Farmers' Corner- A Knowledge Hub Blood fruit is a perennial, woody climber thatgrows taking support of large trees in its naturalhabitat. Plants bear sweetish sour fruits inbunches, which originate directly from the stem.In backyard gardens, it could be grown using largetrees such as mango, jackfruit etc. as live standardsand hence, does not require additional space forits cultivation. Fruits have dark red, juicy pulpowing to which it is known as blood fruit. Fruitclusters are borne on old wood and are of about30-45 cm length weighing 1.0-1.5 kg each. As the fruits are collected from wild/semi-wild conditions and sold in local markets at the rate of Rs. 250- 300/- per kg, effortswere made to popularize it as a backyard crop among the island farmers so that thenatural populations are conserved. Nursery techniques have been standardized andregenerated seedlings are being provided to various stakeholders to promote thespecies as a crop. Pulp was found to be a rich source of anthocyanins of whichPelargonidin and Cyanidin were the dominant ones. Use of blood fruit pulp as anatural colourant was also demonstrated.Benefit: Growing a few liana in the backyards could serve as additional source ofincome to the farmers besides being a source of nutrients. Further, use of pulp as anatural colourant for beverages could be promoted among the island hoteliers orcaterers.

He noted that CAG has taken measures to build capacities in thearea of environmental audit. He said that this is a very welcome step for ourfuture. We must always remember that constraints of resource finitenesscan only be partly addressed by human innovation. For the rest sacrifice byour generation is the only recourse. CAG has a big role to play in making usaware of sacrifices we need to make for our future generations. Addressing the officer trainees, the President said that as public servantsthey would derive greatest satisfaction in being able to serve the poorestand bring a smile on her face. ANIIMS for YOU

-Dr.Ankit Das (Intern), ANIIMS

Q. What is Tuberculosis?

Before COVID-19 became a global pandemic, India was dealing with another, much older epidemic Tuberculosis (TB).

Tuberculosis (TB) is caused by a bacterium called Mycobacterium tuberculosis. The bacteria usually attack the lungs, but

TB bacteria can attack any part of the body such as the kidney, spine, and brain. Not everyone infected with TB bacteria

becomes sick.

Q. What is Pulmonary Tuberculosis?

When the tuberculosis bacteria affect the lungs it is known as pulmonary tuberculosis. Pulmonary TB is the most

contagious type, but it usually only spreads after prolonged exposure to someone with the illness.

Q. How is TB Spread?

TB germs are put into the air when a person with TB disease of the lungs or throat coughs, sneezes, or speaks. These

germs can stay in the air for several hours, depending on the environment. Persons who breathe in the air containing these

TB germs can become infected; this is called latent TB infection.

Q. What are the symptoms?

General symptoms of TB:

Lack of appetite and weight loss

Evening high temperature

Night sweats

extreme tiredness or fatigue

TB that affects the lungs (pulmonary TB):

A persistent cough that lasts more than 2 weeks and usually brings up phlegm, which may be bloody

Breathlessness that gradually gets worse

TB outside the lungs:

Less commonly, TB infections develop in areas outside the lungs, such as the small glands that form part of the immune

system (the lymph nodes), the bones and joints, the digestive system, the bladder and reproductive system, and the brain

and nerves (the nervous system). Symptoms can include:

persistently swollen glands

abdominal pain

pain and loss of movement in an affected bone or joint

confusion

a persistent headache

fits (seizures)

Q. Who all are at risk?

people living in areas with high rates of TB

Substance abuse (smoking, alcohol, drugs)

Immunosuppression like patients on cancer chemotherapy or on prolonged steroid therapy, people with HIV AIDS.

Malnutrition

Q. What should I do if I have been exposed to someone with TB Disease?

People with TB disease are most likely to spread the germs to people they spend time with every day, such as family

members or co-workers. If you have been around someone who has TB disease, and have symptoms as mentioned above

for at least 2 weeks you should go to your doctor or your local health department for sputum tests.

Q. How is TB Disease treated?

TB disease can be treated by taking drugs for 6 to 12 months. It is very important that people who have TB disease

finish the medicine, and take the drugs exactly as prescribed. If they stop taking the drugs, they can become sick again; if

they do not take the drugs correctly, the germs that are still alive may become resistant to those drugs. TB that is resistant

to drugs is harder and more expensive to treat.

In some situations, staff of the local health department meets regularly with patients who have TB to watch them take

their medications. This is called directly observed therapy (DOT). DOT helps the patient complete treatment in the least

amount of time.

Q. What is COVID-19?

COVID-19 is the disease caused by a new corona virus called SARS-CoV-2. WHO first learned of this new virus on

31 December 2019, following a report of a cluster of cases of 'viral pneumonia' in Wuhan, People's Republic of China.

Similarities and difference between Tuberculosis and COVID19

Facts related to COVID-19 & Tuberculosis

Q. Do COVID 19 And Tuberculosis spread in the same way?

While both tuberculosis (TB) and COVID-19 spread by close contact between people the exact mode of transmission

differs, explaining some differences in infection control measures to mitigate the two conditions.

TB bacilli remain suspended in the air in droplet nuclei for several hours after a TB patient coughs, sneezes, shouts, or

sings, and people who inhale them can get infected. The size of these droplet nuclei is a key factor determining their

infectiousness. Their concentration decreases with ventilation and exposure to direct sunlight.

COVID-19 transmission has primarily been attributed to the direct breathing of droplets expelled by someone with

COVID-19 (people may be infectious before clinical features become apparent). Droplets produced by coughing, sneezing,

exhaling and speaking may land on objects and surfaces, and contacts can get infected with COVID-19 by touching them

and then touching their eyes, nose or mouth . Handwashing, in addition to respiratory precautions, are thus important in

the control of COVID-19.

Q. Is COVID19 leading to flare-up of Tuberculosis?

While there is no conclusive evidence yet to establish that COVID-19 can directly trigger an increase in tuberculosis,

TB cases have indeed been going up, especially in the post-COVID phase.

Mostly because of delayed diagnosis due to lock down during the pandemic. However, emerging evidence from various

studies suggest that COVID and TB dysregulates the immune responses and the possibility of COVID-19 activating

dormant/latent TB infection or triggering re-infections in those recovering from COVID is real.

Latent TB can get activated into active TB when body's immune system is week. Irrational use of steroids can lead to

immunosuppression, which heightens the risk of reactivation of dormant infections such as latent TB or opportunistic

secondary infections like mucormycosis.

Q. Are people with tuberculosis likely to be at increased risk of COVID 19 infection, illness and death?

While experience on COVID-19 infection in tuberculosis (TB) patients remains limited, it is anticipated that people ill

with both TB and COVID-19 may have poorer treatment outcomes, especially if TB treatment is interrupted.

Older age, diabetes and chronic obstructive pulmonary disease (COPD) are linked with more severe COVID-19 and are

also risk factors for poor outcomes in TB.

TB patients should take precautions as advised by health authorities to be protected from COVID-19 and continue

their TB treatment as prescribed.

People ill with COVID-19 and TB show similar symptoms such as cough, fever and difficulty breathing. Both diseases

attack primarily the lungs and although both biological agents transmit mainly via close contact, the incubation period from

exposure to disease in TB is longer, often with a slow onset.

Q. Is tuberculosis treatment differs in patients who have both TB and COVID 19?

In most cases tuberculosis (TB) treatment is not different in people with or without COVID-19 infection. Experience

on joint management of both COVID-19 infection and TB remains limited. However, suspension of TB treatment in

COVID-19 patients should be exceptional. TB preventive treatment, treatment for drug-susceptible or drug-resistant TB

disease should continue uninterrupted to safeguard the patient's health, reduce transmission and prevent the development

of drug-resistance.

While treatment trials are ongoing, no medication is currently recommended for COVID-19 and therefore no cautions

on drug-drug interactions are indicated at present. TB patients on treatment should nonetheless be asked if they are taking

any medicines, including traditional cures, that may interact with their medication.

Effective treatments to prevent TB and to treat active TB have been scaled up and are in use worldwide. The risk of

death in TB patients approaches 50% if left untreated and may be higher in the elderly or in the presence of comorbidity.

It is critical that TB services are not disrupted during the COVID-19 response.

Prevention:

Measures must be implemented to limit transmission of TB and COVID-19 in congregate settings and health care

facilities. Administrative, environmental and personal protection measures apply to both (e.g. basic infection prevention

and control, cough etiquette, patient triage).

Provision of TB preventive treatment should be maintained as much as possible.

Measures to be taken by people with TB to reduce their risk for COVID-19

Social distancing with "Quarantine": Remain at home and avoid contact with people as much as possible.

Wash hands frequently with soap and water.

Don't touch face, nose, eyes with unwashed hands.

Avoid close contact with those who are unwell.

Strictly adhere to your TB treatment.

Avoid hospital visits as much as possible and keep in touch with your doctor/nurse/health facility by phone.

Use masks, and take extra caution to maintain hygiene such as - disinfection of hands, used surfaces, proper disposal

of used tissues, etc.
