Hospice Patient Census Grows to 700Across southern Wisconsin, people who are struggling with serious and life-limiting illnesses continue to ask for Agrace. In 2015, our average daily census of hospice patients reached 700 for the first time—and remains near that mark as of mid-2016. On any given day, about 700 patients are enrolled for hospice care with Agrace in 10 counties—each one on a unique end-of-life journey.
Sauk County Engagement Committee In 2015, our hospice patient census continued to grow faster than expected in Sauk County. We have cared for hundreds of Sauk County hospice patients and their families, and created several local jobs on our Baraboo-based care team. In 2015, we formed a core group of Sauk County residents who helped us connect with area health care and long-term providers, businesses, faith communities and other service organizations.
Extending Mission and VisionOver the years, we’ve seen something special happen each time Agrace expands our service area. Folks in nearby communities hear about our compassionate, quality care and they begin to ask for Agrace themselves.
That’s why we set forth plans in 2015 to begin serving patients in Richland Center and eastern Richland County (including Lone Rock, Cazenovia and Ithaca). Not only were people in Richland Center asking for Agrace, but we could see from Medicare data that less than half as many people in Richland County use hospice as in Dane County.
Launch of Care for ALLOur mission, “partnering with patients and families to improve quality of life throughout serious illness,” lives in our comprehensive, compassionate care.
In 2015, Agrace provided charity care totaling approximately $419,619. Over the past several years, requests for “Care for All“ assistance have steadily climbed. Realizing that we needed a more predictable, sustainable source of funding for this program, Agrace began a Care for All endowment campaign in 2015. Our goal is to raise $15 million over five years. The funds raised will be invested to generate annual income that will be used each year to provide care for those who qualify financially for this aid.

Returning the Favor How the Agrace Foundation Helps the CommunityWhen patients who need Agrace’s palliative care or hospice services are not covered by Medicare, Medicaid or insurance and cannot otherwise afford their care, the Agrace Foundation is here to help. Thanks to community donations to the Foundation’s Care for All program, Agrace can offer free or discounted care to patients who demonstrate financial need. Because of this program, we’ve never turned away a patient for inability to pay. Those who donate to the Agrace Foundation don’t expect to get anything in return—and yet there is a return on their investment. In 2015, Agrace provided a community benefit of $3.5 million through support of vital Agrace services and programs like these:
Grief services, including free support groups for adults and children that are open to the public
Respite care, so our patients’ caregivers can take a few days’ break
Volunteer training for the specialized roles they perform for Agrace
The Wish Program, which funds and coordinates end-of-life wishes for patients and families
The Hospice and Palliative Medicine Fellowship, which offers rotations to physicians-in-training

Palliative CareAgrace Palliative Care made approximately 500 Palliative Care Consultation visits in 2015 to help people who are struggling with serious illnesses get better relief from their symptoms, medication side effects and stress.
Grief ServicesIn 2015, Agrace provided grief services to more than 15,000 individuals—both children and adults. Although providing grief support is required by state and federal regulations, grief support services are not covered by insurance, Medicare or Medicaid reimbursement. Agrace provides these services because the organization believes they are critical to helping families return to functional living after their loss.
Grief Support GroupsAgrace’s grief support groups are open to everyone in the community who has experienced the death of a loved one, even if the person has no prior experience with Agrace services.
VolunteersIn 2015, 889 specially trained volunteers provided 76,932 hours of service to Agrace in a variety of roles, including direct service to patients and their families in their homes, and in the Agrace inpatient units in Madison and Janesville. Volunteers also supported agency efforts by volunteering in many capacities including in our offices, kitchen and gardens and at our special events, community presentations and Agrace thrift stores.
EducationIn 2015, 148 nursing students, medical students, residents and fellows, and 10 students from other health care professions participated in rotations at Agrace. Since the initiation of the Agrace Rotation Program in 1995, approximately 2,031 students, interns, residents, fellows and working health care professionals from the University of Wisconsin–Madison, Blackhawk Technical College, Edgewood College, Madison College, Herzing College, Alverno College, Benedictine University, Simmons College and other institutions of higher learning have completed an educational experience at Agrace. Agrace receives no reimbursement for these rotations with the exception of an annual stipend from the UW–Madison School of Pharmacy.
