The mix design has been developed to mcet the Tollway's " Petformance-Related Special Provisionfor High Performance Concrete Mix Designsfor Concrete Superstructure." The mix design was developed by your company and submitted to the Tollway for use in bridge deck construction on Tollway contract I- l2-4078, in accordance with the Contract design requirements. The marerial ingredient sources of the attached concrete mix design fbr production at Super Mix's Hampshire plant are IDOT certified to produce the nuterial classifications and / or grades and rhe aggregate qualities that are indicated on the Tollway's standard A-30 mix design form. This includes the use of "A" quality coarse aggregates. The mixture also contains a saturated lightweight aggregate (Gravelite CF) that is shown by Super Mix to meet the requirements of ASTM C-330, "Standard Specifi.cation for Li1hfieeight Aggregate for Structural Concrete." Ttr- Gravelite CF is being applied in accordance with ASTM C l76l "Standarul Specification for Lightweight Aggregate for Intemal Curing of Concrete," as permitted by the Tollway's HPC special provision. A laboratory trial batch of rhe proposed HPC mix design was conducted by CTLGroup, along with the subsequent performance testing of specimens fabricated during the laboratory tdal batch. The te;t results are attached in the July 3,2013 report from CTLGroup, and are summarized here: I . The initial slump of the mixture (AASHTO T I l9) was 7.5 inches, which is within the required range of3 to 8 inches. The slump atler 45 minutcs was 4.?5 inches, which is greater than the 3 inch requirement. These results indicate that the mix did not lose 4 inches ofslump in 40 minutes, and interpolation of these results indicates that the mixture would not lose more tlan 2 inches of slump in 20 minutes. 2. The Compressive Strength test results (AASHTO T22) indicare a value of6,000 psi at l4 days, which achieves the specification requirement ofa minimum of4,000 psi at l4 days, 6,000 psi will also be the Target Compressive Strength that will be used in tield production evaluations of this mixture. 3. The Time to Cracking test (ASTM Cl58l) was not required for this mix design: the mixturc contains less than 605 pounds per cubic yard of cementitious material, and a minimum dosage of 1.5 gallon per cubic yard of shrinkage reducing admixture (SRA) is used. 4. The Length Change test results (AASHTO Tl60) show a value of0.03l percent after 21 days, which exceeds the requirement ofa maximum of 0-03 percent after 2l days. 5, The Air Void System analysis (ASTM C457) indicates that the trial batch mixture contained the sufficient and properly distributed entrained air required in rhe specification. 6, The Freeze Thaw Durability testing (AASHTO T161, Tollway modified) was nor required by the Tollway, due to the acceptahility of the Air Void System analysis. 7. The Chloride Permeabitity testing (AASHTO T277) conducted at 28 days showed a value of882 coulombs, which achieves the requirement of a maximum of 1250 coulombs at 28 days. 8. Alkali Silica Reactivity testing is not required by the specification, since the total alkali content contributed by the portland cement (2.3 lbs per cubic yard) is less than 4.0 pounds per cubic yard.

Subject: Tollway Mix Design: Performance Related High Pertbrmance Concrete (HPC) for Superstructures

Dear Mr. Anderson:

The Tollway has received and reviewed the attached mix design for production by Super Mix at your Hampshireplant. The mix design has been developed to mcet the Tollway's " Petformance-Related Special Provisionfor HighPerformance Concrete Mix Designsfor Concrete Superstructure." The mix design was developed by yourcompany and submitted to the Tollway for use in bridge deck construction on Tollway contract I- l2-4078, inaccordance with the Contract design requirements.

The marerial ingredient sources of the attached concrete mix design fbr production at Super Mix's Hampshire plantare IDOT certified to produce the nuterial classifications and / or grades and rhe aggregate qualities that areindicated on the Tollway's standard A-30 mix design form. This includes the use of "A" quality coarse aggregates.The mixture also contains a saturated lightweight aggregate (Gravelite CF) that is shown by Super Mix to meet therequirements of ASTM C-330, "Standard Specifi.cation for Li1hfieeight Aggregate for Structural Concrete." A field production trial of the proposed HPC mix design, including on-site quality control testing, was conducted bySuper Mix at their Hampshire plant on May 31, 2013, and observed by Tollway representatives. The trial batch testresults are summalized on the Tollway's Trial Batch Work Form, and indicate that the May 3 l, 2013 trial batchachieved specitication results in the plastic concrete testing. The rial batch plastic concrete test results andsubsequent performance testing of specimens fabricated during the trial batch are also included in the attached in theJuly 3, 2013 report tiom CTLGroup. Those results are summarized here:

l. The initial slump ofthe mixture (AASHTO T119) was 6.5 inches, which is within the required rangeof 3 to 8 inches- The slump after 45 minutes was 6 inches, which is greater than the 3 inchrequirement. These results indicate that the mix did not lose 2 inches of slum p in 20 minutes, or 4inches of slump in 40 minutes, as required by the specitjcations.

2. The plastic air content (AASHTO Tl l9) of the mixture was 5.4 percent, whioh is within the design aircontent of 5.0 to 8.0 percent.

3. The water/cement ratio ofthe field trial was 0.377. This is within the speciticatior range of +0.00/-0.03 of the design water cement ratio of 0.404.

4. The Compressive Strength test results (AASHTO T22) indicate a value of 6,497 psi at 14 days, whichachieves the specification requirement ofa minimum of4,000 psi at l4 days. The field trial stengthalso did not exceed the Target Compressive Strength of 6,000 psi plus 1,500 psi, as required of thefield produced mixture.

5. The Chloride Permeability testing (AASHTO T277) conducted at 27 days showed a value of 1089coulombs, which achieves the requirement of a maximum of 1500 coulombs at 28 days.

Based on the information artd results described above, since the length change results did not pass the testrequirements, the attached HPC mix design containing 1.5 gallons per cubic yard of SRA is not approved.Additional information can be submitted to the Tollway tbr reevaluation of this mix design.

Subsequent to this evaluation, Super Mix proposed that the Tollway consider the mix design acceptable at 2.0gallons per cubic yard olSRA, if acceptable length change test results are provided for that mixture. The Tollwayaccepted that proposal, since the other rnaterial properties should not be greatly affected by this change. Theattached drati report from CTLGroup dated July 26.2013 provides length change test results halfway through the 28day test period. Extrapolation ofthese test results indicate that the Super Mix HPC mix design containing 2.0gallons per cubic yard of SRA should achieve acceptable length change results.

Based on this information and the results described above, the attached Tollway high performance concrete mixdesign for production by Super Mix is given conditional approval for placement on Tollway contracts containing the" Petfonnance-Related Special Provision for High Performance Corlcrete Mix Designs for ConcreteSuPerstnrcl re." The HPC mix design is assigned Tollway mix design number 90PCC1325. Note that this mixdesign can only be produced at Super Mix plants using the identical sources shown in the mix design, and a changein material components to this mix design may require redesign of the mixture.

Full approval of this mix design will be considered when the completed length change test results are provided.Forward these results to the Tollway when they are completed, to tinalize the review.

Descrlptlon Sika Air-260 is a unigue, synthetically manufactured surfactant for the airentrainment of concrete. SikaAir-260's patented formula provides superiorcontrol oflhe air content and finishability in air-entrained concrete. Sika Air-260 increases concrete's durability, making it more resistant to the damag-ing effects of fteezing and thawing, while improving wod(ability and reducingbleeding. Sika Air-260 meets lhe requhemenls ofASTM C 260.

Applications Sika Air-260 is designed for use in exterior and interior concrete such as high-way paving, curbs and gutters, sidewalks, driveways, slabs, walls, or otherareas needing protection from freeze-thaw damage or the hannful effect ofde-icing salts.

Sika Air-260 is effective in increasing the workability of the concrete, espe-cially in lean or harsh mixes.

How to Use

Dosage The recommended dosage range for Sika Air-260 is 0.1 to 6.0 ounces per 100lb3. (7 to 400 ml/100k9) of cementitious material. Various concrete materi-als, slump, ambient air temperature, additions of pozzolanic materials, mixingtime, and type and brand of cement will afect dosage rates.

It is recommended that trial mixes be conducted in order to determine thedosage to obtain the reguired percentage of entrained air. ln addition, a regu-lar program of air tests should be instituted in order to adjust the dosage tochanging conditions. Please contact your local Sika representative or Sika'slechnical service department at 1-800-933-74521or any assislance in dosagerecommendalions.

Measure the required quantity per batch manually orwith automatic disp6nserequipment. Add Sika Air-260 to mixing water or sand. Do not mix with dry ce-ment.

Comblnatlon with other Admlxtures:Wh6n used in combination with other admixtures, care must be taken todispense each admbdure separately into the mix. Combination with other ad-mixtures, may affect the amount of entrained air in the mix. lf multiple admix-tures are used, actual air content should be verified as p6r applicable ASTMstandards to ensure desired level of air entrainment is achieved.

Sika Viscocrete 2100 meets the requirements for ASTM C494 Types A and F.

Appllcatlong Sika Viscocrote 2100 may be used in both ready mix and precast applications, asa plant added high range waler reducer lo provide excellent plastlcity while main-taining slump for up to 90 minutes. Controlled set times make Sika Viscocrete2100 ideal for hodzontal and vertical applications.

Sika Viscocrete 2100 is ideal for production of Self Consolidating Concrete (SCC).

Advantages Water Reducgon: Sika Mscocrete 2100 c€n be dosed in small amounts to oblainwater reduclion from 10-15%, and will achieve water reduction up to 45% at highdos€ge rates.

Description Sika Control-220 is a unique, nonrhloride based chemical admixture that hasbeen specifically designed to reduce concrete drying shrinkage. Sika Con-trol-220 does not contain any chemical ingredients that will cause expansionof concrete.

Sika Control-22o reduces concrele shrinkage when tested against a plainreference mix.

Applications Sika Control-22o is designed for the following applications:r Concrete floors.r Concrete elements which are restrained against shrinkage.r Concrete in marine environments.t V\fatertight concrete in reservoirs, s6wage tr6atm6nt plants, and dams.

r Reduces drying shrinkage cracking.r Can be used in air entrained or non-air entrained concrete-r Reduces the potential for slab curling.r lmproves concrete durability.

Laboratory tests shows that Sika Control-2zo, can reduces long term shrink-age by as much as 40olo. However, the amount of shrinkage reduction isdependent on the mix design, concrete components, and curing regime.

When using Sika Control-22o, lab and field tests are always recommended toveriry the performance and handling characteristics of the specified concretemix.

The required dosage of Sika Control-22o should be added to the concrete atthe batching plant, with the mixing water lt may also b6 added in the transitmixer at the point of discharge, but an additional mixing time of 5 minutes isrequired and the concrete should be visually checked for uniform consistency.

Description SikaTard .140 is a new hydration controlling admixture to mntrol the hydrationof cement. The use of SikaTard 440 allorrs the concrele producer to stabilizeconcrete mixes for long periods without negatively afec{ng concrete durability.

SikaTard,l40 meets th6 requirements ofASTM C-494, Type B and irierim criteriaofASTM C 494 Type D admixture.

How to Use

Do6age Dosage rates will vary according to materials used, ambient conditions and therequirements of a specifc proiect. For nomal applications, depending on retardationrequired, the dosage of Sikatard ,l4O will vary from 2€ fl.oz. per 100 lbs. (130-520mU100 kg) of cementitious mass. For siabilization for longef periods the dosage c€nbe increased up to 48 fl.oz. per 100 lbs. (3130 mli100 kg) ot cemontilious mass.Congete tial mixes must be conduded to detemine th6 optimum dosag6. Formore informaiion about the use of this product please conlact the local Sika salesrepresentative or the Technical Service Department at 1 -80G93+7452.

For best results, add SikaTard 440 lo lhe mix togethsr with the mixing wat6r.During the wet process sholcreling Sigunil accelerator is added at the spraynozzle to initiate fast setting. Preliminary trials on site should be conducted todetermine the required dosage rat$. To optimize the stabilizing effect, afterlh6addition of SikaTard 440, Sika recommends that the combined materials be mixedfor 80-100 revolutions, either in the concrele mixer or in the Ready-Mix truck.

Combination with other admixtrree:SikaTard 440 can be used in coniunction with all Sigunlt accelerators. SikaTard440 is also compatible with all other Sika admixtures.

Attached are the results of qualification testing for the submitted concrete mixlure, tested inaccordance with the lllinois Tollway performance-related special provision for high performanceconcrete for bridge decks.

Testing was canied out in the laboratory by CTLGroup using provided materials in accordancewith the following methods: compressive strength (MSHTO T 22), drying shrinkage (AASHTOT 160), chloride ion penetrability (AASHTO T 277), hardened air-void system analysis (ASTMC457), petrographic examination of hardened concrete (ASTM C856), and chemical analysis ofhardened concrete by XRF (AASHTO T 105). The results are enclosed.

A field trial batch was completed by others on May 31, 2013 at Super Mix's Hampshire Plant.Specimens from the field batch were returned to CTLGroup for testing on June 3, 20i3. Slump(AASHTO T 1 19) and plastic air content (AASHTO T 152) were measured in the field by others.Compressive strength (AASHTOT 22) and chloride ion penetrability (MSHTO T 277) werecarried out in the laboratory. The results are enclosed.

All the testing for the "F30-40" mixture meets the requirement of the Tollway with the exceptionof the drying shrinkage test.

Dimensions: Diameter is 102 mm (4.0 in.); length is 203 mm (8.0 in.).

Top End: Rough and somewhat uneven struck surface. Paste is light gray and hard.

Bottom End: Flat and smooth, formed surface. Paste is light gray and hard.

Cracks, Large Voids: None observed.

AGGREGATES

Coarse: Mixture of crushed rock and lightweight aggregate. Crushed rock is composedpredominantly of dolomite.

Flne: Natural sand composed mainly of quartz, chert, quarkite, feldspar, dolomite, andother minerals and rocks.

Gradation & Top Slze: Visually appears evenly graded to an observed top size of 14 mm(0.6 in.).

Shape, Texture, Oistrlbutlon: Coarse aggregate is sub-angular to sub-rounded, andequant to slightly elongated with rough surfaces; distribution is uniform. Fine aggregate issub-rounded, and mostly equant with smooth to slightly rough surfaces; distribution isuniform.

PASTE

Golor: Medium beige-gray.

Hardness: Hard.

Luster: Vitreous.

Paste-Aggregate Bond: Tight.

Air Content: Measured at 6.7%. Concrete is air-entrained based on the abundance ofsmall, spherical voids in hardened paste.

Depth of Carbonation: Negligible along both struck and formed surfaces.
