SUBJECT: FOLLOW-UP REPORT REGARDING THE CURRENT CONDITIONS ONSKID ROW

Attached please find our follow-up report on the Current Conditions on Skid Row as ofMay 2, 2013. As you recall, our initial baseline assessment was conducted on May 2,

2012 in response to a request by the City of Los Angeles (City) to the Los AngelesCounty Department of Public Health (DPH) to provide assistance in assessing andaddressing public health issues in the Skid Row area of downtown Los Angeles. (See

Attachment I, Skid Row Target Area). Our current report represents the DPH's ongoing commitment to monitor conditions on Skid Rowand require corrective actionsand/or recommendations where warranted.

What is most critical in the May 2, 2013 survey findings is the continuing lack of access tofully operable and sanitary public restrooms for homeless residents in the Skid Row area.The 2012 survey noted that two of three Automatic Public Toilets (APTs) were eitherdirty/unsanitary or without handwash soap. The 2013 survey revealed this condition tohave deteriorated to a finding that three out of four (75%) APTs were inoperable.Reinspections conducted on May 15, 2013 again revealed the same number, though notall the same units, to be inoperable.

A comparison of urine/fecal matter locations to the four APT locations revealed that 16%of the total deposits occur within a 300 foot radius of APTs (Attachment IV). The numberof deposits increases to 59% at a radius of 600 feet (Attachment IVa).

The figures suggest and we strongly recommend the installation of additional fullysupplied and maintained restroom facilities strategically located with a maximum serviceradius of 300 feet between units. As referenced in our earlier report, access torestrooms and associated handwash facilities serves to reduce the potential for diseasetransmission.

Additionally, we recommend that the City work with property owners within the Skid RowTarget Area to identify small/narrow open areas adjacent to public streets that can besecured to prevent public defecation/urination.

In response to the May 2012 survey findings, the City of Los Angeles abated mostviolations and/or reduced insanitary conditions to acceptable levels by August 2012. TheCity of Los Angeles subsequently implemented a two tiered street cleaning program thatincludes limited (spot) street cleaning eight times a year and comprehensive cleaning ona quarterly basis. However, the 2013 Survey findings suggest that the schedule isinsufficient in sustaining the compliance levels observed August 2012 as solid wasteviolations increased by 880/0; vermin violations increased by 100%, safety hazardsincreased by 62% and the presence of urine/feces on public streets increased by 82%.

We recommend that the street cleaning schedule be reevaluated to incorporate

aggressive mitigation strategies that are outcome based and measurable. Similarly, werecommend development of a proactive integrated pest management program thatincludes routine rodent breeding and harborage monitoring and abatement activities.

Lastly, as illustrated in Attachment V, 26 of 38 (68%) trash receptacles provided servetwo blocks (Le. San Julian and San Pedro Streets) and 12 receptacles (320/0) serve eightblocks (Le. Gladys, Stanford and Towne Aves., and Crocker Street) within the Skid RowTarget area. We therefore recommend that trash receptacles, in addition to those alreadyin place, be provided a minimum of two per block and additional receptacles specificallyplaced at locations determined to have high volume or on-going refuse problems.

Similar to the previous survey, three multi-disciplinary teams of the Environmental HealthDivision (EH) conducted the latest survey of the target area. However during this survey,EH was accompanied by Steven Pedersen, Chief Environmental Compliance Inspector,and Tri Tran, Senior Environmental Compliance Inspector for the Department of PublicWorks, Bureau of Sanitation, and officers from the City Police Department.

The survey findings were divided into five categories, Immediate Threat to Public Health,Safety Hazards, Vermin Control, Solid Waste Disposal, and Non-Public Health

observations (Attachment II). Each category lists Findings, Actions Needed, ActionTaken, and Recommendations as to how such conditions can be prevented in the future"Non-public health issues" observed during the survey included the number of personalitems, tarps, and shopping carts.

Attachments lilA and IIIB are tally sheets listing the occurrence of each observation,Attachments IV & IVa provide the locations of feces/urine found during the survey, andindicate distances of the feces/urine to the nearest APT at 300 and 600 feet respectively.Attachment V provides locations of trash cans, and Attachment VI provides locations ofrodent burrows.

During the survey, staff from the City's Emergency Response and Enforcement Section(ERES) removed and properly disposed of hypodermic needles observed by the surveyteams. ERES conducted pre-scheduled clean-up activities in the target area on May 3,

2013, eliminating insanitary conditions related to feces/urine on the sidewalk and in thestorm drains and garbage and trash on that date. Continued compliance actionsconcerning these issues was verified by EH staff on May 15, 2013.

A referral notice (Attachment VII) has been forwarded to the City of Los Angeles,Department of Public Works, Bureau of Street Services for findings related to rodentactivity. Please note that failure to correct the conditions by June 10, 2013 may result in

the issuance of corrective orders. A Notice of Violation (NOV) (Attachment VIII) hasbeen issued to the City of Los Angeles, Department of Public Works, Bureau of StreetServices for out-of-service public toilets. The NOV requires immediate corrections and willbe reinspected for compliance June 6, 2013.

c. Findings: The following four Automatic Public Toilets (APTs) were inspected:

• West San Pedro Street, between 5th

Street, between San Pedro and Crocker street - Out-of-Service;

• South 6th

Street, between Stanford and Gladys street - Out-of-Service;

• South 5th

Street, between Wall and San Julian street - no problems observed.

Action Needed: Maintain toilet facilities clean, fully operable and accessible on a daily

basis. Next compliance inspection scheduled for June 6, 2013.

Action Taken: Re-inspection conducted on May 15,2013 revealed APT at South 6th

Street, between Stanford and Gladys Street repaired. However, consistent with thefindings on May 2, 2013, three of the four APTs were found to be out-of-service.(South 5

thStreet, between Wall and San Julian Street: out-of-service). Notice of Violation

(NOV) issued to the City of Los Angeles, DPW, Bureau of Street Services.

Recommendations: Develop and implement a maintenance schedule that results in

APTs that are clean and fully operable daily. Toilet facilities should be installed on all

streets within the Skid Row Target Area with a maximum distance of 300 feet betweenunits. Suggest priority placement for additional restroom facilities for San Julian, SanPedro, and Crocker Streets.

Safety Hazards

Findings: Survey teams observed 8 discarded, improperly disposed hypodermic

needles/syringes.

Action Needed: Immediate Corrective Actions Required

Action Taken: Corrected on-site on May 2, 2013. Staff from ERES immediately removed

hypodermic needles observed by the survey teams.

Recommendation: Routine monitoring of the area to ensure discarded hypodermicneedles, syringes, and sharps are properly disposed of.

Vermin Control

Recommendation: Develop a vermin monitoring/control program that conducts routinemonitoring and surveillance to identify active and potential rodent breeding locations,

sources of harborage, and placement of bait stations. This can be accomplished byqualified City staff/programs, or by third party contract with a qualified private pest controloperator or Department of Public Health (Vector Management Program).

Solid Waste Disposal

A. Findings: Survey teams observed garbage (e.g. putrescible matter, food, and empty foodcontainers) improperly disposed of on the ground, sidewalks, and gutters in the following

• 640 S. San Pedro Street - 1 "potential" burrow at missing cover to base of metal electrical pole;
