Dear Friends,
It is with pleasure that we take this time to reflect on theaccomplishments of 2012 and thank all of our generousdonors. Together with our partners, HCP contributed to theexamination and treatment of over 701,000 patients andprovided over 46,000 surgeries, a substantial increase overour efforts (400,000 screened and 40,000 operated) in 2011.
As always, our work is amplified through partnerships withaffiliated medical institutions and INGOs working to alleviateneedless blindness. In 2012, the impact of these partnershipswas apparent throughout all of our programs, but particularlyat a high-volume event in Ethiopia. In May, with partnersfrom three continents, we managed a high-volumecataract event in Woldiya, Ethiopia that provided over1,100 sight-restoring surgeries in 6 days. While thesurgical volume is impressive and life changing for thethousands of patients and caregivers who were directlyinvolved, the power of the event was the collaboration withpractitioners and trainees, both former and current. Theevent was also noteworthy as it was Co-Founder Dr.Sanduk’s Ruit’s first visit to Sub-Saharan Africa.
The Ethiopia event is one example among many includingpartnering with London Vision Clinic Foundation andUSAID/ASHA to establish the Refractive Surgery Unit atTilganga; working with ORBIS International to improvepediatric eye care in Ghana; collaborating with SightLife toenhance eye banking in Ethiopia and Nepal; partnering withacademic medical centers, first and foremost the John A.Moran Eye Center, to host and train ophthalmologists; andproviding procurement services to The Fred HollowsFoundation, the LDS Humanitarian Division and others,expanding our reach to 26 countries.
We would like to share that Dr. Ruit is increasing histeaching efforts in Asia and has been elevated to theposition of HCP’s Founding Director. While no longer avoting member of the Board, Dr. Ruit remains integral toall of our programmatic work and is particularly focusedon training the next generation of master surgeons. Weare also pleased to announce that Adrienne Graves, PhD,joined the Board in November. Dr. Graves is a visualscientist by training and a global industry leader inophthalmology.
Finally, we would like torecognize and honor David OliverRelin who passed away inNovember. David was a bestsellingauthor, award-winning journalist,and human rights advocate whotraveled extensively in theHimalayas and Sub-Saharan Africato write Second Suns, his finalbook that chronicles the journeysof our co-founders. As wecelebrate the book’s release (June 2013), we are remindedof David and his tireless efforts to champion our work.

Striving for Sustainability at TilgangaThe Himalayan Cataract Project (HCP) and its main partner, the Tilganga Institute ofOphthalmology (TIO), are working together to enhance TIO’s financial sustainability in orderto further its mission of eradicating treatable blindness. Tilganga generates revenue frompatient fees, sales of intraocular lenses manufactured in their facility, and tuition fromspecialized ophthalmic training programs.
Modern Refractive Surgery Unit Exceeds Expectations!In its first year of operation, the Refractive Surgery Unit (RSU) provided over 700 lasersurgeries – 200 more than projected. The new state-of-the-art facility serves patients fromNepal, India, Tibet, Bhutan and the Middle East. The project was funded by TIO, HCP,USAID and London Vision Clinic (LVC). Revenues from RSU operations are used to subsidizecataract care and other operating expenses associated with Tilganga.

Tilganga’s Training Program ExpandsEducating eye care providers at all levels – ophthalmic assistants, medical students,ophthalmology residents, and ophthalmologists – is one of Tilganga’s highestpriorities. Tilganga’s internationally respected ophthalmologists and ophthalmicassistants serve as their faculty and manage the Institute’s sought-after skills transferprogram. The hands on training that takes place in outpatient clinics, operatingtheatres and outreach work ensures that trainees not only become proficient in theirspecific focus area but also understand the model for delivering high-quality,high-volume, affordable eye care.
In 2012, Tilganga trained more than 100 eye care personnel in a full range ofofferings including:
• 51 ophthalmologists from 11 different countries
• 5 ophthalmology residents
• 19 theatre nurses
• 5 management trainees
Eye Banking Advances in NepalTilganga, HCP and SightLife increased their efforts to improve cornea care in Nepal.In 2012, Tilganga hired an Eye Bank Director and 5 grief counselors to work athospitals throughout Nepal to gain consent from donor families, resulting in a50% increase in the amount of successful cornea transplantations.

Tilganga’s 12 Satellites Expand Care in Rural NepalTilganga manages one Community Hospital and eleven Community Eye Centers (CECs) throughout rural Nepal. The HCPdirectly supports four of the eleven Community Eye Centers and the HetaudaCommunity Eye Hospital (HCEH) that was established in 2007. In 2012, HCEHexperienced considerable development. With two full-time ophthalmologistsonsite and a staff of 27, it provides quality eye care to the ruralcommunities of Makwanapur and surrounding districts. In 2012, 35,297people received care – 68% at the hospital and 32% during outreachevents. HCEH also supports an additional Community Eye Center inChattiwan, which expanded its services to include telemedicine, anorthoptics clinic, and a research and training office.
Increasing Care in PhapluIn 2012, there was a 29% increase in the numberof patients served in the small, one room PhapluCommunity Eye Center in the Solukumbo region.The construction of a new, stand-alone eye clinicis scheduled to be complete in 2013 with the helpof longtime HCP supporter the Nancy AllisonPerkins Foundation.

Providing Care and CriticalTraining in Uttar PradeshHCP supported two high-volume outreach microsurgicaleye clinics in Mangahr, Uttar Pradesh. Tilganga’s vitreo-retina specialist, Dr. Govinda Paudel, led both outreachevents continuing with the ongoing cataract surgicaltraining for eye care personnel from Kripalu’s two charityhospitals. In March, Dr. Paudel hit a new milestoneduring the workshop – performing 247 high qualitycataract surgeries in one day. This is the highestnumber of surgeries that a single surgeon has performedin all of Tilganga’s work.

Bhutan: Focus on the Sub-SpecialtiesSince 2000, HCP has supported sub-specialty training for 8 Bhutanese ophthalmologistsand dozens of paramedical staff in addition to providing over 8,500 sight-restoring eyesurgeries. HCP has supported sub-specialty fellowships in pediatric ophthalmology,corneal disease, oculoplastics and vitreo-retina to reduce the number of patients beingreferred outside the country.

Tilganga Expands Further EastIn 2012, with support from HCP donors, six Indonesianophthalmologists and eight nurses received specializedtraining at Tilganga and at two workshops in Indonesiamanaged by Dr. Ruit and a team from Tilganga. Duringthe two workshops, 3,500 patients were screenedand 1,449 cataract surgeries were provided. In addition to the training, the surgicalteams also received basic surgical equipment for their home institutions.
Since 2010, Dr. Ruit has been working with A New Vision, a Singaporean NGO, to trainIndonesian ophthalmologists and ophthalmic nurses to build capacity to serve theircountries’ eye care needs.

Partners Unite in WoldiyaWith partners from three continents, HCP managed a high-volume cataractevent in Woldiya that provided over 1,100 sight-restoring surgeries in 6days in a region with one of the highest rates of bilateral blindness. Four operat-ing tables were in constant use from dawn until dusk. The clinical team includedlocal eye care professionals from three institutions in Ethiopia; Dr. Sanduk Ruit andhis outreach team from Nepal; and HCP’s Drs. Geoff Tabin and Matt Oliva and ahost of assistants. The event was part of a research initiative aimed at assessingthe costs of blindness and the economic impact of cataract surgery. Training through ServiceIn 2012, HCP supported six cataract events in Ethiopia whereby HCP volunteer ophthalmologistsvisited local partners at their home institutions to provide ongoing training in specialized surgicalskills, while providing critical care in regions with high rates of blindness.
Specialized Training Highlights• Dr. Wossen Mulugeta Zewdu of Gondar University hosted Drs. Tabin and Oliva in April,attended the American Academy of Ophthalmology (AAO) Annual Meeting in Chicago inOctober and began SICS training in Nepal with a fellow nurse in December.
• Dr. Yewubnesh Hailu Somega completed a 12-month cornea fellowship in July and receivedadditional training in Arbaminch with volunteers Drs. Doug Holsclaw and Fasika Woreta inOctober.

Ground Breaking ResearchThe HCP contracted the RAND Corporation to carry out a three-year research study to understand the costs of blindness in thedeveloping world and to measure the cost-effectiveness of sight-restoring cataract surgery, both at the household level. In 2012, fieldsurveys were conducted to assess baseline conditions of three groups of people from the North Wollo District of Ethiopia: 1) operable
cataract blind; and as controlgroups, 2) inoperable blind;and 3) non-blind. Thesituations of blind adults wereexamined – before the MayWoldiya surgical interventionorganized by HCP – relativeto their activities, socialparticipation, physical andemotional health, changes inliving arrangements, and thesubjective impacts of visionloss on the blind person. In2013, a follow up survey willbe administered and weexpect the findings to form apowerful policy tool that willenhance global understandingof the impact of blindness,and suggest how resourcescan be targeted to improveeye care wherever resourcesare limited.

GHANAReplicating the Tilganga Model at KATH –Construction nears CompletionHCP partnered with Komfo Anokye Teaching Hospital (KATH) in Kumasi, Ghana in 2006 to replicate the“Tilganga” model – that has been so successful in the Himalayas – in Sub-Saharan Africa. Theconstruction of the new 27,000 square foot Eye Care Center of Excellence was 90% complete in 2012.
New Support from USAIDIn October, HCP secured its third USAID/ASHA grant for the KATH Eye Center. The$500,000 grant is part of a $960,000 project and includes funds for a 20kw solarinstallation, non-incineration medical waste management, and additional ophthalmicequipment for the new facility.
Increased Focus on Pediatric TrainingPediatric cataract surgery requires specialized care and advancedtraining for practitioners. HCP has been working with partnersat KATH, the Moran Eye Center and ORBIS to increase KATH’spediatric capacity.

Training: Surgical Team ProfileTraining Profile: Small IncisionCataract Surgery – Doctor/NurseTeamManual Small Incision Cataract Surgery (SICS) with anintraocular lens implant continues to be the most efficientprocedure for reducing cataract blindness in the developingworld. In 2012, Tilganga trained 24 ophthalmologists (upfrom 10 in 2011) and 19 nurses in SICS.
I had a good time in Nuwakot with Dr. Ruit, we didmore than 250 cataract surgeries in 4 days – all witha very good outcome. Procurement: 26 CountriesIncreasing Impact through the Provision of Ophthalmic SuppliesThe Technology and Procurement Division was established in November 2011 to fill an expanding need for ophthalmic technologyexpertise. In addition to sourcing ophthalmic equipment for our partner institutions, the new program provides consulting toother international blindness alleviation groups including the LDS Humanitarian Division, The Fred Hollows Foundation of Australia,and ORBIS International.
In 2012, HCP procured over $1,200,000 in ophthalmic equipment and supplies. Forty-seven percent of these goods supportedHCP's ongoing programs, and 53 percent were for other INGOs, expanding our reach to 26 countries.

Since 2005, Dr. Hoffman has worked with HCP in Africa and the Himalayas,training eye care professionals from Nepal, Indonesia, Bhutan, and Ghana inpediatric ophthalmology. He has also hosted trainees at the Moran EyeCenter in Utah. In addition to traveling four to five weeks per year withHCP and the Moran Eye Center, he is also Volunteer Faculty for ORBISInternational, with whom HCP recently partnered to enhance Ghana’spediatric ophthalmology program. In 2012, Dr. Hoffman worked with thefollowing ophthalmologists at HCP partner institutions:

The story showcased Dr. Ruit’s efficient, high-quality,high-volume cataract surgery delivery system and his efforts to cure blindness for tens of thousandsof people in Nepal and other countries in Asia and Africa.
Dr. Geoff Tabin interviewed on CNNIn a March interview with CNN, Dr. Geoff Tabin discussed his international eye care work, including the December2011 visit to South Sudan where he helped provide over 300 surgeries in a remote village with no running wateror electricity.

The purpose of the paper was to analyze the rate of intraoperativecomplications, reoperations, and endophthalmitis with phacoemulsifica-tion, manual small-incision cataract surgery (SICS), and large-incision extra-capsular cataract extraction (ECCE). The setting for the study was AravindEye Hospital, Madurai, India.

The mission of the Foundation is to improve and preservelifelong vision for people nationally and internationally by originating andadvancing vision research, providing community vision programs, and expandingvision education through school partnerships and public programs. Dr. Tabindonated the $10,000 award to HCP.
Dr. Sanduk Ruit: Arthur Lim AwardDr. Sanduk Ruit received the Arthur Lim Award at the Asia Pacific Academy ofOphthalmology meeting in South Korea. The Arthur Lim Award recognizesophthalmologists who have exhibited exemplary leadership in ophthalmology,leading to substantial improvements in ophthalmic teaching and training in theirregion and beyond.
Dr. Sanduk Ruit visits the VaticanDr. Sanduk Ruit attended a special congress at the Vatican focused on blindness,where he delivered a lecture. The congress was attended by ophthalmologists,associations for the blind, and leaders of pharmaceutical and biomedicalcompanies from around the world. They discussed the newest research towardsfinding treatments for blindness and the effect they would have on medicine, theeconomy, and society at large.

Training Profile:Pediatric OphthalmologyTreating the developing eyes of infantsand children requires a differentapproach than traditional ophthalmology.Aside from the differences in the eyesof a child versus an adult, there aredifferences in possible complicationsfrom surgery, conditions specific toinfants and children, and the need forgeneral instead of local anesthetic. In2012, with support from ORBIS, Tilgangatrained an Indonesian ophthalmologyfellow, Dr. Rozalina Loebis, in specializedtechniques for pediatrics and strabismus(wandering eye).
