Who is eligible to enroll in a HSA

• You must be enrolled in a HSA-compatible* health plan (HDHP). Additionally, you must NOT be: Covered by any other non HSA-compatible health plan Claimed as a dependent on another person’s tax return

(excluding spouses per Internal Revenue Code) Enrolled in Medicare Receiving health benefits under TRICARE Receiving Veterans Administration (VA) benefits within the past

three months. Covered by a spouse’s general purpose health care flexible

spending account (FSA) or health reimbursement account (HRA).

*An HSA-compatible plan satisfies minimum annual deductible and maximum out-of-pocket costs per Federal guidelines.

Who puts money in my HSA?

•Your employer and you (if you choose to)

Eligible medical expenses to use HSA funds

A list of Qualified Medical Expenses can be found in IRS Publication 502, http://www.irs.gov/pub/irs-pdf/p502.pdf. As described in IRS publication 969, http://www.irs.gov/pub/irs-pdf/p969.pdf, over-the- counter medications are

considered Qualified Medical Expenses for HSA purposes.

HSA and Medicare • If you are on Medicare Part A and/or Part B you will not be

allowed to contribute to your HSABank account. Your employer will not be able to contribute to your account either.

• If you are on Medicare and covered under Thrifty White Pharmacy group HSA plan with your spouse and any tax dependent children. Your spouse can open an HSA account and make contributions. Their contributions would not be pretax from your paycheck, but you can claim a tax deduction on their contributions every April 15th, however you would still have paid payroll taxes on their contributions that you would not be able to recoup.

When to elect Copay plan NOT HSA plan • You the employee is age 65 or older and covered under

Part A or Part A & Part B Medicare

• You are taking a prescription drug that is categorized by Medica as a Tier 3 prescription drug The Copay plan will pay for Tier 3 drugs, the HSA plan will NOT To find Tier 3 drugs call 1-800-952-3455

• We are not allowed to waive the third month copay on a 90 day supply filled through our central refill pharmacy on the HSA plan

How to Get Funds From Your HSA • What you should know about the HSA VISA:

The VISA can be used as a CREDT CARD or DEBIT CARD.

• We recommend that you always use it as a credit card. Why? There is NO transaction fee when “credit” is used and the maximum limit per day is set at $2000.00. The “debit” should only be used for cash machine withdrawals. For each cash withdrawal transaction

you will be charged $2.00 by HSA Bank plus any additional fees from the financial institution that owns the ATM. When prompted if you would like to withdraw funds from “Checking” or “Savings”, select “Checking”. The maximum limit per day for debit card usage is set at $300.00.

• IT IS YOUR RESPONSIBILITY TO KNOW YOUR CURRENT HSA BALANCE AMOUNT.

There are 4 easy ways to find what your balance is: Statements will reflect all transactions processed, fees/charges to your account, and your current

balance. To speak to a representative, call Client Assistance Center at (800)357-6246 Monday through Friday,

7 a.m. to 9 p.m., CST. For an automated system, call Bankline at (800)565-3512. This is available 24 hours a day. To view your account online, go to www.hsabank.com. Your account can be accessed online 24 hours

a day. To view your account for the first time, you will need to set up your account with the Internet Banking Sign-up Form. You will also need to create your User Name and Password.

How to pay your pharmacy bills with HSABank

• Pharmacy – What to do when paying for prescription medications Show your Medica ID Card to the pharmacist. The pharmacy will process the claim immediately with your medical

insurance company. Pharmacist will present the bill for the cost of the medication

Use your HSA VISA Card to pay for the bill, if funds are available. a) To cover for 100% of the cost, use as a “credit” transaction, NOT “debit”.

Attach the pharmacy receipt and the credit card receipt together in an envelope labeled “HSA Allowable Expenses” for your records.

Benefits (EOB). a) If you have any questions on the EOB, you can call Medica

To pay your bill, you can call your clinic and pay over the phone by giving them the number of your HSA Bank VISA Card, or wait for the new bill from the doctor’s office and complete the payment stub with your HSA Bank VISA Card information for payment.*
