... to the Fifth Third Bank Health Savings Account (HSA) solution. Your new HSA gives you the opportunity to save money for both short- and long-term medical expenses, while enjoying certain tax advantages. Plus, we’ve made it easy and convenient for you to manage your account online, around the clock.

This Resource Guide provides details on how to use the Fifth Third Bank online HSA banking system to perform important account maintenance tasks, including setting up your profile, adding dependents, making contributions to and distributions from your HSA, and more.

Now that you have opened your Fifth Third Bank HSA, it’s important to have a basic understanding of how HSAs work, and what this account can do for you. This Resource Guide provides an overview of key features.

How it Works

Your Fifth Third HSA allows you to save pre-tax income that you can use to pay for qualified short- and long-term medical expenses. It complements your High-Deductible Health Plan, giving you an additional method to save specifically for healthcare costs. Here’s how it works:

ContributionsTax-free contributions to your HSA can be made in a variety of ways, including:

1. Pre-tax payroll contributions, if available through your employer

2. Online transfers — transfer funds directly to your HSA from your linked personal savings or checking account, even if it is not a Fifth Third account (See page 8 for the steps required to link your bank account to your HSA.)

3. Send a check to Fifth Third Bank for deposit into your HSA (Note that others, including family members, employers, and anyone else can make contributions on your behalf as well.)

4. Rolling over or making a transfer from an existing HSA, FSA (Flexible Spending Account), or IRA (Individual Retirement Account)

DistributionsDistributions from your HSA are used to pay for qualified medical expenses. This can be done via the following methods:

1. By paying for purchases and medical services using your Fifth Third HSA Debit MasterCard®

2. By using online bill pay via your online HSA portal

3. By requesting a reimbursement via the online portal when you have already paid out-of-pocket for qualified expenses

4. By paying with an HSA check

Eligible ExpensesHealthcare costs that qualify for payment using HSA funds include:

n Doctor Visits (including co-pays, deductibles, co-insurance, etc.)

n Dental and Vision Services

n Chiropractic Care

n Prescriptions

Using Your HSA Online Tools

The following sections provide steps for performing the most common tasks via your online Fifth Third HSA portal. Please review these instructions and, if you have further questions about these or other account functions you need to perform, contact Fifth Third’s HSA Support Center at 1-888-350-5353.

Logging in for the First Time

To log into the online portal for your Fifth Third HSA, visit www.53hsa.com, click on Account Access, and sign in with the username and password designated for your account. If you experience any problems logging in, contact our Support Center team at 1-888-350-5353.

The first time you log into your online account, we recommend that you verify and/or complete your personal profile, as well as other account details such as beneficiaries, dependents, and the personal checking or savings account that will be used to fund your HSA.

 

Go to www.53hsa.com and click Account Access to log in.

Enter Username and Password and click Login

Forgotten Password

If you’ve forgotten your password, simply go to www.53hsa.com, click Account Access, and enter your username. Then click the I Forgot My Password link. After correctly answering the security question, you will be provided with a temporary password, allowing you to log back into the system.

Navigating the Site & Checking Your Balance

After you log into your account, you’ll be greeted with an opening page that gives you a quick overview of your account, including your current HSA cash balance.

Alternatively, you can click the page name links at the bottom of each page on the site.

Navigate your online account by rolling over the tabs at the top of any page and clicking on the appropriate subpage title.

To see recent account transactions, click the View Activity link.

Threshold

In the same way that your Cash Account Threshold determines when contributions are swept into or out of your available Investment Cash Account, Fifth Third will automatically move funds out of your HSA investments, and back into your liquid HSA, ensuring that funds are always available for your use and that your cash balance remains steady.

The default balance threshold for your Cash Account is set at $2,000. If you’ve invested more than $2,000 into your HSA, the remainder is automatically swept into your Investment Cash Account (which holds funds that are available for investing, but have not yet actually been invested). Similarly, when you pay bills from your HSA, surplus funds are automatically moved back into your Cash Account to maintain a balance of $2,000.

With Fifth Third HSA online tools, you can set your HSA Cash Account threshold to any amount equal to or greater than $2,000 if you wish. This helps you to accommodate larger medical expenses that you can anticipate — such as minor surgeries, etc.

To change your Cash Account Threshold:

1. Roll over the Accounts tab and click on Account Summary.

2. Click the blue Set Investment Transfers text link on the right side of the page. A notification will appear with a brief description of how the account threshold feature works. This area contains a box where you can enter your chosen threshold.

3. Enter your desired threshold, and click the Save button.

Your Profile Summary Page

Personal InformationTo ensure that your personal information is kept up to date, you can add and edit these details via your Fifth Third HSA online portal. Here’s how:

1. Roll over the Profile tab at the top of the page, and click on Profile Summary.

2. This page displays information such as:

> Your personal details

> Your eligible dependents

> Your beneficiaries

> Your bank accounts

3. Click on the appropriate blue text links at the right side of the page to add or update each of these sections.

4. Each time you edit one of these sections, be sure to click the Submit button at the bottom of the page to save your changes.

Note: To add/edit dependents and beneficiaries, you will need personal details such as Social Security number, address, and date of birth for each individual. To add/edit bank account details, you will need your routing number and account number.

Update Profile text link Click to change personal details such as your address, home phone, and email address.

Add Dependent Click to add a new dependent.

Update Click to change the personal details for each dependent.

Update Click to change details for an individual account.

Add Beneficiary Click to add a new beneficiary. Once entered, an Update button will appear for each beneficiary, allowing you to edit personal details.

Add Bank Account Click to link a new account that you wish to use to fund your HSA, or to receive distributions. (See following page for details about security steps involved in adding a new bank account.)

Profile Summary Page — This page provides links to your Dependents, Beneficiaries, and Bank Accounts screens. You can also reach those pages by rolling over the Profile tab, and clicking on their respective page names.

Security Steps for Adding a New Bank Account

When you add or make changes to a linked bank account to fund your HSA, you’ll be taken through an additional step to confirm your identity and account access:

1. Fifth Third will make a small deposit to the linked account, in the amount of $0.01 - $0.99.

2. You will log into the identified bank account to view the transaction amount.

3. Upon logging back into your Fifth Third Bank HSA, you’ll be prompted to enter the transaction amount in order to confirm receipt and verify that you are the account owner.

4. We’ll then withdraw the same amount to cancel out the transaction.

This process provides an extra layer of security in preventing unauthorized users from making fraudulent transactions through your online HSA. You will only need to perform this step once for each account addition or modification that you make.

Debit Cards

To review a list of the Debit Cards that have been issued for your account, roll over the Profile tab, and click on Debit Cards. This will take you to a summary page showing the names of the cardholders, and the last four digits of each card number, along with the expiration date for each card.

Your first two Debit Cards are free. For additional cards, you will be charged a fee of $10 each, and this fee will be deducted directly from your Fifth Third HSA.

Using Your HSA Online Tools

To report a card as being lost or stolen, click the Report Lost/Stolen text link next to the appropriate card.

To order a replacement card, click the Order Replacement text link next to the details for the card you wish to replace. We will send you a new card with the same card number, unless you have also reported the card as lost or stolen.

Note: To order the initial debit card online, click Accounts > Change Payment Method > Issue Debit Card.

Ordering a Checkbook

If you prefer to use paper checks to pay for some or all of your medical expenses, you may order an HSA checkbook from Fifth Third. Note that a fee of $15.99 applies for each box of checks you order, and this fee will be deducted directly from your HSA.

To order your checkbook:

1. Roll over the Profile tab, and click on Order Checks.

2. Review the personal information that will be printed on the checks and, if everything is accurate, simply click the Order Checks button at the bottom of the page. Your checks will arrive in approximately 2-3 weeks.

3. If personal information is not correct, you may need to edit your personal profile. To do this, roll over the Profile tab, and click on Update Profile. Then follow the instructions on page 8 of this document.

Contributing to Your HSA

To contribute funds to your Fifth Third HSA:

1. Roll over the Accounts tab, and click on Contribute HSA Funds.

2. Review any details provided regarding previous contributions, to see how much more you are allowed to contribute. (Click on the links on the right side of the screen regarding IRS and Scheduled Contributions, if you need more information.)

3. Under the heading Contribution Details, review the bank account information provided. If more than one linked bank account exists, choose the one you wish to use to fund your current contribution.

4. If making your contribution between January 1 and April 15, be sure to select the year to which you wish to apply your contribution for tax purposes.

5. Enter the amount you wish to contribute, along with a note about the contribution.

6. Review the Contribution Disclaimer, and select the checkbox next to “I have read, understand and agree to the information and terms above,” which appears in red at the bottom of the page.

7. Click the Add Contribution button. Note: Transfers from linked bank accounts are typically completed in approximately 2-3 days. If you anticipate a doctor appointment or other medical expense, plan ahead and fund your account early.

Requesting Distributions

To request a distribution of funds from your Fifth Third HSA:

1. Roll over the Accounts tab, and click on Request HSA Distribution.

Note: Your Account Summary (cash balance) appears in the first section of the page. You may only request a distribution up to the amount of the account balance; the system will display an edit if the request is greater than the available balance.

2. Select whether you would like the funds to be distributed to yourself, or to another payee (such as your doctor). If you select Someone Else, the page will expand, allowing you to choose the appropriate Payee (or to add a new one).

3. Enter the amount of the distribution, and choose whether you would like this to be a one-time payment or a recurring one.

4. Choose the method of distribution from the dropdown box provided.

5. Use the notes box if you would like to add information about the distribution.

6. Review the Disclaimers, and select the checkbox next to “I have read, understand and agree to the information and terms” at the bottom of the page.

7. Click the Request Distribution button at the bottom of the page to finalize the transaction.

Note: You are only able to request a normal distribution from the consumer portal for online bill payment or reimbursement. Any other distribution types require additional documentation. Click on the text link How to request other distribution types on the far right side of the Distribution page for details.

Choose Distribute Funds to Me to receive a direct deposit into your selected linked bank account. (If you have multiple linked accounts, choose the appropriate one from the dropdown box entitled Bank Account.)

Choose Distribute Funds to Someone Else to perform an online bill payment and enter the appropriate payment information.

Recurring Payments can be set up to be automatically deducted from the HSA and sent to the provider.

Information entered here will appear on the memo line of the check.

Note: Online bill payments will generate a check mailed directly to the provider, and the provider will receive in 5-7 business days.

Viewing Online Summaries

To view monthly account summaries, roll over the Notifications tab, and click on HSA Account Summary Reports. The page lists all available account summaries. Click the blue View Report text link next to the appropriate summary date. The report will open as a PDF.

Reviewing Your Investments

If you have funds in your Investment Cash Account, your Fifth Third HSA online portal will allow you to view details of your HSA investments without having to visit a separate site.

To access your HSA investments:

• Roll over the Accounts tab and click on Account Summary.

• Then click the Investment Details link.

You will be required to respond to a security question in order to view investment details.

My Performance provides returns for all the funds available in the account, and gives you access to a variety of performance charts to show how well your investments are working for you over the long term.

Navigation on the left side of the page will allow you to access different areas of your investment account.

Allocating Funds

To redistribute future contributions to your HSA investment funds:

1. Click on Investment Elections, under the Manage My Account heading on the left side of the page.

2. Use the dropdown boxes to select the percentage of your available investment funds to be allocated to each fund.

3. Click the Submit Election Change Request button at the bottom of the page to save changes.

Note: The total allocations must equal 100%. You can view or cancel your pending transaction by clicking Pending/Activity History from the menu on the left.

 Note: Transfer Investments – moving some or all of the current dollars in the account from one fund to another.

Automatic Rebalance – keeps the portfolio on auto pilot once the investment elections have been set.

Transaction Detail shows all transactions for a specified period; you can use the provided calendars to view specific timeframes, or use dropdown boxes to choose which transactions to display.

Fund Activity Summary provides fund activity information, including withdrawals, fund transfers, changes in market value, and more. You may use dropdown boxes to see the activity you would like to view.

Forms & Documents Related to Your HSA

Fifth Third has included several valuable forms and documents in your online portal, to make using your HSA easier. To view these documents, click the Forms tab. Following are just a few of the forms you will find in this location:

Mail-in Contribution FormThis form allows you to mail a check from yourself or someone else as a contribution to your HSA. Print and send the form along with your check to ensure that we have all necessary information to expedite the deposit.

HSA Investment OptionsThis document provides details on the investments available to you through your Fifth Third HSA.

HSA Interest Rate InformationAllows you to see up-to-date information on interest rates related to your HSA. Check here periodically, since rates will change over time.

HSA Transfer FormThis is the form you will need to complete in order to transfer funds from another HSA to your Fifth Third HSA.
