Columbus County (CC) was rated the least healthy county in North Carolina according to the County Health Rankings and Roadmaps (http://www.countyhealthrankings.org/app/north-carolina/2015/rankings/columbus/county/outcomes/overall/snapshot). CC is at a crisis point and in desperate need of interventions to improve the health related outcomes of its citizens. In order to address the health issues prevalent in CC a blueprint for change must be implemented with the goal of improving the overall health of the population of CC. CC ranks 98th out of 100 North Carolina counties in length life and 99th in quality of life. Through interventions and the collaboration of multiple stakeholders the goal is to provide services and education to improve health related behaviors in the short term and to improve quality of life and increased life expectancy of the population of CC in the medium and long term. Improving health outcomes directly correlates to improving health behaviors. 28% of all CC residents over 20 report not engaging in any leisure time physical activity. Lack of physical activity and access to exercise opportunities need to be addressed in order to achieve positive health outcomes. Only 40% of CC residents have access to exercise opportunities compared to 76% of North Carolina citizens overall.

Increased physical activity can drastically improve health related outcomes. According to the CDC physical activity can help control your weight, reduce your risk of cardiovascular disease, reduce risk for type 2 diabetes, reduce risk of some cancers, strengthen your bones and muscles, improve mental health and mood, and increase your chances of living longer. The Centers for Disease control and Prevention (CDC) recommends 2 hours and 30 minutes (150 minutes) of moderate intensity aerobic activity per week for adults, and five hours a week to achieve greater health benefits. Due to the lack of PA options in CC the program will institute individually adapted healthcare behavior change programs and interventions that provide social support and resources to encourage participants to engage in recommended physical activity levels. Resources, contributions, and investments can be utilized to reach the target population and result in positive health outcomes and increased PA levels.

Columbus County (CC) has a population of 56, 953, and median household income of 35,719 (census.gov). The biggest challenge to the program is securing funding to provide services to an impoverished population. Columbus County Department of Health will provide free health screenings for all citizens of CC (>18), smoking secession, health fairs, as well health counseling. From those screened program participants will be chosen. Each participant chosen will complete a health survey pre and post program participation. The program will be conducted over 18 months and include up to 2,200 participants (5% of CC adult population). Physicians associated with Columbus Regional hospital and Columbus County Health Department will asses participants, and give them information regarding what types of exercises and at what intensity they are healthy enough to participate in safely. Physical Therapist and Dieticians will work with participants to create an exercise routine specifically catered to the individual participant. Students and staff at SE Community College will moderate community meetings, take surveys, and conduct interviews with citizens in four locations in CC. This information will be used to determine why citizens are not more active and how to create opportunities to become less sedentary. We will work with stakeholders to provide PA venues. In

addition professional staff at SE Community will submit grant proposals and seek other funding from organizations and individuals interested improving health outcomes in underserved communities.

In order to provide the citizens of Columbus County (CC) with safe adequate places to engage in exercise, the program will partner with Columbus County Schools to allow program participants to use school facilities during non school hours. The program will target adults, especially those with children. Active parents are associated with active children. Engaging Adults will create an environment that produces generational positive health outcomes. Columbus County Parks and Recreation will create a new website that highlights ways for the population of CC to be more physically active as well as provide facilities for program participants to exercise and engage in the recommended amount of daily PA activity. “Forming partnerships broadens community support and strengthens the community trust” (Brown, Heath, &Martin, 2010). Columbus County Economic Development Commission will partner with representatives of the community including the business sector, media and communication sector, voluntary organization sector, faith sector, to gage their interest in intended activities or outcomes. CC Economic Development Commission will create a resource guide listing Physical activity locations in Columbus County, and tips for healthy living.

The program’s success will be measured by participant participation over 18 months and the number of program participants that are regularly engaging in the CDC’s recommended PA goals. Pre and post program surveys will be used to evaluate if participants have experienced behavioral changes, and determine if Columbus County has provided more opportunities for Physical activity and exercise. Success will also be determined by the number of initiatives completed including a new Parks and Recreation website (1a), increased community engagement, the creation of a physical activity resource guide, and successful completion of joint use agreements with both Columbus County Schools and Columbus Parks and Recreation. The program is meant to have a community wide impact and create infrastructure to allow this program to be implemented community wide and eventually provide opportunities for all citizens of CC to engage in regular and consistent exercise and physical activity.

Most physical research is guided by social ecological Models (Lecture 2 PRT 510). In order to implement successful intervention all aspects of SEM must be addressed. Columbus County is the least healthy county in North Carolina any successful intervention must start with changing prevailing attitudes and social norms. This program with the contributions of multiple stakeholders can began to create social norms associated with increased levels of physical activity and exercise.

Individual: Program interventions include health related education and counseling provided by Columbus County Department of Public Health and Columbus County Regional Hospitals. The distribution of Physical Activity Resource guide by Columbus County Economic Development Commission. Barriers include SES, education, physical activity levels below the North Carolina average and, teen births, unemployment, and single family households at levels higher the North Carolina average (County Health Rankings).

Social Environment: This program will create social networks and supports through community engagement, meetings, and interventions supported by Columbus County Hospital, Columbus County Dept of Health, Parks & Recreation, SE Community College, and CC Economic Development Commission. The goal is to create a community wide shift towards physical activity within Columbus County.

Physical Environment: Through joint use agreements between Columbus County Schools and Columbus County Parks and Recreation citizens will have greater access to safe places to be physically active and exercise. CC Parks and Recreation will work to improve current infrastructure and give more opportunities for PA and exercise to the citizens of Columbus County.

Public Policy: Long term public policy outcomes include urban planning initiatives including access to greenways, trails, bike lanes, and areas dedicated to exercise and physical activity. In order to create positive outcomes revamped healthcare, workplace, and environmental policies must be instituted to allow for a healthier, happier, and more active Columbus County.
