appointment (should receive email from PSB w/in 24 hrs)

Step 3b: after receiving HHC’s email,

PSB does the following w/in 24 hrs:

• Complete arrival transaction in EMILPO

• Slot SM in correct PARA/LN in EMILPO

• Forward the installation inprocessing checklist with instructions to SM

Note: if SM is taking leave/PTDY prior to inprocessinginstallation, coordinate with SM on dates to receive installation inprocessing checklist

Step 2:Sign in HHC USMA

NLT the last day of approved PCS leave, SM reports to HHC USMA during normal business hours* with the following documents:

• PCS Orders, amendments

• DA31 (Signed out by last unit & control number)

• Inform HHC if you are required to be quarantined/isolated due to overseas travel, contact with COVID+, or are symptomatic – and we will provide the information for the 14-day COVID TDY

• If requesting PTDY for house hunting or ordinary leave (local area w/in 50-miles), be prepared to complete a DA31 – need the gaining first line supervisor sign in block 12 and approving authority in block 13

• HHC requirements are complete

Step 5: (NLT N+72hrs) after receiving SM’s email,

HHC does the following:

• HHC CDR signs DA5960

• Emails DA5960 to SM

• Confirm SM has received installation inprocessing checklist from PSB, if not instruct SM to contact PSB

• Emails DMPO/finance completed DA5960, PCS DA31, Orders w/ PACIND

Step 3b: (NLT N+48hrs) after receiving HHC’s email,

PSB does the following:

• Complete arrival transaction in EMILPO

• Slot SM in correct PARA/LN in EMILPO

• Forward the installation inprocessing checklist with instructions to SM

In accordance with Department of the Army, Office of the Assistant Secretary Manpower and Reserve

Affairs memorandum dated August 16, 2017, Subject: Recertification of and Supporting Documentation

Required for the Basic Allowance for Housing (BAH) Entitlement for Regular Army and Reserve

Component Soldiers:

(5) Effective immediately, each Soldier will annually recertify their entitlement to BAH by completing a

new DA Form 5960. The recertification will be accomplished in conjunction with the required DD Form

93 (Record of Emergency Data) annual recertification. Soldiers will also recertify upon life changing

events (e.g. marriage, divorce, birth of a child, other dependent changes). The DA Form 5960 will be

signed by the Soldier and certified by the company level unit commander. Commanders may delegate,

in writing, this certification to the first commissioned officer in the Soldier’s chain of command.

At the time of your annual recertification, the Military Personnel Division will verify that your current

permanent change of station (PCS) order, current promotion order, and all applicable dependency

documents (marriage license and children’s birth certificates) are posted in your Official Military

Personnel File (OMPF). We recommend that you pre-screen this PRIOR to coming to your appointment

and if these items are missing please bring them in at the time of your appointment.

You will be required to complete your DA Form 5960 yourself and have it certified by your company

commander or his/her delegate.

Instructions for completing the DA Form 5960

Block number – instructions

1. Self-explanatory

3. Enter your grade not rank

4. Place an X in RECERTIFICATION

5. Enter your current unit on first line and West Point, NY 10996 on the second line

6. Enter today’s date

7. Select the appropriate BAQ (BAH) type

8. Enter your marital status- Select the appropriate MARITAL/DEPENDENCY STATUS – enter

spouse’s SSN (1) and spouse’s duty station (2) if your spouse is military. (3) Enter your date of

marriage or divorce. (4) Select the appropriate.

9. Place an “X” in a. ADEQUATE

10. List all AUTHORIZED dependents. If you require more lines than the DA Form 5960 allows

please type these dependents on a blank continuation page but be sure to include all the required

information for each authorized dependent (Name, complete current address, relationship and DOB

for children).

11. Select the appropriate certification statement by placing an “X’ in the block.

You will then have to email your signed form to your company commander for certification- we will not forward the form for you. Once your commander has signed and returned your form to

you please email it to us or bring printed copy in when you come in for your appointment.

***NOTE*** For any Soldier missing the required dependency supporting documentation in

iPERMS after the 60-day period, the Army will reduce the Soldier’s BAH with-dependent or BAH-

DIFF entitlement to BAH without-dependent until the key supporting documents are provided.
