    Riveted built-up box girder floorbeams exist at UD PP 17 and 17'. The box girder floor beams exhibit

    moderate surface rust and section loss of top flange and rivet heads, 10% and 25% respectively. There is

    debris from open joint on the top flange of the floorbeam.

    1 113/1 Paint StlStringer

    The paint has failed and is flaking on the interior upper deck stringers. In locations of expansion joints the

    stringers have moderate surface rust with minimal section loss. At some of the splices along the exterior

    stringers, pack rust is forming along the edge of the bottom flange splice plate.

    1 121/1 P/Stl ThruTruss/Bot

    There is pack rust occurring at the lower chord, particularly on the splice plates, filler plates, and cover

    plates. There is also pack rust along the edges of the lower chord. This pack rust is causing the edge of

    the plate to curl-up. Minor to moderate section loss is occurring on the underside of the lower chord,

    particularly in the area of the cover plates and splice plates.

    The inboard and outboard gusset plates at the lower chord panel points are generally in good condition.

    There is surface rust and flaking paint on the gusset plates, and pack rust around the edges of the plates.

    1 126/1 P/Stl ThruTruss/Top

    The top chord members and the gussets plate connections to the verticals, diagonals, and lateral bracing

    are generally in good condition. The minor deterioration occasionally observed includes (1) surface rust

    and minor pack rust along the edge of gusset plates, (2) minor debris and surface rust inside of the topchord around the connection bolts for the top chord splices, (3) pack rust along the outside and inside

    edges of the top chord member, and (4) pack rust along the edges of the keeper plates.

    The truss members (including the vertical and diagonal members) typically have minor surface rust along

    the inside and outside corners of the boxes. Also, along the edge of the internal diaphragms, the truss

    members have minor to moderate surface rust, pitting, and pack rust. The corrosion is active in some

    areas, but the section loss in the primary members is typically between 1/16 and 3/16. There is pack rust

    noted up to 3/16 on the inside of the verticals, especially around the interior diaphragms. There is a

    2x0.75 hole in member L1-U1 upstream with a 3.5 diameter area around the hole going from 100%

    section loss to 0% of the 0.5 plate.

    1 152/1 Paint Stl

    Floor Beam

    The floor beams were observed in overall fair condition. The primary deficiencies noted were cracks at

    the floor beam connection to the truss. Of the fourteen total locations, two locations were observed at theend of the floor beam top flange angle at the connection to the truss, eleven locations were noted at the

    end of the floor beam in the T-connection at the connection to the truss and one 5/8 crack originating at

    rivet hole to edge of fill plate of FB16, UD, DS (see Appendix for table).

    1 205/1 R/ConcColumn

    Overall, the piers are in satisfactory condition. Minor cracks with minor spalls were observed for Piers I,

    II, and III. Pier IV exhibits heavy map cracking and delamination along bottom of pier cap and the full

    height of the upstream column, west face.

    1 210/1 R/Conc Pier Wall

    Minor cracking was present at the bearing areas.

    1 22/1 P ConcDeck/RigidOv

    The deck was noted in overall satisfactory condition and appears to be structurally sound. The bottom of

    the lower deck has spalling with exposed rebar adjacent at the majority of the deck joints. The bottom of

    the upper deck exhibited cracking with efflorescence adjacent to the deck joints at a few locations. The

    wearing surface of the upper and lower decks are in fair condition with minor spalling typically adjacent to

    the expansion joints along with transverse and longitudinal cracking throughout. Grooving in the wearingsurface is showing wear in the wheel path.

    1 301/1 PourableJoint Seal

    The upper deck joints have been repaired since the previous inspection and are in overall satisfactory

    condition. Lower deck joints are in overall poor condition. Primary deficiencies to lower deck joints occur

    on bottom of the deck (see 58.1) and leakage.

    1 303/1 AssemblyJoint/Seal

    Some of the expansion joints at the ends of the truss show a small amount of vertical misalignment. It is

    noted up to 1/2. The troughs under these joints are all filled or overflowing with debris.

    1 311/1 MoveableBearing

    The bearings show fair amounts of surface and pack rust throughout the truss, and heavy pack rust and

    active corrosion under the expansion joints. The bearings at the piers are in good condition.

    1 313/1 FixedBearing

    1 331/1 Conc Bridge

    Railing

    Vertical cracks in the concrete barriers are typical throughout the length of the bridge. The barrier have

    isolated areas of spalling.

    1 356/1 SteelFatigueSmFlag

    There are cracks on the T-connections, filler plates, and welds of the floorbeams. See appendix for crack

    sizes and locations.

    1 357/1 Pack RustSmart Flag

    There is pack rust occurring at the lower chord, particularly on the splice plates, filler plates, and cover

    plates. There is also pack rust along the edges of the lower chord. There is active pack rust between the

    gusset plates and the vertical faces of the lower chord.

    1 359/1 Soffit SmartFlag

    There is spalled concrete around the joints on the bottom of the deck with exposed rebar.

    1 604/1 2nd ElemDist

    As previously noted, fractured bolts and guide plates where present at U17-U18 and U17'-U18' members

    at panel point 17 and 17', respectively.

    1 606/1 Drains The deck drains are completely clogged with debris and vegetation. The downspouts are heavilycorroded, to the point of 100% section loss in some locations. The neoprene boots on the downspouts

    are worn and cracked.

    There is moderate debris on top of the floorbeams at locations of expansion joints. Also there is moderate

    debris on the cat walk at FB17 and FB17'. The lower chord and diagonal members were cleaned during a

    2010 maintenance cleaning project.

    Structure Notes

    *NOTE THAT THIS 2013 JOINT ANNUAL INSPECTION IN CONJUNCTION WITH ODOT WAS PERFORMED ON 09/16/2013.Joint annual inspection notes are as followed:-All needed repairs throughout structure were left in place for detail in the Inspection Notes Tab from last Fracture CriticalInspection dated 12/29/2010 (Entran, PLC).-Structure is in need of varying degrees of rehab construction soon (paint protective coating system, deck surface patching and/or total overlay project, expansion joint device repairs, under deck inspection walk repairs, etc.).

    -Deck surface is deteriorating along intermediate expansion joint device locations. Full depth holes/failures are starting to occur/develop in upper deck of structure at two locations near the north end of bridge as evident from view below. Area of deck alongexpansion joint at panel point #7 of the lower deck was found to have an area of full depth failure that was found during JAInspection, which was repaired same by state crew forces.-Under deck inspection walk system as well as the northbound lower most deck was accessed during time of Joint Annual withODOT. Clip angle connections supporting under deck inspection walk system at back side location of floorbeam element #16remains fractured at this time and are no longer operating as designed.-Protective paint coating system throughout structure was found showing failure, with worst locations being located along areasof expansion joint devices as well as structural members near traveled lanes of structure.-Broken light pole KY span.

    This inspection was limited to the "Joint Annual Inspection" with ODOT personnel accessing deck area throughout the lowernorthbound lanes and under deck inspection walk system through portion of structure and does not include Element Level or NBIRated portions for structure. This inspection was not a thorough inspection review of structure.For full detailed inspection report of structure see inspection report dated 12/29/2010.

    Inspection Notes

    ENTRAN,PLC on December 29, 2010 completed a fracture critical inspection of the truss bridge. The inspection included datagathering for both NBI and PONTIS element level inspection reporting.

    In summary, the Brent Spence Bridge is considered to be in overall fair condition. The following NBI condition ratings wereassigned to the structure: DECK - 6; SUPERSTRUCTURE - 6; PAINT CONDITION - 5; SUBSTRUCTURE - 7

    ENTRAN suggests the following maintenance recommendations: repair new and active cracks, replace the bolts and fracturedguide plates of the sliding pins at the upper chord, replace compression seals and repair damaged armored edges, remove rustfrom bearings at expansion joints, clean drains and drain pipes, clean troughs under expansion joints, replace the broken ormissing utility box covers, monitor pack rust and section loss thrhoughout truss, clean and paint the floorbeams and stringers,and repair spalls in deck and barriers.

    Floor Beam

    The floor beams were observed in overall fair condition. The primary deficiencies noted were cracks at

    the floor beam connection to the truss. Of the fourteen total locations, two locations were observed at theend of the floor beam top flange angle at the connection to the truss, eleven locations were noted at the

    end of the floor beam in the T-connection at the connection to the truss and one 5/8 crack originating at

    rivet hole to edge of fill plate of FB16, UD, DS (see Appendix for table).

    1 205/1 R/ConcColumn

    Overall, the piers are in satisfactory condition. Minor cracks with minor spalls were observed for Piers I,

    II, and III. Pier IV exhibits heavy map cracking and delamination along bottom of pier cap and the full

    height of the upstream column, west face.

    Minor cracking was present at the bearing areas.

    1 22/1 P ConcDeck/RigidOv

    The deck was noted in overall satisfactory condition and appears to be structurally sound. The bottom of

    the lower deck has spalling with exposed rebar adjacent at the majority of the deck joints. The bottom of

    the upper deck exhibited cracking with efflorescence adjacent to the deck joints at a few locations. The

    wearing surface of the upper and lower decks are in fair condition with minor spalling typically adjacent to

    the expansion joints along with transverse and longitudinal cracking throughout. Grooving in the wearingsurface is showing wear in the wheel path.

    1 301/1 PourableJoint Seal

    The upper deck joints have been repaired since the previous inspection and are in overall satisfactory

    condition. Lower deck joints are in overall poor condition. Primary deficiencies to lower deck joints occur

    on bottom of the deck (see 58.1) and leakage.

    Some of the expansion joints at the ends of the truss show a small amount of vertical misalignment. It is

    noted up to 1/2. The troughs under these joints are all filled or overflowing with debris.

    The bearings show fair amounts of surface and pack rust throughout the truss, and heavy pack rust and

    active corrosion under the expansion joints. The bearings at the piers are in good condition.

    -Deck surface is deteriorating along intermediate expansion joint device locations. Full depth holes/failures are starting to occur/develop in upper deck of structure at two locations near the north end of bridge as evident from view below. Area of deck alongexpansion joint at panel point #7 of the lower deck was found to have an area of full depth failure that was found during JAInspection, which was repaired same by state crew forces.-Under deck inspection walk system as well as the northbound lower most deck was accessed during time of Joint Annual withODOT.     ENTRAN suggests the following maintenance recommendations: repair new and active cracks, replace the bolts and fracturedguide plates of the sliding pins at the upper chord, replace compression seals and repair damaged armored edges, remove rustfrom bearings at expansion joints, clean drains and drain pipes, clean troughs under expansion joints, replace the broken ormissing utility box covers, monitor pack rust and section loss thrhoughout truss, clean and paint the floorbeams and stringers,and repair spalls in deck and barriers.

Full depth holes/failures are starting to occur/develop in upper deck of structure at two locations near the north end of bridge as evident from view below.     In summary, the Brent Spence Bridge is considered to be in overall fair condition. The following NBI condition ratings wereassigned to the structure: DECK - 6; SUPERSTRUCTURE - 6; PAINT CONDITION - 5; SUBSTRUCTURE - 7

    Riveted built-up box girder floorbeams exist at UD PP 17 and 17'. The box girder floor beams exhibit

    moderate surface rust and section loss of top flange and rivet heads, 10% and 25% respectively. There is

    debris from open joint on the top flange of the floorbeam.

    1 113/1 Paint StlStringer

    The paint has failed and is flaking on the interior upper deck stringers. In locations of expansion joints the

    stringers have moderate surface rust with minimal section loss. At some of the splices along the exterior

    stringers, pack rust is forming along the edge of the bottom flange splice plate.

    1 121/1 P/Stl ThruTruss/Bot

    There is pack rust occurring at the lower chord, particularly on the splice plates, filler plates, and cover

    plates. There is also pack rust along the edges of the lower chord. Section loss is occurring on the

    underside of the lower chord, particularly in the area of the cover plates and splice plates. It is noted up to

    50% at the cover plate. There is also section loss on the lower chord at the connection to the vertical

    members.

    The inboard and outboard gusset plates at the lower chord panel points are generally in good condition.There is surface rust and flaking paint on the gusset plates, and pack rust around the edges of the plates.

    In some locations, there is pack rust up to 1 thick that is prying up the gusset plates. There is section

    loss up to 1/8 at some locations. See appendix of short form for table of condition states for each gusset

    plate.

    1 126/1 P/Stl ThruTruss/Top

    The top chord members and the gussets plate connections to the verticals, diagonals, and lateral bracing

    are generally in good condition. The minor deterioration occasionally observed includes (1) surface rust

    and minor pack rust along the edge of gusset plates, (2) minor debris and surface rust inside of the top

    chord around the connection bolts for the top chord splices, (3) pack rust along the outside and inside

    edges of the top chord member, and (4) pack rust along the edges of the keeper plates.

    The truss members (including the vertical and diagonal members) typically have minor surface rust along

    the inside and outside corners of the boxes. Also, along the edge of the internal diaphragms, the truss

    members have minor to moderate surface rust, pitting, and pack rust. The corrosion is active in some

    areas, but the section loss in the primary members is typically between 1/16 and 3/16. There is pack rust

    noted up to 3/16 on the inside of the verticals, especially around the interior diaphragms. There is a2x0.75 hole in member L1-U1 upstream with a 3.5 diameter area around the hole going from 100%

    section loss to 0% of the 0.5 plate.

    1 152/1 Paint StlFloor Beam

    The floorbeams exhibit section loss and surface rust throughout the bridge. The filler plates of the floor

    beams typically have section loss up to 3/16, and there is typically pack rust along these filler plates and

    stiffeners. The coverplates often exhibit section loss at this location. Under the expansion joints, there is

    heavy corrosion and debris on top of the floor beams.

    There are cracks on the T-connections, filler plates, and welds of the floorbeams, including a significant

    amount of new cracks in these locations. See appendix of short form for crack location and sizes.

    *NOTE THAT THIS 2013 JOINT ANNUAL INSPECTION IN CONJUNCTION WITH ODOT WAS PERFORMED ON 09/16/2013.Joint annual inspection notes are as followed:-All needed repairs throughout structure were left in place for detail in the Inspection Notes Tab from last Fracture CriticalInspection dated 12/29/2010 (Entran, PLC).

    -Structure is in need of varying degrees of rehab construction soon (paint protective coating system, deck surface patching and/or total overlay project, expansion joint device repairs, under deck inspection walk repairs, etc.).-Deck surface is deteriorating along intermediate expansion joint device locations. Full depth holes/failures are starting to occur/develop in upper deck of structure at two locations near the north end of bridge as evident from view below. Area of deck alongexpansion joint at panel point #7 of the lower deck was found to have an area of full depth failure that was found during JAInspection, which was repaired same by state crew forces.-Under deck inspection walk system as well as the northbound lower most deck was accessed during time of Joint Annual withODOT. Clip angle connections supporting under deck inspection walk system at back side location of floorbeam element #16remains fractured at this time and are no longer operating as designed.-Protective paint coating system throughout structure was found showing failure, with worst locations being located along areasof expansion joint devices as well as structural members near traveled lanes of structure.-Broken light pole KY span.

    This inspection was limited to the "Joint Annual Inspection" with ODOT personnel accessing deck area throughout the lowernorthbound lanes and under deck inspection walk system through portion of structure and does not include Element Level or NBI

    Rated portions for structure. This inspection was not a thorough inspection review of structure.For full detailed inspection report of structure see inspection report dated 12/29/2010.

    Inspection Notes

    PB Americas, Inc. conducted a fracture critical inspection of the Brent Spence Bridge over the Ohio River. The inspectionincluded data gathering for both NBI and PONTIS element level inspection reporting.In summary, the Brent Spence Bridge is considered to be in overall fair condition. The following NBI condition ratings wereassigned to the structure: DECK - 6; SUPERSTRUCTURE - 6; PAINT CONDITION - 5; SUBSTRUCTURE - 7

    No repairs are deemed emergency at this time, PB recommends the following maintenance and repairs in order to maintain andextend the useful service life of the structure. (1) Replace fractured and missing sections of guide plates for the upper chordsliding pins at U17-U18, (2) clean drain inlets and troughs of finger tooth joints, (3) replace pourable joint seals, (4) monitorfatigue cracks in LD floorbeams, (5) replace fractured angles supporting inspection walk at PP16, and (6) paint flooring systemsupporting the upper deck and under expansion joints of both decks.
