At Destiny Meditravel we stand by the service that we offer believing that we have a responsibility to our clients long after they have returned home. We provide our clients with the assurances that they are not putting themselves at unnecessary risk by traveling to Thailand for surgery and that the Doctors and Hospital are of an acceptable standard to westerners. We have researched thoroughly our doctors and hospitals and can confidently provide these assurances to our clients.

Partner Doctors in Australia is another essential part of guarantee to our clients that working with Destiny Meditravel to coordinate their surgery holiday is to their benefit. As we have partnered with both Phuket International Hospital and Bangkok Hospital Phuket for our surgery clients we have also access to their appointed preferred medical practitioners within Australia who work closely with them to provide ongoing care and when needed once clients return to Australia.

Whilst clients may choose their own doctors or even see their own GP for any pre or postoperative issues, the hospitals recommend using their preferred medical practitioners as they have an agreement that these practitioners will communicate with the hospital, provide them with medical reports and ensure there is a continuation of care and service. Clients should note that payment for any service is the responsibility of the client. Hospitals will only reimburse clients or pay for treatment and services in Australia when this has been pre authorised.

We do acknowledge that clients will research their procedures prior, risks will inherently be associated with any surgery- which we try to minimise by selecting the best hospitals and surgeons. If you are healthy without prior medical conditions and have realistic expectations, followed all your doctors advice and recovery instructions then you should be pleased with your results.

We have put this booklet together to provide additional informaton to our client that may assist them in being well prepared for their trip. We highly encourage using the checklist provided to ensure that you have all the things completed and packed to take with you.

Please ensure that you take the time to read the Pre-Operative considerations, Risks and Complications and make sure you are not taking any of the medications and supplements listed. If you are unsure if you should stop taking medication please contact so we can confirm with your surgeon.

Know that whilst the staff on the ground in Phuket will take fantastic care of you well we are still always available back here in Australia to assist you in any way.

PACKING

ESSENTIALSWe suggest that you get your suitcase out a few weeks prior so that you can add things and remove as you thinkof them. There will be essential things that you will need to take and then there is the wish list if you have room.If you fold clothes in half and then roll them instead of folding you will find you keep things wrinkle free as wellas have a lot more room. Make sure all toiletries are in plastic bags so if they spill wont ruin all your clothes, fromexperience we know toothpaste especially has bad habit of doing this so put in a zip lock bag.

Your hotel will have hair dryers as well as shampoo, conditioner soap and basic toiletries so don't over pack on these items. Don't panic if you think you forgot something you can always buy it when you get there. Don't overfill your suitcase on the way over as you want to leave room for all your new purchases.

Arrival and Appointments

TRANSFERS

Airport transfers are included free of charge for our Meditravel clients who book surgery. If your require transfers for travel companions on occasions when “you” the actual client is not travelling with then charges will apply to these transfers. They can be arranged on a fee for service basis.

If you require transport from the airport upon your arrival, please ensure that Meditravel is aware of your travel itinerary and a copy has been emailed prior to your actual departure. This then allows us to ensure that your transfers are all confirmed.

If after arrival your flight is changed or significantly delayed, please let PIAC know this by calling them on + 66 76 249400 ext 3400 from outside of Thailand and on 076 249400 if calling from within Thailand. You may also email Meditravel or text the information through to Mel on 61+404 493105.

On the morning of your consultation and surgery you will be collected from your hotel by your hospital transport, they will collect you in your hotel lobby around 30 minutes before your appointment time (times may change depending on the location of your hotel). You will then be taken to the hospital . Please ensure you have your passport with you as it will be required for registration when you arrive at the hospi-tal.

ARRIVAL AIRPORT

Once in the arrival hall, ignore all taxi and transport touts and make your way to the Holiday Inn Hotel lounge. The Holiday Inn Hotel lounge is open from 8:00 AM to 9:00 PM and staff will be waiting in this area even outside of opening hours.

APPOINTMENTS

You will have received a confirmed appointment in your information pack or emailed to you by your consultant. This treatment plan will set out all your initial transfers as well as Surgery requested includ-ing you scheduled appointment time. Check-up appointment and transfers will be made direct with your on discharge after surgery and are included in your packages.

If you wish to change your appointment or pick up times,please confirm this directly with your Medit-ravel consultant prior to your departure. If you have arrived in Thailand and wish to change your appoint-ments or pick up times please contact the hospital directly

Your appointment is when you get to meet your surgeon for a face to face consultation. This is your chance to ask your surgeon all the questions you have in mind. You may want to bring photos that will aid in explaining what result you expect to have after the surgery.

During this time you should also inform the surgeon about your medical history, previous surgeries and other pertinent health information.

Surgery is only confirmed AFTER the consultation. An consult date and surgery can be on the same day. Your surgery is pre-confirmed and scheduled prior to your arrival, however you are not committed to proceed until you personally confirm this with your surgeon after your consult.

You will have been given an Immigration Passenger Arrival card on the plane to complete. Make sure you have completed this prior to landing in Phuket as you will need to present it with your passport at customs. Australian + New Zealand passport holders do not need to apply for a Visa to enter Thailand as there is an agreement in place that this will be issued on your arrival if your stay is less than 30 days. If you have a passport from another country please check with us or travel agents if you need to apply for Visa.

Airport staff will guide you where to proceed through immigration to baggage claim and through customs into the northern end of the airport arrival hall.

PRE-OPERATIVE CONSIDERATIONSAge of Consent`

The age of consent for surgery in Thailand is 20 years at the date of surgery. It is the clients responsi-bility to advise Meditravel if you will be less than 20 years of age on your surgery date.

Exceptions are allowed for younger clients provided that consent is received from parents/guardians and that the surgeons does accept the cases based on individual client requirements, eg need for surgery, client has “developed” and growth and maturity complete, actual age etc. Final decision on the accepting of a client under 20 years is made on a case by case basis by the surgeon.

Clients who present to hospital for surgery, who are under 20 years of age without prior authority and consent will be denied surgery and Meditravel and hospital will not be responsible for any costs,loss or damages.

Breast Feeding and Pregnancy

Compression Garments

Hospitals require all clients having any liposuction, tummy tuck, belt lipectomy, brachioplasty and some other specific surgery to supply their own compression garments before arriving in Phuket.

We have an assortment of garment available for purchase on our website. Please purchase these at least 4 weeks prior to your planneddeparture. These garments are essential for ensuring a better and more comfortable recovery and the cost of these compression garments are not included in the surgery package price.

You must take these with you to your surgery appointment so surgeon can dress you in them after surgery.

Breast Implants

Hospital will not accept clients who are requesting breast surgery within 6-12 months following a pregnancy or breast feeding. It is the clients sole responsibility to ensure that they have advised Meditravel staff if this will apply to their situation.

The Hospit does not provide compression garments to clients nor require the use of compression garments except for breast surgery. However each surgeon has their own requirements and will advise clients on a case by case basis.

For a standard breast surgery, PIAC surgeons’ will provide each client with that surgeon’spreferred choice of post-surgical bra. We do not require our clients to purchase these brasbeforehand and the cost of the bra included in the surgery package price.

If you require a second bra or wrap to use during washing we have those available for purchase through our website.

Our hospitals provide an extensive range of silicone breast implants but does not use saline implants even on request. Mentor, Silimed and Allergan are the standard product range, available in many styles including round, teardrop, high and low profile etc.

Meditravel provide an information sheet on the types and styles of implants commonly used by our surgeons as well as incision sites (breast crease, armpit, endoscopic etc) as well as the placement technique (above, behind muscle, or dual plane).

Hospital Rooms

Surgery patients are allocated single private rooms during their hospital admission and the sharing of rooms admitted patients is not allowed. Companions are welcome to visit their admitted friends or relatives in other rooms.

The hospital does allow without charge a friend or family member occupying the same room as the patient. Each room has a couch that friends or family members may sleep on.

For an additional charge, an extra bed maybe requested and placed in the client’s room, subject to availability.

Concierge Services

The hospital will provide and English speaking staff member to meet and greet surgery clients arriving at Phuket Airport and to assist with transfer, confirm appointment times and update clients on any changes to their bookings. Details on where and when to meet our concierge service representative are provided in your Treatment Plan in your information folder

Concierge staff monitor incoming flights and will be aware of any flight delay, and whilst all due care and attention will be made to ensure clients are not in any way inconvenienced, Meditravel and PIAC will not be responsible for any costs or charges incurred by clients who fail to follow all the instructions on the Treatment Plan and who fail to show up at the arrival designated meeting location within Phuket Airport.

Concierge services are also able to offer mobile phone and SIM card rental for clients. Please note this must be pre booked with your consultant prior to your arrival.

All body piercing must be removed before surgery. Temporary plastic (nonconducting) maybe used in place of piercing provided approval from PIAC is obtained before surgery

Nail polish and any artificial nails must all be com-pletely removed before surgery

Artificial tanning is not advised before surgery

Our Hospitals provides complimentary transport for surgery clients. The complimentary transfer are only to be provided within Phuket and do not cover areas such as Phi Phi Island or Phang Nga.

Complimentary transfers include airport pickup and drop off to and from hotel, as well as all hotel to hospital transports that are booked by hospital directly to attend surgery and all initial and follow up appointments.

Transfers apply to the patient only and any family members/friends travelling alongside with that patient. We do not provide for any complimentary transport for friends/family members wanting to travel on their own to and from hospital. They may be booked on a fee for service basis.

Immunizations

Provided surgery clients have been fully immunized as a child following accepted Western Countries and WHO immunization schedules, and have had routine booster vaccinations, no additional immunizations are required for travel to Phuket.

If clients have any doubts about theirimmunization status they should seek advice from their GP.

There is no malaria in Phuket and clients are not required to take prophylactic medication to protect against malaria. Taking medication for the malaria may result in your surgery being cancelled, as many prophy-lactic malaria type medication is contraindicated for surgery

Menstruation

Clients who may, or will be menstruating on the date of their surgery may have their surgery cancelled or delayed.

It is the clients responsibility to advise your consul-tant or hospital if they may or will be menstruating as this can pose a risk to client’s health and surgical outcome.

Clients should consider changing their period pattern well before surgery to eliminate the risk of being denied surgery.

Meditravel will not be responsible for any costs, loss or damages incurred by clients who are denied, or delayed surgery if at time of surgery they are men-struating.

Hospitals do have very strict policy on which types of medications can, and cannot be taken prior to being accepted as a patient for surgery.

Many medications contribute to several post-operative complications or are contraindicated with surgery, this includes many types of vitamins and supplements.

Certain medications must be ceased at least 2 weeks prior to the surgery date.

It is important that all clients inform your consultant of all medications they are taking so we can inform your surgeon. Your surgeon will advise if clients are to stop the medication.

We have an information guide which details with many, but not all prohibited or restricted types of medications and vitamins. Please ensure your have read this prior to your arrival.

Hospitals does not offer surgery on the same day as arrival in Phuket for clients arriving from overseas. They also has strict minimum stay requirements in Phuket to ensure that adequate post-operative time is allowed for healing, and follow up care and appointments.

Clients not staying the minimum required time in Phuket will automatically void the guarantee unless prior approval is obtained.

Clients must also stay within Phuket and Krabi prov-inces, and will void their guarantee if they travel outside of these provinces during their minimum stay period.

This means for example that a client cannot have surgery, then travel to Bangkok or Koh Samui and still be eligible for the guarantee.

Minimum stay requirements are listed on the surgical recommendations sent prior to Meditravel making your appointment.

Appointment and Surgery Day

Compression garments suitable to your intended surgery. Breast surgery clients will be given one garment and must purchase any additional ones they need.

Hospital Arrival

On arrival at the hospital you will directed to the reception area where you will then completed your Patient forms.

You will also need to present your passport for a copy to be taken. Once all forms have been completed, you will then be taken to the Plastic Surgery reception area on level 1.

At this point you will be checked in with staff and await your appointment time with your surgeon.

Although all efforts are made by the surgeons to stay on scheduled there could be times when you will be delayed and we request that you do remain patient.

There are computers with internet access for your use whilst you are waiting. You can also request codes for the Wifi access from the reception desk to use with phones, ipads and laptops.

Take this time to call home and let your love ones know that you have arrived safely. Time gets away on you quickly and we don't want you to forget and have them worry unnecessarily.

This is also a great time if you have not already done so to prepare a list of questions that you wish to ask and discuss with your surgeon.

Hotel Pick Up

On day of surgery you will be collect from your hotel lobby. Your pick up time will be listed on your treatment plan.

This is normally 30 minutes prior to you scheduled appointment time.

Ensure that you have packed your overnight bag the night before so that you are ready to go on the morning of your appointment.

Do not eat anything for 8 hours prior to scheduled appointment.

Meeting your Surgeon

Your Surgeon will review all of your medical transcripts and conduct additional tests to ensure that you are ready for your procedure.

Surgeon will fully explain your intended surgery proceedure as well as any options that are availble to achieve your results.

We suggest that prior to this appointment you put together a list of questions that you may want to ask your surgeon. Often clients get caught up in the moment and forget to ask things they wanted to know.

If having Breast Augmentation you will have the opor-tunity to try the various implant sizes, shapes and profiles to visually see your results. The surgeon will provide his input in what would best suit your situation and together you will make the final decisons on size, profile, type and placement of the implant prior to you proceeding with your surgery.

You will then also be informed of the final cost of surgery and if acceptable you will then be taken to make payments. Further information on payments in following pages

Once you and your surgeon have agreeed to proceed you will then be taken to do your pre-op test. Your surgeon will explain the routine test that he requires and if there are any others he may need.

A nurse will escort you to have the test taken and then bring you back to the reception area to await being taken to the ward.

Once these are completed and you are clear for surgery to proceed you are then taken to your assigned hospital room to place your personals and await to be taken to surgery.

If you didnt call home whilst waiting for surgeon this is another good time to touch base with home.

Pre-Operative

Pre-Surgical

Pre-Surgical Tests is the standard hospital procedure. Prior to surgery, your surgeon will require you to undergo laboratory and other examinations to ensure that you are fit for surgery.

In most cases unless otherwise informed you are scheduled for surgery on same day as your appoint-ment.

Please ensure that you discontinue taking any aspirin and vitamins at least two weeks before your appoint-ment date. As they will increase bruising and may even cause internal bleeding.

Assume that you going to be having surgery under a general anesthetic on your appointment date, please do not drink or eat for at least 8 hours before your confirmed surgery or appointment time.

If you are on prescription medication you must discuss this with your surgeon and advise them of your medical history. If you are unsure if you should cease taking any medication before your appoint-ment or surgery, contact Meditravel for clarification.

If your medical history warrents it you may also be required to undertake additional diagnostic test . These will be requested by your surgeon on case to case basis (fees may apply for additional testing)

Money MattersAlthough you are having surgery your recovery period is a forced holiday too,.Most of us don't go on our holidays to be tight with our cash, but there are a thousand better things to spend your money on than fees and charges. The choices we all have today are so much better then when we had to use travelers cheques and cash.

These day's we have the availability of debit and credit card facilities just about everywhere we go, the choice is up to you. Whether you use a credit card, debit card or travel cards or cash, you should be aiming for a card that gives you the least amount of hassle combined with the lowest fees and charges.

In Thailand for shopping you will primarily use cash as you will be bartering for the lowest price. Credit cards can be used in Hotel, Restaurants and high end and retail shops and outlets in the large shopping centres.

Look for a card with low currency conversion fees and easy access to one currency. But since you will be traveling to Thailand you'll want to load a travel money card with multiple currencies at a set exchange rate - you can save on currency conversion fees by loading up to ten 'currency buckets' at a time.

Cash

Carrying cash comes down to your choice, just make sure that you keep it in a secure location when traveling and in the hotels security safes when your out exploring.

When exchanging AUD money for Thai currency make sure you check out 2 or 3 exchange booth’s, as you will find they all offer a different exchange rates. As currency rates change throughout the day and from day to day only exchange what you need for a few days shopping and eating.

ATM’s

You will find ATM’s in all your shopping areas and at most 7 Elevens stores. Nearly all currency exchange booths will also offer an ATM service.

Personally if I am going to use an ATM I would usually go into one of the many banks, usually in the major shopping centers and withdraw a lump sum of Thai currency to avoid all those dreaded bank fees. You can then store in your hotel safe and take with you what you will need for the day.

Banks

Banking hours usually start from 08:30 am to 3:30 pm from Monday - Friday. Some small branches are located in the shopping malls and are open from 08:30 am to 7:00 pm, seven days a week.

Most major banks have branches in Phuket City and Patong, as well as a few sub-branches dotted around

Exchange Rates`The exchange rate between two currencies is the rate at which one currency can be exchanged for another.

If you see the Thai Baht (THB) is at $0.033, this refers to the exchange rate between the THB and another currency (ie the Australian Dollar). In this case $1 AUD will buy you $30.31 THB

To quickly do a conversion when shoping if you know the exchange rate is .033 and they are telling you something will cost 150 THB Calculation method .033 X 150 = $4.95 AUD

There are many apps that make this easy for you to do on your phone so load one before you leave. You can alsop always borrow store owners calculator they all have them.

Most time they will not say aloud the offer and instead type into calculator and show you. You can then type in your counter offer. This is mainly done so people in the store or passing by do not know what price they sold an item to you.

Surgery /Dental PaymentsPlease be aware that all Surgery or Dental is charged in Thai Baht.

The hospitals require payment in Thai Baht currency and payment in full is required upon your hospital admission and certainly before your surgery

There are options in place for clients who may wish to make foreign currency money transferpayments to the hospital prior to arrival and at least two weeks prior to the appointment date. Banking details can be found below.

E-paymentsClients may make E-Payments at any time thought the hospital’s website. Payments are instant and available only when using a credit card. To perform a E-Payment, instructions can be obtaned by contacting your consultant who wll provide you with a detailed instruction sheet.

Personal Credit CardsHospitals and Dental Clinics in Thailand all accept major credit cards of Visa, Mastercard, Bankcard and American Express. Visa debit cards are also accepted.

If using a credit card for surgery please ensure that you have sufficient funds available.

ATM or Debit cards are accepted , however they generally are limited in the amount that you may withdraw at one time by your bank to 20,000 THB per day.

Meditravel or the Hospital/Dental are not responsi-ble for any additional fees that your bank may charge you when using a Visa or Debit card in

Every surgical procedure carries risk and you should discuss and understand the possible risks and compli-cations with your plastic surgeon so you are fully informed before surgery.

Surgery has risks and complications, so does anaesthe-sia. Also each procedure carries its own specific risks and complications. Your consultant would have already provided these to you or you can request them.

To reduce your risk, you must inform your surgeon about any medications you are taking and let them know your complete medical history. It is in your inter-est to fully disclose all of this information with your surgeon.

The risk of infection is small and antibiotics reduce this risk dramatically. However, if infection does occur, it can be very serious. People who smoke, take steroids or have certain vascular conditions are at greater risk. The longer your surgery lasts and the more blood you lose, the more likely you are to have an infection.

If the sutures come loose this can lead to internal bleeding or a hernia. Such problems would require additional surgery.

Usually follows an infection or haematoma and is much more likely among smokers. Additional surgery may be required, the skin excised (surgically removed) and this may affect the cosmetic outcome.

Moderate or severe asymmetries may require a second surgery. Mild asymmetry is normal.

Keloid’s are raised shiny dome scar and is more common in populations with darker skin tones. Candi-date who are prone to, or have a history of keloid scarring, should reconsider surgery and advise their surgeon. With candidates who have no history of keloid scarring, it may not present itself until after the surgery and is not a result of the surgeon ability, rather the predisposing condition of a candidate to this type of scarring and their skin condition.

Some patients have serious reactions to the anaesthe-sia or sedation used during surgery. Most anaesthetic complications occur with general anaesthesia. Patients who have heart trouble, lung disease or are obese are at greater risk of complications due to anaesthesia.

Usually associated with age, skin type, and failure to follow doctor's advice or factors beyond anyone's control.

Is often temporary, however sometimes there is permanent loss of sensation. This results from injury to sensory or motor nerves. Seroma Fluid can collect under the skin and can occur after breast augmentation, liposuction or a tummy tuck.

Bleeding is normal with any procedure and is usually minimal. However, if there is excessive bleeding, it can create major complications. If this occurs during surgery, your plastic surgeon and anaesthesiologist will be aware of by pooling blood or by a blood pressure drop. If bleeding occurs after surgery, it can accumulate under the skin causing bruising and may require additional surgery. You should follow your surgeon’s instructions following surgery to minimize the possibility of post-operative bleeding.

Anaesthesia can sometimes irritate air passages, causing the vocal cords to spasm and this can block the airway. The anaesthesiologist may need to insert a tube down the throat or cut into the windpipe.

Airway Obstruction

Some patients take longer to heal than others may , particularly people with more than one illness. A patient with a chronic illness, an immune system problem, or sickness in the weeks prior to surgery may have a lengthier hospital stay and a more difficult recovery period.

Diabetics who have surgery typically have a longer healing time, especially if blood sugar levels are poorly controlled. For this reason, diabetics must carefully weigh the risks and rewards of having surgery, including the potential complications during recovery

There is a vast supply of ATM’s (automatic teller machines) available throughout Phuket accepting most major cards from the major international networks such as Cirrus. Check the symbol on the back of your ATM card against the symbols on the ATM machine. However transactions fees can be considerably high and it is wise to check with your bank prior to departure.

Maximum withdrawal amount per day is usually 20,000 baht. Rates of exchange at banks and any authorised money exchanges are generally more competitive than those offered at hotels and retai outlets.

Money can be easily changed at currency exchange counters on retail strips such as Patong, Kata and Phuket City. Always ask in advance how much Baht you will receive for $x and count it back carefully. Some money changers are very quick and slightly alter their first offer in their favour if you are not being attentive.

Credit card Major credit cards are accepted in most retailers and restaurants. (Visa, MasterCard, Amex and Diners)

Phuket International Airport is a 40 minute drive from Patong, Kata and Phuket City tourist areas. DMT Client will have pre-arranged transfers so will not need to worry about the barrage of taxi drivers offering their services.

Many taxi’s in Phuket do not use their meters often preferring to charge a set rate, however this is often more expensive than metered taxis. Taxi’s should charge by distance travelled not how many people in vehicle, always negotiate payment with driver before choosing a taxi. As a guide the rate for a ride from one point in Patong to another is generally 50 Baht and from Patong to Phuket City is around 350 Baht.

Transport/Taxi’s

Thais don't normally shake hands when they greet one another, but instead press their palms together in a prayer-like gesture called a wai.

Thais regard the head as the highest part of the body and the feet as the lowest, both literally and figurative-ly. Therefore, avoid touching people on the head and try not to point your feet at people or an object. It is considered very rude. Shoes should be removed when entering a private Thai home and some places of busi-ness. Overt public displays of affection between men and women are frowned upon, much as public displays of anger are.

Phuket Travel Information

If you are planning a short visit in Thailand you may not require a Thailand visa if you are citizen of one of the approximately 40 nations that qualify for a Thai visa waiver or Thai visa exemption. With evidence of onward/return travel, citizens of Australia, New Zealand, Canada, the USA, and most European nations may enter Thailand for up to 30 days without a Thai visa.

These Thailand visa waivers are issued upon arrival in Thailand. Those entering Thailand by land may not receive a 30 day exemption however; the Thai visa waiver was reduced to 15 days for those entering by land after January 1, 2009.

International departure taxes are now included in the cost of air tickets when they are purchased from an airline or travel agent. There are no official domestic departure taxes, although privately owned and some operated airports, such as Koh Samui Airport, has levied for a small domestic “departure tax” in the past and may continue to do so depending on their current policies.`

Thailand customs allows visitors to enter Thailand with personal effects, the value of which does not exceed 80,000 Baht, without paying import fees as long as:

1) the items are specifically for your personal or professional use; 2) the amount of goods are reasonable; and 3) the items are not subject to restriction or prohibition.

There are restrictions on the amounts of alcohol and tobacco products; only the following amounts may be transported into Thailand under the current duty free regulations:

250 gms of cigars or tobacco, or 200 cigarettes

1 litre of wine or liquor.

General AnesthesiaMay be given as an inhaled gas or as a liquid administered intravenously. You will be completely asleep and unaware during general anesthesia. Anesthesia carries risks, although local anesthesia and sedation carries fewer risks than does general anesthesia. However, there are risks of allergic reactions to the drugs being used, even if you had anesthesia safely in the past. You must inform you surgeon about any underlying health conditions you have, such as allergies, asthma, and lung, heart or circulatory conditions, because these can increase your risks.

There is also the risk of an interaction between any of the anesthetic drugs being used and any medications or herbal products you take. Make sure you tell both your surgeon and whoever administers anesthesia of all the medications you take, including over-the-counter drugs, any vitamin supplements, and herbal medicines. You may not need to stop taking every medication you currently take, however you do need to make sure that your surgeon knows about them all. Some medicines can raise your risk of problems too. Make a list of all the prescription and over-the-counter medicines you take. And share your list with your doctors.

If you smoke, drink alcohol, or use illegal drugs, you may be more likely to have problems from anesthesia. It's important that you are honest when you talk with your surgeon and anesthesia specialist

Special Medication Alerts!

If you are on Anti-depressants, please advise your doctor. Some monoamine oxidase (MAO) inhibitors (also known as MAOI) intensify the effects of the anesthesia - especially General. This could be quite dangerous in the operating room if your doctor is unaware of your medication usage. If you advise your doctor he or she can make adjustments for your anesthesia or at least will watch for the slightest decrease in heart or breathing rate.

These medications may include: Isocarboxazid, Marplan, phenelzine (Nardil, Nardelzine) tranylcypromine (Parnate, Sicoton), Deprenyl, selegiline hydrochloride, 5 HTP, Amitripty-line, Prozac, Wellbutrin, Buropion, Buropion HCL, Fluoxetin, EFFEXOR® XR, etc.. They are used for the treatment of depression, obsessive-compulsive disorder, eating disorders, essential hypertension (pargyline), chronic pain syndromes, and migraine headaches. They work by inhibiting nerve transmissions in brain that may cause depression. Tranylcy-promine and phenelzine account for over 90% of all MAO inhibitors currently prescribed. It is reported that drug interactions can occur even weeks after discontinued use of an MAOI. Therefore, in patients undergoing General anesthe-sia, cessation of usage is normally instructed several weeks prior to surgery to avoid possible cardiovascular effects.

Diet Pills, Fat Loss Supplements & Stackers: Please, Please, Please! stop taking these pills at least 2 weeks before surgery. Many of these pills contain anticoagulants and can seriously impede your bodies ability to clot sufficiently resulting in hemorrhaging.

Vitamins & Supplements

Other Supplements and Herbal Remedies Although many of you may take herbal supplements and some are good but MANY are contraindicated with anesthesia or surgery in general. Some reasons why you may not wish to disclose your herbal consumption information to your surgeon or anesthesiologist may be because you believe that since these supplements are over the counter (OTC) they must be safe to use. This isn't true - may of these are contraindicated and can severally hurt you when used in conjunction with anesthesia, other medications or while undergoing surgery. You may also think that because these products are safe - Most medications, supplements, dangerous drugs and poisons are made from plants. So please remember this. You may also think that telling your surgeon or anesthe-siologist would be embarrassing - especially if you shouldn't be taking them, if you are taking them for weight loss, increase your sexual stamina, for acne or for depression. This is your life - don't mess around! There is nothing embarrassing about any of this.

Vitamin E is the number one no-no. It has anti-platelet properties and inhibits vital clot formation. Please cease consuming Vitamin E at least 2-3 weeks prior to any surgery and for 2-3 weeks afterwards unless specifically instructed by your surgeon.

Garlic (allium sativum), Ginger, Alfalfa, Cayenne, Papaya, Feverfew, Chamomile, Dong Quai root, Willow bark, Goldenseal, Guarana, Horse Chestnut and Bilber-ry Tablets/Supplements, have anti-platelet properties and may inhibit vital clot formation. These medications are normally taken to remedy edema (fluid retention).

Gingko, Gingko biloba, & Selenium are powerful anti-coagulants. It is considered to be 3 times stronger than Vitamin E, which should also be avoided.

Ginseng may cause rapid heartbeat/and or high blood pressure in some individuals as well as coagulation disruption.

St. John's Wort, Yohimbe, ("The natural Viagra®") and Licorice root have a mild monoamine oxidase (MAO) inhibitory effect and may intensify the effects of anesthe-sia. (*note some well known and popular anti-depres-sants are MAO inhibitors, disclose any and all medica-tions you are taking - your life may depend on it!)

Melatonin decreases the amount of anesthesia needed for surgery.

Kava Kava, Hawthorn, Lemon verbena, Muwort, Laven-der and Valerian/Valerian Root: (taken to promote relaxation and sleep) have tranquilizing properties and should not be combined with other sedative agents. WARNING! Tell your anesthesiologist/surgeon if you take any one of these

Echinacea may have a severe impact on the liver when general anesthesia is used. Please advise your surgeon of all medications and supplements and alert him to the possible effects of herbal supplements and remedies, he may not be aware of the contraindications. it also has some coagulation disruption properties.

Ma Huang (aka Ephedra), : (used by many for weight loss and is a major component of most weight loss supplements) has been associated with more than 800 adverse health effects including DEATH. It is an amphet-amine-like compound with the potential for stimulating the central nervous system and heart.

WARNING! we strongly discourage the use of this supple-ment in any form." In large doses ephedrine causes nervousness, headaches, insomnia, dizziness, palpita-tions, skin flushing, tingling and vomiting (Lawrence Review, 1989). The Handbook of Nonprescription Drugs notes that "The principal adverse effects of ephedrine are CNS stimulation, nausea, tremors, tachycardia [rapid heartbeat], and urinary retention." (APhA 1986).

Gotu Kola has a stimulant effect as well.

Communicating

Most people in Phuket will understand at least a few words of English and you might get by without speaking any Thai at all. Thai children learn English at school, and the ability to speak English is regarded as an important qualifi-cation for a large number of jobs. However, if you venture off the tourist trail, communication can become quite difficult. In any case, making the effort to learn a little Thai will help endear you to the local people you meet and may come in handy in an emergency.

Polite Particles

At the end of sentences males use the polite particle 'khrúp', and females, 'kâ'. These can also be used to answer in the affirmative, or negative if used with the negation mâi, i.e. mâi khrúp or mâi kâ. Although the sentences below only show them at the end of the first few examples, it is considered good manners to use these at the end of just about any sentence.

Sa-wat dee (hello)Used for both hello and goodbye.

Khop koon (thank you) How to show your appreciation once you’ve been handed your Singha beer and pad Thai.

Gee baht? (how much) Essential for all the shopping you’ll be doing, or when haggling with tuk-tuk drivers.

Yoo tee nai...? (where is...) Invaluable when lost! Just point at a place on your map or in your guidebook and say ‘yoo tee nai’. Also useful when you are trying to find the right bus to go to Phuket or otherwise.

Mai ow (don't want) Useful for fending off persistent street vendors.

Khor tort (sorry)Stood on someone’s foot at the Chatuchak market? Now you can apologise.

Neung, song, saam, see, haa, hook, jet, baat, gow, sip (one to ten)Impress the locals by reciting one to ten.

Lot noi dai mai? (can you make it cheaper?)If you are doing a lot of shopping then try this handy phrase. By speaking a little of the Thai language you might just get a better discount.

A-roi (delicious) Show your appreciation for that delicious Thai meal! You never know, if you go back to the same place you might get an extra helping.

Mai pen lai (never mind)A common phrase in the Thai language – if someone apologises to you (khor tort) then reply with ‘mai pen lai’. If someone thanks you (khop koon) say ‘mai pen lai’. If there’s a mix up (which invariably happens with a language barrier) have a chuckle and say ‘mai pen lai’!

Phuket Weekend Market (Talad Tairod) Phuket TownThis busy market is as much a social event as a commercial one so be prepared for lots of smiles and bartering. For sale are jeans, T-shirts, custom jewelry, artificial flowers, DVDs, sports shoes and a lot of secondhand goods. And of course a Thai market would not be complete without lots of delicious snacks.

Bargaining in ThailandMost department stores and large retail outlets have fixed prices, so bargaining is not applicable. At markets, small stalls, shops or street vendors, bargaining is the norm. Expect to pay from 10-40% less than the original asking price - depending on your bargaining skills and the will of the vendor.

Bargain with a smile and you will be rewarded. Many Thais work long hours, so they appreciate a smile and good humour - you'll get better bargains. Just remember, sometimes you are squabbling over cents, is it really worth doing that?

The best way to get a bargain is never lose your cool or raise your voice, always bargain with a smile and a few words of Thai will not go astray and your attempts will be much appreciated.
