Meaningful Use Stage 1 What is Meaningful Use? Meaningful Use is a government incentive program to encourage care providers to switch to an electronic health record (EHR) or electronic medical record (EMR) system, and use it in a way that leads to more efficient, streamlined patient care. By attesting for Meaningful Use objectives, you can receive payments of up to $44,000 over a four-year period. Stages of Meaningful Use There are four stages, and we are currently in Stage 1. Stage 2 is set to begin in 2014. Attestation for each stage takes place over a two-year period. In the first year, providers must attest for a 90-day period, while in the second year, they must attest for an entire year. October 3, 2013 is the last day you can begin your 90-day attestation period for Stage 1. You cannot move to subsequent stages without first attesting for the previous stage. For example, if you’re still attesting for Stage 1, you must finish before moving on, even once Stage 2 requirements are in effect. What are the incentives? For years 1–4 , you can receive up to $18,000, $16,000, $12,000, and $4,000 respectively. The incentive payment you receive depends on Medicare payables for the year. You receive 75% of these payables. What are the penalties? In 2015, any providers who choose not to participate in the incentive program, or who cannot meet the requirements, will be penalized. If you don’t meet Meaningful Use by then, you’ll receive a 1% deduction in your payback. For each following year, if you don’t meet Meaningful Use, you’ll receive an additional 1% deduction. In total, you could lose up to 5% in your payback from Meaningful Use. Stage 1 breakdown For Stage 1, you must attest to 14 core objectives, and 5 of 10 menu objectives. Most objectives have a numerator and denominator, and a threshold you must meet for each measure. You may be excluded from some of these measures, depending on the data present during the reporting period. In addition, you must attest to 6 Clinical Quality Measures (see core objective 10 for more). NueMD has everything you need to meet all measures, so keep reading to see how you can increase your profits while fostering greater patient care.

Meaningful Use Stage 1What is Meaningful Use?Meaningful Use is a government incentive program to encourage care providers to switch to an electronic health record (EHR) or electronic medical record (EMR) system, and use it in a way that leads to more efficient, streamlined patient care. By attesting for Meaningful Use objectives, you can receive payments of up to $44,000 over a four-year period.

Stages of Meaningful UseThere are four stages, and we are currently in Stage 1.

Stage 1 breakdownFor Stage 1, you must attest to 14 core objectives, and 5 of 10 menu objectives.

Meaningful Use Stage 1 | Core Objectives 3 of 27

Core 1: CPOE for medication ordersComputerized provider order entries electronically cross-check prescribed drugs with other patient info, like allergies or lab results, and alert you to potential problems. This reduces errors before ordering medications, ensuring patients don’t run into any adverse reactions. NueMD interfaces with Emdeon’s Clinician product to bring you this service.

Objective: Use computerized provider order entry (CPOE) for medication orders directly entered by any licensed healthcare professional who can enter orders into the medical record per state, local, and professional guidelines.

Measure: More than 30 percent of all unique patients with at least one medication in their medication list seen by the EP have at least one medication order entered using CPOE.

Exclusion: Any EP who writes fewer than 100 prescriptions during the EHR reporting period.

Numerator: The number of patients in the denominator that have at least one medication prescribed using eClinician by the ordering provider.

Menu 5: Patient electronic accessInstead of making patients wait until their next visit, give them electronic access to key health record details so they can see them any time. With their health info only a click away, patients will be more likely to make better lifestyle decisions.

Objective: Provide patients with timely electronic access to their health info within 4 business days of the info being available to the EP.

Measure: At least 10 percent of all unique patients seen by the EP are provided timely electronic access to their health info subject to the EP’s discretion.

Exclusion: Any EP that does not order lab tests or create other info that would be contained in the problem list, medication list, or medication allergy list during the EHR reporting period.

Numerator: Number of patients in the denominator who have timely electronic access to their health info online.

Denominator: Number of unique patients seen by the EP during the EHR reporting period.

Workflow:

In the Patient Portal

1. Go to Practice Setup > Portal Setup > General tab.

Menu 7: Medication reconciliationToday, it’s common for patients to change locations frequently, or visit specialists for specific problems. With new patients constantly moving in and out, as well as changing pharmacies, chances are, you don’t know their medication history right off the bat. Having a complete electronic list that’s easily updated, however, fixes this problem, and prevents you from prescribing medications they’re currently taking, or stopped taking.

Objective: The EP who receives a patient from another care provider or believes an encounter is relevant should perform medication reconciliation.

Measure: The EP performs medication reconciliation for more than 50 percent of transitions of care in which the patient is transferred into the care of the EP.

Exclusion: An EP who was not the recipient of any transitions of care during the EHR reporting period.

Numerator: Number of transitions of care in the denominator where medication reconciliation was performed.

Denominator: Number of transitions of care during the EHR reporting period for which the EP was the receiving party of the transition.

Workflow:

Check medication list

Sending it electronically in standard protected formats ensures a safe and speedy arrival, and lets the new team pick up where you left off.

Objective: The EP who transitions their patient to another care provider should provide summary of care records for each transition or referral.

Measure: The EP who transitions their patient to another care provider provides a summary of care record for more than 50 percent of transitions of care or referrals.

Exclusion: An EP who does not transfer any patients nor refers a patient to another provider during the EHR reporting period.

Numerator: Number of transitions of care and referrals in the denominator where a summary of care record was provided.
