Description of Process:Intake/Assessment: The majority of our clients are self referred, howe r many county and private fostercare/adoption agencies contract with CFL in support of their fami es. Many of our clients haveparticipated in trainings or workshops with Juli Alvarado and seek er out for further and ongoingattachment/adoption/foster care support. The intake and assessment st ge of our work is critical for thebuilding of relationship upon first contact.

During this stage we typically meet only with parents, sometimes by ph ne as we work locally, nationallyand internationally, and whenever local, in the home. We do not see c . dren without parents, and we donot provide office based therapy-we find it much more effective to m et with families in the context inwhich they actually have to live and relate.

Treatment planning:The parent is the most vital component to treatment. I can not provid for your children what they need,only you can. Yet it is the relationship with you that they need the mo t, that they will also, at times, fearthe most. That is attachment challenge at its core. All of our treatment lanning is geared toward:

Creation of regulation in relationship between parents st, in order to provide that fortheir children

Empowering child and parent to feel, express, process all emotions, no matter how big,but in a safe manner

Treatment Techniques:Regulated relationship is the most powerful tool that I as a therapist, c provide for you, my client. I willwork intentionally, purposefully and consciously toward a safe relation hip to you, so that you in turn mayprovide that for you child, or in an organizational capacity, for your aff. If you can not be vulnerable,open, honest and real with me, I can not help you help your child do th same in relationship to you. I takemuch responsibility for that, as you will need to do with your child or s ff.

All treatment is geared toward emotional regulation and the completio of development stages that, due tomaltreatment, abuse and neglect, have not reached optimal state. ReI ant(development age appropriate),repetitive, relational and rewarding interventions by both therapist and arent are taught.

The biophysiology of stress as well as the neurophysiology of re lation and attachment are primaryfocuses of our interventions toward healthy attachments. Built on the most solid rese ch in neurobiology, psychology,psychiatry, development and learning Emotional Regulatory Healing is a paradigm for treatment andparenting that focuses on a deep understanding of trauma, regulation d attachment systems. We teachyou how to parent from the inside out, focusing first on internal proces es of your child and yourself, andthen on the external manifestation or behavior.

SafetylRisk Management:Every interaction, with parent or child, or staff in an organizational cap city, is directed at regulation. Weseek to create an environment first that prevents dysregulation and that is skilled and prepared to handle itwhen it does occur. We NEVER use restraint, physical holds of any e, or any form of coercion. Weremain with the child who is scared and dysregulated until such time th the child can return to functioningwith out harm to self or others. Although we would not intentionall work to escalate a child, we alsoknow that this is the window of opportunity for healing, if the parent an remain engaged, safe, regulatedand available as the child shifts back down to a state of allowing the pent/child relationship to unfold. Ifneeded, I would contact emergency support, but to date have never ad to do that(1l-09, 15 years ofpractice) and as a matter of fact, believe that it is harmful to the chil unless there are literally no otheralternative. In many situations I will use a co-facilitator if I am wor ing with a large family, or with afamily of a child who tends to be more dysregulated, more frequently, a the inception of treatment.

We adhere to the safety principles as outlined in the ATTACh Professio al Practice Manual.

Evaluations/OutcomeslFollow up:Coaching for LIFE has been involved in a longitudinal research roject, with regard to our modelEmotional Regulatory Healing, in collaboration with SAFY, Treatmen Foster Care and Adoption. We arein the second year of intensive training in trauma, attachment, and em ional regulation of all staff, and alltreatment parents across the 8 states in which they are licensed to pro ide services. Each new placementfor one year post training will be included in this project and we antici ate at least 200 children/families tobe involved. Data collection and follow up in all 8 states began in Ju 2009. We anticipate outcomes toreport by Dec, 20 10.

We also conduct internal follow up with each client family, through ph ne and email contact 30, 90, 120days and one year post final session. This provides the opportunity t support and assess the need forfurther treatment.

Many attachment challenged children are taken in by the most well inte ded foster and adoptive parents. Iam one of those foster parents. I know first hand how often we e intellectually and emotionallyunprepared for the task before us. Many of our children are not capabl , yet, of giving and receiving lovein a way that makes sense for our families. Many of these children end p constantly defying and fightingus. Families suffer, marriages suffer, and traumatized parents relinquish heir children, or have them placedinto residential treatment when there are no other options. Early educati n and effective interventions mayprovide you with another option. We would be honored to support you a d your family.
