    Weekly Evaluation and Reflection

    Apply basic leadership skills in the care of families.

    I provided positive and constructive feedback on aspects of performance and

    explained the purpose and desired outcomes of the tasks and the time frame in

    which the task was to be completed. I assisted the primary nurse in managing

    the patient’s care. I provided leadership in the modication of client care and

    organizational issues. I introduced myself and informed the patient and her

    spouse what I will be doing throughout the day and also inquired if there was

    something that they needed or wanted to accomplish for the day and prior to

    discharge. I educated the patient on breastfeeding techniques. he patient was

    very receptive to learning the techniques and showed understanding by

    demonstrating and verbalizing understanding. I also stressed the importance of

    the need for follow up care with the patient’s primary care physician to address

    the patient’s diabetes management. I really en!oyed working in the post"partum

    section. #ortunately$ I was teamed up with a great nurse that was eager and

    willing to teach me. %he was very busy and I tried my best to be as helpful as

    possible within my scope as a student nurse. I had the opportunity to assist with

    the care of four patients’. hree of the patients were post"partum and one

    patient was &'( ) for a hysterectomy. I also assisted the charge nurse with an

    admission. I learned so much in one day and I am very grateful for all of the

    experiences and learning I acquired from my clinical instructor and primary

    nurse that will help me be a great nurse.

    (emonstrate therapeutic communication skills in interactions and

    relationships with families$ individuals$ and other members of the health care team

    with attention to the identication and correction of non"therapeutic communication

    techniques.I e*ectively utilized verbal and nonverbal approaches for e*ective

    therapeutic communication in non"complicated client situations. I seeked

    verbal collaboration with other health care team members in providing care$

    and I assessed the client’s learning needs$ learning styles$ and variable

    impacting the teaching"learning process and used appropriate teaching

    strategies and materials. I spontaneously incorporated health care knowledge

    and education into routine communication and created individualized health

    teaching plans. I was able to identify the patient’s need for follow up care

    with her &+&. ,pon questioning my patient on her gestational diabetes$ she

    began to explain that she had gestational diabetes with the two prior

    pregnancies and never seeked care for herself after giving birth. %he also

    mentioned that she was prescribed metformin in -/ and only used it for a

    month$ then never went to her follow up appointment. I used therapeutic

    communication to nd out the reasons for her non"compliance. I then began

    to educate her on the risks of uncontrolled diabetes and also about ways to

    improve her blood sugar levels 0ex. exercise$ carb counting1. %he thanked me

    for the information and stated that she will follow up with her &+& after

    discharge. I was very grateful to have the opportunity to educate my patient

    who was now a mother of three$ who was willing to learn and wants to better

    her health for herself and for her children. I also gave my patient a back and

    neck rub$ then showed her spouse how to do it. 2y patient and her spouse

    verbalized gratitude and encouraged me to continue doing what I was doing

    and said that I was an awesome student nurse and that I will be an amazing

    nurse$ and it was a pleasure to meet me.
