PARMA COUNCIL OF PTAs 2014- 2015 SCHOLARSHIP PROGRAM

Parma Council of PTAs is pleased to coordinate this Scholarship Program for the Graduating Seniors of the Parma City School District. We proudly announce that this is the largest PTA Council Scholarship Program in the State of Ohio. This booklet includes instructions, contact information, the scholarship application and a listing of scholarship offerings. Specific criteria for each scholarship can be accessed on the Parma City Schools website www.parmacityschools.org. Click on “For Parents” and then on “Parma Council of PTAs”, then “Scholarship Program”. It is also available from your Counselor. Please review all information carefully and apply for all of the scholarships for which you are eligible.

INSTRUCTIONS FOR THE SCHOLARSHIP APPLICATION

1. Read and follow all directions carefully. By carefully reviewing the special requirements, you may find that you are eligible for more scholarships than you initially thought. There are scholarships available in a variety of areas including the Career and Technical Track, art, music, sports, Firefighter/Paramedic/EMT, math, computers, education, and so forth. Be certain you meet all requirements listed for each scholarship you submit an application for.

2. If you’ve attended a PCSD elementary school that is now closed, please review the scholarship criteria for the elementary school in your current attendance area. You may be eligible for a scholarship from that school.

3. Use ONLY the application form provided. DO NOT REFORMAT. Follow the directions

given for each section. DO NOT add additional pages (except if specifically requested in the special requirements). Please use the back of the application if needed.

4. Complete only ONE application (the master application) even if you plan to apply for

several scholarships. You will print/make additional copies later.

5. DO NOT COMPLETE SECTION A (PAGE 2). Your Counselor will complete this portion and provide the correct Student I.D. Number.

6. Both the STUDENT and a PARENT must sign the application if, you, the student is

under the age of 18 before you return it to your Counselor for completion of Section A (page 2).

7. Return Pages 1 and 2 of the application to your Counselor by prior to making copies

for completion of Section A.

8. Please TYPE or PRINT all information on the MASTER application. Complete Page 1 and Sections B through J (pages 3 through 7). DO NOT include the scholarship name and scholarship number. You will enter/write the name and number of the scholarship you are applying for on the copies you will be making of your master application. MAKE SURE YOUR STUDENT I.D. NUMBER IS ON ALL PAGES OF YOUR APPLICATION PRIOR TO PRINTING/COPYING.

9. Proofread for accuracy and spelling and make sure that your application is neat and

legible. Be sure to add up your hours correctly when filling out your application. Failing to do this may result in you receiving fewer points than what you are entitled to. Remember, this application reflects you and your request for scholarship dollars.

10. After your Counselor has completed Section A (page 2), these pages will be returned

to you. You must then duplicate ALL PAGES of your APPLICATION for EACH of the scholarships for which you are eligible. You MUST make copies yourself. Be sure that every copy of your application is COMPLETE. We are unable to notify students if they are missing pages and information.

11. After making your copies, you must add the scholarship name and scholarship number

to Page 1/Cover Sheet (upper right hand corner) for EACH scholarship for which you are applying. Scholarship names and scholarship numbers are listed within this booklet and on the website.

12. Return all completed copies of your scholarship applications to your Counselor by

FRIDAY, FEBRUARY 13, 2015. The only exception to this application deadline is the Ohio Congress of PTA Grant. The Ohio Congress of PTA Grant requires a different application and due date. Your Counselor would have distributed Ohio PTA applications previously.

The information contained in this booklet and on the Parma City Schools website is deemed reliable, but holds no guarantee. The Parma Council of PTAs Scholarship Program does not discriminate on the basis of race, color, national origin, gender, age, religion, or disability. Distribution of funds is based on the rankings received from individual scholarship sponsors. Final assignments of scholarships will take into consideration total dollar amounts awarded to individual students. PC-1 PARMA COUNCIL OF PTAS - GERANIUM SCHOLARSHIPS Minimum of two grants per high school $1,200 each 1. The applicant must be a graduating senior of a Parma public high school. 2. Reimbursement is required if recipient does not graduate high school (or attain the equivalent) or does not begin higher education within 12 months of receiving scholarship. 3. Recipient is responsible to supply proof of completion of one year of College, University or Technical School within 18 months of beginning higher education.

PC-2 PARMA COUNCIL OF PTAS - GERANIUM CAREER & TECHNICAL SCHOLARSHIP One grant per district $1,200 1. The applicant must be a graduating senior of a Parma public high school. 2. The applicant must have completed a two-year Career and Technical Track program through the Parma Public Schools. 3. The applicant must enroll in supplemental training, which will advance their education in their field. 4. Reimbursement is required if recipient does not graduate high school (or attain the equivalent) or does not begin higher education within 12 months of receiving scholarship. 5. Recipient is responsible to supply proof of completion of one year of college, university, technical, or vocational school within 18 months of beginning higher education.

PC-3 PARMA COUNCIL OF PTAS COMMUNITY SCHOLARSHIP One or more grants $500 each 1. Applicant must be a graduating senior of the Parma City School District. 2. The award will be based on academics, extracurricular activities, community service, work experience, honors, awards and essay. 3. Reimbursement is required if recipient does not graduate high school (or attain equivalent) or does not begin higher education within 12 months of receiving scholarship. 4. Minimum GPA of 3.0

EC-1 PARMA EARLY CHILDHOOD PTA SCHOLARSHIP One grant $500 1. Applicant must be a graduating senior of a Parma City School District public high school. 2. Reimbursement is required if recipient does not graduate high school or does not begin higher education within 12 months of receiving the scholarship. 3. Recipient is responsible to supply proof of completion of one year of college, university, or technical school within 18 months of beginning higher education. 4.Community service and community involvement in the Parma area will be taken into consideration for this award.

PE-1 DENTZLER ELEMENTARY SCHOOL PTA- THE GERALDINE FRITCHIE SCHOLARSHIP One grant $1,000 1. The applicant must have attended Dentzler Elementary school for a minimum of one year. 2. The applicant must be a graduating senior of a Parma public high school. 3. The applicant must live or have lived in the current Dentzler attendance area. 4. Applicant must retain a 2.8 GPA or better in high school. 5. Reimbursement is required if the recipient elects not to continue his/her first year of college. 6. Reimbursement is required if the recipient fails to maintain a 2.0 or better average.

PE-2 GREEN VALLEY ELEMENTARY SCHOOL PTA SCHOLARSHIP Two grants $1000 each 1. The applicant must have attended Green Valley Elementary School (or have attended a Parma City Schools elementary school in the current Green Valley attendance areas closed as a result of consolidation, i.e. Dag Hammarskjold, Lt. Col John Glenn) for their final three years of elementary school. 2. The applicant must be a graduating senior of a Parma public high school.

PE-3 JOHN MUIR ELEMENTARY SCHOOL PTA SCHOLARSHIP One grant $750 1. The applicant must have attended John Muir Elementary School for at least three years (or have attended a Parma City Schools elementary school in the current John Muir attendance area closed as a result of consolidation). 2. The applicant must be a graduating senior of a Parma public high school. 3. Reimbursement is required if the recipient does not attend college or complete at least one year of college. 4. The recipient is highly encouraged to submit a mid and end of school year brief educational experiences update to the John Muir PTA.

PE-4 JOHN MUIR ELEMENTARY SCHOOL PTA -STAFF MEMORIAL SCHOLARSHIP One grant $750 1. The applicant must have attended John Muir Elementary School for at least three years (or have attended a Parma City Schools elementary school in the current John Muir attendance area closed as a result of consolidation). 2. The applicant must be a graduating senior of a Parma public high school. 3. Reimbursement is required if the recipient does not attend college or complete at least one year of college. 4. The recipient is highly encouraged to submit a mid and end of school year brief educational experiences update to the John Muir PTA.

PE-5 PARMA PARK ELEMENTARY SCHOOL PTA -JAY HOLLOWAY AND MAX FEDORCHUK MEMORIAL SCHOLARSHIP (two students who passed away in 1997 with leukemia) One grant $750 1. The applicant must have attended Parma Park Elementary School for at least two years (or attended a Parma City Schools elementary school in the current Parma Park attendance area closed as a result of consolidation, i.e. Pleasantview, James E Hanna, Pearl Rd). 2. Applicant is a graduating senior of a Parma public high school. 3. Reimbursement is required if recipient does not complete one full year of college.

PE-6 PLEASANT VALLEY ELEMENTARY PTA ALEX SHAIDE MEMORIAL SCHOLARSHIP One grant $500 1. The applicant must be a Parma public high school graduating senior. 2. The applicant must have attended Pleasant Valley (or have attended a Parma City Schools elementary school in the current Pleasant Valley attendance area closed or redistricted as a result of consolidation, i.e. Pleasantview, Pearl Rd, or Parma Park) for their final years of elementary school. 3. Following your essay, you have the option to attach a summary of how you were involved or active at your elementary school. 4. Reimbursement is required if the recipient does not complete one full year of college. 5. The recipient must complete graduation or its equivalent. 6. The recipient must attend a college, university or school of higher education within 12 months of their graduation year.

PE-7 PLEASANT VALLEY ELEMENTARY PTA SCHOLARSHIP One grant $500 1. The applicant must be a Parma public high school graduating senior. 2. The applicant must have attended Pleasant Valley (or have attended a Parma City Schools elementary school in the current Pleasant Valley attendance area closed or redistricted as a result of consolidation, i.e. Pleasantview, Pearl Rd, or Parma Park) for their final years of elementary school. 3. PE-8 RENWOOD ELEMENTARY SCHOOL PTA SCHOLARSHIP (2) Two grants $500 each 1. The applicant must have attended Renwood Elementary School for at least two years (or have attended a Parma City Schools elementary school in the current Renwood attendance area closed as a result of consolidation) 2. The applicant must be a graduating senior of the Parma City School District. 3. Reimbursement is required if recipient does not graduate (or attain the equivalent) or does not begin higher education within 12 months of receiving scholarship. 4. Recipient is responsible to supply proof of completion of one year of college, university or technical school within 18 months of beginning higher education.

PE-9 RIDGE-BROOK ELEMENTARY SCHOOL PTA - FRANKIE YUHAS MEMORIAL SCHOLARSHIP (Frankie Yuhas was an extremely involved State Life Recipient at Ridge-Brook Elementary) One grant $500 1. The applicant must have attended Ridge-Brook Elementary School for a minimum of 4 years. 2. The applicant must be a graduating senior of a Parma public high school. 3.Reimbursement is required if the recipient withdraws from college or institute of higher learning during the first year.

PE-10 THOREAU PARK ELEMENTARY SCHOOL PTA SCHOLARSHIP- THE BARBARA GROVES/DARIN CARLETON MEMORIAL SCHOLARSHIP (2) Two grants $1,000 each 1. The applicant must have attended Thoreau Park Elementary School for at least three years (or would have attended Thoreau Park had their school not been affected due to closure, consolidation or redistricting). 2. The applicant must be a graduating senior of a Parma public high school. 3. Award is based on scholastic achievement and character. 4. Must have a minimum 2.5 grade point average.

PM-1 GREENBRIAR MIDDLE SCHOOL PTA GRANT One grant $750 1. The scholarship will be awarded to a former Greenbriar Middle School student who: a. Attended Greenbriar Middle School at least two years or have attended Greenbriar Middle School for one year plus attended one of the PCSD Elementary Schools for one year (for those students who are affected by the changes of consolidation). b. Is a graduating senior of a Parma public high school. c. Applicant must achieve a cumulative GPA of at least 2.5 2. Reimbursement to the Greenbriar Middle School PTA is required if the student does not complete one full year of college.

PM-2 HILLSIDE MIDDLE SCHOOL PTA GRANT Two grants $750 1. The applicant must have attended Hillside Middle School for at least one full year. 2. The applicant must be a graduating senior of a Parma public high school. 3. Reimbursement is required if the recipient fails to finish the first year of higher education.

PM-3 SHILOH MIDDLE SCHOOL PTA GRANT One grant $750 1. The applicant must be a graduating senior of Parma Senior High School. 2. The applicant must have attended Shiloh Middle School for both 7th and 8th grade. 3. Reimbursement is required if the recipient fails to finish the first year of college. 4. The applicant must have a minimum cumulative GPA of 2.5 to apply.

PH-1 NORMANDY HIGH SCHOOL PTA GRANT Five grants $875 each 1. The applicant must be a graduating senior of Normandy High School. 2. The applicant must have completed three full academic years at Normandy High School at the time the application is submitted. 3. Reimbursement is required if the recipient does not complete one full year of college.

PH-2 PARMA SENIOR HIGH SCHOOL PTSA SCHOLARSHIP Two grants $750 each 1. The applicant must be a graduating senior of Parma Senior High School. 2. The applicant must have attended Parma Senior High School for at least two years. 3. The applicant must have an accumulative GPA of 2.0 at the time of the application. 4. The applicant will be judged on his/her scholastic achievement and school/community involvement. 5. Reimbursement is required if the recipient does not complete the first calendar year of higher education. 6. The applicant will be judged on community involvement.

PH-3 VALLEY FORGE HIGH SCHOOL PTSA SCHOLARSHIP One grant $750 1. The applicant must have attended Valley Forge High School for at least three years. 2. The applicant must be a graduating senior of Valley Forge High School. 3. The applicant must have a minimum GPA of 2.5 to apply. 4. The applicant must attend an institution of higher learning within 12 months of the class' graduation date. 5. Reimbursement is required if the recipient does not complete one full year of the institution of higher learning. 6. Additional consideration will be give to those applicants who are PTSA members.

PH-4 VALLEY FORGE HIGH SCHOOL PTSA- STEELE NOWLIN PATRIOT SCHOLARSHIP One grant $750 1. The applicant must have attended Valley Forge High School for at least three years. 2. The applicant must be a graduating senior of Valley Forge High School. 3. The applicant must have a minimum GPA of 3.0 to apply. 4. The applicant must attend an institution of higher learning within 12 months of the class' graduation date. 5. Reimbursement is required if the recipient does not complete one full year of the institution of higher learning. 6. Additional consideration will be give to those applicants who are PTSA members.

The applicant must display leadership ability in school activities.

SH-6 VALLEY FORGE HIGH SCHOOL ADULT ATHLETIC BOOSTERS SCHOLARSHIPS Two (one Female and one Male) $500 each 1. Athlete must be involved in any varsity sport at Valley Forge for two years, one of which must be during junior or senior year. 2. GPA of at least cumulative 2.5 average for high school education. 3. Applicant must have attended Valley Forge for at least two years. 4. Applicant must be graduating from Valley Forge this year. 5. Scholar must attend a school of higher learning. 6. His parents wished the monies they received to be put into a trust for a memorial grant for David's classmates and future Valley Forge students.) One grant $750

C-1 ALPHA DELTA KAPPA - ALPHA NU CHAPTER SCHOLARSHIP (This scholarship started as a tribute to the teaching profession and to encourage top-notch seniors to become teachers.) Two grants $750 each

Recipient is responsible to supply written proof of completion of one year of college/university within18 months of beginning higher education. 4. The applicant must attend an institution of higher leaning and major math or math related field. 4. The applicant must be involved in community service.

C-9 THE PARMA CITY SCHOOLS FOUNDATION SCHOLARSHIP The newly formed Parma City Schools Foundation has been established to fund and support curricular and extracurricular programs of the PCSD and to facilitate scholarships. The applicant must have a cumulative GPA of 2.5 or above. 3. The applicant must be planning on attending either a two year or four year college in the year immediately following graduation. 4. The applicant must include an essay that addresses the following question/scenario: Describe one instance in your high school career where you became the teacher. Be sure to describe challenges you faced and how you overcame them. OR Write about an experience when you have had to tell the truth when it was not easy to tell the truth.

RS-2 PARMA AREA CHAMBER OF COMMERCE SCHOLARSHIP One - Three grants $1,000 each 1. The applicant must have earned at least one full credit in business education (including career and technical business). 2. The applicant must have maintained at least a 2.5 accumulative GPA. 3. The applicant must be planning a career in the business field by attending college full time or by working and attending college part time. Should the recipient not attend college, the scholarship will be forfeited. 4. The applicant from each high school must submit with the application a 100 word essay on the topic "WHY I HAVE CHOSEN TO PURSUE A CAREER IN __________" Each recipient must be prepared to give a one or two minute presentation to the Chamber members at the May luncheon, if requested. (The only exception is a student residing in the school district who may be serviced through another school district due to program availability.) 2. The applicant must be planning to attend a technical school, an apprenticeship or college. 3. The recipient is responsible to supply proof of completion of one year of college, university or technical school within 18 months of beginning higher education. 4. Teacher, administrator or counselor recommendation must be included.

RS-5 PARMA EDUCATION ASSOCIATION ACHIEVEMENT SCHOLARSHIP One grant $500 1. The applicant must be planning on either entering a technical school, an apprenticeship program, or a college program. 2. The applicant must have maintained a GPA of 3.0 or better. 3. The applicant must attend an institution of higher learning. 4. Must be involved in at least two extracurricular activities, one of which must be school-related (such as music, theatre, sports, or student leadership groups) 5. RS-22 VALLEY FORGE HIGH SCHOOL -THE ROBERT A. MUELLER MEMORIAL SCHOLARSHIP (Robert A Mueller was a Social Studies teacher and department chairman at Valley Forge High School for 32 years.) One grant $1,000 1. Applicant must be a June graduating senior of Valley Forge High School. 2. Must be planning to attend a four-year college or university. 3. A short letter of recommendation from one of the student's current or past Honors Social Studies or Government teachers is required. 4. A list of all high school Social Studies Honors classes taken and grades achieved is required with counselor verification. 5. The award will be based on scholarship achievements and activities.

RS-23 VALLEY FORGE HIGH SCHOOL FACULTY & STAFF SCHOLARSHIP -SPECIAL ESSAY At least one grant $750 1. The applicant must be a graduating senior at Valley Forge High School. 2. The applicant must have a cumulative GPA of 2.75 or above. 3. The applicant must be planning on attending either a two or four year college. 4. The applicant must include an ESSAY (of no less than one, but no more than two pages) on the following topic: What does it mean to be a Valley Forge Patriot? 5. Reimbursement will be required if the recipient does not complete one full year of college.

RS-24 VALLEY FORGE HIGH SCHOOL NOTABLE ACHIEVEMENT (O.O.C.) SCHOLARSHIP Two grants $500 each 1. The applicant must be a graduating senior of Valley Forge High School. 2. The applicant must be planning on entering a technical school or college for post high school training. 3. The applicant must have a record of documented disability (current year IEP) throughout high school. 4. The applicant must have a statement of support prepared and signed by the current IEP case manager. This can be a simple hand-written or typed statement saying the student is a good candidate and has college or vocational plans. 5. The applicant will be judged by a team of intervention specialists at Valley Forge High School on scholastic and/or vocational achievement, teacher input and completed application. 6. Applicant must complete a short statement on his/her career/post high school education plans. This can be hand-written or typed but should give their specific intentions for their educational plans after high school.

RS-25 VALLEY FORGE - CLASS OF 1964 50TH REUNION "UNIVERSITY" SCHOLARSHIP One grant $1,200 1. The applicant must be a graduating senior of Valley Forge High School in 2015. 2. The applicant must have maintained a 3.0 or better. 3. Must be involved in at least two extracurricular activities a. one of which must be school related (such as music, theatre, sports or student leadership groups) b. one out of school activity - including work experience 4. Applicant must submit written letters of recommendation from (2) high school teachers. 5. Must include an ESSAY, hand written and double spaced of not more than 1000 words explaining "What does Patriotism mean to me?" RS-28 THE JAMES J. HIGGINBOTHAM MEMORIAL SCHOLARSHIP (EMT, PARAMEDIC, FIREFIGHTER, FIRE TECHNOLOGY) One grant $1,000 1. The applicant must be a graduating senior of the Parma City School district interested in pursuing education for either EMT (Emergency Medical Technician), Paramedic, Firefighter, or Fire Technology. 2. Letter of recommendation from a teacher, counselor or coach must accompany the application. 3. The applicant must attend a certified program, fire academy, or institution of higher learning for EMT, paramedic, firefighting, or fire technology. 4. Applicant must submit a special essay with their application addressing "Why I am passionate about becoming an EMT, paramedic, fire fighter or fire technology professional!" 5. Reimbursement is required if recipient does not graduate or does not begin attending an accredited program within 12 months of receiving the scholarship.

RS-29 THE RUTH H. BENSON ART SCHOLARSHIP (Ruth Benson was a watercolor artist. She was president of Parkview PTA in 1969 and worked for many years with the Parma Area Fine Arts Council.) One grant $500 1. The applicant must be a graduating senior of a Parma public high school. 2. The applicant must be planning to major in Fine Arts or Graphics Arts at an accredited college or art school. 3. Applicants must submit a copy of their favorite piece of art or project they created, along with an essay (of no more than 800 words) explaining their piece. 4. Artwork may be submitted in a photocopy, 35mm slide, digital memory card, CD, or DVD format. 5. Copy of artwork must be submitted with application and essay. It can be returned after judging if requested. 6. Reimbursement is required if recipient does not graduate from high school or attain the equivalent and does not attend college or art school within 12 months of receiving the scholarship.

RS-30 THE ANTHONY ZIELINSKI MEMORIAL SCHOLARSHIP (Anthony Zielinski, former City of Parma Treasurer and Councilman, was co-founder of the "Leadership of Tomorrow" Parma City School District local government educational program.) One grant $1,200 1. The applicant must be a graduating senior of the Parma City School District. 2. The applicant must have a cumulative grade point average of 3.0 or above. 3. The applicant must have participated in school and/or community service. 4. The applicant must be seeking higher education at a two or four year college or university. 5. The applicant must plan to major in political science, education, criminal justice, or social sciences. 6. The applicant must attach to his/her application an essay of approximately 250 words on the topic "What leadership means to me." Cite specific examples of your leadership. 7. Reimbursement is required if the recipient does not begin studies within one year of receipt of the award.

RS-31 THE JOSEPH A. KORTAN MEMORIAL SCHOLARSHIP (Joseph A Kortan was a teacher at Greenbriar, specializing in math and computers.) One grant $1,000 1. The applicant must be a graduating senior of the Parma City School District. 2. The applicant must be planning on majoring in math or computers. 3. The applicant must include with their application an essay on why they are choosing to major in math or computers. 4. Reimbursement is required if the recipient does not begin studies within one year of receipt of the award.

RS-32 COMMUNITY AMBASSADOR SCHOLARSHIP Purpose: To honor and support students who have demonstrated scholastic excellence, exemplary leadership, and community service. Two grants $500 each 1. The applicant must be a graduating senior of a Parma public high school. 2. The applicant must have achieved an accumulative 3.0 or better grade point average. 3. The applicant must have demonstrated leadership ability and exemplary citizenship through community involvement. 4. The applicant must be seeking higher education from a trade school, technical school, business school, or college. 5. The scholarship will be given in the Fall once enrollment is confirmed by current proof of attendance at a post-secondary institute. 6. Applicant must supply a short essay (1-2 pages) that address the following topics: a. Share one way that you have served your community during high school b. Identify one need or challenge you see in the community. Then give an example of how you and/or others in the community might be able to help address that need.

RS-33 PARMA AREA KIWANIS One grant $750 1. All graduating seniors, regardless of school attended, MUST RESIDE IN THE CITY OF PARMA, PARMA HEIGHTS, OR SEVEN HILLS. 2. The applicant must be accepted for enrollment at a four year college or university. 3. Reimbursement is required only if the student DOES NOT complete freshmen year. 4. Selection will be based on need, service and scholarship. 5. Finalists may be interviewed.

Distribution of applications for these grants takes place in January through the senior counselors. If the students has not received an application and wishes to apply, he/she should contact the counselor IMMEDIATELY to determine if participation is still possible.
