CORRIGENDUM OF TENDER NOTICESealed tenders are invited by District Health Society Shopian for Empanelment of tour/Vehicle operations in the District (For Mobile Block Medical Teams under RBSK of J&K National Rural Health Mission for the concerned blocks; Block Shopian and Block Keller) from interested parties.The interested parties are requested to submit the sealed tenders affixed with Rs 5.00 revenue stamp (clearly marked on the envelope) at NHM unit for four Vehicles, CMO office Shopian. The sealed tenders must reach this office on or before 30th of March 2016 & will be opened in presence of bidders on 1st April 2016 at 11:30 AM in the office chambers of chairman of District Health Society Shopian. The acceptance and finalization of tenders will be subject to following conditions.TERMS & CONDITIONS:1. The Vehicle must not be below the model of 2010 and in excellent running condition. (Scorpio,

Tata Sumo Tavera etc.).2. The vehicle shall be hired for the financial Year 2016-17 as required by the District/block offices

of District Rural Health Society Shopian on the approved rates.3. The vehicle shall entirely be at the disposal of block Mobile Medical team/Teams (RBSK) and

in no case shall carry passengers during the period of agreement with the department. If at any time, compliant of this kind is received against the vehicle hired, strict action will be initiated which may be cancellation of registration of Vehicle or termination of the contract.

4. The parties may quote rates for hiring on monthly basis. Minimum running of 65 KMS per day shall be included in the quote and per KM rate shall be as per Guidelines for extra mileage. The movement shall be in any part/area within the District.

5. The rates shall be applicable for financial Year 2016-17 i.e., up to ending March 2017.6. The tender should accompany CDR of Rs 2000/- pledged to Vice Chairman District Rural Health

Society Shopian, which shall be released to successful tenderer after completion of rate contract i.e.; 5 months after finalization of rates.

7. The payment for use of Vehicle shall be released on production of bill on monthly basis along with copy of log book duly certified by the touring officer and concerned BMO.

8. The vehicles should be made available as per requirement within one hour as and when required.9. Vehicles have to attend all gazetted holidays & hartal days except Sundays.10. The selected Tenderer/s should bear the cost of full vehicle branding of the vehicles as per

guidelines of RBSK at their own level. No financial assistance will be provided.11. The details of documentation and other necessary conditions are available at District
