Laboratory Policies and Procedures
1. REQUESTS OF MATERIALS
a. Materials for experiments must be requested three (3) days before the schedule by filing up
the Requisition Slip for Professors. Borrowers Slip for Students should be given one (1)day before the experiment.
Experiment Day Day Requested
Monday Wednesday of the other week
Tuesday Thursday of the other week
Wednesday Friday of the other week Thursday Monday of the week
Friday Tuesday of the week
Note: Requisition slip should be properly filled up by stating the materials needed and
quantity/amount/size.
b. The students should provide themselves with the following materials during there
experiments:
Cleaning/washing materials such as detergent, soap, scotch brite
Matches, labelling paper, masking tapes, marking pens, scissors
Rags
Personal hygiene materials such as rubbing alcohol, tissue paper, towels
c. Requests for minor experiments (meter stick, ruler, stop watch) can be requested one day
before the schedule. d. Materials may be requested by either the professor or students. These will be issued in
exchange of their ID.
e. All requested and returned materials must be recorded in the Request Logbook.f. Strictly no materials will be issued without prior request. The NO RESERVATION,
NO LABORATORY USE policy shall be implemented.
g. Any changes with the laboratory experiment must be properly communicated with the
Science laboratory technician one day before the new schedule.
h. If a student missed a laboratory experiment, there is absolutely NO make-up.i. Special Experiments/ Investigatory Projects:
Special experiments / investigatory projects must be approved by and performed
under the supervision of the professor.
Materials / chemicals to be used for the experiments other than the prescribed
experiments will be shouldered by the students.
j. Request form of non-science students should be countersigned by their professor.

2. Students are not allowed to enter or perform experiments in any laboratory room without the
professors supervision.
3. If the faculty is absent during the scheduled experiment, the requested experiment should beperformed the following meeting.
4. RETURNING OF MATERIALS
Glassware and equipment should be cleaned before returning to the stockroom.Physics materials should be properly and orderly return. (Return as Issued)
5. BREAKAGE /DAMAGE OF MATERIALS
The student is responsible for the issued materials. Any breakage / damage that he / she mayincur will be recorded immediately in the Student Accountabilities Logbook. Breakage/
damage of materials must be replaced the soonest possible time.
6. CLEARANCE
At the end of the semester, all students must secure a clearance from the laboratory andpresent this to the professor before taking the final examination.
7. Only authorized personnel is allowed in the Laboratory Stockroom. The laboratory office/
stockroom can only be opened by the laboratory personnel and authorized persons ONLY.
The laboratory stockroom is for laboratory personnel ONLY.
LABORATORY GUIDELINES1.CHEMICAL STORAGE:
a. Chemicals are properly stored according to its hazard classification. The safety data colorcode is used, thus:
RED for flammability
BLUE for health hazard
YELLOW for reactivity hazardWHITE for contact hazard
ORANGE for general chemical storage2. EQUIPMENT & APPARATUSAll equipment and apparatus must be stored properly and labelled accordingly.
3. CHEMICAL PREPARATION
a. Proper attire for the preparation of chemicals/ reagents must be observed.(laboratory gown/ apron goggles/ gas mask & gloves).
b. Use fume hood whenever dispensing chemicals that are toxic and have obnoxious odor.
c. Prepared reagents must be properly labelled.4. LABORATORY SUPPLIES, FIXED ASSETS & SERVICES
a. Definition of terms:
Supplies: refer to consumable materials such as office supplies, chemicals, microscope
slides and cover slips, etc. Fixed Assets: refer to non-consumable or fixed materials, such as tables, microscopes and
all electrical and/ or electronic equipments.
Services: refer to any service (carpentry, electrical, mechanical, etc.)b. Service Requisition Form is issued by the Auxiliary Office. Supplies and Fixed Asset
Requests are forwarded to the Deans Office for encoding in the Database System of the
Financial Affairs office.

PHYSICAL SCIENCE LABORATORYSAFETY RULES
1. Always wear your laboratory gown, goggles and gas mask when performing experiments.
2. Use reagents, water and gas economically.3. Avoid contamination of reagents. Observe proper dispensing of chemicals.
Spilled chemicals on the skin are washed off immediately with plenty of water.
4. Always clean your working area and make sure it is free from any chemical spill beforeleaving.
5. Never touch chemicals with your bare hands. Do not taste chemicals unless otherwise directed
to do so.
6. Use the fume hood when working with toxic vapour.7. Should an odor test be necessary, do not inhale directly from the container or test tube. Instead
waft a bit of vapour towards your nose.
8. Protect your hands with towel or rag when a glass rod, glass tubing, or thermometer is to beinserted.
9. Make sure that there is already a lighted match at the nozzle of the gas burner before opening
the gas line.10. Always use non-luminous flame.
11. Always set aside heated objects carefully when not in use.
12. Observe proper waste disposal of chemicals. Use specified container.13. Upon completion of the experiment, wash all apparatus used and return them to the
stockroom.14. Before leaving the laboratory, ensure the following:
Water and gas outlets are turned off.
The working table/ areas are clean and free from any chemical spill.

PHYSICAL SCIENCE LABORATORY
DOS AND DONTS
1. NO smoking.
2. NO eating and drinking.
3. NO littering and loitering.
4. NO bags and books on the worktables during experiments. They must be placed on the
tabletop.
5. Do not play with the electric outlets and water/gas valve.
6. Observe the laboratory safety rules.
7. Observe proper laboratory operations.
8. Observe proper waste disposal.
9. Conserve water.
10. Report immediately any injury/ accident to your professor.
11. Perform experiments with teachers supervision.
12. Observe proper decorum while waiting for your instructor and performing experiments.

BIOLOGY LABORATORY
SAFETY RULES
1. Always wear laboratory gown, goggles and gas mask when performing the experiments and
dissection.
2. Read and understand the procedures before the start of the experiment.3. Use reagents and water economically.
4. Do not apply flashing solution to your specimen during your dissection.
5. Be extra careful in handling the microscopes, prepared slides and specimens.6. Avoid contamination of reagents. Observe proper dispensing of chemicals. Spilled chemicals
on the skin are washed off with water immediately and thoroughly.
7. Never touch chemicals with your bare hands. Do not taste nor inhale chemicals unlessotherwise directed to do so.
8. Should an odor test be necessary, do not inhale directly from the container or test tube.
Instead, waft a bit of vapour towards your nose.
9. Protect your hands with a towel or rag when a glass rod, glass tubing, or a thermometer is tobe handled.
10. When an alcohol lamp is used, never tilt the lamp when being lighted.
11. Always set aside heated objects carefully when not in use. Use tongs/ potholders intransferring/ holding any heated object.
12. Make sure that your hands are dry when connecting any electrical equipment to the power
supply.
13. Observe proper waste disposal of chemicals. Use container specified for the purpose.14. Always return stuffed animals and specimen to its designated display cabinets after use.
15.Wash properly your hands with soap and water after dissection.
16. Upon completion of the experiments and/ or dissection, wash all apparatus used and returnthem to the stockroom properly.
17. Before leaving the laboratory, ensure the following:
Water outlets are turned off.
The working table/ area is clean and free from any chemical spill.
