OVERVIEW

Complete this form to document the outcome of an eligibility determination conference conducted to evaluate or reevaluate a student suspected to be in need of special education or related services. Typically, an evaluation is conducted to determine initial eligibility. Reevaluation is done every 3 years to determine continuing eligibility. The conference is held with one or both of the student’s parents, the student’s special and regular education teachers, and a representative of the local education agency. These individuals will determine if the student has a specific learning disability (SLD) or other (non-SLD) disability that adversely affects educational performance and requires special education or related services. Before scheduling an eligibility determination conference, the district must obtain written permission to conduct an evaluation (or reevaluation) from the student’s parent or guardian. A ‘Permission to Evaluate’ form should be completed for this purpose. It is also recommended to complete an ‘Invitation Letter’ to schedule and invite the student’s team to the conference. *Note: In some districts, eligibility is included as part of the student’s IEP form. The eligibility sections within this document pertain to the separate EDC/form or when included in an IEP.

FORM INSTRUCTIONS

FORM CREATION

The EDC/form creation process includes the following steps.

Step 1: Select a Student Step 2: Select a Meeting Step 3: Select Evaluations/Tests

STEP 1 – SELECT A STUDENT

1. Click Student Management > Manage an Existing Student. 2. Use the Search window to find the student based on either, Last Name, First Name or Student ID.

3. Click Search. Then click the appropriate Name to select a student. 4. Once a student has been selected, the student’s Name, ID, Birth Date and Age will display. The Forms that

can be created for a student will be listed. Click on the ‘Eligibility Determination Conference’ link.

The EDC Summary screen will display, listing any prior forms created for the student. The most recent form will display at the top of the list. To review an existing EDC, click the appropriate link. To create a new EDC, click the ‘New’ icon.

STEP 2 – SELECT A MEETING

1. First, select a Letterhead to print on the form. This will default based on System/User options as defined by the district. The letterhead determines the name and address information on the first page of the form. The letterhead can be changed.

2. If a meeting had been previously scheduled in IEPPLUS, select it from the drop down list. If not, select

‘Create a Meeting’, then enter the Meeting Date and select a Meeting Type from the drop down list. The type will display on the Summary page that lists all EDC forms for the student.

STEP 3 – SELECT EVALUATIONS/TESTS

1. Select Evaluations to display on the form. ‘Select ALL Evaluations’ will pull all evaluations a student has ever received and place them on the

EDC. ‘Select the LATEST of each type of Evaluation’ will pull only the most recent of each type of

evaluation. For example, if a student received two Social History evaluations, only the most recent would be selected.

2. If ‘Latest’ was selected above, enter a Date Range (e.g., to include evaluations from the last 3 years, enter a

Start Date three years prior to the meeting and use the Meeting Date as the End Date). 3. Click ‘Create Form’.

NAVIGATION/TOOL BAR

After the EDC has been created it will display to the screen. A tool bar will display on the left side of the screen, divided into the following areas:

1. Sections There is a separate link for each section of the form. Click on any link to move to that section.

2. Maintenance These options will maintain Form Information, such as Form Name, Date Ranges, Letterhead, etc. Refresh and Delete functionality is available depending on user security.

3. Validation These options will change the status (i.e., Locked, Ready for Review, Finalized).

4. File Attachments Click the plus icon (+) to add an attachment. Click the minus icon to delete an attachment.

5. Archives Archives are permanent PDF copies of the form. Click Finalize in the Validation section to create an Archived copy. The form should be finalized and archived only after it has been fully completed and a copy has been sent home to the student’s parent(s) or guardian(s).

The following options will display at the top and bottom of the form (see screenshot below).

Information from each section should be saved before moving to another section. Return

Click the Return icon to return to the Form Summary screen. Print Preview

Print Preview will display the form in Adobe Acrobat. Mouse over the bottom of the form preview to see the toolbar illustrated below. Select the Print icon or click the Save icon to save a copy of the form to your computer. The plus (+) icon will enlarge the preview for ease of viewing.

FORM COMPLETION

MAIN The main section of the EDC includes the meeting date, purpose of the conference, the team participants and certain demographic information, such as student name, birth date, and school currently attended. If more information is needed on these fields or any other areas of the form, refer to the mapping section of this guide. Complete the Main section of the form as follows.

1. Meeting Date The meeting record is created typically from the Invitation Letter form or when creating the EDC. The meeting information can be edited by clicking on the link illustrated below.

2. Purpose of Conference Click on the appropriate check boxes to indicate the purpose(s) of the conference. This information is stored in the Meeting Reasons area of the meeting record.

3. Student Information Student information will populate based on Demographics and other Basic Information about the student. Grade will display based on the current Placement record accessible through the Student Toolbar (Program > Placements).

4. Resident District, Serving District, Home School, Attending School This information should reflect the district and school the student would attend if not disabled and the district and school providing special education services. If the information is not displayed, click on the applicable ‘create’ link to create this information.

5. Manage Meeting Attendees Meeting Attendees are typically created in the Invitation Letter. To add, edit or delete attendees, click the link illustrated below.

DOCUMENTATION OF EVALUATION RESULTS *Note: The Eligibility sections can be included within the IEP form or as separate forms on the Student Toolbar. The eligibility sections within this document pertain to the separate EDC/form or when included in the IEP form.

1. Add Evaluation Data Click the applicable text box (see below) ‘Click HERE to enter ‘Academic Performance statement’. Type or paste text directly in the text box and click the disk icon to save. There is a text limitation of 65000 characters. At the upper right of the text boxes notes the amount of text used and available. If the text extends past 65000 characters, the amount available will turn red. The data will not save until the amount of characters is below 65000.

2. Add Evaluation and Test Data Click on the Add Evaluation link, to add a record. On the screen displayed, select the evaluation via the drop down menu and enter the date. Then click save and the application will return to the IEP form. Note: You must add an evaluation record before you can add a test record.

ELIGIBILITY DETERMINATION (SLD) The SLD Eligibility Determination section consists of textboxes and radio buttons, used to indicate whether the student has a Specific Learning Disability that adversely affects educational performance. This section will print if any information is completed on this page. Complete this section as follows.

1. Determinant Factors Click the yes/no checkboxes to indicate if the reason (determinant factor) that the student is experiencing difficulty in school is the result of (a) lack of appropriate instruction in reading, (b) lack of instruction in math, or (c) limited English proficiency. If the student is not succeeding due to any one of these three reasons, the student is not eligible for services under IDEA.

2. Limited English Proficiency The LEP checkbox will default to No. To create a Release/Waiver record and change the checkbox to Yes, click the link illustrated below.

Within this record, select the radio button for the release as well as the start date. Click the Save icon. The system will return to the EDC with the LEP checkbox changed to Yes.

3. Exclusionary Criteria Click the yes/no checkboxes to indicate if the student’s instructional need is primarily the result of a) a visual, hearing or motor disability, b) cognitive disability, c) emotional disability, d) cultural factors, or e)

environmental or economic disadvantage. If any box is checked ‘yes’ the student does not have a specific learning disability.

4. Inclusionary Criteria Click the yes/no checkboxes to indicate the team’s findings relating to a) the student’s deficiencies in educational progress, b) discrepancy data or differences between the student’s progress and the progress of typical peers and c) instructional need for services, supports or accommodations. If any box is checked ‘no’ the student does not have a specific learning disability.

5. Optional Data Chart Insert a data chart that displays the student's performance in reading, writing or math relative to the student’s peer group.

SLD DOCUMENTATION OF INTERVENTION/EVALUATION RESULTS Complete the textboxes in this section of the IEP when a specific learning disability is suspected. Type or copy and paste the text directly into the textboxes. This section will print if any information is completed on this page.

1. Problem Identification Click in the textbox to enter text identifying areas of concern observed in the learning environment, including the general education classroom.

2. Strengths and Weaknesses Click in the textbox to enter text describing student's skill strengths and weaknesses in the identified area(s) of concern.

3. Interventions Click in the textbox to enter text describing previous and current intervention plans implemented for the student.

4. Educational Progress Click in the textbox to enter text describing deficiencies in the student’s rate of learning based on progress data.

5. Plan Evaluation/Discrepancy Click in the textbox to provide discrepancy data, describing the difference between the student’s progress and the normative progress for typical peers.

6. Instructional Needs Click in the textbox to enter text describing the services, activities, supports, accommodations, and modifications needed for the student to make curricular or developmental progress.

7. Additional Information Click in the textbox to enter text providing any other relevant information not covered in the preceding sections.

ELIGIBILITY DETERMINATION (SLD) This section will not print unless the student has an SLD Disability or if step 2 is completed. Complete this section as follows.

1. Eligibility Determination (Step 1) Indicate whether the student has a specific learning disability that adversely affects educational performance. The yes/no checkbox will default to no. To change the checkbox to yes, click the link illustrated below.

Clicking this link will display the disability record. Indicate if the disability is Primary (Priority = 1) or Secondary (Priority = 2). Click the Save icon to save the record.

After saving the disability record, the eligibility checkbox will update to yes. Check the appropriate boxes to indicate the nature of the learning disability (e.g., basic reading skills, etc.).

2. Special Education and Related Services (Step 2) Click the appropriate checkbox to indicate whether the student is or is not eligible due to SLD.

ELIGIBILITY CRITERIA This section covers detailed criteria used to aid in identifying a student’s disability. The team should complete each section in which the student is suspected to have a disability (i.e., Speech or Language Impairment, Emotional Disability, Cognitive Disability, Developmental Delay, Traumatic Brain Injury, Orthopedic Impairment, Other Health Impairment, Attention-Deficit Hyperactivity Disorder, Hearing Impairment, Deafness, Visual Impairment, Autism, Deaf-Blindness, Multiple Disabilities). These sections will not print on the report unless the checkbox found in the header of the applicable disability section is marked.

*Note: There are currently three versions of the Eligibility Criteria section in use (standard, 1A and 1R).

ELIGIBILITY DETERMINATION (NON-SLD) This section consists of textboxes and radio buttons, used to indicate whether the student has a disability (other than a Specific Learning Disability) that adversely affects educational performance. Complete this section as follows.

1. Determinant Factors Click the yes/no checkboxes to indicate if the reason (determinant factor) that the student is experiencing difficulty in school is the result of (a) lack of appropriate instruction in reading, (b) lack of instruction in math, or (c) limited English proficiency. If the student is not succeeding due to any one of these three reasons, the student is not eligible for services under IDEA.

2. Limited English Proficiency The LEP checkbox will default to No. To create a Release/Waiver record and change the checkbox to Yes, click the link illustrated below.

Within this record, select the radio button for the release and the start date. Click the Save icon.

3. Disability (Step 1) Based on the team’s analysis of the evaluation data, identify the student’s disabilities (other than specific learning disability), if any. Click the link corresponding to the specific disability (e.g., Autism, Visual Impairment, etc.). This action will display the disability record. Within this record, indicate whether the disability is Primary (Priority = 1) or Secondary (Priority = 2). If no disability was found, skip to Step 4.

4. Adverse Effects (Step 2) Click the checkbox to indicate whether the disability has an adverse effect on the student’s educational performance, or not. Click in the textbox to enter text, describing how each disability has an adverse effect on the student’s performance. If the disability does not have an adverse effect on the student’s educational performance, the student is not eligible for special education services. In such a case, indicate ‘No adverse effect identified’, and skip to Step 4.

5. Educational Need (Step 3) Click in the textbox to enter text describing the extent to which the student requires special education and related services to address the educational needs.

6. Eligibility (Step 4) Click the checkbox to indicate whether the student is eligible for special education and related services, or not.

DATA MAPPING

SPED# (Special Ed File Number) up to 10 alphanumeric characters in length will print in the bottom right corner of the EDC report based upon the student’s Basic Information, Identifications, Type = Special Ed File Number. The SPED# does not display on the web form. If present, the Special Ed File Number will print on the report.

Active Radio Button ‘No Disability Identified’ creates Disability record = ‘Not Eligible for Special Education Services’, Alternate Code = 98 with meeting date start/end. Inactive Radio Button ‘Disability Identified’ is checked if an active disability is found. The active links display all available disability codes. Click the link to create, update or delete a disability, Alternate Code = 0,A,H,G,N,K,F,M,C,L,I,P,E,D. Check Box (Primary or Secondary) will display based on the Priority assigned to the disability (Priority =1 is Primary, Priority > 1 is Secondary).

Statement selection is based on Type = Statement, Category = ‘Determinant Factors (SLD)’ or ‘Determinant Factors (Non-SLD) and Subcategory = ‘Reading’, ‘Math’ or ‘LEP’.
