Hospitalist Overview The Hospitalist Physician Experience Playbook provides a comprehensive approach to maximizing a Hospitalist’s
productivity in Cerner Millennium through optimal design, configuration, content and training. Designed to meet the
clinical and inpatient needs of Hospitalists, the playbook provides standard settings and content based on our
experience in implementing our solutions across our client base. Over time, we have found that these standards help
Hospitalists improve regulatory compliance, productivity and patient safety.
The Hospitalist Playbook Content provides ways to enhance the experience for a Hospitalist through the following
content:
Hospitalist Workflow View - Streamlines the Hospitalist’s workflow to improve efficiency and increase
productivity.
Hospitalist Summary View - Contains components and information that matters to a Hospitalist, and
provides an "at-a-glance" view of the patient's status.
Hospitalist ViewPoint- Provides a single location from the PowerChart Table of Contents for the
Hospitalist to locate the Workflow and Summary views.
Table of Contents- Simplifies the Table of Contents to include only the sections of the chart that are most
important to a Hospitalist.
Quick Orders & Charges- Provides the most common orders placed by a Hospitalist in one place, allowing
them to quickly complete the ordering process.
PowerPlans- Increases the efficiency of the Hospitalist’s ordering process by providing PowerPlans for the
most common diagnoses, identified by the specialty audits.

In the sections of this page you will find configuration recommendations for each of these enhancements for

Specialty Position Build Physician positions are each different in their own ways. Therefore it is recommended that each specialty physician
position has an individual and unique position build. This is an essential part of the developed standards that will
allow clinicians to take full advantage of the playbooks and pertinent tools throughout Millennium. This Specialty
Position link will assist with the build process and the creation of the platinum position.

Physician Handoff View Physician Handoff provides a standardized approach to the transfer of care and responsibility for a patient from one
physician to another during a shift change. Studies have validated that inadequate handoff processes attributed to
poor communication can contribute to adverse patient events (7.5 problems per 100 patient days).
To help establish a standard format for what information is shared during the handoff process, the I-PASS format
has been adopted into this solution. I-PASS is an acronym for:

This standardized approach provides the following benefits:

Easy to use, intuitive solution that requires minimal training

Page Level Options:
Patient List Menu - allows for the selection of the desired patient list. This will pull from the clinician's
existing PowerChart patient lists.
Physician Contact - provides the ability to batch assign patients to myself or another provider.
Patient List Management - allows the physician to add patients from one list to another from the Handoff
view.
Print option - provides a printed version of the Handoff that can be leveraged in the absence of a computer
device.
Patient List View:
Location - displays the location of the patient.
Patient Information - includes patient name, age, gender and patient identifier (facility may choose to
display MRN, FIN, or other patient identifier).
Contact Physician - displays the physician currently in charge of the patient's care (independent of the
Patient Provider Relationship in PowerChart). Clicking on this field allows the physician to assign
themselves as the contact physician. With the 5.0 release, this field will auto populate with the Attending
Physician name the first time it is entered from registration. This field can also be displayed as a custom
field within the PowerChart patient demographic banner for all other providers to view.
Primary Diagnosis - displays the reason the patient is receiving care (documented through the PowerChart
Diagnosis control). By default, the diagnosis displayed will be the diagnosis type of "Discharge" and
classification type of "Medical". If the "Quick Add Diagnosis" functionality is enabled, the clinician can
click on this field to open the Consolidated Problems component, allowing them to add/modify the patient's
diagnoses.
Patient Detail View:
Patient Demographic Banner - includes patient name, age, gender and patient identifier.
Following the I-PASS standard for handoff, the following fields are displayed and available for editing:
Illness Severity (available with the 5.0 release) - ability to manually select the illness severity
that best represents the patient's current status. The options are:
Stable: The patient's condition is at an acceptable level showing no significant change.
Unstable: The patient's condition is not at an acceptable level and is requiring
interventions.
Watch: The clinician expresses a concern that the patient's condition may be worsening.
This may or may not be support by objective measurements.
Discharging: The patient is anticipated to be discharged within the next 24 hours.
Patient Summary - ability to add/modify a free text summary that briefly describes the patient and
why they are receiving care. The patient summary is not saved to the permanent medical record.
Actions - ability to add/complete free text "to do" items that serve as reminders of activities that
need to be done for the patient. These action items are not saved to the permanent medical record.
Situational Awareness - ability to add/delete free text comments to be shared between clinicians to
help communicate what is going on with the patient and plan for what might happen. These
comments are not saved to the permanent medical record.
Length of Stay / Expected LOS - the LOS and ELOS are calculated based on the admit date/time and a
documented estimated discharge date/time.
Active Diagnoses - shows all active diagnoses. The diagnoses displayed will be based on the diagnosis
type and classification selected in Bedrock.
Procedures - displays scheduled or performed procedures for the selected encounter as documented from
SurgiNet.
Code Status - displays the details related to the resuscitation status order. Cerner recommends that clients
create a resuscitation order and place that on a per encounter basis in the inpatient setting.
Clinical data (available with the 5.0 release) - provide key clinical data to the clinician at the time of
handoff, eliminating the need to open the patient's full medical record.
Initial phase of this feature will include Lab results for the last 24 / 48 hours and a list of active
medications.
Future phase to include Vital Signs, Intake/Output and Care Team.
Remove Patient - provides the ability to remove a patient from the list (patient will also be removed from
the respective PowerChart Patient List). This action is only available on "custom" patient lists.
PowerChart Table of Contents Organize the Table of Contents so that the components most vital to the Hospitalist's Workflow are sequenced first.
Then include a spacer, followed by other components that the Hospitalist might use less often, sequenced in
alphabetical order.
Hospitalist View A ViewPoint is a group of MPages designed to fit the needs of a specific position. Below you can see a ViewPoint
tailored for a Hospitalist. Each tab represents an MPage.
At a minimum, you should include in the Viewpoint:

Build for the ViewPoint is completed within bedrock.exe and prefmaint.exe. For detailed steps, refer to the All

Hospitalist Workflow
You can localize Cerner's standard MPages content to meet the specific needs of your organization and the users
within each venue of care. The MPages architecture offers organizations the ability to define unique views of the
information contained in their electronic medical record. This localization of Cerner developed standard content is
achieved with a combination of build-specific tools and end-user customization features.
Admit Workflow
Add the following components to create the Admit workflow. Within the View Builder, you can specify for each
component whether it defaults to ON, OFF or REQUIRED for the physician. From the page level component menu,
the physician can then choose to turn components on or off to better configure the page to their specific workflow
needs. The below defaults are recommended to optimize performance.

Discharge Process
Discharging a patient or ending a visit is a key process for any provider. When it is configured correctly, you can
access the discharge process though the MPages Discharge Process icon in the ViewPoint, rather than hunting for it
in the PowerChart toolbar.

You can perform a chart search from within the ViewPoint. Healthe Intent Chart Search is a powerful tool,
providing you with unprecedented search capabilities across the entire patient record.

Patient Summaries ViewPoint In addition to the Hospitalist Workflow ViewPoint, the hospitalist may also benefit from a ViewPoint containing
several summary views consisting of a collection of more condensed components to get a quick view of the patient's
current status. These components are similar to those contained in the Hospitalist Workflow but show a more
limited set of content face-up (i.e. the Vitals summary component shows 3 results face up instead of a flowsheet
view).
The Patient Summaries ViewPoint contains a standard Patient Summary view configured specifically for the
hospitalist. Additionally, it contains the Physiology View which utilizes graphs and tables to display physiologic
measurements aligned by date/time and organized based on body systems.
Configuring the Patient Summary View The Patient Summary View is intended to provide a quick view of the patient's current status. Conversely, the
Admit/Manage/Discharge Views described above should be used to provide a summary of the patient as well as
drive a specific workflow. Below, are the recommended settings and configurations for building a Patient Summary
view to be used for the Hospitalist.
The Patient Summary view should be initially built utilizing the View Builder within Bedrock. This will allow your
facility to specify the components that have been designed for the Hospitalist, while also allowing physicians the
ability to further refine their view, adding / removing components to better fit their specific workflow needs. The
following recommendations will help to ensure appropriate information is available while also facilitating optimal
system performance.

Configure the Quick Orders View The Quick Orders and Charges view is intended to provide a full screen of commonly placed orders and charges to
expedite the ordering process. This page is setup and configured utilizing the Quick Orders and Charges
Configuration MPage.
For optimal performance of Quick Orders and Charges please consider the following:

Maintain Hospitalist Favorite Folders Favorite Folders for Problems, Diagnosis, Family History, Allergies and Procedure History can be built at the
position level. You can find the most common terms entered by Hospitalists using the following queries that can be
found on the Physician Specialty Audits page. Proper utilization of these tools can decrease entry time.
Maintain Order Folders Orders Folders can be configured at the position level in DCPTools.exe > Order Management > Orders Folder
Maintenance Tool. The goal of “Order Folder Favorites” is to streamline the ordering process for a given position or
specialty. Best practices of this build process, for purposes of click-fatigue, are achieved when using shallow folder
hierarchies. Commonly placed orders can be found by utilizing this query,
PHYSEXP_COMMON_ORDERS_AUDIT.
In order to verify the most appropriate order sentences are associated to commonly placed orders, an analysis can be
done on all order details using Order Detail Analysis by Specialty. The outputs can then be copied and pasted into
this Order Details Analysis Macro, click “Organize Audit” to aggregate the data. When pasting the output into the
macro, make sure that the first column is not empty.
You can view an example of the Order Detail Analysis here. On the first page, you'll see the top 100 orders placed
by a specialty, along with an order sentence pulled together from the most commonly entered value for each
required order detail. Click on the individual order to see its entire order detail analysis.
Once Order Folders are configured, open Discernvisualdeveloper, and run the following query

The complete the following steps:
1. Find the ALT_SEL_CAT_ID of the configured folders
2. Then log into PrefMaint
3. Under the 'View' menu, select 'View PVC_NAME'
4. In the left hand pane, expand Chart, and select PowerOrders

Create Common Hospitalist Folders for Lab, Rad and Medication orders in the inpatient setting.
Every properly designed departmental home folder has three characteristics:
It never includes missing required OEF details. Every order in a departmental home folder should have
ALL OEF's defaulted-in (e.g., stat, route, dosage, reason for exam, method of transport, etc): no exceptions.
It lists orders that are almost literally in daily use. Infrequent orders do not belong in the departmental home
folder, although such orders might be useful in sub-folders. Orders that are used once a month or less do
not even belong in sub-folders. These are functional, efficient, and concrete design guidelines.
A Home Folder should NEVER go onto a second page (i.e., require scrolling).

PowerPlans The use of PowerPlans can increase the efficiency of the ordering process by grouping together orders which are
commonly placed together. Most commonly used PowerPlans can be found by running the Top Powerplans audit.
Consider creating a PowerPlan for the top diagnoses found by running the Top Diagnosis audit. The PowerPlans
below are examples, and are not based on medical evidence.
Document Types To save time during the documentation process, it is important to filter down the Document Type list to include only
the note types that are leveraged by the Hospitalist. This is done in dcptools>Documentation Management>Clinical
Document Management Tool. Select the Position Default List tab, select the position, and select the important
document types. Sequence the list to meet the needs of the Hospitalist. Below is an example of which note types to
include for Hospitalist's:

Provider Letters The new Provider Letter functionality will give the ability to easily create, print, and send provider letters from
PowerChart via fax or mail. Letter creation will leverage note templates, auto text and smart templates, similar to
patient letters, and will support unique headers and footers per physician, including graphics if desired. The National
Plan and Provider Enumeration System (NPPES) database can be used to search for the appropriate provider

Related Results Settings Related Results are results attached to orders to help make clinical decisions at the time of order entry. This allows
the hospitalist to not leave the ordering conversation screen to read results associated to the order being placed.
Related Results are most commonly associated to medication orders. Related Results can also be associated to
