TO BE STOCK PILED OR INSTRUCT

THE CONTRACTOR TO REMOVE THE EXCESS TO A LEGAL DISPOSAL SITE. THE SITE HAS BEEN

PREVIOUSLY FILLED BY OTHERS TO ELEVATION 23.0 NGVD. THE CONTRACTOR SHALL BE RESPONSIBLE

FOR FINAL GRADING OF THE SITE TO WITHIN 0.2 ± FOOT OF ALL THE PROPOSED FINISHED GRADE

ELEVATIONS THROUGHOUT THE PROJECT SITE AS DEPICTED ON THE PLANS PREPARED BY THE

ENGINEER. ELEVATIONS OF CONSTRUCTED FACILITIES, IE ROADWAYS, DRAINAGE, ETC. ARE TO BE AS

INDICATED ON THE PLANS.

7. EXISTING UTILITIES, ROADWAY AND DRAINAGE FACILITIES ARE LOCATED AND IDENTIFIED BASED ON

THE AVAILABLE INFORMATION. THE CONTRACTOR IS TO VERIFY THIS INFORMATION WHERE

NECESSARY FOR PROPOSED CONSTRUCTION. ALL COSTS ASSOCIATED WITH VERIFICATION,

PROTECTION OF EXISTING SYSTEMS OR CONNECTION TO THOSE ITEMS AS CALLED FOR ON THE PLANS

IS INCLUDED IN THE UNIT PRICES.

8. CONTRACTOR SHALL EMPLOY A REGISTERED SURVEYOR TO LAY OUT LINE, GRADES, STRUCTURES,

LOT CORNERS, ETC. AS NECESSARY TO CONSTRUCT THE PROPOSED FACILITIES IN ACCORDANCE WITH

THE PLANS AND SPECIFICATION. BENCH MARKS SHALL BE ESTABLISHED BY A REGISTERED SURVEYOR.

BENCH MARKS SHALL BE PERMANENT AND EASILY ACCESSIBLE AND MAINTAINED AND REPLACED IF

DISTURBED OR DESTROYED.

9. ANY UNSTABLE SOIL AND/OR SOIL CONTAINING ORGANIC MATERIAL ENCOUNTERED DURING

CONSTRUCTION, INCLUDING BUT NOT LIMITED TO MUCK, SILT, SLUDGE AND MARL, SHALL BE REMOVED

AND SHALL BE REPLACED BY MATERIAL ACCEPTABLE TO THE ENGINEER. NO ADDITIONAL PAYMENT

SHALL BE PROVIDED FOR REMOVAL, REPLACEMENT, AND DISPOSAL OF SUCH UNSTABLE SOIL AND/OR

ORGANIC CONTENT SOIL. THE REMOVAL, REPLACEMENT AND DISPOSAL OF SUCH UNSTABLE SOIL

AND/OR ORGANIC CONTENT SOIL SHALL BE CONSIDERED TO BE PART OF THE CONTRACT PRICE AS

PROVIDED IN THE UNIT PRICE BID SCHEDULE. IF ANY ROCK, HARDPAN, ETC. IS ENCOUNTERED DURING

CONSTRUCTION OF ANY ITEMS, THE REMOVAL AND BACKFILL REQUIRED SHALL BE CONSIDERED

CONDITIONS, BUT SUCH SUBSURFACE INVESTIGATIONS SHALL BE PERFORMED ONLY UNDER TIME

SCHEDULES AND ARRANGEMENTS APPROVED IN ADVANCE BY THE OWNER'S REPRESENTATIVE AND

ENGINEER. THE EXISTING GRADES SHOWN ON THE DRAWINGS ARE APPROXIMATE ONLY AND NO

REPRESENTATION IS MADE AS TO THEIR ACCURACY OR CONSISTENCY. THE CONTRACTOR SHALL

VERIFY ALL EXISTING GRADES TO THE EXTENT NECESSARY TO INSURE COMPLETION OF THE JOB TO

THE PROPOSED GRADES INDICATED ON THE DRAWINGS

11. THE CONTRACTOR SHALL PROVIDE ADEQUATE BARRIERS AND PROTECTION FOR THOSE EXISTING

SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION.

2. MAINTENANCE OF TRAFFIC M.O.T. FOR THIS PROJECT WILL COMPLY WITH THE

F.D.O.T. STANDARD INDEX (600 SERIES) AND THE LATEST EDITION OF THE MANUAL

ON UNIFORM TRAFFIC CONTROL DEVICES (MUTCD).” SPECIAL ATTENTION WILL BE

GIVEN TO FDOT DESIGN STANDARD INDEX 611, 612, 613, AND 660.

3. ALL THERMOPLASTIC TRAFFIC STRIPES, MARKINGS AND SIGNAGE WILL BE

INSTALLED PER THE FDOT ROADWAY AND TRAFFIC DESIGN STANDARDS.

4. PERMITTEE WILL MATCH ABUTTING PAVEMENT SECTION ADJACENT TO PERMITTED

WORK WHICH INCLUDES THICKNESS AND TYPE OF ASPHALT & BASE & SUBGRADE

MATERIAL OR AS DIRECTED BY THE LOCAL RESIDENT OPERATIONS ENGINEER OR

DESIGNEE.

5. IT IS THE PERMITTEE'S RESPONSIBILITY TO OBTAIN FINAL ACCEPTANCE OF

PERMITTED WORK (COMPLETED) AND THE RESTORATION OF THE RIGHT-OF-WAY

FROM THE F.D.O.T. PRIOR TO USAGE.

6. PERMITTEE WILL PROVIDE THE NECESSARY DENSITIES IN ACCORDANCE WITH

SECTION 125-8 OF THE FDOT STANDARD SPECIFICATIONS FOR ROAD & BRIDGE

CONSTRUCTION (LATEST EDITION) PRIOR TO FINAL ACCEPTANCE BY THE F.D.O.T.

7. PERMITTEE WILL RESTORE THE RIGHT OF WAY AS A MINIMUM, TO ITS ORIGINAL

CONDITION OR BETTER IN ACCORDANCE W/F.D.O.T.'S LATEST STANDARD

SPECIFICATIONS FOR ROAD & BRIDGE CONSTRUCTION OR AS DIRECTED BY THE

RESIDENT OPERATIONS ENGINEER.

8. PERMITTEE SHALL PROVIDE THE PRODUCER'S CERTIFICATION (DELIVERY TICKET)

FOR THE NS CONCRETE-2500 PSI (USED FOR SIDEWALK, CURB & GUTTER, DITCH

PAVEMENT AND TRAFFIC SEPARATOR) PRIOR TO FINAL ACCEPTANCE BY THE

DEPARTMENT. THE DELIVERY TICKET SHALL CERTIFY THE CONCRETE WAS

BATCHED, DELIVERED AND PLACED IN ACCORDANCE WITH SECTION 347 OF THE

F.D.O.T.'S STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION

(CURRENT EDITION).

9. SODDED AREAS WILL BE IN ACCORDANCE WITH STANDARD INDEX 105 AND

SECTIONS 162, 981, 982, 983, 987 OF THE F.D.O.T.'S STANDARD SPECIFICATIONS

FOR ROAD AND BRIDGE CONSTRUCTION, LATEST EDITION. ALL DISTURBED AREAS

WILL BE SODDED WITHIN ONE (1) WEEK OF INSTALLATION OF SAID PERMITTED

WORK.

10. OWNERSHIP OF ALL SUITABLE EXCAVATED MATERIALS WITHIN THE FDOT R/W, AS

DETERMINED BY THE F.D.O.T., SHALL REMAIN IN THE DEPARTMENT UNTIL A FINAL

ACCEPTANCE OF THE PERMITTED PROJECT IS FULFILLED. EXCAVATED

MATERIALS SHALL BE HAULED BY THE CONTRACTOR, AT THEIR COST & EXPENSE

FROM THE SITE TO THE PALM BEACH OPERATIONS CENTER, 7900 W FOREST HILL

BLVD OR STOCKPILED IN THOSE AREAS AS DIRECTED BY THE DOT, INCLUDING

ASPHALT MILLINGS.

11. PERMITTEE SHALL RETURN ALL SALVAGEABLE SIGNS, INCLUDING

APPURTENANCES AND DRAINAGE PIPES TO THE PALM BEACH OPERATIONS

CENTER AT THE OWNERS EXPENSE.

12. RESTRICTED HOURS OF OPERATION WILL BE FROM 9:00AM TO 3:30 PM,

(MONDAY-FRIDAY), UNLESS OTHERWISE APPROVED BY THE OPERATIONS

ENGINEER, OR DESIGNEE.

13. PERMITTEE WILL COORDINATE (PRE-CONSTRUCTION MEETING, INSPECTIONS,

3. BACKFILLING OF STRUCTURE (MANHOLES, ETC) EXCAVATIONS AND REQUIRED FILL

UNDER STRUCTURE SLABS SHALL BE DONE IN HORIZONTAL LIFTS NOT EXCEEDING

TWELVE (12) INCHES IN DEPTH, AND SHALL BE COMPACTED TO A DENSITY OF NOT

LESS THAN 98% OF THE MAXIMUM DENSITY AS DETERMINED BY A.A.S.H.T.O. T-180.

4. BACKFILL MATERIAL FOR PIPE EXCAVATION SHALL BE SELECTED, DEPOSITED AND

COMPACTED SO AS TO ELIMINATE THE POSSIBILITY OF HORIZONTAL OR LATERAL

DISPLACEMENT OF THE PIPE. BACKFILL MATERIAL SHALL BE SOLIDLY TAMPED

AROUND THE PIPES IN SIX (6) INCH LIFTS UP TO A LEVEL AT LEAST SIX (6) INCHES

ABOVE THE TOP OF THE PIPE. BACKFILLING SHALL BE CARRIED OUT SIMULTANEOUSLY

ON BOTH SIDES OF THE PIPE. THE REMAINDER OF THE BACKFILL SHALL BE PLACED IN

SIX (6) INCH LIFTS (COMPACTED THICKNESS), MOISTENED AND MECHANICALLY

COMPACTED TO A DENSITY OF NOT LESS THAN 95% OF THE MAXIMUM DENSITY AS

DETERMINED BY A.A.S.H.T.O. T-180. WATER SETTLING MAY BE UTILIZED AT THE OPTION

OF THE CONTRACTOR, PROVIDED THE REQUIRED DENSITY IS ACHIEVED. IN AREAS

WHERE PAVING IS TO BE PLACED OVER THE BACKFILLED TRENCH, COMPACTION SHALL

BE CARRIED OUT TO ACHIEVE A DENSITY OF AT LEAST 98% OF THE MAXIMUM DENSITY

AS DETERMINED BY A.A.S.H.T.O. T-180 FOR A MINIMUM OF THE TOP THREE (3) FEET OF

THE BACKFILLED TRENCH (THE REMAINING DEPTH AS NOTED ABOVE) AND EXTENDING

TO THE TRENCH WIDTH + TWO (2) FEET - SEE DETAIL.

5. DENSITY TESTS FOR DETERMINATION OF THE SPECIFIED COMPACTION SHALL BE MADE

BY AN APPROVED ENGINEERING TESTING LABORATORY. TESTS SHALL BE TAKEN IN

ALL COMPACTED LIFTS (1 FOOT MINIMUM) AND SPACED ONE (1) FOR EVERY 150 FEET

OF TRENCH (MAXIMUM) WITH AT LEAST ONE (1) TEST IN EACH PIPE RUN (STRUCTURE

TO STRUCTURE). WHERE TRENCH CROSSES AREAS TO BE PAVED, A MINIMUM OF ONE

(1) OF THE REQUIRED DENSITY TESTS MUST BE TAKEN IN THAT AREA.

6. EXCAVATION, SIDE SLOPES, SHORING, ETC., SHALL CONFORM TO O.S.H.A. STANDARDS.
