MESSAGE FROM JOLYNN MUNRO

INFINITY REHAB

2019 was an incredible year for Infinity Rehab as we celebrated 20 years as a company. Our outstanding achievements today are testaments to our work over the last decade – originating from the diligent effort

our early leaders devoted to Infinity Rehab.

In 2019 we recognized our outstanding clinical outcomes and 20th anniversary at our annual Symposium. We debuted our Can’t Stop the Feeling music video as we celebrated our feelings of great accomplishment and excitement that we just “can’t stop.”

Our teams have proved we “can’t stop” these feelings throughout 2019 and continuing into 2020. Throughout the year I heard stories of advocacy by our fellow Infinity Rehab team members.

The Leadership Academy Class of 2019 scholars shared several stories of advocacy in their local areas. Among the many inspiring stories, Kiley Connealy, OTR/L and Practice Council member, advocated for patients with strong needs for wheelchairs. Through her advocacy and collaboration with the National Occupational Therapy Association of Nebraska, where she resides, she has seen wheelchair approvals through Medicaid jump from 25% to 80%. Thank you, Kiley, for your efforts and for making a difference for Nebraska clinicians.

I also had an opportunity to stand with Michelle Jabczynski, Director of Compliance and Strategy, in Washington D.C. alongside fellow industry colleagues. Together we fought against the proposed 8% cut to the 2021 physical fee schedule resulting from E&M changes by the Centers for Medicaid Services. I am grateful for this opportunity to take a stance and make a difference along with so many other wonderful clinicians.

In February 2020, Infinity Rehab was named a Great Place to Work for the second consecutive year. This speaks volumes about our culture – about the immense care our clinicians offer their patients, the many hours our leaders spend guiding our teams, and the careful work our clinicians devote to their professional development through Infinity Rehab’s many programs.

Since 1999, customers have chosen Infinity Rehab’s unique brand of clinically intensive, comprehensive occupational, physical, and speech therapy for our ability to achieve quality outcomes and manage rehab programs that provide better business results.

Therapists choose us for our commitment to life-long learning, the significant resources we commit to professional development, and our forward-looking approach to standardizing care delivery to optimize outcomes.

Infinity Rehab began as a response to payment changes made to Medicare after the Balanced Budget Act of 1997. These changes provided for the implementation of a per diem Prospective Payment System (PPS) for skilled nursing facilities. Inspired by these changes, Rick Miller asked therapists Bob Thomas and Mike Billings to put together a strategic plan and lead the newly-formed company.

Starting with just seven employees and meeting in each other’s homes to write policies and procedures, the fledgling company ended 1999 with 84 employees at seven locations throughout Oregon.

Today, Infinity Rehab employs over 2,200 therapists spanning 221 locations across 19 states. Once again, we find ourselves amid an evolution in reimbursement, and Infinity is leading the change from volume-based to value-based care. We are committed to improving the way therapy is delivered, reducing unnecessary hospital readmissions, and enhancing the patient experience.

With the October 2019 changes in the way skilled nursing facilities are reimbursed, industry leaders transitioned and continue to navigate and understand the new rules and adapt their business models accordingly. As these leaders begin to deal with the complexities of PDPM, they are increasingly turning to outside experts for help guiding them through the process.

New and existing partners are depending on Infinity Rehab to help them understand the new metrics and provide solutions to meet the demands of the program. We built our strategy for PDPM around our

clinical model, with proven outstanding outcomes. As an industry leader in developing methods and systems for the improvement of patient care, we have the experience they need most.

Since our humble beginnings 21 years ago, Infinity Rehab has led a post-acute care revolution by creating transformational paths for our patients and partners. We do this through highly qualified people, intelligent platform, and innovative process. These three core foundations set us apart from other rehab providers:

Data-Driven Solutions: We serve more than 43,000 patients and provide over 1.6 million patient

Industry Thought Leader: Patty ScheetsInfinity Rehab announced this past year that Patty Scheets, PT, DPT, NCS, was promoted as the Vice President of Quality and Clinical Outcomes. According to Patty, her promotion from a director to vice president is an outward demonstration of the importance of clinical quality to Infinity Rehab’s success.

This promotion supports Patty’s diligent work within the company, according to JoLynn Munro, Division President at Infinity Rehab.

“Patty has achieved a position of high influence in the industry and in the company,” JoLynn said. “She has successfully moved our corporate culture of compliance to new levels in the last five years that she has served as the director of quality and clinical outcomes and has consistently led this strategic initiative.”

Among Patty’s other achievements is her re-election as President of the Academy of Neurologic Physical Therapy, a division of the American Physical Therapy Association (APTA). As the ANPT President, she establishes and implements the academy’s strategic plans by working with fellow board members, committees, special interest groups, and task forces.

QUALITY TEAM HIGHLIGHTS OF 2019

→ The Infinity Rehab quality team gave a platform presentation on the results of our gait speed training initiative at APTA Combined Sections Meeting in Washington, D.C.

→ Kele Murdin, PT and Clinical Knowledge Broker, was recognized as Clinician of the Year by APTA Washington.

→ We identified and selected core outcome measures for speech pathology; this will enrich our database with information about critical aspects of executive function and the impact of dysphagia at the patient level.

→ We did a deep dive with our clinical model as we explored implementation in different types of patients and further refined our standards of care.

→ We implemented Infinity Predict in the hands of directors. Predict is built on analysis of our past patients to help identify characteristics and expected outcome for current and future patients. Predict is an important tool in help us deliver consistent high-value care.

→ We implemented the work of clinical knowledge brokers. Clinical knowledge brokers work alongside clinicians in coaching and training clinical behaviors consistent with our clinical model. Clinicians who worked with clinical knowledge brokers demonstrated an average improvement of 56% in specific clinical behaviors associated with best-evidence care.

→ The clinical knowledge brokers also assisted the field in implementing group and concurrent interventions to ensure we maintained our best-evidence care in the context of different modes of care.

→ We spearheaded a pilot project exploring the impact of the addition of systematic delivery of generalized activity and mobility outside of the skilled therapy session, a program called More Movement Matters. The concepts are based on the body of literature that demonstrates that increasing generalized activity and mobility improves health outcomes. This project is in process as we continue to analyze data.

Infinity Rehab and the PDPM Transition

The Patient-Driven Payment Model was the biggest change to healthcare in 25 years. Infinity Rehab prepared for years for the October 2019 transition. Here’s a look into how the therapy company successfully transitioned to this new payment model and how they continue to support and train their staff and customers.

Clinical Model Sets Infinity Rehab Up for Success Under PDPM

Infinity Rehab’s clinical model primed the team for success under PDPM. Infinity Rehab leadership implemented new standards of care based on best-evidence practice in 2017 and through 2018. This clinical model combined with Infinity Rehab’s diligent efforts to lower cost through personalized therapy plans provided a strong foundation for embracing a patient-focused and value-based payment model.

“PDPM provides us the opportunity to deliver a consistently high value care experience to all of our patients by matching the care we provide with the patient’s level of responsiveness,” stated Patty Scheets, Vice President of Quality and Clinical Outcomes for Infinity Rehab. “And we do that by truly practicing at the top of our license.”

Infinity Rehab initiated PDPM plans long before the new model’s implementation, going back five years before October 2019.

“For the last 5 years, we have been quite purposeful in our movement toward value-based care,” Patty said.

In the five years leading up to PDPM, Infinity Rehab leadership provided their staff with training,

resources, and a clinical model, including focuses on therapy intensity and patient engagement in their care.

One of these training sessions was held in May 2019 – see what our directors found most beneficial from the training on Infinity Rehab’s YouTube channel.

Successful teams are agile and pivoting to thrive under PDPM as the Centers for Medicare &

Medicaid Services enforces model updates. Infinity Rehab leaders continue to train

and support their staff on PDPM to serve as a trusted resource for their customers and patients.

Manage & Mitigate Risks

Infinity Rehab has been mitigating risk and remaining compliant

by actively monitoring clinical outcomes and care

delivery under PDPM. This includes reviewing if the treatment

was provided in an individual, concurrent, or group manner. Infinity Rehab leadership provided documentation and coding training to staff. From June through end the of December, Infinity Rehab held weekly director training calls to provide education and support in managing new clinical components.

Maintain Staff for Quality Outcomes

While the new payment model led some therapy companies to substantial cutbacks and lower therapy minutes, Infinity Rehab continued to hire clinicians during this major industry change.

New clinicians are trained on Infinity Rehab’s clinical model as part of the 90-Day Immersion Program, an on-boarding guide for a new therapist’s first 90 days.

Maintaining staff who are experienced with Infinity Rehab’s clinical model contributes greatly to the company’s goal for outstanding clinical outcomes. Standardized assessments based on external evidence, knowledge tools, and other data blends into a model for rehabilitation care to lead to positive patient results.

Collaboration

PDPM united the entire healthcare industry in preparation for this major industry change. Infinity Rehab collaborated with multiple customers in collecting the most accurate clinical information about patients. This resulted in increased communication and more individualized care.

One of these collaborations included Avamere Living. Together they developed pilot programs to help staff under PDPM.

“We’re coming together as a team in which I haven’t seen us do in healthcare in many years,” said Jeannie Russell, Director of Clinical Reimbursement for Avamere Living.

“We’ve said it for years – resident-centered care,” she continued. “But now we’re moving to getting it right.”

Infinity Rehab also collaborated with customers to prepare them for the PDPM change. Through webinars, videos, blog articles, newsletter updates, and other training sessions, Infinity Rehab kept their

customers on the pulse of the PDPM change and what adjustments would be needed. Customers were set up for success.

Outstanding Outcomes Begin with an Irresistible Culture

Even through the PDPM transition, outstanding patient outcomes remained a top priority for Infinity Rehab. Company leadership started with their people in their approach for outstanding patient outcomes.

By creating an irresistible culture, Infinity Rehab prepares its partners and clinicians to achieve outstanding quality outcomes for the people they serve.

Bersin by Deloitte published a case study on Infinity Rehab, stating:

Patty Scheets, Vice President of Quality & Clinical Outcomes

Infinity Rehab announced in September 2019 the promotion of Patricia “Patty” Scheets, PT, DPT, NCS, to Vice President of Quality and Clinical Outcomes.

Patty has been a valuable Infinity Rehab employee for over five years with extensive experience in physical therapy and advancing clinical strategies. She is also president of the Academy of Neurologic Physical Therapy, a section of the American Physical Therapy Association.

According to Patty, her promotion from a director to vice president is an outward demonstration of the importance of clinical quality to Infinity Rehab’s success.

“Infinity Rehab is first and foremost a clinical company,” Scheets said, “and this move to vice president reflects to the outside world the commitment to high quality that those of us who work here recognize.”

Derek Fenwick, Senior Director of Human Resources

Infinity Rehab appointed Derek Fenwick, PT, MBA, GCS, SHRM-SCP to Senior Director of Human Resources, promoted from his role as Director of Professional Development. Derek brings his passion for Infinity Rehab to this new role, as well as a wealth of experience.

Derek joined Infinity Rehab in 2010 and brought over 10 years of experience across multiple areas of healthcare. Derek has a drive for serving and improving the healthcare industry. He became a physical therapist in 2003 and earned his MBA in 2008. Three years later, he became a board-certified geriatric clinical specialist. In 2010, Derek was named an Emerging Leader of the American Physical Therapy Association (APTA). He was also recently appointed to the APTA’s first-ever Leadership Development Committee and serves as President of the Oregon Physical Therapy Association.

Derek now uses all these skills and leadership experience to integrate a comprehensive talent management strategy into all aspects of the employment experience of Infinity Rehab’s healthcare professionals.

HOKU OKUMURA APPOINTED TO OREGON PT LICENSING BOARD: Hoku has been a physical therapist for 10 years, seven of which she has worked with Infinity Rehab. With an immense background in physical therapy, a fellow Infinity Rehab team member and PT board member, Jennifer Julkowski, encouraged Hoku to apply for the board. Hoku’s admission to the board was approved by Oregon Governor Kate Brown. “I am very excited to participate in this process,” Hoku stated. “I have benefited from the work of this board for almost 10 years now, and it will be valuable to observe and participate in the inner workings of this system.”

CLINICAL CHAMPIONS: Clinical Champions is Infinity Rehab’s professional development advancement

program that focuses on identifying and developing the clinical behaviors that are essential for effective clinical practice at Infinity Rehab. Clinician practice is measured in each of five areas across three levels of progressive excellence, providing a clear framework for every Infinity clinician to advance to his or her development within the company. Level 1 achievement is considered essential while levels 2 and 3 show advancement, meeting challenges, and is voluntary for therapists to complete.

We are thrilled to report that Infinity Rehab now has 75 Level 2 Clinical Champions.

PEOPLE DASHBOARD: With this innovative tool, we can assess performance across the company, in a specific region, or one individual community. It enables Infinity Rehab to strategize initiatives and support our team members as needed.

INFINITY REHAB ANNUAL SYMPOSIUM: Infinity Rehab celebrated their 20-year anniversary at their annual conference, Symposium. Team members and clinicians at Infinity Rehab, as well as outside clinicians, attend from different areas throughout the United States. During the weekend-long event, clinicians network and learn by attending courses tailored specifically to them and their daily practice. The event took place in May 2019.

Infinity Rehab Therapists Advocate in Washington D.C.

The National Association for the Support of Long-Term Care (NASL) hosted Capitol Hill visits in February 2020 for therapy providers across the country. Infinity Rehab leaders JoLynn Munro, Division President, and

Michelle Jabczynski, Director of Compliance and Strategy, went to Washington D.C. for the visits. Their goal was to “fight the cut” and advocate against the proposed 8% cut to the 2021 physical fee schedule resulting from E&M changes by CMS.

The highlight of the trip was meeting with Congressman Kurt Schrader (D-OR 5th District). Congressman Schrader is a vocal therapy advocate. With the help of the NASL, an advocacy letter to fight the cut was prepared with the goal of having several congressman and provider specialty organizations sign it before sending to CMS. Schrader signed the letter.

“It’s an honor to advocate on behalf of patients and our profession to make a difference,” noted Michelle. “It’s through this advocacy that we truly see change. Part of advocacy is telling patient stories, which truly makes it impactful and makes a difference. “

Infinity Rehab Patient Walks After Being Bedridden for 10 Years

Infinity Rehab therapists are making a huge impact on the lives of our patients.

One Infinity Rehab patient shared her inspiring journey to walking again with staff and fellow patients and residents at her community, Avamere Transitional Care and Rehab – Brighton.

At only 25 years old, Kristie, whose last name is being withheld for privacy, was diagnosed with multiple sclerosis. By the time she was 35, she could no longer walk. And only three years later, she was bedridden and put on hospice. When she came to Avamere Transitional Care and Rehab – Brighton, Kristie said she hadn’t walked for 17 years.

In June 2019, after several months at the gym, Kristie moved from using a Hoyer lift for transfers to sit-to-stand. And shortly after that, she was able to take a few steps while holding parallel bars. Now she walks with a front-wheeled walker. Kristie was then able to live on her own.

“This story exemplifies the core values established by Infinity Rehab,” shared James Brannan, Director of Therapy with Infinity Rehab, “specifically how closely therapy and restorative teams work together to improve the lives of our patients.”
