HEALTH & WELLNESS OCCUPANCIES URGENT CARE • 8,000 SF facility with a capacity of 50,000 visits per year • General radiology and point of care testing • Provide alternative, more cost effective setting for Primary Care related Emergency Room visits • Ensure continuity of care with Primary Care practitioners
MIND BODY WELLNESS CENTER • Create a center which has a holistic approach to well being that takes into account all aspects of an individual – physical, mental, emotional, and social-spiritual • Establish an empowered approach to wellness, based in self care and self awareness • Develop program to support and compliment traditional medical models – yoga, mediation, movement, massage, and exercise • Provide kiosks for health information and access to Patient Portal • Develop a simulation lab for patients and caregivers

• Develops evidence-based preventive programs designed to limit the incidence of avoidable illness and injury
• Partners with diverse community organizations to ensure that the places in which people live, learn, work, play and pray actively promote health
• Conducts research on population health management and data analysis, such as our Community Health Needs Assessment
• Provides education and training through workshops, seminars, internships and practice opportunities
