Clinical Details: Malignant phyllodes left breast.

Specimen : Left breast, mass - MRM for HPE.

Gross Description: Received modified radical mastectomy specimen measuring 25x21x6cms along with

skin flap, nipple, areola and axillary fat.

skin shows 2 nodular areas, 0.4 cms each.

On cut section, 1.5 cms away from the nipple and just below the skin, there is a

circumscribed, firm, grey white mass with cystic, hemorrhagic and myxoid areas

measuring 7.5x6.5x6cms. Tumor margins are circumscribed.

Deep resection margin shows fat and is 0.2cms away from tumor.

Adjacent breast tissue appears unremarkable.

Grossly all resection margins appear to be free of tumor.

Axillary fat shows 14 lymphnodes, largest measuring 2x1cms.

Representative sections submitted.

Microscopy : Sections from the breast mass show cellular tumor with sheets of plump oval to

spindle shaped cells with pleomorphic nuclei.

There is increase in mitoses. In the most cellular areas mitotic activity is 4 to 5/10

HPF.Intervening stroma is scanty, shows areas of myxoid degeneration, hemorrhage

and ischemic necrosis.

- Tumor is seen 2 mm beneath the overlying skin. Skin does not show tumor

infiltration.

- Skin over nipple and areola is free of tumor. There is no evidence of Pagets disease.

- Deep resection margin is free of tumor.

- Superior and inferior resection margins are free of tumor.

- Medial and lateral resection margins are free of tumor.

- Underlying muscle is free of tumor.

- Axillary fat does not show any isolated tumor deposits.

- All the 14 lymphnodes identified in the axillary fat show reactive hyperplasia. There

are no tumor deposits (0/14).

Diagnosis: Left breast, mass - MRM : Low grade, Malignant phyllodes tumor.

Histological grade - I.
