First Week of DutyAfternoon Group

- Reiterate policies, rules and regulations of the hospital regarding affiliation and affiliates - Orientation on the physical set-up, policies, rules and regulations of the assigned Pavilion- Discuss the role of each staff members in the area

- Psychiatrist- Psychologist- Nurses- Nursing Aides- Social Workers

- Family / Significant others- Observation in the ward activities of the personnel with patients

- Discuss the responsibility of the nursing students in dealing with clients’ view and perceptions and remember the following techniques:

- focus on his/her safety and security- focus also on the safety, security and feelings of the client - accept client as he/she is- be honest and consistent- encourage clients to express more fully

- reflect, clarify and validate client’s statement - encourage hope and never with false assurance - acknowledge verbal and non-verbal behavior - do not provide a response that implies that the client is unworthy - never be defensive and avoid asking why - attempt to establish good relationship at all times - Discuss the Rights of the Patients

- Accurate diagnosis- Confidentiality- Quality care and holistic treatment- Appropriate care according to his/her cultural background and

- Discuss the grading system, the requirements of the related learning experience, flow of the duty for the entire 2 weeks of stay in the area- Provide assignments to students for the whole rotation.

- Self Awareness Activity- Chart Reading

- legal document- strict confidentiality- safeguard and care of the thick and loose chart- return after use

1:30pm – 3:30pm - Receiving of patient from the Staff Nurses and Nursing Aides for activity- Activity of Daily Living

- self care activities- encourage independence or with assistance- self esteem worth- improve function and health

- Hygiene and Grooming - Emphasizes on hand washing, wound dressing, care of the hair, care of the mouth, care of the skin, care of the nail

- Nurse – Patient Interaction (Orientation Phase)- Introduce self- Set therapeutic contract- Trust and rapport- Therapeutic Environment- Assess client’s strength and weakness- Help and/or encourage to communicate- Observe the clients during therapeutic communication on general appearance, motor behavior, client’s verbal communication, disturbances of client’s perception, thought process, mood and affect,

- Discussion of what transpired last meeting- Preparation of materials for routine procedures for the day

1:30pm – 3:30pm - Nurse – Patient Interaction (Working Phase)- Promote positive self-concept- Realistic goal setting- Organize support system- Verbalize feelings (encourage)- Implement action plan- Develop positive coping behaviors- Evaluate the results of plan of action

- Receiving of clients from the staff nurses and nursing aides for activity- Activity of Daily Living- Hygiene and Grooming

- Emphasizes on hand washing, wound dressing, care of the hair, care of the mouth, care of the skin, care of the nail

- Short Prayer- Introduction of the Nursing students and the patients- Calisthenics - Music Appreciation thru and Art Therapy - Song Therapy (at least one to two clients)- Bibliotherapy- BREAK

- Returning/endorsing of clients to the Staff nurses and Nursing Aides 3:30pm – 3:45pm - Nursing Student’s Break3:45pm - 5:00pm - Post Conference

1:30pm – 3:30pm - Nurse – Patient Interaction (Termination Phase)- Promote self care- Recognize increasing anxiety- Increase independence- Demonstrate emotional stability- Environmental support

- Receiving of clients from the staff nurses and nursing aides for activity- Activity of Daily Living- Hygiene and Grooming

- Emphasizes on hand washing, wound dressing, care of the hair, care of the mouth, care of the skin, care of the nail

- Short Prayer- Introduction of the Nursing students and the patients- Grand Socialization Activity

- Prepare parlor games /team or group games- BREAK- Opportunity by the nursing students’ to terminate nurse patient relationship leading the patients into no hopefuls and dependency

- Returning/endorsing of clients to the Staff nurses and Nursing Aides 3:30pm – 5:00pm - Post Conference

- After care of the materials used- Appreciation and evaluation of the activity
