Transforming Community Based Care & Support

Through the transformation of community based care and support we are proposing to transform our primary care services, with local GPs driving new models of care and Local Care Organisations (LCOs) forming to include community, social care, acute, mental health services and the full range of third sector providers. We want LCOs to be the place where most people use and access services, in their communities, close to home.

The Greater Manchester Primary Care Strategy

Primary Care is nowincreasingly viewed as a core component of an integrated, community based, care system rather than a separate stand-alone entity.

• A programme for primary care reform which provides a firm foundation on which to transform primary care

• A GM approach to delivery of the GPFV which is aligned to our wider ambitions in GM as set out in ‘Taking Charge’ and the GM Primary Care Strategy

• Aligned to Locality Plans to ensure General Practice is fully engaged • Commitment to increase investment to General Practice – ‘ask’ of the

Transformation Fund to fulfil the commitments of the GM Primary Care Strategy

• A GM resilience programme which will support General Practice and act as a programme of improvement

• An emphasis on acknowledging and harnessing excellence and sharing best practice

• Increasing capacity through new roles and skills and ensuring our seven day services alleviate some of the pressures in core hours

• Avoid duplication and explore the opportunities for further alignment with urgent primary care – a review of 24/7 urgent primary care provision

• Increased investment in our primary care estate• Development of a ‘toolkit’ to support GPs to make informed choices about

their existing premises• Roll out of GP development and leadership programme

• An ‘ask’ to the GM Transformation Fund for up to £41.2m to fund the programme over the next four years

• This significant investment in General Practice aims to provide additional capacity and capability by:

• Looking at investible models which do not destabilise already pressurised individual GP practices; there is no expectation on individual practices to open 8am – 8pm, 7 days per week

• Have the flexibility to respond to the needs of the diverse populations and a focus on particular patient cohorts which make sense for their geography

• Support our General Practice workforce through the establishment of a support hub and network for our GPs with peer support at its heart

• Provide training and development for receptionists and clerical staff to alleviate the rising demand and administrative burden placed on GPs and wider practice

• Expand the wider practice workforce with up to an additional 100 clinical pharmacists

• Investment in technology for online consultation systems, mobile apps and ability to book appointments online

Delivering Improved Access•Embedded within LCOs and rooted within the neighbourhood delivery model•Investment of £6ph•Delivered via a hub based model serving geographical neighbourhoods•Help to alleviate pressures in core hours•Manage patient flow and demand across 7 days, e.g. book more acute activity into 7 day hubs

GM Primary Care Estates•Capital pipeline in place to improve primary care estate•Virtual map to illustrate ‘neighbourhood hubs’ serving populations of 30k-50k•Toolkit to inform local discussions with GP practices and Strategic Estate Groups to enable GPs to move to neighbourhood hubs where appropriate •Committed to increased investment in primary care estates

GM Resilience Programme•A single world class hub to support General Practice and act as a programme for improvement.•Identify best practice and areas of excellence •Offer a coherent and consistent offer in terms of rescue, resilience and improvement•Develop our clinical leaders to enable them to offer peer support or more formal arrangements to support general practice

GM programme for primary care reform presented to Strategic Partnership Board on Friday 24th February including the funding requirements over the next four years.

Work already underway in respect of:

GM Resilience Programme•Commencing work with two neighbourhoods to support general practice towards sustainable primary care •Mobilise the support and development hub from April 2017•Established a working group to develop the specification for the GP improvement team which will involve LMC representation Improving access•All parts of GM delivering additional access over 7 days•Parity of funding in 2017/18 in line with national £ph•Enable all areas to meet national requirements 30mins/1000 population rising to 45mins/1000 by 2020/21•Embed within the LCO and neighbourhood delivery model to meet needs of the populationReview of 24/7 urgent primary care provision•Commencing baseline review of all urgent primary care provision in GM•Explore models of best practice and identify opportunities to streamline services, avoid duplication, easier navigation for patients and professionalsWorkforce•Expansion of clinical pharmacist in general practice - potential to recruit c100 additional clinical pharmacists across Greater Manchester•HEE North out to procurement for a training provider for Care Navigators Estates•Toolkit in development to inform General Practice of options available around existing premises
