Criteria for Review: ContentContent Review:
The Restructuring Plan meets ESEA requirements for “alternative governance,” namely “fundamental reforms” with “substantial promise” of improving student achievement. The restructuring plan will be reviewed based on the extent to which it meets these criteria .
Criteria Standard

-- Accurately identifies the subject area(s) for which the school did not make AYP for two consecutive years
Accurate for April 2012 AND April 2013 AYP release
-- Describes a credible school or district Teacher Mentoring program;

Action Plans for Priority Objectives
-- Identifies specific changes in organizational practices to focus on effective leadership and distributed responsibility for student results
For each objective: Assessment describes current status; Plan describes future vision when fully implemented
-- Describes reforms in instructional practices that offer ‘substantial promise’ of improved student achievement
A significant proportion of Objectives in the plan describe refinements in instructional practices
-- Incorporates activities that target overall student improvement along with specific actions that will lead to improved achievement of students not proficient in the area(s) in which the school is identified

Where? In the 2013 NH Restructuring Plan Instructions and Forms doc sent by email to all 2012 SINI 4 school contacts; also posted on the NH DOE website:

What? Contact information for Principal and (optional) additional contact.
How? Copy full set of FORMS from the 2013 Instructions and Forms document. SAVE as a *.doc (WORD) document with a memorable name on your own computer. Fill out requested information.

What? Superintendent’s assurances that the school will/ has complied with all ESEA requirements for the improvement plan and that the LEA will provide adequate leadership and support to implement the activities in the plan.
How? Extract and print the MOU pages from the Instructions and Forms document. Have the Superintendent sign the MOU. (Or, insert an electronic signature on the WORD document.) SAVE as part of your Restructuring Plan master/ host document.

Restructuring Plan master document to create the completed restructuring packet.
Which Steps to Success documents? -- NH SINI Goals and Objectives form-- Indicator Checklist Report for each of the priority objectives/ indicators
How? Retrieve as WORD docs; SAVE to your computer; copy and paste into the “host”/ “master” document.

The Restructuring Plan Packet: NH SINI Goals and Objectives Form
Where? Steps to Success Dashboard > Forms to Complete How? Type in each of your Goal statements and the associated
priority (i.e., ‘main idea’) objectives [Note: either type in the priority objectives in the space indicated OR copy and paste from the full list of Rapid Improvement Indicators located on the DOCS and LINKS tab.]
SAVE the Goals and Objectives form on your own computer with a distinctive name
in a folder where you can find it again (!) When ready to compile your packet, open the host document.Open the Goals and Objectives form. In the G & O form, select ALL (CTRL A); copy all (CTRL C).Move your cursor to the end of the host document.Paste (CTRL V) the copied material at the end of the host doc.

Where? INDICATOR CHECKLIST REPORT in “Resources and Reports” menu on School Main Page
How? (1) Select Indicator Checklist Report(2) CHECK each of the Objectives for your GOAL 1(3) Select REPORT(4) EXPORT the REPORT for Goal 1 in WORD format. (5) REPEAT and EXPORT a separate Action Plan for each of the priority objectives for your Goal 2 and Goal 3.
