The Liaquat University Hospital, Hyderabad/Jamshoro intends to pre-qualify the Local/Foreign manufacturer for their products mentioned under groups and sub-groups, as supplier of Electro Medical, Hospital, Scientific Equipment, Machinery and Furniture of Medical use for the year 2006-07.The applications are invited on the prescribed form from well reputed firms/ running their business for least last three years. The application form / documents can be obtained from General Branch LUH, Hyderabad during office hours, by submitting a written request along with non-refundable fee Rs.100/-.The qualified applicants are required to submit Rs.1000/-per each group specify as qualifying fee must be deposited through demand draft/pay order in the name of Medical Superintendent, Liaquat University Hospital, Hyderabad/Jamshoro. The application with necessary documents must be duly completed, signed and stamp by the local manufacturer or the authorized agent of foreign manufacturer delivered to addressed below latest by 10.02.2007. INVITATION FOR PRE-QUALIFICATION FOR MACHINERY, EQUIPMENT & FURNITURE .

The Liaquat University Hospital, Hyderabad/Jamshoro intends to pre-

qualify the Local/Foreign manufacturer for their products mentioned under groups

and sub-groups, as supplier of Electro Medical, Hospital, Scientific Equipment,

Machinery and Furniture of Medical use for the year 2006-07.The applications are

invited on the prescribed form from well reputed firms/ running their business for

least last three years. The application form / documents can be obtained from

General Branch LUH, Hyderabad during office hours, by submitting a written

request along with non-refundable fee Rs.100/-.The qualified applicants are required

to submit Rs.1000/-per each group specify as qualifying fee must be deposited

through demand draft/pay order in the name of Medical Superintendent, Liaquat

University Hospital, Hyderabad/Jamshoro. The application with necessary

documents must be duly completed, signed and stamp by the local manufacturer or

the authorized agent of foreign manufacturer delivered to addressed below latest by

10.02.2007.

The department reserves the right to reject any or all the applications

with out assigning any reason.

PREQUALIFICATION FOR MACHINERY, EQUIPMENTS AND FURNITURE INSTRUCTIONS FOR PRE-QUALIFICATION.

o The Liaquat University Hospital intends to prequalify the firms for Machinery / Equipment & Furniture.

o The prequalification has been divided in into following:a. prequalification of Agents Sole Agents / Distributor of Foreign

principal in Pakistan.

b. Pre qualification of local man.

C. Group Registration Performa.

o The firms / manufacturers are required to type this application form in accordance

With requested information (firm means Agent / Sole Agent / Distributors &

Manufacturers).

o For the firms to be pre qualified three years business track record is mandatory and

the period will be counted from the Date of GST registration.

o The firms are required to select the group and sub group according to their capability.

o The evaluation/ inspection will be conducted for complete sub group.

o The firm will be pre qualified for complete sub group.

o The preference will be given to firm having well established office and workshop in

Sindh.

o For any item which is not mentioned in the groups / Sub groups will be considered in

relatively similar nature sub group.

o The local manufactures and authorized dealers of foreign manufacturer are required

to fill the particular Performa and attach the necessary supporting documents.

o The evaluation of the application will be conducted as per information supplied by

the manufacturers for which the applicant have to submit an undertaking on stamp

paper on Rs. 50, that the information supplied are correct to the best of his knowledge

and if any information found incorrect at any stage the qualification will be cancelled

at the cost of applicant and liable to penal action as per existence rules.

o Certificate from the government of Pakistan/ Government of foreign Manufacturer for

recognition of manufacturing facilities must be attached along with application.

o For pre qualification of product, the information reports from reports from different

renowned / prestigious institutions using this product must also attached with the

application form.

o In case of high technology product attach list of functional units world wide provide

the certificate that the product is being used in country of origin and given

undertaking that the product is of latest model and there is no provision that the new

model will marketed by the time of supply of the particular item.

The firms are bound to provide the following.

o The firm is bound to give technical training inland / foreign as the case may be, to all

and user.

o The original catalogue must accompany with offer and the equipment should

comply/certified at CE/FDA.standards.

o The firm must confirm free installation /Demonstration at consignee’s end.

o Two years free service from the date of installation 05 Years tube / parts warranty /

Service contract and available of spare parts may also be confirmed by the bidders.

o Sole Agent Certificate/Authority letter from the manufacture must be provided by the

bidder.

o The Service manual with circuit diagram must be provided with all electro-medical

equipments.

o PREQUALIFICATION FOR MACHINERY, EQUIPMENTS AND

FURNITURE.

o The applicant will be bound to supply the price list of spare parts and

consumable at the time of tender i.e. for two years.

o The supplier will be bound to provide free service during warranty period and

the supplier spare parts accessories of the supplied equipment on demand.

o Join Inspection of all items will be carried out by the consignee, representative

of industries, Directorate Sindh, representative of SDSSP,(except at teaching

hospital) Representative of Electro-medical department government of Sindh

and End –User i.e. specialist.

o Separate sheet product will be used.

o A set of Performa will contain the following documents for each product

manufacturers.

1. Valid Agency Agreement, in case of agent of foreign manufacturer certified

9. Product Brochure of the firm.

Note: Separate sets for each product / Manufacturer will be submitted along with the

application form, No internet Download/Photostat Copy will be accepted , only

Printed Broachers will be Accepted.

o The product will only be considered by providing the required information

otherwise it will stand rejected.

o All Documents including application form must be signed be the authorized

personal of the firm.

o The complete application must be delivered within the mentioned date and

time. The applications after the dead line will not be considered.

o The prequalification status so achieved will be valid for three years.
