The SEND Code of Practice 2014 Says

This plan has been developed under Part 3 of the Children and Families Act 2014

Section A The views, interests and aspirations of the child and their parents, or of the young person.

Section B The child or young person's special educational needs.

Section C The child or young person's health needs which are related to their SEN.

Section D The child or young person's social care needs which are related to their SEN or to a disability.

Section E The outcomes sought for the child or the young person. This should include outcomes for adult life.The EHC plan should also identify the arrangements for the setting of shorter term targets by the earlyyears provider, school, college or other education or training provider.

Section F The special educational provision required by the child or the young person.

Section G Any health provision reasonably required by the learning difficulties or disabilities which result in the child or young person having SEN. Where an Individual Health Care Plan is made for them, that planshould be included.

Section H1 Any social care provision which must be made for a child or young person under 18 resulting from section 2 of the Chronically Sick and Disabled Persons Act 1970.

Section H2 Any other social care provision reasonably required by the learning difficulties or disabilities which result in the child or young person having SEN. This will include any adult social care provision beingprovided to meet a young person's eligible needs (through a statutory care and support plan) under the Care Act 2014.

Section I The name and type of school, maintained nursery school, post-16 institution or other institution to be attended by the child or young person and the type of that institution (or, where the name of a schoolor other institution is not specified in the EHC plan, the type of school or other institution to be attended by the child or young person)

Section J Where there is a Personal Budget, the details of how the personal budget will support particular outcomes, the provision it will be used for including any flexibility in its usage and the arrangementsfor any direct payments for education, health and social care. The special educational needs and outcomes that are to be met by any direct payment must be specified.

Sarah is beginning to tolerate other children at school playingalongside her and can be part of a small group (no more than 5children) if supported by an adult.

Sarah experiences difficulties with noise and other peoplebeing too close which can result in behaviours such as hitting,kicking, screaming, leaving the room.

Sarah needs help in being able to be near other children. Adultsupport should be available to help Sarah with her emotions ifshe becomes cross or frustrated.

Where possible noise levels should be controlled for Sarah and a quiet area made for her so she can have 'time out'.

Sarah is delighted that she does not have to use her wheelchair in the classroom at all times and is hopeful that with further physiotherapy this can increase

Sarah is able to use her ipad in school and this enables her to engage in classroom lessons

The ipad is not always fully charged overnight so this can mean that by the end of the day Sarah is frustrated as she cannot join in lessons like she would want to.

Teaching staff need to make sure that Sarah's ipad is fully charged every day.

Cognition & Learning Sarah really enjoys attending theICT club that is held after school.

At times the taxi is late and this delays Sarah from getting home.

Social worker to ring the taxi firm and confirm the exact timing.

Social, Emotional& Mental Health

Sarah is usually happy and this can been seen by her wonderful smile.

Sarah has said that she can be upset it can take time for others to notice this.

Explore how Sarah can use her ipad that she has in school and at home to let people know how she is feeling.

The direct Payment carer is unreliable and often cancels.

The Education Setting. Providing 10 hours of Teaching Assistant Support per week for Sarah to aid her independence for sitting unaided in school and support the physiotherapy programme.The local authority provides funds
