Pre-Conference Session: Do You Know Your KPCA Patient-Centered Medical Home Resources? Learn More

Session Description: This informal session will provide introduction to the resources available to KPCA members in support of clinics attaining NCQA patient centered medical home recognition. Learn more about the opportunities for members to be in the Kentucky Regional Extension Center’s PCMH Cohorts. Come meet the KPCA PCMH Consultant, Dawn Gentsch, MPH, MCHES, PCMH CCE, her one-on-one, customized, consultative approach provides solid guidance in the NCQA recognition standards.

KEYNOTE SPEAKER

Yesterday, Today and Tomorrow - Reflecting on 40 Years of KPCA

William B. Wagner, CEO, Family Health Centers, Inc., Louisville, KY

Session Description: This session will focus on the robust history of the Kentucky Primary Care Association.

BIOGRAPHY: William B Wagner, MSSW is the CEO of the Family Health Centers Inc. (FHC), a Federally Qualified Health Center (FQHC) in Louisville, Kentucky, where he has worked in various administrative capacities for the past 35 years. He has served as FHC’s CEO since 1998. He is responsible for seven community health center sites that serve approximately 40,000 individuals annually in Louisville and Jefferson County.

Mr. Wagner is one of the founders of Kentucky’s Region 3 Medicaid Managed Care Partnership known as Passport Health Plan. Mr. Wagner is on the Board of Directors and is the recent past Chairman of the Board of Passport Health Plan, and previously served as Chairman of Passport’s Partnership Council for 13 years.

Mr. Wagner is a board member of the Louisville Primary Care Association and an ex-officio member of the Louisville Metro Board of Health. He currently chairs the Board of Health’s Outreach and Enrollment Committee whose mission is to ensure that uninsured Jefferson County residents take advantage of insurance available under the Affordable Care Act and kynect.

He earned his BS degree in 1974 from the University of Dayton, and is a 1981 graduate of the University of Louisville where he earned a Masters Degree in Social Work with concentration in comprehensive health services. In 2006, Mr. Wagner was named the UofL Alumni Fellow from the Kent School of Social Work.

NCQA PCMH 2017 Redesign: A Roadmap for Your Practice that Results in a Healthier Population

Session Description: This session will provide your practice with a roadmap to obtain NCQA PCMH recognition. An overview of the proposed 2017 NCQA PCMH standards and redesigned process and program will be discussed.

Strategies for true practice transformation that focuses on population health, quality improvement, and patient engagement will be shared to enable participants to obtain meaningful and sustained change.

Following the presentation, participants will be able to:

1) Understand the basic components of the NCQA PCMH 2017 standards 2) Outline a process to obtain recognition and program requirements 3) Identify at least one strategy to utilize in their practice that impacts each of one of the three; population health,

quality improvement and/or patient engagement.

BIOGRAPHY: Dawn is a health educator with over 20 years of experience in public health and primary care. As a PCMH coach and practice facilitation consultant she has worked with Federally Qualified Health Centers (FQHCs) and private primary care practices in Iowa, Kentucky and Nevada. Many of these practices have obtained NCQA PCMH recognition, with over half attaining level 3. Dawn began supporting Kentucky health centers seeking PCMH recognition the fall of 2012.

Dawn is an NCQA PCMH certified content expert as well as a master certified health education specialist. She received her Master’s in Public Health degree from Indiana University and Bachelors of Science from Iowa State University in community health education. She previously served as the Network Performance Improvement Manager for the Iowa Primary Care Association, responsible for data collection and

reporting for systems improvement, electronic medical records systems development, quality, performance and systems improvement and training. She also led the PCMH Learning Collaborative for all Iowa community health centers. This Collaborative focused on practice transformation and Patient Centered Medical Home recognition.

She has served in governmental public health, non-profit health organizations and academia. Her work has included systems design development for children’s health service and access issues as well as chronic disease self-management system design. Dawn has extensive experience with coalition building, program management, as well as performance and quality improvement activities from a systems perspective.

The Medical Neighborhood: Enhancing Care Coordination and Integration through Relationships

Jessica K. Beal, Psy. D., Behavioral Health Program Manager – Integrated Care, Passport Health Plan, Louisville, KY

Session Description: This session will explore the medical neighborhood through a review of players (both providers and community partners) and discuss ways to establish a medical neighborhood. A review of how an established medical neighborhood can improve care coordination, enhance efforts towards the practice of integrated care and has the potential to improve provider satisfaction and patient outcomes.

Following the presentation, participants will be able to:

1) Define medical neighborhood 2) List at least one benefit to patients and providers of establishing a strong medical neighborhood 3) Identify at least two ways to network in the formation of a medical neighborhood

BIOGRAPHY: Dr. Beal completed her master’s and doctoral degrees in clinical health psychology at Spalding University. After completing her internship year in integrated primary care, specializing in pediatrics at Cherokee Health System in Tennessee, she returned to Kentucky for the Lovick C. Miller fellowship in pediatric psychology at the University of Louisville School of Medicine, Division of Child and Adolescent Psychiatry and Psychology. Following an extended fellowship in behavioral medicine for stem cell transplant, she served as the pediatric psychologist and Director of Pediatric Hematology/Oncology Behavioral Health with the Division of Pediatric Hematology/Oncology/Stem Cell Transplant. Dr. Beal also has a background in pediatric rehabilitation and brain injury. In April of 2015, took a position as the Behavioral Health Program Manager for Integrated Care at Passport Health Plan.

What Reliable Information Technology is Like, and What That Can Mean for Your Practice

James David Hall III, Masters of Science, Management Information Systems, Northeast Kentucky Regional Health Information Organization, Morehead, KY

Session Description: This session will focus on the importance of establishing a highly functional information technology foundation in your practice. Particular emphasis of what makes up a functional information technology system, how that looks and what steps your practice can do to more your organization towards having one will be discussed. Systems that function to provide reporting features have a significant positive impact on quality scores, dashboards for practice improvement, and provider engagement.

Following the presentation, participants will be able to:

1) Distinguish between good and bad information technology practices 2) List at least two emerging or established technologies that can be used to improve your network 3) Describe ways in which these technologies can be cost neutral

BIOGRAPHY: James Hall has been an Information Technology professional for the past five years; in this time, he has played an integral role in the development of many projects, including the construction of a Metropolitan Area Network, the successful recovery of a system compromised by ransomware, and is a regular author on NeKY RHIO’s information technology blog.

BIOGRAPHY: Sean McPhillips serves in a voluntary support capacity for federal programs dedicated to supporting rural communities. In addition, Mr. McPhillips is an industry leader in System Integration and Interoperability and has served in leadership roles on numerous large-scale projects, including CSC BioSense Biosurveillance and the British Columbia Interoperable Electronic Health Record. In addition to being a CPHIMS Certified as well as Fellow in Healthcare Information and Management Systems Society. McPhillips holds a Maters of Public Administration in Government and a MBA in Marketing.

BIOGRAPHY: Jessica Robinson, a proud eastern Kentucky native, is passionate about branding and design. Long before obtaining her Bachelor of Fine Arts degree from Eastern Kentucky University she valued a well- planned and strategic approach to creative problem solving. As a small child, some of her earliest memories are of watching her grandmother perfectly blend handpicked pattern and color combinations to carry out her artistic vision for each quilt she sewed by hand. Years later, while earning her degree, Jessica learned how to implement the design principles she had watched her grandmother effortlessly use on her beloved quilt patterns. Today, as the owner and principal designer at Print & Pixel Creative, Jessica works with rural healthcare organizations, small businesses and nonprofits to craft authentic, cohesive brands that build trust, attract the right audience and lead to more profitability. She loves empowering eastern Kentucky clients by guiding their organizations through brand visioning and goal setting, then combining strategy and design to provide creative solutions. Jessica has more than 15 years of experience as a graphic designer. She has worked in industries ranging from healthcare to thoroughbreds, nonprofits to retail. Her work has earned recognition including Addy, KSPHRM, and TRAVERSE awards. Jessica resides in Mount Sterling, Kentucky with her husband and two children, Olivia and Mason. BIOGRAPHY: Marc Wetherhorn has over 30 years of political, organizing and advocacy leadership experience at the state and national levels. Marc served or 15 years as the National Advocacy Director and Senior Director of Advocacy and Civic Engagement for the National Association of Community Health Centers. Prior to joining NACHC, Marc worked on statewide election campaigns in Georgia, worked on Capitol Hill and served as State Director for a U.S. Senator. Marc later started and ran a grass roots consumer and environmental advocacy organization and directed grassroots advocacy and organizing campaigns in seven southern states.B.E. in Chemical Engineering from Vanderbilt University and an MBA from the University of Virginia.

340B: Complaince and MegaGuidance Update

Session Description: This presentation will provide participants and opportunity to learn of recent developments in the 340B world from Health Resources and Services Administration (HRSA) and other sources. Insight will be provided to

help participants understand what a compliance audit would entail, and to gain a better understanding of compliance risks, concerns and how to effiectively monitor compilance. The presentation will assist in providing a better understanding of the need to evaluate the benefits with the 340B Drug Program. Key insights on the recent MegaGuidance and how it is expected to impact community health centers will also be discussed.

Following the presentation, participants will be able to:

1) Develop and strengthen existing internal policies, procedures and internal controls around the 340B Drug Program

2) List several recent updates to the 340B Drug program 3) Identify key components of a compliance program

BIOGRAPHY: Micheal Earls is a member of BKD National Health Care Group, and provides assurance and consulting services for healthcare and not-for-profit organizations, including hospitals, health systems, community health centers, nursing homes and social health and welfare organizations. He performs audit services in accordance with Government Auditing Standards and advises on issues related to the 340B Drug Discount Program. He is a member of the AICPA, Indiana CPA Society and HFMA. Michael is a graduate of distinction from Indiana University-Purdue University, Fort Wayne, with a B.S. degree in business. Michael has presented at numberous primary care associations as well as other health care conferences. In addition, he has delivered several webinar sessions on 340B and the related MegaGuidance.

Working Together to “Weaken the Link” between Cardiovascular Disease and Diabetes

Session Description: This presentation will describe the link between cardiovascular disese and diabetes, prevalence and health disparities among various populations, and quality improvement initiatives in place to promote provider and patient engagement for improved health outcomes. Two main goals will be the focus of the presentation, improvement of cardiovascular health and reduction in disparities in diabetes care. The presentation will promote the use of standarized blood pressure protocols with every patient encounter and discussion which empower patients to the their ABCS to enhance quality of care. As well as emphasize the importance of identifying those at risk, monitoring data regularly, and referring to educational programs to encourage active patient participation.

BIOGRAPHY: Kim Headspeth, is a Quality Improvement Advisor for Kentucky Medicare Quality Innovation Network-Quality Improvement Organization (QIN-QIO). In this capacity she works with the cardiovascular task which includes improving cardiac health and reducing health disparities for the state of Kentucky. Ms. Headspeth received her Bachelor of Science degree in Health Information Management from Eastern Kentucky University. Thereafter, she earned her Master’s degree in Business Administration with a focus on Health Administration from Troy University, Columbus Georgia.

Ms. Headspeth began her career as a Regional Long Term Care Ombudsman in the state of Alabama, also working within the agency she was a certified Case Manager and a Nutritional Coordinator. Thereafter, Ms. Headspeth began working as a Patient Safety Manager at Maxwell Air Force Base, Montgomery.

BIOGRAPHY: Ms. Semrau is a QI Advisor for the Kentucky Medicare Quality Innovation Network-Quality Improvement Organization (QIN-QIO). In this position, she facilitates diabetes classes, trains diabetes class Peer Facilitators, and works with physician practices to improve diabetes outcomes for Medicare beneficiaries in the state of Kentucky. Ms. Semrau has a B.S. in Business Administration from California State University, Sacramento, and a Master’s degree in Health Informatics from Northern Kentucky University. She is a Registered Nurse with more than 11 years of experience in a variety of roles, such as medical-surgical, emergency department, charge nurse, hospice, rehabilitation, and quality improvement.

As a managing advisor of quality reporting at Purdue Healthcare Advisors (PHA), Tara guided providers toward attainment of EHR Meaningful Use standards. Prior to joining the PHA team in May of 2014, Tara worked in a variety of work-flow redesign and quality improvement roles with Indiana’s Quality Improvement Organization and another Midwest-based HITECH Regional Extension Center. She assisted in the Meaningful Use attestation and Physician Quality Reporting System (PQRS) submission for more than 100 healthcare providers. Tara earned a Baccalaureate of Nursing from Indiana University, and she is certified as a Health IT Provider Consultant. Cody earned his BS from Purdue University in Interdisciplinary Science with a focus on healthcare engineering and statistics. Cody is currently a doctoral candidate in Health Policy and Management at the Richard M. Fairbanks School of Public Health with research interests in quality of care and access to care for vulnerable populations, especially citizens of rural America and individuals with an intellectual/developmental disability. Cody has been active in health services research for 8 years, working on projects ranging from infant mortality to hospital readmissions to the utilization of assistive technology by individuals with a disability. BIOGRAPHY: Carrie Banahan is the Executive Director of the Kentucky Office of the Health Benefit and Information exchange and is responsible for the implementation and operations of the Exchange. Carrie is a graduate from the University of Louisville and has over 30 years’ experience in State Government. From 2008-2012, she was the Executive Director of the Office of Health Policy and was responsible for the administration of the Certificate of Need program and Health Planning and development for the Cabinet for Health and Family Services. During that time, Carrie also served as the implementation Director for Medicaid Managed Care in which 540,000 individuals was transitioned from a fee-for-service delivery system to managed care.

From 2006 to 2008, Ms. Banahan served as Deputy Commissioner for the Department for Medicaid Services. While at Medicaid, Carrie was instrumental in overseeing the implementation of Kentucky Health Choices and the new MMIS, as well as assisting with the development of new waiver programs.

From 1998 until 2006, she was employed by the Department of Insurance and worked in the Health Division as a Branch Manager, Division Director and also served as Deputy Commissioner. While at Insurance, Ms. Banahan was responsible for overseeing and implementing HIPAA insurance requirements, prompt pay laws for insurers, an internal and external review process for consumers, and a state high risk pool for individuals who were unable or had difficulty obtaining health insurance.

From 1986 to 1998, Carrie was employed by the Department for Medicaid Services and served in various capacities including policy development, drafting of Medicaid regulations and State Plan amendments and assisted with implementation of a new MMIS, and the Medicaid Partnership Program.

BIOGRAPHY: April Washington serves the Regional Director (RD) and Immediate Office of the Secretary relative to the development and management of program strategies as well as fostering relations with Health and Human Services internal and external partners to facilitate strategic initiatives and objectives. One of the April’s primary duties is to support the President’s initiative in the education, promotion and implementation of the Affordable Care Act throughout Region IV.

April oversees ACA efforts in Florida, North Carolina, Tennessee and Kentucky.

Prior to joining the U.S. Department of Health and Human Services, Region IV, April worked as a veteran award-winning journalist and public affairs professional.

For nearly 20 years, April worked in high-pressure newsrooms and the halls of government. For much of her journalism career, she served as a governmental/political reporter and writer for the Rocky Mountain News, Denver, Colo., and The Dallas Morning News, Dallas, Texas, covering state and local government public-policy issues, political campaigns, elected officials, the Colorado General Assembly, mayoral and gubernatorial administrations, Texas Congressional Delegation and local, state and national elections.

April served as principal writer and contributing editor for a recently published book, A Defining Moment, Barack Obama: The Historic Journey to 1600 Pennsylvania Avenue. The book was nominated for 2012 NAACP Image Award for Debut Literary Work. April also co-managed a diversity outreach initiative for the United States Tennis Association of Colorado (USTA). In that capacity, April served as Associate Producer of the documentary film "Crossing the Net: The Denver City Park & The Black Tennis Experience.” The award-winning film captures the words and deeds of African American tennis players who broke color barriers to establish a thriving tennis community in the Denver metro area.

April has earned recognition and numerous national and regional awards: Received the International Academy of the Visual Arts 2012 Communicator Award of Distinction in the Print Division for the USTA Colorado Breaking Barriers project in two categories: Marketing/Promotion-Special Event; named 2005 Print Journalist of the Year by Colorado Association of Black Journalists.

April was born and raised in Denver, Colo. She earned a BA in journalism from Metropolitan State University of Denver. She holds a Master’s degree in Management/Public Administration from the University of Phoenix.

BIOGRAPHY: Kristi Putnam serves as Executive Advisor in the Office of the Commissioner for Department for Community Based Services. She has experience on both the policy and direct services sides of human services and education through her leadership positions in the Florida Executive Office of the Governor, Department of Children and Families, and Department of Education. She also has been a classroom teacher, a preschool director, and a Veterans Services Representative with the U.S. Department of Veterans Affairs. Prior to joining DCBS, Kristi served as Child Welfare Services Manager in the Florida Department of Children and Families. Prior to serving as Regional Director, Cheryl served as Enroll America's Arizona State Director, working with partners across the state to help nearly 500,000 Arizonans get access to health coverage.

The rule will be analyzed and the necessary steps practices need to take in order to prepare for the distinct reporting mandated by MACRA. The value based purchasing provisions in the Medicaid Managed Care rule and other MCO contracts will also be discussed.

BIOGRAPHY: Trudi Matthews serves as Managing Director of the Kentucky Regional Extension Center (REC) at the University of Kentucky. Trudi manages all staff, programs and operations of the Kentucky REC, a center charged with assisting health care providers, practices, and hospitals across Kentucky with using technology, connectivity, process improvement and value-based care models to improve the quality and value of health care. Trudi also serves as a policy advisor on the External Affairs team for UK HealthCare. In this role, Trudi helps leadership remain informed of changing state and federal legislation, regulation and policy related to quality, value, meaningful use of IT, and payment innovation. Trudi formerly served as Director of Policy and Business Development with the Health Collaborative, HealthBridge and the Greater Cincinnati Health Council, where she worked with one of the nation’s top regional health improvement and transformation teams. Between 2008-2014, Trudi provided leadership and support for a wide range of technology and quality improvement initiatives including Cincinnati’s Beacon Community Program, the Tri-State Regional Extension Center, Aligning Forces for Quality, the Kentucky and Indiana State Health Information Exchange Programs, and the Nationwide Health Information Network, among other initiatives. Trudi led grant and business development efforts which garnered more than $26 million in funding for regional health improvement programs. From 2005-2007, Trudi was Senior Policy Advisor with the Kentucky Cabinet for Health and Family Services serving as coordinator for Kentucky’s statewide e-Health efforts. Trudi worked for the Council for State Governments from 1999-2005, where she managed CSG’s health policy activities for state legislators and executive branch officials. Trudi received her B.S. and M.B.S. in political science from Iowa State University. She lives in Lexington, KY with her husband, four children and a cat. BIOGRAPY: Susan Thompson is the Network Development Director for Precision Healthcare Delivery (PHD). PHD manages three Independent Practice Association (IPA) networks in the Commonwealth of Kentucky and Southern Indiana. Susan brings a plethora of experience to her current position as over the last 25 years working in the healthcare industry, she has held several positions including revenue cycle management, practice management and compliance. BIOGRAPHY: Dr. Daniels completed her MSN/FNP at Michigan State University in 1987 and PhD in nursing at the University of Kentucky in 2004. She is dual board certified in Family and Pediatrics and maintain an active practice at the Polk Dalton: Internal Medicine /Pediatrics Clinic. She was named Medical Director of the Chronic Opioid Assessment and Treatment Clinic in July 2016. Dr. Daniels is currently on faculty at the University of Kentucky College of Nursing.

BIOGRAPHY: Dr. Hughes is currently employed at Baptist Health Lexington as their Nurse Epidemiologist working in Infection Control. She serves as a subject matter expert in infection control practices and epidemiology. She is a Registered Nurse with many years of experience, who also has a PhD in epidemiology from the University Of Louisville School Of Public Health and Information Sciences. She was the recipient of the University of Louisville 2015 Alumni Fellow award. In addition, she has a Masters’ degree in Public Administration, with additional certification in infectious disease epidemiology from Johns Hopkins University. She has extensive education and experience working on local, national and global health initiatives within the Department of Health and Human Services, and within a hospital clinical setting. This includes epidemiologic studies, infectious diseases and public policy. She has been extensively involved in the sepsis initiative for all Baptist Health hospitals in KY.

BIOGRAPHY: Mahak Kalra graduated from the University of Louisville with a bachelor’s degree in health and human performance along with a biology minor and received her Master’s in Public Health from the University of Louisville School of Public Health. Mahak is a policy director at Kentucky Youth Advocates. There, she manages the statewide, Kentucky Oral Health Coalition, develops and publishes oral health policy fact sheets/issue briefs, manages several oral health grants, and works closely with key officials to create policy change. Mahak has worked on multiple health issues at Kentucky Youth Advocates, including childhood obesity, expanding children’s health coverage, and smoke free policies.

She is also the dental director for the UK North Fork Valley Community Health Center. She directs their mobile dental outreach program that has been recognized by the Robert Wood Johnson Foundation and has kept data showing over ten years of successfully bringing tooth decay rates down in rural Appalachia. Dr. Stone has over 20 years of experience as an FQHC dentist and enjoys helping and encouraging FQHC’s to implement dental outreach programs.

BIOGRAPHY: M. Raynor Mullins is Emeritus Faculty, UK College of Dentistry and UK Center for Oral Health Research. He is also a member of the Executive Board of KPCA. Dr. Mullins is a Past-President of the American Association of Public Health Dentistry and a member of the Kentucky Public Health Hall of Fame.

RHC TRACK Navigating the RHC Licensure and Provider Enrollment Processes Monique D. Funkenbusch, CPC, Managing Consultant, BKD, LLP, Springfield, MO Session Description: Have you experienced reimbursement delays due to lack of or inefficient provider enrollment steps? Is your RHC currently meeting all of the required Medicare conditions of coverage? Are annual program evaluations conducted and documented at your RHC? BIOGRAPHY: Monique, a member of BKD National Health Care Group, has health care operations experience in administrative and management capacities in hospital and clinic settings. She provides reimbursement and operational consulting for physician clinics, rural health clinics and federally qualified health centers. She assists in providing guidance related to Medicare enrollment and credentialing processes and physician compensation structure analysis, operations and improvement and revenue cycle enhancement consulting. Monique also performs general financial and operational consulting services related to electronic health record systems. Prior to joining BKD, she was the manager of systems integration for a top-rated integrated health care system. While there, she worked with management and provided services throughout the implementation of the electronic health record. She is experienced in change management, staffing and efficiency analysis, process improvement and training. Monique also lectures about operational improvements, revenue enhancements and billing topics at events hosted by state primary care associations. Phyllis has taught at the University of Kentucky and Spalding University where she also served as Chair of the School of Social Work. She has expertise in school based health, rural health, integrated care, non-profit leadership, and organizational capacity building. Her primary interest is in helping address community needs through partnerships and collaboration. Dr. Platt currently serves as the CEO of Shawnee Christian Healthcare Center.
