For support call Digital Health Clinical Consultation Services on 3176 8800 ieMR Enhancements – Pharmacy Care Organiser (PCO) The Pharmacy Care Organiser (PCO) will be available within the ieMR from Wednesday 1 July 2020. You will receive notification from your Director of Pharmacy once the PCO has been validated and is ready for use. The PCO is a real-time, interactive display of a patient list which includes some clinically relevant information specifically designed for Pharmacists. It is expected to significantly aid in the Pharmacist’s daily workflow by providing an overall view of their patient load, displaying some face-up clinically relevant information, and enabling handover communication to be documented and available in a more consistent manner. The overall purpose of the PCO is as a prioritisation tool for Pharmacists within ieMR. It is important to note that the PCO is not to be utilised to clinically review a patient. All clinical patient reviews are to continue as per usual workflows. The improvements include: Ability to view patient load including reconciliation status and length of stay within one view Snapshot of some clinically relevant patient information within one view Prioritisation tool to assist in managing daily workload Digital handover communication tool that can be shared with colleagues, including Pharmacist Interns and Pharmacy Technicians, through PowerChart Although the PCO is a great enhancement for the Pharmacy workflow, there are a few limitations to be aware of: PCO is not currently available through FirstNet. It is anticipated that PCO will be made available through FirstNet in June 2020 through the change request process It is not currently possible to scroll from left to right on the PCO screen therefore screen resolution may be an issue if wanting to include numerous columns Relevant for paediatric and neonatal areas, the Patient Demographics column currently rounds the patient weight to two decimal places What is changing? The PCO is an additional prioritisation tool for Pharmacists. It is not a mandatory tool and local workflows will define the purpose and recommended utilisation of this tool. Users are able to customise their own PCO view by: Enabling/disabling columns depending on clinical need Rearranging the sequence of columns Sorting the entire patient list per the contents of an individual column

ieMR Enhancements – Pharmacy Care Organiser (PCO)

PCO is available for the following ieMR roles within PowerChart:

• R4 Pharmacist • R4 Pharmacist Intern • R4 Pharmacy Technician

The following table explains the available PCO columns and included functionality:

PCO Column Details Patient Column • Displays patient information such as name, age, sex, Date of Birth, and UR number

(MRN) • The patient list default is sorted alphabetically by patient surname • Clicking on the patient name will open the patient’s chart • Similar patient names within a patient list are highlighted with an asterisk (*) to

caution the end user to potential selection error Location Column • Displays the patient's current location including the nurse unit, room number, and

bed number • Clicking on the Location header will sort the patient list alphanumerically (in

ascending order) Visit Column • Displays Admit Date and Length of Stay

• Clicking on the Visit header will sort the patient list by length of stay (in ascending order)

Allergies Column • Displays the status of recorded allergies for each patient • Clicking in the cell will generate a pop out window with more detailed information

about the allergies • Various icons are displayed to represent the recorded allergy status;

Patient Demographics Column

• Displays Pharmacy relevant demographics such as patient weight, height, Creatinine, BSA, and IBW

• Only weights documented within the last 7 days and heights documented within the last 90 days will appear

• Weight is rounded to two decimal places Reconciliation History Column

• Displays icons to indicate the medication history, admission reconciliation, and discharge reconciliation statuses for the patient's encounter

• Clicking within the cell will generate a pop out with a more detailed view

Unverified Orders Column

• Displays a count of unverified pharmacy orders for each patient's encounter • If the patient has no unverified orders, the column is blank • If any unverified orders have clinical checking alerts or other Discern alerts,

the alert icon is displayed in the cell. This alert icon displays all alerts that have been built against the medication orderable and does not identify if any alerts were presented to the prescriber at the point of ordering.

• Unverified orders that are also awaiting product assignment are displayed with the product assignment icon

• Clicking within the Unverified Orders cell for a patient will generate a pop out with additional information about the unverified orders, including how long the orders have been unverified

• Clicking on the Unverified Orders column heading will sort the patient list in descending order by the count of unverified orders

• Launches the PharmNet profile for the selected patient through the “Launch Pharmacy Medication Manager” link

Problems Column • Displays information about Alerts and Problems that have been documented for the patient

• This column currently displays all active and resolved Alerts and Problems therefore it is important to click on the Problems cell which will open a detailed pop out with further information around the status of the Alert/Problem

Diagnoses Column

• Displays information about diagnoses that have been documented for the patient within the current encounter

• Clicking in the Diagnoses cell opens a detailed pop out with further information Comments Column

• Can be used by Pharmacy staff to add, review and delete Pharmacy specific comments relevant to the patient

• Clicking in the Comments cell opens a detailed pop out with all comments

• Hovering over the comment in the detail pane displays the deleting icon . When deleting, users are first prompted with an alert to confirm deletion

Discharge Column

• Displays a black door icon for any patient with a Discharge Order • Note: it is not current workflow within Metro South to have Discharge Orders placed

against patients therefore this column will be of no to low use Consult Orders Column

• Displays a count of the current consult orders for each patient that fall under the Pharmacy or Pharmacy Consults activity type e.g. Pharmacist Clinical Review, Consult to Pharmacy

• Follow up to Pharmacy orders are not displayed in this column as they are built under the Allied Health Consults activity type and will bring in all other unrelated Follow up orders

• All patients will have a minimum value of 1 within this column which will be the automatic Pharmacist Clinical Review order
