Dear Spine Care profeSSionalS:aloha! On behalf of the North American Spine Society (NASS) and the Japanese Society for Spine Surgery and Related Research (JSSR), we invite you to attend Spine Across the Sea, July 29-August 2, 2012, on the beautiful island of Kauai, Hawaii. This meeting will provide spine care professionals an opportunity to share current information and concepts relating to spinal problems. While most will come from the United States and Japan, participants from Asia, the Americas and Europe are expected to attend.

Spine Across the Sea will be held at one of Kauai’s premier resort hotels, the Kauai Marriott Resort on Kalapaki Beach. Unique among hotels in Kauai, the resort sits on Kalapaki Bay and offers a wide variety of recreational activities, a superior beach experience and 18 holes of Jack Nicklaus championship golf at the adjacent Kauai Lagoons Golf Club.

This unique meeting is held every three years and offers a wealth of benefits: scientific paper presentations, electronic posters and symposia; a technical exhibition; plus breakfasts and networking breaks. The Wine and Cheese Opening Reception on Sunday, July 29 is a great opportunity to meet with colleagues.

Kauai, haWaii Kauai is known as the “Garden Isle” and is considered by most people to be the most beautiful island in Hawaii. It is the oldest of the Hawaiian Islands. Mount Waialeale’s lush slopes are home to the wettest tropical rain forest in the world. This in return gives the island some of the most incredible waterfalls.

The Napali coast on the north shore has majestic cliffs and rugged terrain that make it accessible only by boat or helicopter. You can hike if you’re very adventurous. The Wiamea Canyon is on the south side of the island which is often referred to as the Grand Canyon of the Pacific.

Three quarters of the island is covered with beaches for any type of water sports or just relaxing in the warm tropical sun. Kauai is the only Hawaiian Island with navigable rivers to get up in the mountains and explore and swim in the pools and waterfalls. With all the spectacular waterfalls, beautiful beaches, incredible tropical flowers and Aloha spirit, you will understand why Kauai is known as the Garden Isle.

Weather Step off the plane and feel the warm embrace of Kauai with average July/August temperatures ranging between 75 and 84 degrees Fahrenheit. Ocean temperatures also are perfect, ranging from 71 to 81 degrees Fahrenheit year round.

Visitors can play beautiful resort courses or affordable independent courses, where the rough is lined with lava rocks while mountain ranges and seascapes frame the greens.

The Kauai Lagoons Golf Club at the Kauai Marriott Resort reopened in May 2011 with three new holes—one a new ocean hole—and the longest stretch of continuous ocean holes of any course in Hawaii. Jack Nicklaus originally designed the course, and he again headed up the renovation work. The result is a course that is even more breathtaking and exciting.

airlineS anD flightS Delta Airlines (800-221-1212) has several flights a day to Lihue and several other Hawaiian destinations from Atlanta, Cincinnati, Salt Lake City, and Los Angeles.

United Airlines (800-241-6522) flies to Honolulu from several cities and directly to Lihue (LIH) from both San Francisco and Los Angeles. Flying directly to Kauai (LIH) saves you from having to stop in Honolulu.

Hawaiian Airlines (800-367-5320) is one of the two main inter-island carriers on Hawaii. Hawaiian Airlines offers non-stop service to Honolulu from several mainland cities including: Seattle, Portland, Sacramento, San Francisco, Los Angeles, San Diego, Las Vegas, and Phoenix.

go! Airlines (888-IFLYGO2) flies inter-island flights between Honolulu, Oahu (HNL), Hilo, Big Island (ITO), Kona, Big Island (KOA), Kahului, Maui (OGG), and Lihue, Kauai (LIH).

Lihue Airport (LIH), pronounced lee-HU-ay, handles virtually all arrivals on Kauai. The terminal in Lihue is small, modern, and efficient. Airlines that fly directly to Kauai include Delta, Hawaiian, United, US Airways (from Pheonix), and Alaska Airlines (from Seattle). Because of the limited number of airlines servicing Kauai, many people fly other airlines to Honolulu and then use an inter-island carrier to travel to Kauai.

Syposium: Minimally Invasive Lateral Access to the Lumbar SpineModerator: John C. Liu, MDMinimally invasive lateral access to the lumbar spine has gained tremendous popularity. The ability to reach the anterior lumbar spine to perform interbody fusions has allowed surgeons to treat patients with a variety of spine diseases including degenerative disc disease, pseudoarthrosis, adjacent level degeneration and spine deformity. This session covers the history, indications, surgical anatomy and techniques for safely navigating through minimal access to the lateral spine. A number of case exam-ples also are discussed.

Deformity of the cervical spine can arise from congenital, traumatic, infectious and inflammatory causes. Often these deformities do not come to surgical attention until they are quite advanced.

This symposium covers the natural history, work-up, surgical techniques and outcomes of deformities of the cervical spine, with an emphasis on the use of osteotomy techniques for the correction of sagittal plane deformities of the cervical spine and cervicothoracic junction.

10:26–11:36 a.m.Symposium: Neurological Complications Associated with Spinal Interventions: Patterns of Injury, Anatomy and AvoidanceModerator: Way Yin, MDThe incidence of serious neurological complications associated with spinal interventions has increased dramatically over the last decade, likely associated with the popularity of injections for the diagnosis and treatment of spine pain. Most complications remain sub judice.

Accordingly, reported incidences and patterns of injury may be underestimated in the scientific literature. Several patterns of severe neurological injury can now be discerned, and may be categorized into injuries associated with direct trauma to the spinal cord and secondary vascular events.

This symposium combines case studies with relevant anatomical considerations and technical considerations relevant to modern spinal interventions to minimize the occurrence of these complications.Upon completion of this session, participants will gain strategies to:• Describe basic anatomical relationships of

the spinal nerves and vessels.• Explain how knowledge of regional anatomy

may reduce the likelihood of complications associated with spinal injections.

9:37–10:57 a.m.Symposium: Kyphotic Deformity with Compression FractureModerator: Mamoru Kawakami, MD, PhDIt is important to prevent kyphotic deformity after vertebral compression fracture, which may result in residual back pain and neurological deficits due to compression of the spinal cord and cauda equina.

The sequelae can be a significant burden for patients, impairing physical function and quality of life. Surgical intervention should be considered to restore spinal alignment and neurological function without pain in patients with kyphotic deformity with vertebral compression fracture. This symposium focuses on treating the kyphotic deformity with compression fracture.

Upon completion of this session, participants will gain strategies to:• Assess how much kyphotic deformity

should be corrected.• Review the surgical indications to determine

if an anterior or posterior approach is most beneficial.

• Discuss how patients with osteoporosis are best treated for kyphotic deformity.

DiSCloSureNASS Financial Conflict of Interest Disclosure PolicyNASS recognizes that professional relationships with industry are essential for development of new spine technologies and medical advancement. According to the NASS Ethics Committee, which authored the disclosure policy, “the goal is to create an environment of scientific validity, in which learners can accurately judge whether the information they receive is objective and unbiased, and to be sure that our members are current and forthright in their dealings with one another and with their colleagues and patients.” The establishment of uniform disclosure requirements frees individuals from having to decide which relationships might influence his or her decision-making and which are irrelevant; transparent disclosure allows the audience to participate in the interpretation of the significance.
