Wood Group
Mustang Bringing aStrong Lineup
Wood Group Mustang is comprised of several companies that have
operated under either the Wood Group or Mustang banner and have
individually made a mark of excellence, with experience and successful
project results across a broad spectrum of the global energy industry.
As of January 2012, six companies, including newly acquired ISI Solutions
of South America, will be operating under a new brand, combining skill
sets, resources and geographic expertise to deliver added value to our
clients. The component parts of Wood Group Mustang are varied, and
each has a record of contributions to the industry that marks each as a
leader in the markets they serve.
MUSTANG
As Mustang celebrates its 25th anniversary in 2012, the companys
philosophy is as consistent today as when it started. Our People Oriented...
Project Driven culture focuses on taking care of people, providing
support for the communities in which it works and delivering technicalexcellence to clients.
Mustangs industry reputation grew as its service offerings
expanded throughout the 1990s. Mustangs service offerings
expanded into pipelines, onshore oil and gas facilities, refining
and chemicals, industrial facilities and automation and
control systems. Offerings for LNG facilities and
ecological solutions, including renewable energy were
also added.

In 2000, Mustang became part of Wood Group, a highly respected
Aberdeen-based energy services company whose business model
and culture closely matched that of Mustang. Since then, Mustang
has expanded globally, opening offices in the U.K., India, the Middle
East, Latin America and South East Asia in order to expand its
international presence.
Mustang now has over 5,000 Mustangers and operates in eight
countries. It has delivered numerous world-class projects, many of
which have been considered first, deepest or largest and were
reflective of Mustangs ingenuity and experience. Mustang has
also been repeatedly recognized as one of the Houston areas top
places to work and ranked number one in employee appreciation
categories.
ALLIANCE ENGINEERING
Alliance Engineering is a perfect complement to Mustang in
serving the deepwater, shallow offshore, and onshore pipeline and
facilities markets. Like Mustang, Alliance provides engineering,
procurement services, project management and construction
management to its clients.
Founded in 1993 and part of Wood Group since 2000, Alliance
provides a full range of engineering disciplines with expertise in
process, electrical, instrument, mechanical, civil, structural. In
addition to its Houston operation, Alliance has offices in Denver
and Pittsburgh to bolster its significant presence in the pipeline
facilities industry and established a visible presence in the dynamicMarcellus and Utica shale plays with unconventional gas reserves
in the northeast U.S. and Appalachia.
Alliance is a leader in delivering advanced designs as a major
player in the concept development of single-lift, lightweight topsides
production facilities for deepwater platforms. The advantages of
these designs - schedule reduction, full utilization of deck space,
lower installation costs, and faster installation and hookup have
been embraced by operators.
Alliance has also demonstrated ingenuity on floating production,
storage and offloading (FPSO) vessels, including the Cascade/Chinook,
the first FPSO design approved for installation in the Gulf of Mexico.
Alliance designs have been incorporated into more than 20 world-
class FPSO projects with operators and vessel contractors.
For the pipeline industry, Alliance has designed and installed over
two million horsepower of compression, over 1,000 miles of
pipeline and over 50 mainline meter stations over the last ten years.
GLOBAL PERFORMANCE
In early 2006, Mustang acquired Global Performance, a Greenville,
South Carolina-based engineering firm that provided project
management, construction management and design services to the
chemical, automotive and industrial markets. Global Performance
offers a diverse and track-record on non-energy projects and
provides in-house contract engineering services to notable interna-
tional chemical and life science companies. Additionally, it has
established a reputation for outstanding project performance on
the design and construction management of vehicle assembly
plants for some of the worlds most recognized automotive brands
including, BMW, Honda, General Motors and Daimler Chrysler.
The Mustang Greenville office has continued to add synergy to our
focus on the process and industrial sectors of the business, making
inroads into industries such as pulp & paper, pharmaceuticals,
cement and general manufacturing.
One of its most recent successes is the involvement in the grass-
roots final assembly facility for the Boeing 787 Dreamliner aircraft
in Charleston, South Carolina. Global Performance provided the
construction management and project management for the massive
642,000 square-foot (59,000 square meter) main production facility
as well as an aircraft interiors manufacturing, equipment mainte-
nance facility, delivery center where customers take possession of
the plane, and a welcome center at the gateway of the sprawling
industrial campus.

But do they have enough torqueto close your critical valves?
You might have to choose between muscle or footprint with other actuator manufacturers, but you dont need to sacrifice
anything with Emersons Bettis pneumatic scotch-yoke actuators. Our G-Series has a very efficient modular lightweight
construction with an optimal center of gravity for applications where weight and space are important. For the same torque
output requirement, Bettis G-Series is typically 1/3 lighter and smaller than our competitors offerings. In addition, we
understand accurate torque output is critical for SIL or fail-safe operations. Our G-Series has guaranteed torque outputs in
spring-return models to 3 million lb-in and double-acting configurations to 6 million lb-in. PED97/23/EC, IP66 and IP67M
compliance allows Bettis actuators to be used in just about any service onshore and offshore.Learn more about compact, yet powerful, Bettis pneumatics.

A PersonalTrainer?
Do You Need
WHY do you need a personal trainer in
the first place? Isnt physical fitness and
exercise a private affair? For many, the
answer is yeswell get to that later. But more and
more people are opting to enlist the aid of a personal
trainer, and they do so for the same reason that
someone who is furnishing a living room might call inan interior decorator for advice.
Having a personal trainer can have several
benefits. First, and most obvious, you get personal
attention. That means the exercise regimen can
be tailored to your specific health needs and your
personal goals. Plus, it gives you the advantage
of having a professional who can provide expert
guidance. Compare it to this: How many of us, as kids,
learned to play the piano or guitar all by ourselves?
True, some are self-taught, but many if not most had
a personal trainer. The same is true with physicalfitness. Whether your goal is to lose weight or just to
tone up, a personal trainer might be able to help you
accomplish your goals.
Some find that having a personal trainer gives
them motivation. After all, now theyre accountable

tomorrow. And trainers are more than people
who just call out the number of repetitions
you should do. These are people who can
work with your specific agenda. But how do
you find a trainer whos just right for you?
Writing in Bottom Line, personal
trainer Joe Sweeney (who is also the
author of a book entitled I Know I ShouldExercise, But ) suggests that you set up an
appointment with your prospective trainer. In
a 20- or 30-minute discussion, find out his or
her area of expertise. What experience does
this trainer have with your age group and
fitness level? You should not have to pay for
these appointments, Sweeney says.
There are several other things to
look for in a trainer. For example, what is
his or her certification? If you want quality
assistance, your trainer should have a four-year degree in some type of fitness-related
field. You can even ask to see a copy of
any certificate he may have. Look into his
liability insurance policy, too, while youre at
it. Any good trainer will not hesitate to offer
you a written contract, spelling out all fees
and conditions of your program.
Make sure that any trainer you are
thinking of enlisting is prepared to tailor
his workout to your abilities and goals.
He should start by assessing your health,
nutrition, fitness and medical history, writes
Joe Sweeney. He may test your body-fat
percentage, flexibility and muscular strength.
The trainer should be a great listener and take
into account what you hope to gain from your
workoutsweight loss, more stamina, better
posture, etc.
But a caution is in order. Not all
trainers are created equal, says Barrett
Niehus at the web site alltrainers.com. He
recommends: You should choose a personal
trainer using the same criteria that you use tochoose your personal physician. After all, the
personal trainer is a health care professional
that works by preventing disease. Evaluate
the trainers background, their qualifications,
their references, and their ability to
communicate and relate to your needs. You
are looking for a professional that will suit
your needs and be able to help you achieve
your goals.

22/52
Of course, personal
training will cost. One
program sets the price at
about $40 for a half hour
session and $60 for a full
hour. Rates are lower if
you sign on for a longer
term, perhaps 20, 50, or
75 weeks, and lower still
if you share with someone
elsein other words, one
trainer to two clients.
Another route is
to look into an on-line
personal trainer. A
number of web sites offer
personal advice tailoredto your personal goals and
informationbut its all
done by e-mail. Not bad
its semi-personal, and
its only semi-costly! For
exampleand this is just
one examplefor about
$30 per month, Desert
Valley Personal Training
offers a personal program
every four weeks. Theseprograms are completely
individualized based on
your goals and health information, asserts trainer
Ryan Crandall. Ill be beside you all the way with
unlimited email support.
All told, the process of choosing a personal
trainer need not be difficult. There are hundreds of
extremely qualified and professional personal trainers
out there, writes Barrett Niehus. The choice merely
requires a little homework and something more than
pulling a trainer off the floor of your local health club.Your personal trainer is your employee, and you are
paying him/her to help you achieve a goal. If you are
willing to pay money to become fit, why not choose a
professional that will make you succeed?
You can start your search for a personal trainer
at your local health club, or you can investigate web
sites like freetrainer.com or ustrainersearch.com.
Also, check out the web site of the Personal Trainers
Marketing Association (alltrainers.com).
Do It Yourself? But cant you do all this yourself? As men-
tioned at the outset, some can. The challenge is to
know which regimen is best for you and to keep moti-
vated. So if youre going to try to be your own personal
trainer, here are some tips from Dr. Miriam Nelson
from Tufts University, as printed in Bottom Line:
Work out with a friend. If someone else is
counting on you to be there, youre less likely to miss
a workout.Make a realistic schedule. Whether you
exercise every day for 30 minutes or 30 minutes three
times a week, make sure its a schedule that is practical
for you. If you set unrealistic goals, Dr. Nelson says,
youll quit.
Put it in writing. Chart your progress. Check
off each day on your calendar after youve completed
your exercise. Better still, keep a detailed exercise log,
including the date that you exercised, how long you

worked out, and what routine you performed. Charting
your progress will motivate you to maintain your
program, Dr. Nelson says.
Dont pamper yourself. Monitor whether a
routine is too easy, just right, or just plain too hard.
Exercise wont do you much good unless you work out
at an appropriate level of intensity, says Dr. Nelson. So,
just as a personal trainer would push you to a realistic
limit, ask yourself: Could I have exercised for a longer
period? Should I have included more difficult exercises?
Of course, if your limbs are heavy or sore after a
workout, your intensity level might be too high.
There are three types of workouts that you can
include in your program: Aerobic exercise, strength
training, and flexibility training. Aerobic training
includes jogging, bicycling, or brisk walking. This is
great for increasing the heart rate, though you should
be careful never to overexert yourself. Strength training
focuses on muscle strength and function by lifting
weights. You should start at a moderate level, because
movements that require all your strength can cause
injury to the bones and muscle. Flexibility training
involves stretching, which will expand the motion rangeof your joints.
Whether you choose to enlist the help of a
personal trainer or choose to be your own personal
trainer, a good exercise program will help you to look
and feel your best!

S
tudies of major accidents in process facilities indicate
that most result from the simultaneous occurrence of
multiple and seemingly minor errors and incidents
that interact in complex and unforeseen ways. (1)
Hidden errors and conditions that can occur during
the Safety Instrumented System (SIS) or facility life-
cycle can be referred to as Blind Spots and are often recognized
as a significant contributor to a major accident. Red Flags are
also a common prelude to a major accident. As an aid to revealing
Blind Spots, an awareness of common Red Flags may be helpful.
Projct excutionTodays typical large scale engineering projects have sizeable
teams that interact with many organizations and companies. In a
relatively short time they can generate thousands of documentsand a huge increase in project data communicated through many
different media. Quality checkpoints are added at key points, but
schedule compression can increase error rate and challenge this
process. Compression on fast track projects can result in late design
changes, delayed decisions and extended approval cycles. Quality
checks become less effective if performed at the wrong time or
under stress, so errors can be missed. It might be a purchase order
with a single digit error in a lengthy model number so that the
wrong material is procured. A person with essential technical
knowledge misses a key meeting. An inspector overlooks an
important detail at a factory check. Undetected errors can occur
in the engineering data exchange between companies if the data
exchange protocols are not well defined or managed.
Construction projects have a higher number of personnel who work
within a physically more hazardous environment. Onsite decisions
are constant, so errors can occur. Items dont f it, material cannot
be located or a key person in the communication channel is absent.
The transition from construction to pre-commissioning and startup
involves handoffs of many documents and status reports, all oppor-
tunities for missed information.
Safty Assssmnts
Safety assessments, such as Hazard and Operability Studies(HAZOP) identify hazards and quantify their respective risk. A
hidden deficiency in this process can result in hazards that are
missed or incorrectly assessed, especially if the team is missing key
technical, operating or maintenance expertise. (2) For example,
combustion and process experts are needed to assess the complex
impact of a major fuel gas drum swing on multiple f ired vessels
across the facility. Without this expertise, assessment may fail to
consider all modes of operation, common mode failures, process
response time or the complex scenarios that can result when a

major upset occurs in a shared utility system, e.g., steam, instrument
air or cooling water.
tehnlgyNew technology and new designs often create unforeseen challenges,
as when industry embraced open systems. The Microsoft Operating
System became a standard component in many control systems,
but there was an unforeseen risk of installing frequent software
patches to correct security holes and software stability problems.
There is increased exposure to destructive viruses of the type
recently revealed as the Stuxnet virus. (3) When control system
vendors press users to upgrade application software
and hardware to ensure future product support,
these upgrades often have subtle and undocumented
technical and performance differences. Implementing a
change before it is fully tested introduces an unknown
risk. If a facilitys software backup and recovery
procedures are inadequate or not followed, the wrong
program or an outdated version may be loaded.
High Integrity Pressure Protective Systems (HIPPS)
are increasingly being used to reduce project cost or
increase production. A well designed and managed
HIPPS offers safety benefits, but is high tech solution
and replaces a low tech solution that is well under-
stood. Management of HIPPS and other SIL 3, high
integrity safety systems require a mature, disciplined
and technically talented organization for the duration
of the systems lifecycle. Because a SIL 3 system is typically
implemented to mitigate a high consequence safety hazard, its
failure or degradation can result in a major accident.
Human FarsHumans will make mistakes. A well designed system avoids or
minimizes less reliable human activities. Process or activity errors
increase when humans are under tasked, over tasked or placed
under stress. (5) Human error is not random but is now understood
to be systematic. When presented with a problem, humans tend to
develop a mental model of what is happening and select data that
supports that model, ignoring non-supporting data. This flaw can
contribute to major accidents. As a counter point, humans oftenprovide the only safety response to a previously unknown hazard
event.
organizainOrganizations undergo continuous change and can contribute to
blind spots. A seemingly subtle change in priorities, staffing,
training, work process, safety culture, age, technical expertise or
tools can significantly affect process safety as it interacts with
other modes.
When a SIS progresses through its lifecycle, the Safety Requirements
Specification (SRS) provides the essential foundation document
needed to define and maintain system integrity. Figure 1 summarizes
the information included in this document. An inadvertent change
in any item can degrade or disable one or more Safety Instrumented
Function residing in that safety system. Mapping each data element
to the department, technical discipline or organization charged with
its creation or management provides an indication of the potential
challenge. The opportunity for hidden errors and changes increase
when elements are distributed across organizational boundaries.
Figure 1 Safety Requirements Specification (SRS)
operains and MainenaneOperating modes may exist that are below the radar and therefore
not assessed from a safety and risk perspective. For example, a
facility may regularly have a manual bypass valve open around a
control valve to increase throughput, or an operator tweaks a
mechanical stop on a fuel gas valve changing a process heaters
minimum firing rate. Operator use of safety system bypasses may
become a common and casual act that is not tracked and goes
unnoticed.
Off-the-books repairs and undocumented software changes may
be implemented in response to a problem that occurs during anunscheduled maintenance callout. Spare parts used may not
actually meet the Replacement in Kind requirement of PSM or
the more rigorous requirements in ANSI/ISA codes. Changes may
be made without employing or controlling a Management of
Change process.
Risk Aepane creepIndividual risk tolerances can shift when faced with an immediate
decision on whether to proceed or revert to a known safe state.
40
-
7/27/2019 Mustang Winter 2012 Web
41/52
Risk acceptance or denial can increase. The conse-
quence of taking a def initive safety response is
typically known, including the operational and fiscal
impact. The consequence of no action or a delayed
response is less certain. The decision to proceed or
shutdown reflects the decision makers understanding
of the companys management expectations, safety
culture, priorities and training. This phenomenon,
in a slightly different form, can occur during the
engineering phase of a project when a tight schedule
conflicts with the time needed to finish a safety-
critical assessment or quality check.
Accident InvestigationsAnalyzing and identifying the root cause of a near
miss or accident is an essential element in a safety
management program. , In the past accident investiga-
tions often sited operator error as the root cause.
The new, broader theory often traces the root cause
to a management, organization or system failure in
which humans function (4). Those employing the old
approach (still commonplace) overlook where the true
weakness in their systems exists, so similar accidents
may re-occur.
Management and LeadershipAll organizations, whether project teams or operating
facilities, face the dichotomy of balancing process
safety with production, cost and schedule demands.
By words, actions and examples, management and
safety leaders demonstrate their expectations. Subor-
dinates interpret this message and bias their actions
and attitudes accordingly. Given the challenges of
communications in large and complex organizations,
a few misunderstood words or an ambiguous or
conflicting message may degrade the process safety
attitude of employees. Many companies are imple-
menting behavior-based safety programs that have
been very effective at reducing injuries and accidents,
but these same programs may be less effective at
revealing or mitigating errors caused by technology orproject execution blind spots. Assuming that a given
safety program sufficiently encompasses the full
breadth of the safety management challenge may,
itself, be a serious blind spot.
Red FlagsSeveral blind spot modes have been discussed, but
complete listing of possible blind spots within
each mode can fill volumes. To limit their accident
4

contribution, an awareness and acceptance that blind spots exist
is essential. Another important element in a safety management
program should include an awareness of the Red Flags that often
precede a catastrophic accident. Management and safety leadersshould give pause when they hear the following-listed words; they
may have just arrived at that point, that last chance when a critical
failure can be prevented.
Experience says that will never happen. (Most catastrophic
accidents) (5, 6)
We need to reduce maintenance, staffing and training to cut
costs (Bhopal MIC release) (6) So we are all in agreement, RIGHT. (Shuttle Challenger and
Columbia Disasters) (5)
We dont have time for that (Most catastrophic accidents) (5, 6)
