HKCFP Research Fellowship 2014
Introduction
The HKCFP Research Fellowship was established by the Hong Kong College of Family Physicians to promote research in Family Medicine. The Grant is up to the value of HK$ 100,000. Applicants are expected to have regular contact with a nominated supervisor with a Master or equivalent degree or above.
Eligibility
Applicants for the HKCFP Research Fellowship must be active Fellows, Full Members or Associate Members of the HKCFP. New and emerging researchers* are particularly encouraged to apply.
Selection criteria
Applications will be judged on:• Training potential of applicants;• Relevance to Family Medicine and community health;• Quality;• Value for money;• Completeness (incomplete or late applications will not be assessed further).
* Please note that new researchers and those at an early stage of their research careers are defined as those who have not led a major research project or have fewer than 5 years of research experience.

HKCFP Trainees Research Fund 2014 /
HKCFP Research Seed Fund 2014
The Research Committee of HKCFP is proud to continue to offer the two established research funds,
The Trainees Research Fund and the Research Seed Fund again in 2014.
The Trainees Research Fund will be opened to all registered HKCFP trainees and is made of multiple
awards (each up to HK$10,000). It is envisaged it will help trainees especially (but not limited to) those
doing research projects for their exit examination. Those who have funding support elsewhere will not
be considered.
The Research Seed Fund is open to all HKCFP members when a maximum of $10,000 award will be
made to the successful applicant to assist the conduction of a research project.
Assessment Criteria for both funds:
1. Academic rigor of the proposal (e.g. originality, methodology, organisation and
presentation); and
2. Relevance and impact to Family Medicine & primary care (e.g. importance of the topic and
the impact of the findings on the practice or development of the discipline).
Preference will be given to the applicants applying for a project on one of the following
themes this year:
1. Professionalism/ Doctor-patient interventions and clinical decision making;
2. Effectiveness of HKCFP vocational training;
3. Perception of FM by the general public and patient care-seeking behaviours;
4. New care models in primary care e.g. Patient Empowerment in chronic disease
management;
5. Clinical presentations and clinical epidemiology; or
6. Health equity in primary care.
Please note you can still apply if the theme of your submitted research project is not one of the above
mentioned. Each research project submitted will be assessed according to the above assessment criteria

Experience Sharing : Mobile Clinics in the New Territories East Cluster
Inadenselypopulatedcity likeHongKongwithagoodtransportsystem,itisnotdifficultforpatientstofindafamilydoctorneartheirhome.However,tosomepeoplewholive inoff-shoreislandsornearthenorthborderofHongKong likeShaTauKokandTaKwuLing,itcouldbedifficult.Howcanwehelpthisgroupofpopulation,especiallythoseelderlywithproblemsinmobilization?Thisarticlegivesyouan ideaaboutsomepublicservicesprovidedtopeoplelivingintheruralareasofHongKong.
(1) Floating Clinic
Background of floating clinic
Inthe1950s’,manyoftheHongKongresidentswere fishermen, living mainly in boats andoff-shore islands.Therefore, theMedical andHealthDepartmenthadtosendateamofmedicalhealthcareprofessionalstotheislandstoprovidegeneralout-patientclinicserviceandvaccination.In1958and1959, theHongKongJockeyClubdonatedthemarines“CHEEHONG”(慈航號)and“CHEEWAN”(慈雲號).Afloatingclinicteamwithadoctor,anurse,adispenser,acaptainandasailorwasthenformed.Theyfollowedascheduletoservetheresidentsontheoff-store islands ifweatherpermitted (Picture1).During theearlydays, themedical teamtravelledto the islandsandsawpatients in theCheKungTemples intheislands.In1988,theHongKongJockeyClubdonatedthesecondgenerationmarines“CHEEHONG” (慈航號)and“CHEEWAN” (慈雲號) tocontinuetheservice(Picture2).Amini-clinicwassetupontheshipssothatpatientscouldbeseenthere.
Afterprovidingtheseservicesfor20years,“CHEEHONG”and“CHEEWAN”wereretiredin2008.Theservicewasthencontinuedbyacontract-outmerchantmarine.
Current service
Now, the floatingclinic runsoneveryTuesdaytoThursday.Ateamwithadoctor (Picture3),a

nurse,adispenser(Picture4),aclericalstaffandaworkmanfromtheFamilyMedicineDepartmentoftheNewTerritoriesEastCluster, theHospitalAuthority continues to provide general out-patient service on the contract-out merchantmarinetotheresidentsofnineoff-shoreislandsofHongKong.Around60%ofcasesareconsultingforepisodic illnesses,while40%of casesarefollowingupforchronicdiseaseslikehypertensionanddiabetes.
(2) Travelling dispensary
Background history of travelling dispensary
Manyvillagesare locatedinthenorthborderofHongKong.Asthesevillagesaregeographicallylocated far fromthecentreofcities,access tohealthcareisverydifficult.Publicservice,inawayoftravellingdispensary,hasbeensetuplongagotohelpprovidesimplehealthcaremanagementtothesevillagers.Thetravellingdispensaryvisitthreeplaces inthenorthborder,namelyTaKwuLingPoliceStation (打鼓嶺警處), LinMaHang(蓮麻坑)andSunUkLing (新屋嶺).SinceLinMa

Hanghadbecomeaclosedfrontierareain1951,eachofourstaffrequiresapermittoworkinthetravellingdispensary.Overthelastfewdecades,the number of villagers in those places hadreducedasyoungergenerationsmovedto liveandwork inmoreurbanareas.Nowadays,thosewholivedinthosevillagesaremainlyoftheoldergeneration.
Current service
ThetravellingdispensarytakesplaceeveryFridayafternoon,visitingTaKwuLingPoliceStation,LinMaHangandSunUkLinginathree-hoursession.At each stop, the minibus will stay for about30minutes.Thenumberofpatientsattendingateachstopranges from1 to10.Themedicalteamconsistsofadoctor,anurse,aclerkandadispenser.With limitedspaceavailable,almostonlybasic and simplemeasurements suchasbloodpressure,glucostixandbodytemperatureare available. Majority of cases present withepisodic i l lnesses l ike symptoms of upperrespiratory tract infection.Occasionally, somepatientsattendforfollowupofchronic illnesseslikehypertensionanddiabetesmellitus.Asmostinvestigations, including ECG, blood & urinetests,arenotavailable,patientswith relativelymorecomplicatedconditionshavetobereferredbacktobiggerpubliccentresintheNorthDistrictforfurtherfollowupandmanagement.
Recentlytheserviceshavebeenfurtherenhancedwithcomputersbeingconnectedtothehospitaldatabase,allowingelectronicpatientrecordstobeseeninmobileclinics.Thus,betterandsaferpatientcarecanbegiven.
Experiences for sharing
Look ing a t the se t t ings , the hea l thcaremanagement in the ship or mini-bus mightappeartobeoversimplified.Thepracticeisverydifferent fromtheusual“office”practicewhichisourtraditional trainingbackground.However,notall staffareable towork in thesekindsof
settings.Theminimalrequirementisthatthestaffshouldbeabletotoleratevehicleorseasickness.What’smore,thestaffshouldbeveryfamiliarwiththemanagementofdifferentclinicalconditionsespecially when using the emergency kit isnecessary.Incaseofemergency,basiclifesupporthas tobe initiatedwithoutdelayasambulanceserviceswillnotbeavailableassoonasinclinicslocatedinurbanareas.Nevertheless,workinginsuchenvironment isstillveryenjoyableasmostpatientsliveinthelocalityandarewellknowntotheservice.Itsoundseasierforcontinuityofcaretobeachievedinsuchclinicsettings!

「爹d加油!爹d加油!」「哥哥,你游快d啦!你包尾啦…」
當我在池邊等待出賽時,看到一眾同業的年幼子女們,既蹦蹦跳跳又「肉緊」地為家人打氣,深深體會到那種溫馨的感覺。尤其當我見到幼童組的一位戴著粉紅色泳帽的小女孩,站在跳水台上仍然矮人一截,我見猶憐,彷彿時光倒流到六歲的時候;當年我才小學一年班,抬頭望著旁邊線的小二、小三師姐,就是這樣「大無畏」地完成了人生第一場小學水運會。
今天是一年一度的香港醫學會游泳比賽,從理工大學保健處步行到比賽場地理工大學游泳池,就只
需五分鐘,這更加強我要堅持繼續出賽的決心。在泳池邊,碰到幾位當年同屬中大泳隊的師兄們,腹肌仍然健在,非常佩服。當年我是中大泳隊游「包尾」的那位,主要職責並非在大專賽或兩大賽爭光,而是負責召集來自不同學系的隊友們,集訓之後大夥兒去沙田食「宵夜」。我們差不多每星期集訓兩至三晚,每晚游幾千米,每次「宵夜」都是在筋疲力竭的情況下「搶食」;總之比賽成績次要,聯誼食飯至上,因此當年中大泳隊又名「中大飯團」。那時候我電燙了一把長髮,由於每次集訓之後要趕著去「宵夜」,所以沒有時間「吹頭」,隊友們笑我那未弄乾的髮型,尤如一碗倒轉的公仔麵,所以我的泳隊戰衣背面,就繡上了「公仔麵」的綽號了。
十幾年後的今天,我從中大走到理大的泳池,仍然頭戴著代表「飯團精神」的泳帽,不在乎比賽勝負,只在乎勇往直前。各位同業,雖然每天源源不絕的臨床診症,再加上繁鎖的行政事務,往往會令人身心疲倦;不妨每天保留一點屬於自己的時間,讓上班前的晨早清風,及下班後的皎潔明月,伴你同行,隨心出發。

Update News for Safety Concerns About Saxagliptin
Saxagliptinisadipeptidylpeptidase-4(DPP4)inhibitorindicatedasanadjuncttodietandexerciseto
improveglycemiccontrolinadultswithtype2diabetesmellitusinmultipleclinicalsettings.Itworksby
increasingtheamountofinsulinproducedbythebodyaftermeals.
Itisusedeitherasamonotherapy,orascombinationtherapywithmetformin,athiazolidinedione(TZD),
asulfonylurea(SU),orinsulin(withorwithoutmetformin).Thisisindicatedwhenthesingleagentalone,
togetherwithdietandexercise,doesnotprovideadequateglycemiccontrol.
Therecommendeddoseis2.5mgor5mgoncedailyregardlessofmeals. 2.5mgdailydosageis
recommendedforpatientswithmoderateorsevererenalimpairment,orend-stagerenaldisease(CrCl
≤50mL/min).Weshouldassesspatients’renalfunctionbeforestartingthemedicationandalsoona
regularbasis.
Thedrug iscontraindicated inpatientswithhistoryofa serioushypersensitivity reaction (e.g.,
anaphylaxis,angioedema,exfoliativeskinconditions)tosaxagliptin.Andpossibledruginteractionwith
strongCYP3A4/5inhibitors(e.g.,ketoconazole)significantlyincreasestheconcentrationofsaxagliptin,
solimitingsaxagliptinto2.5mgoncedailyisrecommendedinsuchsituations.
InFebruary2014,theU.S.FoodandDrugAdministration(FDA)hasrequestedclinicaltrialdatafrom
themanufacturerofsaxagliptintoinvestigateapossibleassociationbetweentheuseofthisdrugand
heartfailure.1TherequestresultedfromastudypublishedintheNew England Journal of Medicine
(NEJM)2,whichreportedanincreasedrateofhospitalizationforheartfailure,whentheheartdoesnot
pumpbloodwellenough,withuseofsaxagliptincomparedtoaninactivetreatment.Thestudydid
notfindincreasedratesofdeathorothermajorcardiovascularrisks,includingheartattackorstroke,
inpatientswhoreceivedsaxagliptin.ThemanufacturerisexpectedtosubmitthetrialdatatoFDAby
earlyMarch2014,afterwhichFDAwillconductathoroughanalysis.
Atthistime,FDAconsiders informationfromtheNEJMstudytobepreliminary.Theanalysisofthe
saxagliptinclinicaltrialdataispartofabroaderevaluationofalltype2diabetesdrugtherapiesand
cardiovascularrisk.Atpresent, ifhealthcareprofessionalswishtoprescribesaxagliptin,theyshould
continuetofollowtheprescribingrecommendationsinthedruglabelsandtoremindtheirpatientsto
reportsideeffectsafterusingtheproducts.
