MISSION STATEMENT New Lake College will provide a learning environment that blends teaching strategies promoting adult learning theories such as professional mentoring, collaborative learning, student engagement, differentiated instruction and technology integration. New Lake will deliver healthcare education and training programs empowering self-directed learners in their pursuit of higher learning and professional growth.

General Description of Equipment and Facilities The campus is contained in a 2,024 square foot facility housing 2 classrooms/nursing care laboratories, 1 library/resource center, a faculty/advising room, and an admissions and student records room. It is located at 1300 Greenbrook Blvd. Suite 102 in Hanover Park, IL 60133.

The healthcare labs include 4 nursing care beds (stations) so instructors may lead students in demonstrating nursing skills. Both stations are also set up to provide supervised practice of nursing skills on simulation mannequins. Reference books are also located in the resource center.

ADMISSION POLICIES

Entrance Requirements

Basic Nursing Assistant Training Program (BNATP)

Individuals applying for admission into the program must comply with the following:

• be at least 18 years of age

• must have completed high school education or its equivalent

• pass the Test for Adult Basic Education (TABE) test administered during application

• submit a completed health-physical form that documents immunity for mumps, measles, varicella, diphtheria, tetanus, tuberculosis, and hepatitis B (or waiver)

• submit to the required criminal background check (with fingerprints, if necessary) and results must reflect no disqualifying convictions.

• be able to handle the job responsibilities and tasks assigned in the profession, specifically: 1. Perform a full-range of body motion including handling and lifting patients,

manual and finger dexterity, and eye-hand coordination. 2. Bend, reach, pull, push, stoop, and walk repeatedly throughout an eight (8) hour

period. 3. Lift and carry up to fifty (50) pounds. 4. Demonstrate visual and auditory acuity within normal range (with correction if

needed). 5. Maintain composure when subjected to high stress levels. 6. Adapt effectively to environments with high tension to ensure patient safety. 7. Adapt to irregular working hours. 8. Respond quickly and in an emotionally controlled manner in emergency situations. 9. Communicate in a rational and coherent manner both orally and in writing with

individuals of all professions and social levels.

Practical Nursing Diploma Training Program (PNDTP)

Individuals applying for admission into the program must comply with the following:

• Completed high school education or its equivalent (GED or college credits)

• Maintains good standing in the IDPH Health Worker Registry as a C.N.A.

• Achieves a minimum proficiency of at least 50% in overall performance with ATI’s Test of Essential Academic Skills. Acceptable profiency is based on ATI’s Research Paper "Predicting PN Student Attrition using the TEAS V" n.d.

▪ If granted, a Retake can be taken within 14 days in from the date of testing after satisfying a TEAS review (face-to-face)

• Completed a background check with no criminal convictions.

• Completed a recent (within the past 6 months) physical and medical examination with good health

• Maintains current Americal Heart Association CPR certification.

PN Prerequisites Program Individuals applying to take courses under the PN Prerequisite program must satisfy the following requirements:

• Completed high school education or its equivalent (GED or college credits)

• Completed and achieved at least 10th grade level of proficiency in the Test of Adult Basic Education (TABE) Health Form in all areas including:

o Reading o Mathematics Computation o Applied Math o Language

Medical Assistant Diploma Training Program (MADTP)

Applicants for the Medical Assisting Diploma Program must complete the following:

• Submit proof of completion of secondary education or its equivalent (GED certification, official transcript of records from an accredited higher education institution in the US.)

• Complete and pass the Test for Adult Basic Education (TABE) Health Form with a proficiency score equivalent to at least a 10th grade level in all areas including:

o Reading o Mathematics Computation o Applied Math o Language

• Submit proof of good medical and physical health examination

• Return signed IDPH Authorization for Healthcare Worker Background Check. Results of check must not include any felony conviction.

Phlebotomy Technician Training Program (PTTP)

Individuals applying for admission into the program must comply with the following:

• be at least 18 years of age

• must have completed high school education or its equivalent

• pass the Test for Adult Basic Education (TABE) test administered during application

• submit a completed health-physical form that documents immunity for mumps, measles, varicella, diphtheria, tetanus, tuberculosis, and hepatitis B (or waiver)

EKG Technician Training Program (ETTP)

Individuals applying for admission into the program must comply with the following:

• be at least 18 years of age

• must have completed high school education or its equivalent

• pass the Test for Adult Basic Education (TABE) test administered during application

• submit a completed health-physical form that documents immunity for mumps, measles, varicella, diphtheria, tetanus, tuberculosis, and hepatitis B (or waiver)

Credits for Previous Education or Training

There are no credits to consider for the BNATP, MADTP, PTTP and ETTP. PN Prerequisite courses can be credited towards the requirements of the PNDTP. Anatomy and Physiology course(s) should have been taken within 5 years from admission to the PN Program.

Refund/Cancellation/Withdrawal

Refund Policy.

Student registration, identification card, kit, uniform, subscription fees for online learning and assessment products, and used/open books are not refundable. Laboratory fees, technology fee are refunded ratably based on the percentage of hours attended over the total program hours. Under the law, student is obligated among others, to pay the full amount due and to obtain under certain conditions a partial refund of the finance charges. Any student applying for a program that is discontinued by the school shall receive a complete refund of all fees and/or tuition paid. It is the policy of New Lake College (NLC) to issue refunds of tuition and fees in a prompt manner. As a matter of courtesy, students should give written notification to NLC (in person or by registered mail) of their intention to withdraw from a program. However, NLC does not require written notification of withdrawal as a condition for making refunds. Should the student’s enrollment be terminated or should the student withdraw for any reason, all refunds will be made according to the following refund schedule:

Cancellation Policy.

The student has the right to cancel the initial enrollment agreement until midnight of the 5th business day after the student has been admitted. If the right to cancel is not given to any prospective student at the time the agreement is signed, then the student has the right to cancel the agreement at any time and receive a refund on all monies paid to date with 10 days of cancellation. Cancellation should be submitted to the authorized official of the school in writing.

Withdrawal Procedure.

If no notification of withdrawal is received, and a student has had an unexplained absence of more than fifteen (15) consecutive class days, NLC shall consider the student to have withdrawn from the program. In all cases, the date of withdrawal shall be the last day of attendance. Refunds shall be made within 30 days of the last day of the attendance if written notification has been provided to the institution by the student; otherwise, refunds shall be made within 30 days from the date the institution terminates the student or determines that the student has withdrawn. Determination that a student has withdrawn shall be made within 30 days of the last day of attendance NLC shall provide written acknowledgment of a student’s notification of withdrawal within fifteen (15) calendar days of the postmark date of the notification of withdrawal. In all instances, refunds shall be based on and computed from the last day of attendance. Any unused portion of books shall be refunded. Lab fees and technology fees will be prorated on the number of hours attended by the student. However, fees involving registration and qualifying exams, nursing kits and subscription to learning resources (ATI) are non-refundable.

Standards for Satisfactory Academic Progress

All students must meet the following established minimum standards of achievement in order to successfully complete a course, progress through and complete the entire program.

Minimum Grade Requirement upon Graduation.

A student must have attained a GPA of 2.0 or better in all courses of the training program in order to be considered for graduation. See eligibility for graduation and program completion for complete requirements.

Monitoring of Student Progress.

The Academic Committee headed by the Program Director is responsible for monitoring student progress at the end of each course and cumulatively at the end of each quarter. The committee will review academic records of students who fall below the minimum standards. The student will be called for an academic review and advising prior to being allowed to progress through the program. A student’s academic performance will also be monitored while progressing through individual courses. A student must attain a grade of “C” or better in the Midterm and Final Examinations. If a student falls below the passing grade in either examinations, he/she will be called for Academic Advising where areas that call for improvement and/or further concentration will be identified. The advisor may either recommend the student for individual focused review or faculty/peer tutorial. The student must demonstrate successful completion of either interventions before being allowed to take the succeeding examination. A student who has a final grade of “INC” and does not successfully complete all coursework in the seven-day time period, or as otherwise specified, receives a course grade calculated based on a grade of “0” for missing work. Any action that may result from a grade calculated on this basis (such as Academic Probation or Academic Dismissal) is taken immediately. When an “INC” grade is removed and a final earned letter grade is assigned, the GPA is recalculated based on the final grade earned.

Academic Probation.

A student who is unable to maintain a 2.0 grade point average (GPA) in each course at end of a quarter will be placed on academic probation for the next quarter. Tutorial and/or focused review may be required of the student in an effort to reach the expected level of performance. The student must be able to commit extra time, if necessary, for scheduled academic advising, focused reviews and/or tutorials. The student has a right to appeal the decision to the Program Director. The appeal should be submitted in writing within one week of the committee’s decision.

Academic Dismissal.

At the end of probation, if the student has not brought his/her grades to meet the school’s satisfactory academic progress policy, then the student shall be dismissed from the program, depending on the recommendation of the academic review committee. The student has a right to appeal the decision to the Program Director. The appeal should be submitted in writing within one week of the committee’s decision.

Maximum Time Frame.

A student must complete the program in no more than 150% of the normal program length. For example, if the Practical Nursing program takes four quarters to complete the entire 1000 clock hour requirements, a student is only allowed up to an additional two quarters to complete the program and must have attained all curricular requirements for graduation by then.

Repeated Courses.

If a student fails to earn a passing grade in a required course, he/she must repeat that course. Students may repeat a course only once and only as scheduling permits. Grades of “NG” and “W” do not count in the repeated course limitation. The grade received on the most recent repeat becomes the final grade and replaces all other grades for that course in the GPA calculation. All other attempts count in the calculation as attempted but not earned credits. Students who fail a required course twice are dismissed from the program.

Graduation Requirements

Basic Nursing Assistant Training Program

To successfully complete the program and be eligible to take the State of Illinois Competency exam for Nursing Assistants, the student must fulfill ALL of the following requirements: • Earn a final grade of 75% (2.0 in Quality Points) or better in the theory and laboratory part

of the program • Pass the Manual Skills Assessment part of the program (IDPH mandatory 21 skills) • Complete 96 hours of Theory and 40 hours of Clinical instruction

In order to be placed on the IDPH Health Care Worker Registry as a Certified Nursing Assistant in the State of Illinois, the student must fulfill ALL of the following requirements: • Pass/Complete the Basic Nursing Assistant Training Program • Submit a Fingerprint-based Health Care Worker Criminal History Records Check • Pass the IL Nurse Aide Competency Exam (Valid Social Security Number required)

Practical Nursing Diploma Training Program

To successfully complete the program and be eligible to take the Nursing Commission Licensure Examination for Practical Nursing (NCLEX-PN), the student must fulfill ALL of the following requirements: • Earn a final grade of 75% (2.0 in Quality Points) or better in each of the theory and

laboratory nursing courses of the program. • Pass the Manual Skills Assessment part of the program • Complete 500 hours of Theory and 450 hours of Laboratory and Clinical instruction • Earn a 90% predictability of passing the NCLEX-PN, based on the results of the ATI

Comprehensive NCLEX Predictor Exam.

In order to acquire a license as a Practical Nurse (LPN), the student must fulfill ALL of the requirements:

• Pass/Complete all requirements of the Practical Nursing Diploma Training Program • Submit a Fingerprint-based Health Care Worker Criminal History Records Check • Pass the NCLEX-PN

STUDENT GRIEVANCE A student who feels that he/she has been treated unfairly has the right to question this treatment and receive prompt resolution of areas of concern. The processing at each step cannot exceed 5 school days; however the time maybe extended by agreement of both parties or by extenuating circumstances as decided by the program director. NLC’s Grievance Procedure must be followed as specified:

1. To initiate the formal grievance procedure, the student must document his/her complaints on the Student Discrimination Grievance Form or Student Grievance Form and discuss it with the instructor within 5 days after the occurrence of the incident or decision in question has taken place. The student and the instructor should attempt to resolve the problem through proper communication process.

2. If the question/s still remain unresolved following the conference with the instructor, the

issue should be referred to the Program Coordinator. The program director will meet

4. If the student is not satisfied with the decision of the program director,, the student may

request in writing for a special meeting with the NLCs Appeals Committee, chaired by the Administrator. The decision of the Appeals Committee will be final and there will be no further appeal, thereafter.

5. If student is still not satisfied with the decision of the administration, the student may

bring her complaint or grievance to the Illinois Board of Higher Education (IBHE) and should refer to the Enrollment Agreement for details on whom and where to address their complaint. Information for the Illinois Board of Higher Education is detailed below

Basic Nursing Assistant Training Program

Program Description

136 Clock Hours (96 Hours Theory and 40 Hours Clinical) The Basic Nursing Assistant Training Program is an 8-10 week program that prepares students into their roles and responsibilities as nursing assistants in long term care facilities and other health care settings. Through lectures, discussions, supervised laboratory and actual clinical experience, students are provided with the knowledge and skills needed to provide basic nursing care for patients in various health care facilities. Students are eligible to take the IL Nurse Aide Written Competency Exam after successful completion of the program. The BNATP is approved by the Illinois Department of Public Health with Program Code NA-7056.

Program Objectives

At the end of the training program, students should be able to:

• Perform basic nursing skills for clients in a variety of settings.

• Safely and effectively perform personal care skills.

• Recognize the psychological, emotional, physical and spiritual needs of clients.

Practical Nursing Diploma Training Program

Program Description

950 Clock Hours (500 Hours Theory and 450 Hours Laboratory and Clinical Rotation) The Practical Nursing Diploma Training Program is a 950-clock hour program which consists of nursing and other related courses essential for the practice of practical nursing. It requires completion of applied general education courses listed below. Content from the science and related courses are integrated into the core nursing courses. The program is sequential and competency-based, such that successful achievement of objectives pertaining to both knowledge content and applied laboratory and essential clinical skills in each nursing course must be demonstrated before a student can proceed to the next one. The scope and sequence of courses will enable the student to master general nursing principles and skills that are the foundation for later and more content-specific courses. It is likewise designed to give the students the opportunity to gain competence through acquisition of knowledge and skills necessary to begin employment in the healthcare profession. The program also consists of courses that provide coordinated theory, laboratory and clinical instruction. After completing the program, students will be ready to challenge the National Council Licensure Examination (NCLEX). The PNDTP approved by the Illinois Department of Finance and Professional Regulation (IDFPR) and is assigned a Program Code US-049110500.

Program Objectives

NLC intends to contribute to the increasing demand of nursing workforce with highly competent, compassionate and well-educated nurses. NLC can achieve this goal with the following objectives:

Program Description

The PN Prerequisite program is a 5-course program designed to prepare the student for the Practical Nursing Diploma Training Program (PNDTP) program. The program is intended to measure the student’s basic skills in communication, calculations relating to percentages, decimals, ratios and proportions, and human behavior. All basic knowledge is relevant to helping the student comprehend nursing concepts and principles. The program also provides adequate knowledge needed on human anatomy to prepare them in learning about common diseases and illnesses so they may provide better nursing interventions and care. Students will improve their skills in determining key ideas and details, integrate ideas into knowledge, produce clear and coherent writing to develop and organize knowledge, participate in effective discussions and collaborations, present information in sequential and organized method appropriate to the audience. They will interpret and write mathematical expressions in equivalent forms to solve problems. Students will develop and strengthen their knowledge of human growth and development and recognize the functions of the body organ systems.

Program Description

The Medical Assisting Diploma Training Program (MADTP) will prepare the students, under the supervision of physicians, to provide medical office administrative services and perform clinical duties including patient intake and care, routine diagnostic and recording procedures, pre-examination and examination assistance, and the administration of medications and first aid. Includes instruction in basic anatomy and physiology; medical terminology; medical law and ethics; patient psychology and communications; medical office procedures; and clinical diagnostic, examination, testing, and treatment procedures. The program will consist of four courses with total contact hours of 800 for the program. At the conclusion of the program the student will be eligible to challenge the certification examinations for; Medical clinical Assistant, Medical Administrative Assistant, Certified Phlebotomy Technician, Certified EKG Technician, and Certified Electronic Health Record Specialist.

Program Objectives

The program intends to educate the student to be proficient in the following occupational tasks and responsibilities required of a medical assistant (O*NET Online, n.d.). At the end of the training program, students should be able to:

• Record patients' medical history, vital statistics, or information such as test results in medical records.

• Prepare treatment rooms for patient examinations, keeping the rooms neat and clean.

• Interview patients to obtain medical information and measure their vital signs, weight, and height.

• Show patients to examination rooms and prepare them for the physician.

• Prepare and administer medications as directed by a physician.

• Collect blood, tissue, or other laboratory specimens, log the specimens, and prepare them for testing

• Authorize drug refills and provide prescription information to pharmacies.

• Explain treatment procedures, medications, diets, or physicians' instructions to patients.

Program Description

The program is a 50-hour course designed to provide the student with the necessary theory and skills to obtain employment as a phlebotomist. The course will cover phlebotomy basics, specimen collection, specimen handling, and the student will be tested on the necessary competencies required by the National Health Career Association (NHA). The course will be taught using lecture, demonstration and return demonstration, and clinical experience. The course will prepare the student to sit for the certification examination from NHA – Certified Phlebotomy Technician (CPT).

Program Objectives

The program graduate will be competent in performing the following:

• State five important job skills a phlebotomist must possess.

• Understand the need for confidentiality and HIPPA regulations.

• Demonstrate the procedures necessary for infection control.

• Define the basic medical terminology applying to phlebotomy.

• Understand the major body functions and the diseases associated with the common laboratory tests.

• Describe the components of whole blood and the function of the main components.

• Explain when a syringe system or a winged infusion set is used in blood collection.

• State the correct order in which various types of tubes should be collected.

The course is designed to provide the student with the necessary theory and skills to obtain employment as a EKG Technician. The course (20 lecture hours and 30 laboratory hours) will cover EKG basics, identification of normal plus abnormal cardiac rhythms, determine actual dysrhythmias from artifacts, know which rhythms indicate and emergency, have an introduction to the Twelve Lead EKG and the student will be tested on the necessary competencies required by the National Health Career Association. The course will be taught using lecture, demonstration and return demonstration, and clinical experience. The course will prepare the student to sit for the certification examination.
