Medical Travel Benefits (MTB) Covers contractually eligible expenses to obtain care that

is medically necessary and unavailable locally Available to members and dependents Funded an ASO basis

Eligible Travel Medically adequate treatment is not available locally Travel must be 100 kilometres from the member’s home

campus Patient referred by a licensed physician to a hospital,

medical treatment centre or medical specialist Where there are multiple trips to the same specialist a new referral

is required once every 12 months

Ineligible Travel Travel to other types of practitioners such as, but not

limited to, chiropractors, physiotherapists Charges for travel incurred outside the covered person’s

province of residence, unless such expenses are lesser than those incurred in the covered person’s province of residence

Travel for Dental unless: such services are required by a licensed Physician and/or when

hospitalization for treatment is required; and such treatment is performed by an oral surgeon, except in the case

of emergency dental assessment or treatment, in which case treatment may be performed by a specialist in the field of dentistry.

Coverage Dollar Limits Overall maximum - $10,000 per year Benefit Amount - $125 per day, to a maximum of 50 days in any

calendar year Combines meals, accommodation, attendant and transportation*

Averaging provision Where eligible expenses exceed $125 per day, but do not exceed the

average of $125 per day for the year, the average will be paid. For example, if the member incurred expenses of only $100 per day for a two-day trip and then later in the year incurred expenses of $200 per day for another two-day trip, the member could claim a total of $300 for the second trip (two days at $125 plus $50 ‘leftover’ from the first trip.)

*transportation costs are not included in the daily maximum for Northern Lights and College of New Caledonia

Covered Expenses Transportation to and from the treatment facility

Scheduled air, train, bus, taxi, ferry, auto (per/km limit) Cost of car rental only covered if less than the cost of taxi Parking

Accommodation before and after medical treatment including commercial facility or hotel, Easter Seal House, Heather House,

Vancouver Lodge, Ronald McDonald House, or other similar institution

Meals Allowable amounts/maximums vary by policy Groceries allowed in lieu of restaurant receipts

All expenses require a receipt unless specifically noted otherwise in the policy

Medical Attendant Charges for transportation of a family member or a medical

attendant if medically necessary and requested by a licensed Physician

— Referral required for all patients 18 or older (assumed if the patient is under 18 years of age)

— Charges are counted towards the patient’s $125 maximum— If the attendant is also eligible for coverage, expenses are still counted

against the maximum for the patient, not for the attendant. For example, if member and spouse are both eligible for MTB. Spouse accompanies the member, all expenses will be processed against the member’s own $125 maximum

When making a claim Plan member completes sections 1, 2, 3 & 7 Have referring physician complete section(s) 4 and/or 5
