Teacher Pilot Guidelines & Agreement Dear Teacher, Welcome to myPerspectives English Language Arts! We are happy you have chosen to pilot this brand new groundbreaking program. Below is an outline of expectations on the part of the pilot school/district and Pearson during the pilot. We believe that the following guidelines will help achieve a successful pilot experience. Product Availability Please note, myPerspectives is still in development and digital components will be rolling out between January and August 2016. Your Account General Manager will share with you availability dates in a separate document. During the product rollout time period, updates regarding content release will be published on www.eladashboard.com/myperspectives . We want to ensure you have the most current and accurate information, so please check this site often during your pilot. Mutual Expectations The pilot school/district will: Complete survey(s) or participate in discussions with Pearson, teachers and students, if asked. If mutually agreed upon, allow Pearson to take photos and/or record video while myPerspectives is in use in the classroom. Distribute to students the Model Release Form (at the end of this welcome letter). This form allows permission for potential use of photos/videos in Pearson’s marketing materials. NOTE: If your school is not going to be videotaped, this form is not necessary. Complete one unit (6 weeks) of instruction as intended/designed, using available lesson resources and technology. If the pilot term is shorter than 6 weeks, then teachers should implement as many lessons as possible within the given time period. Provide opportunities for Pearson to communicate with the onsite contact and pilot teachers, as agreed upon, during the course of the pilot. Agree to participate in a postpilot discussion with the Account General Manager, Literacy Specialist, and/or Product Management and Marketing. Pearson will: Provide all materials for teachers and students, including access to the digital course on Pearson Realize for the duration of the pilot. Page 1

Provide product orientation on agreed upon date. Communicate with ongoing updates related to product releases. Offer ongoing customer/technical support by the Account General Manager, Literacy Specialist, and/or Product Manager.

Collect anecdotal data from teachers and students. If needed and mutually agreed upon, provide photographers/videographers.

Pilot Duration Typically, pilots are set up for 6 or 12 months, but can be shorter or longer depending on the the agreed upon need. Please note that if your pilot extends beyond the initial dates, and you are using the digital courseware, your digital accounts need to be extended. Please be sure to confirm that this extension is done with your Account General Manager prior the expiration to avoid interruption of digital access. MARKETING / MODEL RELEASE FORM FOR PARENTS/GUARDIANS (Only Required if Mutually Agreed Upon as Part of the Pilot)

In consideration of value received, the receipt and sufficiency of which is acknowledged, I hereby give Pearson, Inc., (“Pearson”) the perpetual and worldwide, right and permission:

A. to use, copyright and /or publish videotape, film, audio and/or visual recordings, slides, photographs or other media known of later developed (hereinafter collectively referred to as “Videotapes”/ “Photographs”) of my child or child entrusted to my care, his/her image, likeness, voice, appearance and performance taken on __________________________, 2016 in Pearson’s product and related training, marketing, or advertising materials;

B. to use statements, or any portion thereof, made by my child or child entrusted to my care in conjunction with the Videotapes/Photographs in Pearson’s product and related materials;

C. to use or omit the use of the name of my child or child entrusted to my care in conjunction with the Videotapes/Photographs or statements;

D. to edit, mix, or duplicate, and to use or reuse the Videotapes/Photographs of my child or child entrusted to my care in whole or in part; and

E. to provide Videotapes/Photographs to clients and subcontractors for training, marketing, or advertising materials.

I waive any right that I may have to any ongoing compensation or royalties for use of the Videotapes/ Photographs or statement, and to inspect and/or approve any finished Videotapes/Photographs or statements made by my child or child entrusted to my care. I hereby release and discharge Pearson from any liability arising by virtue of any blurring, distortion, alternation, optical illusion or use in composite form, whether intentional or unintentional, that may occur or be produced in the taking of said pictures or in the processing or publication thereof. I further release and discharge Pearson from any liability arising out of the use or publication of my child’s or child entrusted to my care’s picture, name, or signed statement in the manner specified above. Pearson is released and discharged from any liability arising out of the use of publication of the Videotapes/Photographs or statements in the manner specified above. I acknowledge that I have read this agreement and understand what I am signing.
