MEMORANDUM OF AGREEMENT

KNOW ALL BY THESE PRESENTS:

This Agreement is entered into, by and among:

The AKBAYAN CITIZENS ACTION PARTY, a national organization, withnational office address at 36 B Madasalin St, Sikatuna Village, Quezon Cityrepresented herein by its Secretary General , MR. CONRADO CASTILLO,JR. nowand hereinafter referred to as AKBAYAN;

The MUNICIPALITY OF LAUR, NUEVA ECIJA , represented herein by itsMUNICIPAL MAYOR, ALVARO DAUS, now and hereinafter referred to as LGU;

The Creative Mill Consultancy Inc., duly registered with the Securities and

Exchange Commission, with office address at 54 D Matulungin St. BarangayCentral , Quezon City represented herein by its Chairperson, _______________,now and hereinafter referred to as the Civil Society Organization or CSO;

WITNESSETH THAT:

WHEREAS, the national government, as mandated by the 1987Constitution, desires to engage and encourage the participation of the citizensmovements, non-government organizations, peoples organizations, localgovernment units, private sector, and the citizenry in general, in achieving itspurpose of attaining universal health care;

WHEREAS, Local Government Units which include municipalities, areempowered under the Local Government Code of 1991 to enter into contracts oragreements and to implement or cause to implement programs and projectswithin their respective area of jurisdiction;

WHEREAS, AKBAYAN, a Party List through incumbent Member of theHouse of Representatives, Hon. Arlene Bag-ao, has allocated funds under theGeneral Appropriations Act thru its Priority Development Assistance Fund as afinancial assistance to the Municipality of Laur, Nueva Ecija;

WHEREAS , AKBAYANs platform of government includes widening theaccess to basic social services, particularly, health services, by our people;

WHEREAS, the LGU has identified the CSO, Creative Mill Consultancy Inc.as partner organization which will provide technical assistance and servicesthrough a project called Improving Access to Health in the Municipality of Laur;

WHEREAS, the Municipality of Laur Nueva Ecija had agreed andcommitted to partner with Akbayan and civil society groups, including the hereinCSO, that will support said project at the barangay and municipal levels;

WHEREAS, the herein parties believes that improving access to healthshall be directed towards ensuring the achievement of the health system goals ofbetter health outcomes, fair health financing and responsive health system byensuring that all residents especially the disadvantaged group, have equitableaccess to health care. Further, parties will employ universal health care approachand localization of reproductive health;

NOW THEREFORE, the parties hereto in consideration of the foregoingpremises agree as follows:

About the project

1. That AKBAYAN will allocate the amount of THREE MILLION PESOS(P3,000,000.00) from its Priority Development Assistance Fund for theproject Improving Access to Health in the Municipality of Laur thru asoft financial/technical assistance broken down into the following items:

a. Five Hundred Thousand (

P500,000) for the Construction/Rehabilitation of the Rural Health Unit (RHU),b. Five Hundred Thousand (P500,000) for the Procurement of Medical

Equipmentc. Two Million Pesos (P2,000,000.00) for Capacity Building

2. That it aims to see improvements in six areas of universal health care atthe end of the project. Namely health financing, service delivery, policy,standards and regulation, health information system, health humanresource, governance for health.

LGU Counterpart

3. The local government committed to allocate significant funds forfacility improvement, enrollment of the poor and the near-poor toPHILHEALTH and capacity building of barangay elected officials ingovernance in health.

4. The LGU is also committed to significantly increase its 2012 budget forhealth services and onwards.

5. The LGU shall then allocate a financial counterpart in the amount of EIGHT

HUNDRED THOUSAND PESOS for the said project to conduct bdp-pra in__ barangays.

Implementing Mechanisms:

6. The Municipality of Laur, Nueva Ecija will enter into a partnership withthe Creative Mill Consultancy Firm to provide technical expertise onseries of activities related to the project Improving Access to Healthherein attached asAnnex A

7. That the CSO/Cooperative will provide technical assistance through thehiring of project management to oversee the day to day operations ofthe projcet

implementation (PMC)11.LGAC to oversee the provision of technical assistance12.Tapping of national and local consultants (UP TeleCenter, Dr. Guy, DOH-

CHD 3, etc)13.Hiring of 1 staff preferably nurse or graduate of health science related

courses (deployed locally and knowledgeable on public health)14.

Financial Disbursement:

Separability Clause.

This Agreement shall take effect on the date of signing hereto by theparties.
