Dear Friends and Colleagues,

On November 15, 2012 the California Association for Health Services at Home (CAH-SAH) and Aging Services of California, will host the West Coast’s Premier Home Care and Aging Services Technology Conference – the third AgeTech Conference & Technol-ogy Exposition at the Hilton Hotel in Pasadena, CA.

Organized into the three main segments of “Innovate” “Invest” and “Implement” this one-day conference will address many of the organizational cultural barriers to innovat-ing service models and adopting new technologies that often inhibit senior living and home care providers and will offer guidance for the sometimes-daunting investment decisions and implementation processes. As providers determine how best to thrive in the new value-driven environment, organizations offering evidence-based coordination expertise and innovative service models – strengthened with technology – have a better likelihood of developing partnerships and providing services to a larger customer base. This theme embraces one of AgeTech’s goals of enabling strategic change and facilitat-ing successful business model development through education.

This year’s program ensures that the event will not just be “another technology con-ference,” but rather an important strategic growth venue for care providers’ executive leadership, key staff and board members as well as advisors, consultants, academics, technology organizations and innovators in the aging and home care space. We are also happy to announce that CEUs are offered for across a variety of license categories for staff in California, Oregon and Washington (details below).

When changes occur in the business environment – such as those in the technological, eco-nomic, and policy arenas – new opportunities are created. New payment arrangements and policies that encourage high-quality coordinated care will put unprepared community-based organizations – including aging services and home care provid-ers – at risk of lost patients/residents and revenue. As providers determine how best to thrive in this new value-driven environment, organizations offering evidence-based coordination exper-tise and innovative service models, strengthened with technology, have a better likelihood of developing partnerships and providing services to a larger customer base.

Torrance, California-based HealthCare Partners Medical Group was one of 32 accountable care organizations chosen to participate in CMS’ Pioneer ACO Model. Additionally, HealthCare Partners is part of a commercial PPO ACO with Anthem and is working with other insurers on ACO agreements. In this keynote presentation, HealthCare Partners’ Corporate Medical Director Tyler Jung, MD, will discuss some of the successes and challenges in business model innova-tion to align with the current value-driven environment, with important strategic insights for

aging services and home care organizations and the role of technology in achieving these goals.

Dr. Jung will speak to factors such as stable and visionary leadership, a consistent emphasis on prevention and promo-tion, an integrated health information technology infrastructure, use of effective care coordination and care management, and performance measurement and reporting. One of the key culture changes Dr. Jung will discuss is converting the mindset of a traditional fee-for-service provider into one that is rigorously measured on quality and value.

The program helped older adults get fit, increase social interaction and provided them the tools to man-age their personal health.

Kusserow will discuss how Humana’s investment in innovative new approaches, including technology-enabled care, and strategic partnerships and acquisitions is instructive for all provid-ers to position for success in this marketplace.Kusserow previously served as managing director and chief investment officer of the Ziegler HealthVest Fund, where he focused on early-stage investments in health-care services and IT.

It will cover topics such as Meaningful Use Stage 3; development of care plan requirements to achieve an incremental progress towards a longitudinal care plan concept; identification of the availability of standards to support the exchange of more robust patient status content, including: functional status, cognitive status, pressure ulcers; recognition of available transitions of care data sets that support long-term post-acute care (LTPAC) patient populations; and harmonization of the Consolidated-CDA for use in supporting the Patient Assessment Questionnaire Summary (PAQS) standard (for MDS and OASIS-C federally required assessment instruments) and expand summary care record requirements to include receipt of the PAQS document.

Telehealth advancement in the community requires additional, and sometimes different, strategies and patient-centered collaborations than health system internal networks. The technical arrangements and legal agreements likely have different parties, goals and strategic value. The competencies and willingness of the patient/family will be paramount in achieving both research and therapeutic goals. Many legal issues are involved in the implementation of care to the home and across state lines. The presenter will offer observations from extensive work in the field of community-based services, work in American Telehealth Association (ATA) workgroups, and consideration of recent telehealth issues in the USA and Western Europe.

The ADOPT Toolkit offers tools in four areas: Remote Patient Monitoring, Medication Optimization, Care Transitions, and Mobile Health. This presentation highlights the RPM ADOPT Toolkit.

3B dEPLOYING MEdICATION MANAGEMENT WITH TECHNOLOGYAs many as 76% of the hospital readmissions in the U.S. are thought to be preventable. With a 27% rehospitalization rate within 30 days of discharge, many solutions are needed. Medication non-adherence contributes to approximately half (50%) of all medication-related hospital admissions and 23% of all nursing home admissions. Technology information systems in congregate settings and devices for home settings are accelerating our ability to increase medication compliance and management.

ENGAGING PERsONs WITH dEMENTIA THROUGH AdAPTIVE COMPUTER TECHNOLOGIEsPREsENTER: JACk YORk, IN2LThis presentation showcases real word examples of how multimedia technologies, delivered through adaptive computer systems, are able to deliver quality of life benefits to individuals with dementia and to their caregivers. The systems demonstrated have been specifically designed for individuals with cognitive disabilities and little or no computer experience. The presentation focuses on practical application about how the residential care team may work to integrate the computer system into daily programming.

Outcomes-based research is discussed that shows how the adaptive computer technologies demonstrate measurable benefits to persons with dementia including increased engagement in daily life activities, decreased agitated behaviors, and improved physical and psychological well-being.
