The Competent Authority invites online Limited Tender from Manufacturers/Suppliers/Agencies/Distributors having minimum turnover of Rs. 05 lakhs (Rupees Five Lakhs only) in F.Y. 2015-16 for printing/stationary items.. The “Tender Documents” comprising the General Terms and Conditions of Contract and the Special Terms and Conditions of Contract( if any) which will govern any contract made, the Tender Application Form and the brief specifications of items and proforma for quoting rates (Financial Bid) are given hereunder.

If you are in a position to quote for the contract in accordance with the requirements stated in the attached schedule, please submit your quotation to this office. Tenderers having Annual Turnover of Rs 05 Lakhs (Rs, Five Lakhs only) or more for F.Y. 2015-16 are

requested that, before quoting their rates or sending tender/submitting bid online, the tender form

may please be read out thoroughly (line by line), otherwise purchaser will not be held responsible for

any error / oversight of his own.

The form is a Standard Form of Tender. Certain clause /clauses may not be applicable in some

double/two bid system in the prescribed Performa. Tenders are to be submitted online through e-

procurement portal and all the bids and supporting documents shall also be required to be submitted

physically in hard copy along with Earnest Money Deposit (EMD) in the tender box. The documents may be submitted by eligible bidders in the document downloaded from the website along with Earnest Money Deposit of Rs.11000/- (Rupees Eleven Thousand Only) in favour of “E.S.I.Fund Account No.1” drawn on Indore, in a separate envelope marked “Printing of various forms, Service Book & Registers under Two Bid System ”.

For all practical purposes, the e-tender shall be considered for evaluation, however in case of any dispute, the physical documents would also be scrutinized. Bidders are required to essentially submit the bid in online as well as in hard copy in the tender box failing which tender may be treated incomplete and may be summarily rejected. However, in case of dispute, e-tendered document shall be treated as final. Tender documents duly completed and supported by requisite documents and the forms, as mentioned in the tender document, should be submitted with covering letter in a sealed envelope duly superscribed with the words “Tender for Printing of various forms, Service Book & Registers under Two Bid System ”.

The Tender should be deposited in the Tender Box on Room No. 2, , Ground Floor, of this hospital

i.e ESIC Model Hospital & ODC, Nanda Nagar, Indore (M.P) last upto 1.00 PM on 28/08/2017 which will

be opened on same day at 2.30 P.M in conference hall. Financial bids in respect of qualified bidders

shall only be considered which is not necessary will open on the same date.

If any information furnished by the applicant is found to be false at any stage, the application shall

be cancelled and applicant shall be liable to be debarred from tender.

E.S.I.C. reserves the right to reject any or all applications without assigning any reason. Selection of

the successful bidder will be at the sole discretion of the Medical Superintendent, ESIC Model Hospital,

Indore, who reserves the right to accept or reject any or all the proposals without assigning any reasons

thereof.

II. INSTRUCTIONS FOR OFFLINE SUBMISSION OF BIDS

1. The tender document can be downloaded from www.esic.nic.in, www.esichospitals.gov.in and Central Public Procurement Portal: www.esictenders.eproc.in .

2. Proposals should be submitted in two separate envelopes marked as ‘Technical Bid’ (Envelope-I) and ‘Financial Bid’ (Envelope-II). Technical Bid → The complete Postal Address, telephone / Mobile/ E-mail address, etc. has to be mandatorily mentioned in the completed tender form along with the documents mentioned in the

checklist, failing which the tender shall liable to be cancelled. Financial Bid → The tenderer will quote the rates in respect of specification given in Tender Notification and shall fill Annexure - I appended herewith along with the Tender Application form. The rates quoted must be in lump sum item-wise as per Annexure-I inclusive of all charges. Taxes should be quoted separately for each items. Price must be clearly shown in figures in Indian Currency. Quoting of rates in any other format will not be acceptable. Bids quoted in the tender should be valid for a period of six months from the date of opening of Tender. Both the technical Bid and Financial bid should be duly sealed separately and superscribed accordingly. Both theses envelopes should be put in single THIRD envelope. This third envelope should also contain the demand draft towards EMD. The third cover should be superscribed by the name of the work as “ Tender for Printing of various forms, Service Book & Registers under Two Bid System . The name and address of the tenderer should also be written over the covers.

3. The tender must be accompanied by Earnest Money Deposit (EMD) of Rs 11,000/- (Rupees Eleven Thousand only) drawn in favour of “ESI Fund A/C No. 1”, payable at Indore in the form of Demand Draft.

4. Each and every page of the document (hard copy) should be numbered and signed by the authorized signatory of participating firm with seal.

5. The tenderer shall also enclose the copies of documents in support of details about the firm

such as income Tax returns for the last three years, Photo copy of PAN No. of Firm/ Dealer/

Proprietor, TIN No., Proof of Ownership of Premises/ if on rent copy of rent agreement, Proof of

working with Government Department, if any. Proof of annual Turn over more than 5 lakhs. VAT/

works contract Tax Registration and Service Tax Registration and work completion certificates

etc. whichever applicable. All these documents shall be kept along with the Technical Bid.

6. After submission of Online tender, all Duly signed copies of tender document addressed to the Medical Superintendent along with EMD shall be dropped in the Tender Box kept for the purpose in the Office at Room No.2 situated at ground floor at ESIC Model Hospital & Nanda Nagar, MR-9 Road, Indore (M.P)-452011

7. The EMD of unsuccessful tenderers shall be refunded within one month after the award of work to the successful tenderer without interest .

1. All the copies of documents (duly signed) given in check list above should be enclosed failing which they will not be technically qualified.

2. The price shall be quoted in figure in the relevant column provided in the e-tender document.

3. Rates quoted shall be firm, fixed. If during the contract period any changes in the tax arises, the extra tax if any, this hospital shall not be liable to pay the same. The contractor has to pay it and has to abide to all applicable laws time to time.

4. Conditional tenders are liable to be rejected. 5. The Medical Superintendent, ESIC Model Hospital & ODC, Nanda Nagar, MR-9 Road,

Indore (M.P)452011 reserves the right to accept or reject or cancel any tender or all tenders

without assigning any reasons.

6. Delay/ late tenders will summarily be rejected. 7. Quotations qualified by such vague and indefinite expressions such as “subject to

immediate acceptance‟; “subject to prior sale‟ etc. will not be considered. 8. The quantity indicated above is approximate and may vary subject to our

requirement 9. The bidder to whom work is awarded will execute the work himself. He will not be

allowed to sub-let the work to any other agency. 10. Verification of EMD : The demand draft for EMD will be verified first. Only after

satisfying this condition, the tenders will be considered for opening of technical bids. 11. The orders for printings of Forms, Service Book and Registers will be issued as and

when the requirement arises. IV. VALIDITY OF TENDER:

The validity of rates quoted will be for a period of one year from the date of finalizing of tender by Committee appointed by the Medical Superintendent. However the Medical Superintendent is at discretion to extend the validity for further period of one year on same terms, conditions and rates in willingness of the vendor.

V. OPENING OF TECHNICAL BID:

After satisfying EMD, the technical bids will be opened and evaluated to ascertain the

eligibility criteria mentioned in the specification above. The samples submitted by the eligible

tenders will be evaluated technically with regard to quality and specifications as mentioned in the

tender. Based on this evaluation, the Technical Bids satisfying all the eligibility criteria will be

qualified for opening of Financial Bid subject to verification of original documents. All the

requisite document of Technical Bid should be filled up properly, otherwise the Tender submitted

will be considered incomplete.

All documents submitted should bear stamp and signature of the authorized representative / quoting firm and should be enclosed with the Technical Bid. Tendering firms are at their liberty to be present or authorize a representative to be present at the opening of the tender at the time and date as specified in the Schedule VI. SAMPLES

All the tenderers shall submit a sample for paper on which the printing shall be done along

with the technical bid (e.g 60GSM,70GSM,100 GSM paper, Paper to be used for Service Book)

which shall be seen at the time of opening of Technical Bid, for evaluation by the Committee.

Tenderers shall supply the sample(s) at their own cost. Samples supplied will be retained by

ESIC for record purposes. Samples supplied for test should not be less than the quality for

forms and registers as given in the specifications in Annexure I Each sample should have a

card affixed to the sample which should bear the following information: i. Name and address ii. Tender Number iii. Date of opening of tender iv. Item no. against which tendered v. Any other relevant description. VII. FINANCIAL/PRICE BID:

The qualified tenderers will be informed about the date and time of opening of

Financial Bids. Further, they will be requested to bring all the originals for the copies submitted

by them. Only after satisfactory verification of original documents the bidders will be declared

qualified for opening of Financial Bid. The Financial Bid of the tenderers, whose Technical Bid is

qualified, will be opened in the presence of the tenderers, who choose to attend the opening of

Financial Bid. The tenders will be finalized after evaluation of Financial Bids and the bidders will

be informed, accordingly. VIII. SECURITY DEPOSIT: On acceptance of the tender, within the period specified by the Medical Superintendent, the contractor shall deposit as security Deposit, for a sum of Rs. 27100/- (Twenty Seven Thousand One Hundred Only) or 5% of the contract value whichever is higher . The security deposit shall be in favour of “ESI Fund A/C No. 1”, payable at Indore in the form of Demand Draft from a scheduled Bank. The hospital authority shall be entitled to forfeit the Security Deposit or any part thereof without prejudice to any other remedies provided in the contract or available under the law. a. If the contractor fails in fulfilling the terms and conditions mentioned in the Tender Documents, such failure will constitute a breach of the contract and the Medical Superintendent

shall be entitled to make other arrangements at the risk and expense of the contractor. b. On due performance and completion of the contract in all respects, the Security Deposit will be returned to the contractor without any interest. IX. DELIVERY TERMS

a) The delivery of the materials are required within a period as specified and as the place mentioned therein. No Transportation charges will be paid.

b) The tenderer shall deliver the material to the consignee / authority in good order (of which the Medical Superintendent, ESIC Model Hospital & ODC, Nanda Nagar, Indore-452011 shall be the sole judge) within the limits of the time as deemed reasonable and specify in such quantities/qualities as may be ordered by him/her from time to time.

c) Time for and date of delivery: The essence of contract: - The time for and the date of delivery of the materials in the schedule shall be deemed to be the essence of contract and delivery must be completed not later than the date(s) specified.

d) Tenderer should supply the stores with proper packaging and making for transit so as to be received at the destination without any loss or damage of item/ items.

e) No additional charges are payable for parking, transport, labour etc. f) Any mistakes in printing has to be rectified at own cost of the successful bidder. Hence vendor is

requested to take/approve proof reading before final printing of the forms/service book/registers.

The Medical Superintendent may allow additional time under special circumstances such as strikes, lockouts, fire accidents, riots etc. on written request from the tenderer at appropriate time. X. Responsibility for executing Contract:

The contractor is to be entirely responsible for the execution of the contract in all respects

in accordance with the terms and conditions as specified in the acceptance of tender.

XI. Right to accept / reject: Tender form shall be duly filled in and completed in all respects. Incomplete tenders or tenders without EMD shall be treated as invalid.

A. Submission of more than one Tender by a bidder for the “Printing of Forms, Service Books and Registers” will render the Tender (s) liable for rejection.

B. The hospital authority reserves the right to reject any or all tender without assigning

any reason whatsoever. Also, the hospital authority reserves the right to award any or part or full

contract to any successful agency at its discretion and this will be binding on the tenderer. C. In the event of any violation of laws, rules, statutory provisions by the contractor, this

will amount to breach of contract and in such case, the hospital will have the right for terminating the contract, forthwith, without giving any notice or assigning any reason.

D. The Medical Superintendent, ESIC Model Hospital, Nanda Nagar, Indore , does not pledge himself/herself to accept the lowest or any tender and reserve to himself/herself the right of accepting the whole or any part of the tender or portion of the quantity offered and the Tenderer shall supply the same at the rate quoted by him. Tenderers are at liberty to tender for the whole or any part.

E. Acceptance by the purchaser shall be communicated after finalising the tender. You are requested that the instructions contained in the said communication should be acted upon immediately / as asked for.

F. Failure and Termination: If the contractor fails to deliver the materials or any installments thereof within the period prescribed for such delivery, the Medical Superintendent, ESIC Model Hospital, Nanda Nagar, Indore may without prejudice:-

1. Recover from the contractor as agreed liquidity damages, and by way of penalty a sum equivalent to 2% of the prices of any stores which the contractor failed to deliver within the period fixed for delivery.

Purchase elsewhere, without notice to the contractor, on his account and at the risk of the contractor, the materials not delivered or materials of similar description (where stores exactly complying with the particulars are not in the opinion of the Medical Superintendent, ESIC Model Hospital, Nanda Nagar, Indore-452011, readily procurable such opinion being final), without cancelling the order in respect of the consignments due for delivery, or

2. To cancel the contract or a portion thereof, and if so decided to purchase or authorize to purchase of stores not so delivered or other of a similar description (where materials exactly complying with the particulars are not, in the opinion of the Medical Superintendent, ESIC ESIC Model Hospital, Nanda Nagar, Indore-452011readily procurable, such opinion being final) at the risk and cost of the contractor.

3. Also the EMD/ Security Deposit of the firm will be forfeited in case the firm stops supplying or makes undue delay in supplying the printing items mentioned in the Tender. XII. DELIVERY / INSPECTION OF STORES:

The Delivery of the goods has to be made at the Non Medical stores available at ESIC ESIC

Model Hospital, Nanda Nagar, Indore-452011withing 21 days from the receipt of the Order. Supplies shall be accepted subject to the complete satisfaction of Medical Superintendent as per samples submitted for technical evaluation. Any defect found in the materials will render the supplies open to rejection and decision of the Medical Superintendent, ESIC ESIC Model Hospital, Nanda Nagar, Indore-452011 shall be final and legally binding. The rejected materials shall have to be taken back at their own cost and risk, and shall replace such rejections with the items of standard specifications / quality as acceptable to the Hospital Authority. Alternatively, the materials may be purchased from others, at the risks and costs of the tenderer, at the discretion of the authority. XIII. PAYMENT: Payment shall be made after receipt of complete stores, due inspection by Inspection Officer/Board. No advance payment shall be made. It will be made through ECS/NEFT/RTGS. Bank details as per annexure must be submitted along with the documents.

XIV. Samples/Forms:- Those forms, registers & Service books which are to be printed the sample, specimen copy, the matter to be printed whichever shall be available in the office can be viewed/inspect by the vendor interested to participate in the tender during office hour (2:00 to 4:00 PM) on working days except Saturday. The visit can be made till 24/08/2017 In case of any query/information or clarification the vendor may contact the General Branch of this hospital (Room No. 7) Ground Floor during (2:00 to 4:00 PM) on working days except Saturday. The visit can be made till 24/08/2017 Contract agreement shall be executed on Rs.500/-(Five Hundred Only) Non Judicial Stamp Paper, the expenses shall be borne by the successful bidder.
