    How do you document?

  • Scenario #3Your house manager asks to you to check the date on the medications.

    What are you looking for?

How do you go about this?

    The Six Rights.

These are the most important thing you will take away today.

Right medication how do you know this?

Right dose how do you know this?

Right manner/route how do you know this?

    Right time

    Right person

Right documentation What does this mean to you?Wash your hands or use hand sanitizer. Wash them between each resident.

The handbook says you check the label on the medication and compare it to the MAR three times. And those three times are 1. Before removing the medication from the storage area 2. Before placing the medication in the medication cup. 3. Again, before placing the medication back into the storage area.

Make sure you know what the medication is. 1. Know what it is 2. Know what it is given for 3. Know the intended effect 4. Know the possible side effectsResident has to be in the right position.Resident cant have their head hyperextendedCant be lying downHow do you want them to be if it is an ear medication?

Safety Considerations. What do you think would be safety considerations?Dont leave medication lying around.Only prepare one residents medication at a time.

TimingWhat is crucial for timing? Some medications need to be given with food.Do not give laxatives within 1 hour of other medications.Give most GERD drugs 30 minutes prior to a mealDont give iron with milk productsOral drugs given by mouth. Ensure that they are swallowed.Topical skin medications. Put on with gloves. For anyone who rubs eyes or bites themselves, what are the considerations?

    Sublingual What does this mean?

Eye drops put eye drops in before you put in eye ointment

    Ear drops

    Rectal

    Vaginal -

    Nasal - When and How

    Use your initials.

Initial when you take the medication out of the container.

    Initial bubblepaks

PRN Follow up within 2 hoursHere are the three codes we use at CCS.

So:How do you chart if you are holding a medication or treatment per instructions from RN or Physician? What do you do when the client is at work? How do you chart once you have given the medication? How do you chart when the medication is due and the client is away from the home? How do you chart when the client refuses the medication? How do you chart when you give the medication late per the late medication protocol?

Give me some other situationsClients name both first and last

    Name of medication

    Dosage of medication

Route: PO=oral SL=sublingual pr=per rectum per g-tube=by the gastrostomy tube OD=right eye OS=left eye AD=right ear AL or AS=left ear

    5. How often and/or how long? QD QHS BID QOD TID AC PRN

If it is an antibiotic, need to have how long Needs to be a definite length of time not Q2-3 hours

    6. Why is it being taken?Any out of date or expired medications need to be disposed of

Need to document what ever you do. For non-controlled medications If it is two unlicensed staff, one needs to be manager or asst manager For controlled medications one needs to be a nurse

If it is an unopened bottle of liquid medicine, try returning it for credit to Olsons. They wont take bubblepaks back. They also wont take narcotics back.

Give me some examples of controlled medications.

Where do you dispose of the medication? It is important that you know where the information about the client is. Where are the medical books in your home? If you work in more than one home, where are they in all the homes?

You have an obligation to know about the medicines the clients are getting. When did you do your med assignment? Have their been new medications since then? Have you looked each one up in the drug book and could tell me the major side effects of them?

Rights of clients around medicines right to refuse right to choice right to privacy right to confidential communication right to adequate medical care right to receive information about their medication and all treatmentsHow will you prepare the medications?

    How will you chart them?
