Kdg. Physical Requirements
◼ All children must have a current physical to enter Kindergarten
◼ Must be dated on or after 8/18/2020
◼ Students that do not have current physicals will NOT be able to start school
◼ Things to remember:
signed (by Doctor, PA, APN), dated, health history completed and signed by parent, LEAD and DIABETIC screening completed
Immunization Requirements
◼ DPT/Td Booster on or after 4th
birthday
◼ Polio Booster on or after 4th
birthday
◼MMR/Measles Booster 1st
immunization on or after age 1 booster and at least one month later (2 total)
◼ Varicella (Chickenpox) on or after age 1 (2 shots total)
◼ Hepatitis B need for 5th grade
Vision Requirements◼ Effective January 1, 2008. A
new state law SB641, provides that a Vision Exam is needed for all incoming Kindergarten students.
◼ Proof of the required eye examination by a licensedoptometrist must be submitted by Oct. 15 of each school year. Exam must be dated between 10/15/2020 and 10/15/2021.
◼ Forms are on the table for your use.
Dental Requirements
◼ All children in Kindergarten will be required to have a
dental health examperformed by a licensed dentist.
◼ Must be dated between 05/15/2021-05/15/2022.
◼ The state deadline is May 15, 2022. We request that they be turned in by May 1st.

Medication Policies
◼ Care Plans need to be filled out
by parents and doctor for any
students with Asthma,
Diabetes, Seizures or other
chronic health conditions.
◼ All medications (prescription
or over the counter) must have
district medication
authorization form signed by
parents and doctor.

Taking a Bus Safely

Our PTA will strive to positively impact the lives of all
students and their families by providing meaningful
opportunities and curriculum-based activities throughout the
year. We will sponsor programs and cultural opportunities
to enhance the academic experience of all students. We will
sponsor fun and educational family events to strengthen the
bond between our school, our families and our community.
