Medication Calculation Sample Assessment (Source of information: Springhouse Nurse’s Drug Guide 2006) General Instructions: Show all work/calculations. Each answer must be expressed in the proper units per measure asked for in the question (e.g. tablets, ml, mg, etc.). Do not simply write “yes” or “no” when asked if a dosage is therapeutic. Show the calculations that support your response. Rounding Considerations: During the process of using a calculator to compute calculations, do not round. After completion of all calculations, rounding is done to determine the final answer. For adults, round to the tenths place. For children, round to the hundredths place. For tablets, round to the nearest tablet. Recommendation: Do the assessment once on lined paper. Set it aside. Do it again on a different sheet of clean, lined paper. Then, compare both versions of your work. Revise any errors that you detect. Submit both versions indicating which you would like Katie to evaluate. 1. A 42 year old 154 pound female patient with acute maxillary sinusitis is prescribed levofloxacin 500 mg in 275 mL I.V. to be infused over 1.5 hours. The drop factor for the tubing is 60 gtt/mL. What is the flow rate? 2. Your 45 year old 136 pound female patient with a thyroidectomy is prescribed a maintenance dosage of levothyroxine sodium 200 mcg P.O. daily. This drug is available in tablets of 0.075 mg. How many tablets should the patient take each day? 3. A 42 day old 9 lb 3 oz female patient suffering from nosocomial pneumonia is prescribed linezolid 10 mg/kg P.O. q.8.h. Linezolid is available in an oral suspension 100 mg/5mL. What will you administer to your patient in each day? 4. A 12 year old 91 pound 8 ounce patient with chronic diarrhea has an order for loperamide 2000 mcg P.O. t.i.d. You refer to the drug guide and find that loperamide is recommended as 0.08 – 0.24 mg/kg/day P.O. to be given in three divided doses. This drug is available in an oral liquid 1 mg/1 mL. Is the order safe? If so, what will you administer to the patient in each dose? 5. A 54 year old 157 pound post‐surgical patient is prescribed meperidine hydrochloride 112.5 mg S.C. q.4.h. p.r.n. for pain management. It is available as 50 mg/mL. What will you administer to the patient each dose? 6. A dehydrated patient is receiving maintenance fluids of NS at a rate of 300 mL/hr. At 10:45 a.m. you check the bag and there are 0.975 liters left in the bag. In how many hours will you need to hang a new bag? At what time will this be?

Medication Calculation Sample Assessment(Source of information:  Springhouse Nurse’s Drug Guide 2006)

General Instructions:  Show all work/calculations.
