IMMUNE SYSTEM = ระบบภมูคิุ้มกัน รา่งกายเรามกีลไกป้องกันการรุกล้ำ้าท้ำาลายจากสิง่แปลกปลอม 2 แบบ คือ
1. Nonspecific defense mechanisms/Innate immune response/Native immunity =
กลไกการท้ำาลายสิง่แปลกปลอมแบบไมจ่้ำาเพาะ/ระบบภมูคิุ้มกันท่ีติดตัวมาแต่กำาเนิด 1.1 First line of defense เป็นกลไกการป้องกันท่ีอยูภ่ายนอกรา่งกาย เชน่ ผิวหนัง และ mucous membrane ท่ีทางเดินอาหาร ทางเดินหายใจ ท่อปัสสาวะและสบืพนัธุ์ 1.2 Second line of defense เป็นกลไกการป้องกันท่ีอยูภ่ายในรา่งกาย เมื่อสิง่ แปลกปลอมสามารถแทรกเขา้สูภ่ายในรา่งกายได้ เชน่ การเกิด phagocytosis โดย เมด็เลือดขาว, การผลิต anti-microbial protein, inflammatory response

กลไกการท้ำาลายสิง่แปลกปลอมแบบจ้ำาเพาะ/ ภมูคิุ้มกันที่เกิดขึ้นภายหลัง or third line of
defense ได้แก่ การท้ำางานของ lymphocytes และการผลิต antibody

Clonal Selection Theory
Antigen (Ag)แอนติเจน คือ สารท่ีทำาปฏิกิรยิาจำาเพาะกับแอนติบอดี ซึ่งแอนติบอดีท่ีเกิดขึ้นนัน้มสีมบติัท่ีจะทำาปฏิกิรยิา
เฉพาะต่อแอนติเจนชนิดนัน้เท่านัน้
สิง่ท่ีจดัเป็นแอนติเจน มสีมบติัดังนี้
1) เป็นสิง่แปลกปลอม (Foreigness) สำาหรบัรา่งกาย2) เป็นสารท่ีเซลล์สรา้งแอนติบอดีจำาไมไ่ด้วา่สารนัน้เป็นของตัวเอง3) มโีมเลกลุนาดใหญ่ (> 10 kDa)4) เป็นสว่นใดสว่นหนึ่งของเซลล์มนุษยห์รอืสิง่มชีวีติ เชน่ เซลล์เมด็เลือดแดง เน้ือเยื่อ เชื้อไวรสั
แบคทีเรยี เหด็รา เกสรพชื เป็นต้น

โครงสรา้งและหน้าท่ีของ Antigen
- Epitope or antigenic determinant เป็นสว่นของAg ท่ี Ab เขา้ไปจบั (Ab จะใชส้ว่น antigen binding
site ในการจบั)
- แบคทีเรยีตัวหน่ึงๆ อาจมี epitope ส้ำาหรบัจบักับAb ได้ถึง 4 ล้านโมเลกลุ
Antibody (Ab) คือสารไกลโคโปรตีน (Glycoprotein) ท่ีอยูใ่นเลือด (blood serum) ซึ่งสรา้งมาจาก Plasma cells
(WBC) เกิดจากการตอบสนองของระบบภมูคิุ้มกันต่อ antigenic determinant ท่ีแปลกปลอม
Ab สว่นใหญ่อยูใ่น serum สว่น γ-globulin และเน่ืองจาก Ab เป็น globulin ท่ีทำาหน้าท่ีเกี่ยวกับ ภมูคิุ้มกันของรา่งกายจงึเรยีกวา่ Immunoglobulin (Ig)
Ab ทำาหน้าท่ีต่อสูห้รอืป้องกันการติดเชื้อโรคจากแบคทีเรยี ไวรสั หรอืสิง่แปลกปลอมอ่ืนๆท่ีเขา้มาใน รา่งกาย โดยจะทำาปฏิกรยิาท่ีมคีวามจำาเพาะต่อแอนติเจน (Ag)

Variable domain: VH3 Constant domains: CH1, CH2, CH3
• Covalent, disulfide bonds between cysteine residues
• Flexible “hinge region”Fab (antigen-binding fragment) เปนสวนท่ีอิมมูโนโกลบุลินใชจบักับแอนติเจน
Fc (crystallizable fragment) สว่นท่ีแยกออกมาไดในสภาพตกผลึก เปนสวนท่ีมคีวามสำาคัญทาง biological function

-Ab เป็น globular serum protein เรยีก immunoglobulins (Igs) ประกอบด้วย polypeptide 4 สาย; 2 สายเป็น heavy chain และอีก 2
สายเป็น light chain-variable region ของทั้ง light & heavy chain จะจ้ำาเพาะกับ epitope
หน่ึง ๆ-constant region ของ heavy chain จะจ้ำาเพาะกับชนิดของ Ig (ใชจ้้ำาแนก
ชนิดของ Ig)
-Immunoglobulin (Ig) มทัี้งหมด 5 ชนิด
1. Ig M (pentamer) เป็น Ig ท่ีพบเป็นชนิดแรกเมื่อexpose กับ Ag พบครัง้แรกในปลาฉลามและปลากระ
ดกูแขง็ ดังนัน้ IgM จงึจดัเป็น Ig ท่ีเก่าแก่ท่ีสดุตามสายววิฒันาการ
2. Ig G (monomer) พบมากในกระแสเลือด ท้ำาลายแบคทีเรยี, ไวรสั และ toxin เป็นอิมมูโนโกลบุลินท่ีมีปรมิาณมากท่ีสดุในซรีัม่ ถกูสรา้งมากท่ีสดุเมื่อม ีsecondary immune response
3. Ig A (dimer) พบใน mucous และ colostrum ป้องกันการจบัของไวรสัและแบคทีเรยีต่อ epithelial surface
4. Ig D (monomer) พบมากท่ีผิวของ B cell คาดวา่ชว่ยกระตุ้นการเปลี่ยนจาก B cell ไปเป็น plasma cell & memory B cell
5. Ig E (monomer) จบัอยูท่ี่ mast cell & basophil เมื่อถกูกระตุ้นโดย Ag ท้ำาให้เกิดการหลัง่ histamine หรอืสารท่ีก่อให้เกิดอาการแพ ้

การกระตุ้นคอมพลีเมนต์

Kohler ซึง่เป็นนักวจิยัแห่งมหาวทิยาลัยแคมบรคิจ ์ ได้ค้นพบโมโนโคลนอลแอนติบอดี และได้รบัรางวลัในสาขาการแพทยด้์านวทิยาภมูคิุ้มกันเมื่อปี 1984 - ในอดีตการเตรยีมแอนติบอดีจะได้โพลีโคลนอลแอนติบอดี (Polyclonal antibody) ซึง่เป็นการยากท่ีจะให้ได้ antibodies (Ab) หรอื immunoglobulin ท่ีมคีวามจ้ำาเพาะและมีคณุสมบติัตามท่ีต้องการสงู- โดยปกติรา่งกายของมนุษยแ์ละสตัวจ์ะสามารถสรา้ง Ab ขึน้มาได้มากกวา่ 2 ล้านชนิด เนื่องจากปรมิาณของ Ab ท่ีรา่งกายสรา้งขึน้มามมีากมายหลายชนิดจงึท้ำาให้ Ab ท่ีรา่งกายมนุษยผ์ลิตขึน้มามปีรมิาณท่ีต้ำ่าและไมม่คีวามบรสิทุธิ ์ ดังนัน้ความสามารถท่ีจะท้ำาลายทั้งแบคทีเรยีและไวรสัเมื่อมกีารติดเชื้อเขา้มาในรา่งกายจะต้ำ่า
