Logging on to UCCS’s Sports Medicine Injury Database
These are directions to start utilizing the Athletic Trainer System. Our Sports Medicine Staff will
be using this system to keep track of athlete’s medical histories, emergency contacts and other
pertinent information that might be needed in case of emergency while participating at UCCS.
The Sports Medicine Staff we also use this system to track injuries for our athletes, report
treatments and rehabilitation plans as well as contact the student-athletes if needed. We feel
that this system will help keep track of our athletes throughout their tenure at UCCS.
1. This system can be logged on to from any computer with internet access. You just need
to go to
https://www.atsusers.com. All
information will be inputted
directly into UCCS’s secure
database. The picture to the left
shows you what the screen looks
like.
2. Once at this page (https://www.atsusers.com) please click on the “ATS Athlete Portal”
button. This will take you to your private login page.
3. From here on out all the information that you see will be specific to you. Your “Athlete
ID” and “Password” will be
randomly generated and sent you
in an email separately. The
“Database” will always be the
same, it is “ATSUCCS”
4. Once logged on, you will be in your personal profile that allows you to add and/or change
any information that you want to. The
screen shot to the right is the main
menu. Please check to make sure that
you are the student-athlete listed at the
top.
5. At this point please enter the “Athlete Information” section.
6. This will take us to the
“General” information tab. The
yellow highlighted areas are
mandatory to complete. Below
are some suggestions.
General Information – Please
complete as much as possible, even
include a picture if you would like.
Addresses – The “Primary Address” is
you home address and the “Alternative
Address” is your school address.
Password - This is also where you can
change your password. I highly suggest
changing your password first, preferably
to something that you can remember
and that has an arrangement of letters,
both small case and capital, and
numbers.

7. Once again please make sure that you are in your personal profile, if not please log out
by pressing the button on the top right that looks like a door.
8. Once you have completely filled out all the information under the “General” information
tab please click “Save” at the bottom of the page.
9. Now you may click on the
“Medical History” tab at the top
of the page. Please complete as
much information as you can,
regarding any surgeries that you
may have had. This is for general
information; more specific
information will need to be
provided in the “Athlete Forms”
section. You will be asked
questions related to your injury
history in more detail later.
10. Once you have completed this section click the “Save” button at the bottom of the page.
11. Now enter the “Insurance” tab. To enter a new Insurance carrier, you must click on the
“edit” in the “Actions” Column.
Please fill out the information as
completely as possible. Please DO
NOT enter the primary insurance
holders social security number in
its entirety. If you feel
comfortable the last four digits
are appropriate. Also if you don’t
see your insurance company
please make sure you have
checked all the possibilities before
you “Add (an) Insurance
Company” at the bottom left side
of the page. ALSO, please have a
copy of the insurance card that
covers the student-athlete at
physicals, we will scan and enter
those into the system as well. If
you are a returing student-athlete please bring yours to the first team meeting, a member of
the Athletic Training Staff will also scan them into the system and return them to you.
12. Once your insurance information has been completed, please click the “Save” button. If
you have an additional insurance company that you would like to add, you may do so by clicking
the “Edit” in the “Actions” column above the information you already entered.
13. Now you may click on the “Contacts” tab. Here you will fill out your emergency contacts.
We ask for at least TWO
emergency contacts. Please fill
out the information as completely
as possible. Click “Save” once you
are done entering the
information. If you would like to
add additional emergency
contacts click the “Edit” in the
“Actions” column above the
information that you already
entered. Again, once you are done entering all the information please click the “Save” button at
the bottom of the screen.
14. Please click on the “Athlete Forms” tab. This will take you to the screen shown to the
left. Click on the dropdown box
labeled “Form Name” and select
the “Pre-Participation” form and
then press “New”. During this
process we have updated a couple
of items. First, there is a signature
box at the bottom of the form.
This must be completed for the
form to save. Also once the
“Save” button is pressed if you
have missed a question a box will appear with the name and number of question that needs to
be completed. And finally, this information is in a big file, so please have patience after pressing
the “save” button, it might take a minute to save and then another box will appear that states
the data has been saved. If you don’t wait for that button and press “save” again the form will
save again and we will have multiple forms for the same time and date. Please complete this
form and it will point you to others from the dropdown list.
15. Once done with the “Pre-Participation” form there are several other forms that need to
be completed.
Some of your
answers will point
you to answering
another form, such
as the Concussion
form. If you
answer “Yes” to ever having a concussion then in the section above it will ask you to fill out the
“Concussion” for listed under the “Form Name” dropdown box.
16. Here is a list of mandatory forms that need to be completes, once again there might be
other that you need to complete based on your specific medical history (see information above)

18. Please fill out the appropriate forms as completely as possible. Pay attention to the
questions. Some will ask you a “Yes” or “No” question; if “Yes” you will have to explain in the
box provided on the right.
19. This is how the “Pre-Participation” form looks when you start it.
20. When you have completed all of the questionnaires pertaining to your medical history
click the icon on the top right side of the screen. It looks like a computer screen and it will take
you back to the main menu.
21. Also in the electronic documents on the main menu, there are various documents
pertaining to NCAA
recommendations on ADD/ADHD,
NCAA Sickle Cell Trait information
and testing protocol, athletic
training room rules and
regulations, excess insurance
Policy and claim procedures, these
directions, etc. This section is
constantly being updated with
pertinent information for the
student-athlete. There is even a
section that they can upload
documents if needed, i.e. medical bills, physician notes, insurance cards, etc.
*Remember your user name and password so you can log on and update your information as
needed.
