SHAC PHARMACY COORDINATOR

OVERVIEW The SHAC Pharmacy Coordinator is a 1-year position that the Pharmacy Coordinator-Elect will assume at the start of his/her PY3 year. The Coordinator is responsible for developing and maintaining a strategic vision for all pharmacy services in SHAC. He/She serves to unite all pharmacy services on a central set of goals to ensure the effective investment of time and financial resources. The primary role of the coordinator is to drive innovation by challenging current practices and developing initiatives that redefine current efforts. Management is intrinsically a part of this role, as the Coordinator must ensure current standards are maintained and new initiatives come to fruition.

RESPONSIBILITIES Drive research initiatives/manage pharmacy-led projects that enhance volunteer, leader, and patient experiences

Maintain and build community partnerships Collaborate with Coordinator-Elect to lead recruitment efforts (e.g., host

interest meetings and Senate Carnival booth) Collaborate with SHAC Finance and Medical Clinic Directors to maintain

organization’s financial health Lead the annual SHAC Lab Coat Fundraiser Facilitate SHAC Pharmacy leadership meetings Manage PHCY 700 Sakai site and serve as Lead PHCY 700 TA Serve as representative at monthly SHAC Coordinating Council (CC) and ESOP

SHAC PHARMACY COORDINATOR-ELECT

OVERVIEW The SHAC Pharmacy Coordinator-Elect is a 2-year position open to incoming PY2s. Following the Elect’s first year, he/she will assume the Pharmacy Coordinator position. The intention of this position is to familiarize the Elect with SHAC Pharmacy’s overarching mission and vision so that he/she may foster the organization’s growth as the upcoming Coordinator.

RESPONSIBILITIES Serve as Clinic and Outreach Volunteer Coordinator Collaborate with Pharmacy Coordinator to organize PY1 lab coat fundraisers

and host interest meetings and Senate Carnival booth Attend monthly SHAC Coordinating Council meetings and ESOP Senate Council

meetings as a representative of the Pharmacy Coordinator when necessary Serve as PHCY 700 TA Engage in research and quality improvement projects that aim to enhance

SHAC CLINIC CO-COORDINATORS

OVERVIEW The SHAC Clinic Co-Coordinator (PY2) is a 2-year position open to incoming PY2s. The roles and responsibilities of this position are year-round and include overseeing the pharmacy operations for both acute clinic and Bridge to Care (BTC) at SHAC Clinic on every other Wednesday evening at Piedmont Health Services in Carrboro, NC. There are 2 openings for this position: Incoming PY2s will retain the position for the duration of their PY2 and PY3 years.

Retaining incoming PY2s for two years allows them to become well-versed in clinic operations and workflow, so that they are equipped to propose/implement changes and research initiatives that benefit patients and student volunteers.

RESPONSIBILITIES Recruit, train, and orient both student and resident volunteers to SHAC Clinic Collaborate with Pharmacy Coordinator-Elect to manage volunteer schedule Maintain SHAC Pharmacy prescription inventory and formulary; meet

periodically with SHAC P&T Committee to identify formulary/inventory needs Attend SHAC Pharmacy leadership meetings Collaborate and regularly work alongside SHAC leaders of diverse healthcare

BENEFITS Being a SHAC Clinic Co-Coordinator is an extremely rewarding experience that offers both administrative experiences, in the oversight and management of SHAC Clinic pharmacy operations both day-to-day and long-term, as well as clinical experiences, in the discussions of patients encountered each week at SHAC Clinic. Clinic Coordinators work closely with UNCH pharmacy residents and ESOP faculty to assist student pharmacists in selecting the most appropriate therapeutic regimens for their patients. In addition, Coordinators mentor PY1 SHACterns and provide them with a broad introduction to practicing patient care in a multidisciplinary environment.

OUTREACH DIRECTOR AND DIRECTOR-ELECT

OVERVIEW The SHAC Outreach Director/Director-Elect position is a 2-year position open to incoming PY2s. The roles and responsibilities of the Outreach Director/Director-Elect are essentially the same (they are co-directors, with the “Elect” title reflecting the first year versus the second year of the position). The Directors’ primary responsibility is to manage monthly SHAC Outreach walk-in diabetes and hypertension clinics at two locations: a homeless shelter in Durham and a church with a Spanish-speaking population in Chapel Hill. The walk-in clinics focus on long-term disease state management with many patients returning every month to check in and keep up with their health. Directors also coordinate trainings for all Outreach volunteers and projects for the PY1 SHACterns and have traditionally been involved in research and quality improvement projects within SHAC Pharmacy.

RESPONSIBILITIES Coordinate and supervise walk-in clinics Maintain and build community partnerships Collaborate with interdisciplinary SHAC programs to provide additional services

BENEFITS Although there is a significant time commitment associated with SHAC Outreach, it is an incredibly rewarding role. Directors have a profound impact on their patients, volunteers, and SHACtern mentees and set standards for professionalism, volunteerism, and initiative. Leading SHAC Outreach efforts is a great avenue to explore interest in the ambulatory care setting and practice direct patient care skills in a population that does not have reliable access to healthcare. The Outreach Director and Director-Elect positions provide valuable opportunities for students who wish to maximize their capacity to affect change both in the school

OVERVIEW The Development Specialist-Elect is a 2-year position open to incoming PY2s. The Specialist and Specialist-Elect share duties, with the Elect designation indicating the 2-year commitment. Their primary responsibilities are wide-ranging and emphasize creativity as much as organization. Duties include coordinating the PY1 SHACtern program, spearheading the annual leadership selection process, and leading quality improvement efforts within SHAC Pharmacy

2019-2020 SHAC RESEARCH AND QUALITY IMPROVEMENT PROJECTS

COLLABORATION WITH P&T COMMITTEE

SHAC Pharmacy collaborates with SHAC/AMCP Pharmacy & Therapeutics (P&T) Committee in order to manage medication formulary and effectively utilize budget to promote patient care at SHAC Clinic. This project aims to analyze decisions made around addition or changes to SHAC Clinic formulary with respect to P&T reports in order to promote more efficient and informed decision-making in formulary management.

OUTREACH PATIENT REFERRAL AND BARRIERS TO CARE PROJECT

Patient referrals are an important part of SHAC Outreach screenings to ensure follow-through of care. While patients at SHAC Outreach are often referred to SHAC Clinic for follow up on concerning blood pressure or blood sugar readings, a variety of other resources and services are also available to meet patients’ needs. Interventions for this project includes evaluating and improving the referral resources at SHAC Outreach to ultimately establish a standard operating procedure for SHAC Outreach’s referral system.

LIFESTYLE MODIFICATION COACHING THROUGH TEXT MESSAGES PROJECT

As an extension to the SHAC TExT project conducted a few years ago, the lifestyle modification coaching through text messages project is designed to implement a sustainable text message support system for Outreach patients interested in lifestyle modifications. While SHAC TExT was previously offered to patients at Urban Ministries of Durham, intervention of this project includes translating text messages into Spanish in order to expand services to incorporate patients at St. Thomas More Church. The goal of this project is to encourage patients at SHAC Outreach to advocate for their own health and to help improve overall health outcomes.

BRIDGE TO CARE APPOINTMENT CANCELLATION ANALYSIS PROJECT

SHAC Bridge To Care runs by scheduled appoints, where patients are scheduled for one-hour slots on Wednesday evenings. This project aims to analyze reasons for frequent appointment cancellation, given the patient population served at SHAC Clinic. Identification of potential patient barriers or conflicts will allow areas of improvement for efficiency and workflow within the program, and implementation

PATIENT NEEDS ASSESSMENT PROJECT

SHAC Outreach interacts monthly with the communities of Urban Ministries of Durham and St. Thomas More Church. This quality improvement project will identify strengths and opportunities for improvement for the Outreach program, based on the opinions of the patients we serve. This project is currently in the data collection phase.
