Tutorial 6: Special Topics ill Form Creation (2)

• Forms are divided into 3 sections: (labeled in on the screen in the Design view but not in the Layout view)

1. The form heade r sect ion : appears at the top of the form and usually contains the form title. It may also cont ain the form logo and/o r a title .

Lab Project

.IS? Recruiter is a recruiting firm that specializes in job placement lor health care professionals. Because the recru it rs at lSP have pre -ious experience in the health care industry, the linn is able to provide qual ity candidates for employment in hospi tals, cl inics, medical laboratories, doctors' offices, and other health care facilities.

JSP recrui ters works with clients in need of" health care profess ionals. It assigns each cl ient to a specific recruiter. The recruiter works with the client to determine the necessary qualifi cations lor each job candi date. The recruiter then contacts and does a preliminary revi w of the qualifications for each candidate before setting lip a job interview between the client and the candida te. Il'the candidate is hired, the client pays a percentage of the new employee 's annual salary to the recruitin g firm, which then distributes a percentage of that client fee to the recruiter.

To ensure that operat ions 11m smoothly, JSP Recruiters organizes data on it's clients and recruiters in a database, managed by acces s. In this way .IS? keeps it's data current and accurate wh ile the finn 's management can analyze the data for trends and prod uce a variety of useful reports.

Requirem ents:

1. Create the Clien t table as shown in the fi gure below. 2. Irn port the Recruiter table from an excel sheer. 3. Create the reports shown in the next page. 4. Create the Client Form that includes all the fields in the client table 5. Create the Recruiter f0 n11 that includes allthc fields in the rec ruiter table 6. Create a query tha t shows the Client name, client number and recruiter number for

all clients in .ISP 7. List the c lients ' names , numbers of recruiter number 24 8. Which recruiters have a rate equal to or higher than 1% (list their number, first

name, last name, postal code and hide the rate) 9. Which clients ' of recruite r 2 1 have an amount paid that is more than S20,000.00 ? 10. What is the total amount (amount paid + current due) for each client (list clients'

names and numbers as well) 11. In the clien t report:

11.1 gro up the clients accordin g to their city then SOli by client name 11.2 Add totals and sub totals to amount paid and current due in the Client report 11.3 Conditionally format the curr ent due field so that the values that are greater

than SO.OOare in red 12. Create a I' port that invol ves both the Clien t and the Recruite r tables (choose the

client number, cl ient name, amount paid and CUITent due from the client table. Choose the recruiter num ber. first and last name fro m the recruiter table), ort by client name and name that report "Clients By Recruiter"

13. In the client table, the amount paid should be a number be tween 0 and 100000, a message declaring tha t rul e should be displayed in the user enters a wrong value and the default value is 0

14. In the client table, limit the states th at the user can enter to CO, CA and NY and should consist of 2 upperc - letters only

15. In the recruiter tab le, make the recruiter number, first name, las t name, rate and comm ission required fields

16. Add the "Mobile" fi eld with text as its type and choose phone number as the input mask

17. Add the following fields to the recru iter tab le (Bonus as yes/no type, Start late as date/time type, Comment as memo type, Picture as an OLE object and Contac t Notes as an attachment)
